Background：Globally,the proportion of child deaths that occur in the neonatal period remains a high level of 37-41%.Differences of cause in neonate death exist in different regions as well as in different economic development *** specific aim of this study was to investigate the causes,characteristics,and differences of death in neonates during hospitalization in the tertiary Neonatal Intensive Care Unit （NICU） of ***：All the dead neonates admitted to 26 NICUs were included between January 1,2011,and December 31,*** the data were collected retrospectively from clinical records by a designed *** collected from each NICU were delivered to the leading institution where the results were ***：A total of 744 newborns died during the l-year survey,accounting for 1.2% of all the neonates admitted to 26 NICUs and 37.6% of all the deaths in children under 5 years of age in these *** neonate death accounted for 59.3% of all the *** leading causes of death in preterm and term infants were pulmonary disease and infection,*** early neonate period,pulmonary diseases （56.5%） occupied the largest proportion ofpreterm deaths while infection （27%） and neurologic diseases （22%） were the two main causes of term *** late neonate period,infection was the leading cause of both preterm and term neonate *** two-thirds of neonate death occurred after medical care *** the cases who might survive if receiving continuing treatment,parents＇ concern about the long-term outcomes was the main reason of medical care ***：Neonate death still accounts for a high proportion of all the deaths in children under 5 years of *** study showed the majority of neonate death occurred in preterm *** of death varied with the age of death and gestational *** and prompt evaluation of the long-term outcomes should be carried out to guide the critical decision.

Objective: We report the case of a leg ulcer in a rheumatoid arthritis (RA) patient under treatment with leflunomide, discuss the influence of the drug on the aetiopathogenesis of the ulcer and describe its successful treatment. Case summary: A 68- year- old woman with a 12- year history of RA developed a leg ulcer after 4 months of leflunomide treatment. Other ulcerogenic factors were ruled out. There were some clinical hints for rheumatoid vasculitis. The ulcer was resistant to ambulant conservative phase adapted wound bed preparation and a split skin transplantation failed. After omission of leflunomide and washout procedure with cholestyramine a second split skin transplantation resulted in complete healing. Discussion: Leflunomide inhibits the division of activated T cells and thus inhibits among others the production of proinflammatory cytokines and the adhesion of cells to the endothelium. These mechanisms may partly explain the possible influence of leflunomide on the perpetuation of the ulcer. Until now, occurrence of vasculitis and leg ulcers has been described in one case each for the novel immunomodulator leflunomide. No successful treatment of a leg ulcer under leflunomide has been described yet. Omission of leflunomide and a washout treatment in our case led to a complete healing. This may indicate a critical role of leflunomide in the maintenance of this slow healing ulcer. Conclusions: An association between leflunomide intake, occurrence of leg ulcers in RA patients and delayed wound healing should be considered.

Atrial fibrillation(AF) is the most common supraventricular arrhythmia and a major cause of *** foci originating within the pulmonary veins(PVs) are an important cause of both paroxysmal and persistent AF.A variety of endovascular and surgical techniques have been used to electrically isolate the PV from the left *** venography for localization of the PV ostium can be difficult to perform during the ablation *** the anatomy of the PV is patientspecific,non-invasive imaging techniques may provide useful diagnostic information prior to the intended *** this context,multidetector computed tomography(MDCT) visualization of the left atrial and PV anatomy prior to left atrial ablation and PV isolation is becoming increasingly *** imaging provides pre-procedural information on the left atrial anatomy,including atrial size and venous attachments,and it may identify potential post-procedural complications,such as pulmonary vein stenosis or cardiac ***,we review the relevant literature and present the current"state-of-the-art"of left atrial anatomy,PV ostia as well as the clinical aspects of refractory AF with MDCT imaging protocols and procedural aspects of PV ablation.

The aim of this study was to demonstrate the efficacy of the minimally invasive technique for recurrent pectus excavatum. Fifty patients with recurrent pectus excavatum underwent a secondary repair using the minimally invasive technique. Data were reviewed for preoperative symptomatology, surgical data, and postoperative results. Prior repairs included 27 open Ravitch procedures, 23 minimally invasive (Nuss) procedures, and 2 Leonard procedures. The prior Leonard patients were also prior Ravitches and are therefore counted only once in the analyses. The median age was 16.0 years (range, 3-25 years). The median computed tomography index was 5.3 (range, 2.9-20). Presenting symptoms included shortness of breath (80%), chest pain (70%), asthma or asthma symptoms (26%), and frequent upper respiratory tract infections (14%). Both computed tomography scan and physical exam confirmed cardiac compression and cardiac displacement. Cardiology evaluations confirmed cardiac compression (62%), cardiac displacement (72%), mitral valve prolapse (22%), murmurs (24%), and other cardiac abnormalities (30%). Preoperative pulmonary function tests demonstrated values below 80%normal in more than 50%of patients. Pectus repair was done using a single pectus bar (66%), 2 bars (32%), or 3 bars (2%). Stabilizers were used in 88%of the patients. Median length of surgical time did not significantly differ from that of primary surgeries. Complications were slightly higher than those in primary repairs and included pneumothorax requiring chest tube (14%), hemothorax (8%)-, pleural effusion requiring drainage (8%), pericarditis (4%), pneumonia (4%), and wound infection (2%). There were no deaths or cardiac perforations. Initial postoperative results were excellent in 70%, good in 28%, and fair in 2%. Late complications of bar shift requiring revision occurred in 8%. Objective: Herbal therapies are used by a substantial proportion of persons in the United States, and use of these supplements may be even higher in those with chronic liver disease. The aims of this study were to prospectively determine the proportion of US veterans with chronic hepatitis C that are currently taking vitamins and herbal medications and to evaluate factors associated with use of herbal preparations. Methods: Patients with hepatitis C who were seen in the gastroenterology, infectious disease, and primary care clinics at the VA New York Harbor Healthcare System were invited to participate in this prospective study. For comparison, healthy patients without hepatitis C were enrolled from the primary care clinics at the same medical center. Patients were interviewed by trained research coordinators who obtained detailed demographic and clinical data, as well as information on the use of antioxidants (vitamin C and E),multivitamins, and herbal medications. 要做到有效的网片修补是困难的。在过去的50年中,疝修补技术有了长足的进步,但仍然需要一些新的观念和方法来巩固疝修补术已经取得的成绩。在欧洲疝学会第30次国际大会上,疝修补术及网片试验评估领域的9名专家共同参加了一次圆桌会议,讨论当今疝修补术中亟待解决的问题;内容包括"理想"网片的定义及"理想"的网片材料。讨论的其他内容还包括轻量型网片的特点,网片替代物,外科医生在疝修补术中的作用,当今疝修补术中亟待解决的问题,以及最佳的动物试验模型。尽管人们对于理想网片的构成还没有定论,但对其重要的特点却有了更深的理解。希望这些建议可以促进疝治疗的发展,并催生出最为有效的疝修补网片材料。
