Passive expansion of sub-maximally dilated transjugular intrahepatic portosystemic shunts and assessment of clinical outcomes

AIM To assess for passive expansion of sub-maximally dilated transjugular intrahepatic portosystemic shunts(TIPS) and compare outcomes with maximally dilated *** Polytetrafluoroethylene covered TIPS(Viatorr) from July 2002 to December 2013 were retrospectively reviewed at two hospitals in a single institution. Two hundred and thirty patients had TIPS maximally dilated to 10 mm(m TIPS), while 43 patients who were at increased risk for hepatic encephalopathy(HE), based on clinical evaluation or low pre-TIPS portosystemic gradient(PSG), had 10 mm TIPS sub-maximally dilated to 8 mm(sm TIPS). Group characteristics(age, gender, Model for End-Stage Liver Disease score, post-TIPS PSG and clinical outcomes were compared between groups, including clinical success(ascites or varices), primary patency,primary assisted patency, and severe post-TIPS HE. A subset of fourteen patients with sm TIPS underwent follow-up computed tomography imaging after TIPS creation, and were grouped based on time of imaging( 6 mo). Change in diameter and crosssectional area were measured with 3D imaging software to evaluate for passive *** Patient characteristics were similar between the sm TIPS and m TIPS groups, except for pre-TIPS portosystemic gradient, which was lower in the sm TIPS group(19.4 mm Hg ± 6.8 vs 22.4 mm Hg ± 7.1, P = 0.01). Primary patency and primary assisted patency between sm TIPS and m TIPS was not significantly different(P = 0.64 and 0.55, respectively). Four of the 55 patients(7%) with sm TIPS required TIPS reduction for severe refractory HE, while this occurred in 6 of the 218 patients(3%) with m TIPS(P = 0.12). For the 14 patients with follow-up computed tomography(CT) imaging, the median imaging follow-up was 373 d. There was an increase in median TIPS diameter, median percent diameter change, median area, and median percent area change in patients with CT follow-up greater than 6 mo after TIPS placement compared to follow-up within 6 mo(

Introduction: COVID-19, an emerging infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), typically presents with mild clinical manifestations in pediatric populations. As in many other regions of the Democratic Republic of the Congo (DRC), the COVID-19 pandemic affected children in Haut-Uélé, a province bordering South Sudan and Uganda. This study aimed to describe the epidemiological profile of COVID-19 in pediatric population in this area. Materials and Methods: This descriptive cross-sectional study was conducted over a 6-month period from August 1, 2021 to February 7, 2022. It included all patients aged 0 to 19 years with confirmed COVID-19 using RT-PCR and managed at the COVID-19 Treatment Centers (CTCO) in ISIRO and WATSA. Data were collected from patient records supplemented by the dataset from the service of Health Information System (SNIS) and the Epidemiological Surveillance Commission of the Provincial Health division (DPS). Epidemiological and clinical profiles were analyzed. All statistical analyses were performed using SPSS software, Version 18. Results: During the 4th wave of the pandemic, 5 out of 13 health zones in Haut-Uélé province were affected, with a total of 569 recorded cases;45 of them were children aged 0 to 19 years (7.9%). Asymptomatic cases accounted for 33.3% (15 cases). The mean age was 9.88 ± 5.49 years, with the most affected age group being 10 - 14 years (44.45%). There was a predominance of males (73.3%), with a male-to-female sex ratio of 2.75. Half of the patients (53.3%) were from Watsa Health Zone. A seasonal peak was observed between December and January. Comorbidities were present in 10% of infected patients, and nearly half of participants were contacts of infected individuals. The primary reasons for admission were fever (66.66%), followed by rhinorrhea (57.77%) and cough (31.11%). All patients responded favorably to treatment. 降低模具的开发周期和成本及提高质量和生产效率一直是金属冲压行业的重要目标。在现代高效的生产环境下,对产品数模频繁更改的快速响应是对金属成形模拟工程师的挑战。为了实现这些目标,实时完成金属成形过程数值模拟的要求已经开始显现。然而,由于数值模拟中经常采用自适应网格技术而导致模拟计算效率降低,以及在HPC作业队列中的等待时间过长,都是实现快速金属成形过程数值模拟的瓶颈。本文基于最新英特尔硬件和软件技术提出了基于最新英特尔专业工作站的优化解决方案。该优化计算模拟解决方案通过LS-DYNA代码优化,采用最新的英特尔编译器技术,英特尔(?))最新至强E5系列处理器,第二代英特尔固态设备(SSD)技术,以及全新AVX指令集等方法解决这些瓶颈。该解决方案使得金属成形模拟工程师能够及时地在自己的工作站上完成所有的工作。本文利用真实的金属成形数值模型对性能瓶颈及解决方案进行了深入分析和测试。

With the continuing development and improvement of genome-wide techniques, a great number of candidate genes are discovered. How to identify the most likely disease genes among a large number of candidates becomes a fundamental challenge in human health. A common view is that genes related to a specific or similar disease tend to reside in the same neighbourhood of biomolecular networks. Recently, based on such observations,many methods have been developed to tackle this challenge. In this review, we firstly introduce the concept of disease genes, their properties, and available data for identifying them. Then we review the recent computational approaches for prioritizing candidate disease genes based on Protein-Protein Interaction（PPI） networks and investigate their advantages and disadvantages. Furthermore, some pieces of existing software and network resources are summarized. Finally, we discuss key issues in prioritizing candidate disease genes and point out some future research directions.

This paper focuses on the research of diversified e-commerce activities in e-commerce platform, mapping various business activities into the framework of resource allocation. When a certain number of users access the platform concurrently, the system allocates resources. This paper abandons the traditional simple filtering mode to analyze the historical data of ecommerce platform, abstracts the user commodity binary network and divides the user communities according to the attribute information of users and commodities and the purchase logs of users. By analyzing the community of users, we can allocate resources reasonably. In the binary network of users and goods, there are still many problems in user based clustering. For example, the sparseness of user attribute information, the proportion of intersection between users’ purchasing goods is too small and so on. According to the characteristics of data and the existing problems, this paper selects the SimRank algorithm and improves it. In this paper, a combined similarity index based on user attributes and network structure is proposed to improve the accuracy of user clustering, and a reasonable scheme of resource allocation is proposed by using the community division of users,which increases the accuracy of user clustering, achieves the expected design goals, and improves work efficiency.
