Background:Proctocolectomy with ileal pouch–anal anastomosis(IPAA)is the surgical procedure of choice for medically refractory ulcerative colitis and familial adenomatous *** rare,a pouch volvulus can *** aimed to determine the frequency,presentation,and management approach of pouch volvulus in patients with ***:A systematic search of published literature was performed by a medical reference librarian on 10 August 2018 and two independent reviewers identified relevant publications,extracted data,and assessed the methodological quality based on a validated tool.A retrospective review of the Mayo Clinic electronic medical records identified one case of pouch volvulus between January 2008 and August ***:The frequency of pouch volvulus from one large published study reporting long-termoutcomes of IPAA was 0.18%(3/1,700).A total of 22 patients(18 ulcerative colitis)were included(median age 32 years,73%females).Median time to volvulus after IPAA was 36 months while median interval to volvulus diagnosis from symptom onset was 24 *** pain was the most commonly reported symptom(76%).The diagnosis was made primarily by abdominal computed tomography(13/17 patients,76%).Endoscopic treatment was successful in 1 of 11 patients(9%).Surgery was performed in 20 patients and pouch-pexy and pouch excision were the most frequent surgical operations.A redo IPAA was performed in five patients(25%).Conclusion:Pouch volvulus is a rare but serious complication of IPAA and should be suspected even in the absence of obstruction *** treatment often fails and surgery is effective when performed early.

The model for gain fluency of a laser pulse amplifier is studied. Using the homotopic mapping method, firstly, the system of the original model is packed up standardization; secondly, introducing a homotopic mapping, taking the property of the mapping, inducing a contrived parameter, the solving of a nonlinear problem translates into the solving of a linear problem. Then the approximate expressions of the solution for the corresponding model are obtained. And the precision for the approximate solution is compared. It illuminates that the obtained approximate solution using the homotopic mapping method possesses higher approximate degree. At one time, the expansion of solution through the homotopic mapping method can be kept in the analytic operation. Thus it also enables us through differential and integral operations to obtain other physics behavior for the gain fluency of laser pulse amplifier.

An environmentally stable, repetition rate tunable, all-polarization-maintaining, Er-doped pulse fiber laser with a single-wall carbon nanotubes saturated absorber is demonstrated. The ring laser cavity includes a delay line enabling a tunable repetition rate to vary from 35.52 MHz to 35.64 MHz with continuous mode-locked operation. The laser output parameters confirm that the tunable mode-locked operations are stable. High environmental stability is also confirmed by the -130 dBc/Hz low phase noise, a 70-dB signal-to-noise ratio of radio frequency signals, a low amplitude fluctuation of 5.76 × 10-4, and a low fluctuation of reoetition rate of 12 Hz. The laser shows a high de^ree of oolarization of 93%.

BACKGROUND Enhanced recovery after surgery(ERAS) reduces hospitalization and complication following colorectal surgery. Whether the experience of multidisciplinary ERAS team affects patients' outcomes is *** To evaluate and establish a learning curve of ERAS program for open colorectal *** This was a review of prospectively collected database of 380 "unselected"patients undergoing elective "open" colectomy and/or proctectomy under ERAS protocol from 2011(commencing ERAS application) to 2017 in a university hospital. Patients were divided into 5 chronological groups(76 cases per quintile). Surgical outcomes and ERAS compliance among quintiles were compared. Learning curves were calculated based on criteria of optimal recovery:defined as absence of major postoperative complications, discharge by postoperative day 5, and no 30-d *** Hospitalization more than 5 d occurred in 22.6%(n = 86), major complication was present in 2.9%(n = 11) and 30-d readmission rate was 2.4%(n = 9) accounting for unsuccessful recovery of 25%(n = 95). Conversely, the overall rate of optimal recovery was 75%. The optimal recovery significantly increased from 57.9% in 1 st quintile to 72.4%-85.5% in the following quintiles(P operations are required for a multidisciplinary team to achieve a significantly higher rate of optimal recovery and high compliance with ERAS program for open colorectal surgery.

Background: Low vaccination coverage has been attributed to missed opportunities for vaccination (MOV). This study examines the prevalence of MOV, and its associated factors among children in Cameroon. Methods: Data from the 2018 Demographic and Health Survey (DHS) was analyzed for children with at least one vaccination date in the home-based record (HBR). Immunization performances such as accessibility, drop-out, and timeliness, were assessed. Service quality was assessed using MOV. Multiple logistic regression examined the effect of DHS variables on MOV outcomes, and a decision tree approach was used to study their interaction. Results: Overall, 1824 children aged 12 to 23 months were surveyed;1285 (70.45%) had cards seen with vaccination dates leading to 85.03% of immunization activities. A proportion of 46.5% of children were not completely vaccinated. There was 27% of drop-out between BCG and MCV1, and less than 10% with the specific antigens. Vaccination timeliness proportions ranged from 42.18% for BGC to less than 70% for PENTA1-3. The national prevalence of MOV for simultaneous vaccines was 75.1% (95% confidence interval (CI) = 72;79). Among those who experienced MOV, 67.4% (95% CI = 60 - 73) were uncorrected MOV. MOV was an issue in all regions and comparable in rural areas that urban areas (p = 0.2). Dose-specific MOV ranged from 2.66% (for the third dose of the pneumococcal conjugate vaccine) to 91.12 (for the yellow fever vaccine). Second birth order children experienced more MOV than first born children (adjusted odds ratio (aOR) = 1.67, 95% CI: 1.11 - 2.47). Children born to non-educated/primary level mothers had increased odds of experiencing a MOV than those born to educated mothers (aOR = 1.48, 95% CI = 1.007 - 2.19/aOR = 1.55, 95% CI = 1.12 - 2.09). Children from poorest households were at high risk of experiencing MOV for any vaccine than richest households (aOR = 2.04, 95% CI = 1.11 - 3.76). Longwall gateroad entries are subject to changing horizontal and vertical stress induced by redistribution of loads around the extracted *** stress changes can result in significant deformation of the entries that may include roof sag,rib dilation,and floor *** operators install different types of supports to control the ground response and maintain safe access and ventilation of the longwall *** paper describes recent research aimed at quantifying the effect of longwall-induced stress changes on ground stability and using the information to assess support *** research included monitoring of ground and support interaction at several operating longwall mines in the U.S.,analysis and calibration of numerical models that adequately represent the bedded rock mass,and observation of the support systems and their response to changes in *** models were then used to investigate the impact of geology and stress conditions on ground deformation and support response for various depths of cover and geologic *** research results were summarized in two regression equations that can be used to estimate the likely roof deformation and height of roof yield due to longwall-induced stress *** information is then used to assess the ability of support systems to maintain the stability of the *** application of the method is demonstrated with a retrospective analysis of the support performance at an operating longwall mine that experienced a headgate roof *** method is shown to produce realistic estimates of gateroad entry stability and support performance,allowing alternative support systems to be assessed during the design and planning stage of longwall operations.
