Pulmonary hypertension(PH)is a severe pathophysiological condition characterized by pulmonary artery remodeling and continuous increases in pulmonary artery pressure,which may eventually develop to right heart failure and *** newly discovered and incredible treatment strategies in recent years have improved the prognosis of PH,limited types of effective and economical drugs for PH still makes it as a life-threatening *** drugs from chinese materia medica(CMM)have been traditionally applied in the treatment of lung *** evidence suggests active pharmaceutical ingredients(APIs)derived from those medicines brings promising future for the prevention and treatment of *** this review,we summarized the pharmacological effects of APIs derived from CMM which are potent in treating PH,so as to provide new thoughts for initial drug discovery and identification of potential therapeutic strategies in alternative medicine for PH.

目的本研究通过儿童患者的拉米夫定群体药代动力学研究,建立了拉米夫定群体药代动力学模型,探讨进行患者依从性评价的参考浓度。方法 68名感染HIV的儿童患者接受常规抗逆转录病毒治疗,每名患者采集4～8个拉米夫定的稳态血药浓度数据,用NONMEM 7软件建立群体药代动力学模型,探讨进行患者依从性评价的参考浓度。结果拉米夫定清除率随体重增加成指数增加,并随年龄的增加以Emax渐进性曲线模型达到成熟。10 kg儿童的成熟清除率为12.1 L·h-1,其成熟半衰期为4个月。本研究发现拉米夫定存在明显的昼夜节律,夜晚的清除率比白天低16.7%,因此在采集治疗药物监测样本时需要区分早、晚浓度。拉米夫定清除率的个体间变异非常小,可以用于预测患者依从性评价的参考药物浓度。结论本研究确立了评价儿童患者依从性的参考浓度,可用血药浓度监测结果与参考浓度相比较,简单、准确判断儿童患者的依从性。

Polymyxin B,which is a last-line antibiotic for extensively drug-resistant Gram-negative bacterial infections,became available in China in *** dose adjustments are based solely on clinical experience of risk toxicity,treatment failure,and emergence of resistance,there is an urgent clinical need to perform therapeutic drug monitoring(TDM)to optimize the use of polymyxin *** is thus necessary to standardize operating procedures to ensure the accuracy of TDM and provide evidence for their rational *** report a consensus on TDM guidelines for polymyxin B,as endorsed by the Infection and Chemotherapy Committee of the Shanghai Medical Association and the therapeutic drug monitoring Committee of the chinese pharmacological *** consensus panel was composed of clinicians,pharmacists,and microbiologists from different provinces in China and Australia who made recommendations regarding target concentrations,sample collection,reporting,and explanation of TDM *** guidelines provide the first-ever consensus on conducting TDM of polymyxin B,and are intended to guide optimal clinical use.

After a number of failed drug studies on Alzheimer’s disease (AD) over the past decade, clinical trials of AD started to show encouraging results and were approved or pending approval for clinical use. However,controversies on the clinically meaningful benefits and risks of brain edema and microhemorrhages have reminded us to think further about monitoring treatment and developing new drug targets. The goal of this review is to find insights from clinical trials that aimed at two key pathological features of AD, i.e.,amyloid-β (Aβ) and tau protein, and to explore other targets such as anti-inflammation in AD. The complex pathophysiology of AD may require combination therapies rather than monotherapy. Throughout the course of AD, multiple pathways are disrupted, presenting a multitude of possible therapeutic targets for designing prevention and intervention for AD.
