Does topical vancomycin prevent fracture-related infections in closed fractures undergoing open reduction and internal fixation?A randomised controlled trial

Purpose:The role of topical vancomycin in fracture-related infection(FRI)is debatable.Very few studies have reported their efficacy in open and high-risk extremity fractures.This study aimed to assess topical vancomycin's role in reducing FRI in closed fractures undergoing open surgical intervention with an implant.Methods:This prospective randomized cohort study was carried out between February 2021 to January 2022.Patients with isolated closed fractures,who were planned for open reduction and internal fixation within 2 weeks from the time of injury were included for this study.The data collected included age,gender,socioeconomic status,mechanism of injury,diagnosis,Tscherne classification,and time interval to take up for surgery.Patients were randomized into the intervention and control groups using the block randomization technique.The control group received only systemic antibiotic prophylaxis,whereas the intervention group received topical application of vancomycin powder in the surgical wound alongside systemic antibiotic prophylaxis.The primary outcome measure was the incidence of FRI among these individuals.Clinical and radiological findings and culture reports(in cases with infection)were recorded during the post-operative period and at 6 weeks of follow-up.All relevant statistical calculations were done using STATA statistical/data analysis-parallel edition version 16.0(StataCorp LLC).The quantitative variables like age and duration of the surgery were assessed for normalcy by Shapiro-Wilk W test.An independent samples t-test with equal variances was applied to the age data.Fisher's exact test was used for the analysis of the primary outcome measure(presence of FRI following surgery),and"Risk of FRI"and"Risk difference"between the 2 groups was calculated.The strength of the association between qualitative variables was assessed using the Fisher's exact and Chi-square tests,respectively.Results:There were 88 patients included in this study.No statistical significance was found about FRI between both grou

A Proposed Approach for Measuring Maturity Level of Software Delivery

Software delivery is vital for modern organizations, driving innovation and competitiveness. Measuring an organization’s maturity in software delivery is crucial for efficiency and quality. The Capability Maturity Model (CMM) framework provides a roadmap for improvement but assessing an organization’s CMM Level is challenging. This paper offers a quantitative approach tailored to the CMM framework, using Goal-Question-Metric (GQM) frame-works for each key process area (KPA). These frameworks include metrics and questions to compute maturity scores effectively. The study also refines practices into questions for a thorough assessment. The result is an Analysis Matrix that calculates weighted scores and an overall maturity score. This approach helps organizations assess and enhance their software delivery processes systematically, aiming for improved practices and growth.

Targeting nuclear receptors for NASH/MASH:From bench to bedside

The onset of metabolic dysfunction-associated steatohepatitis(MASH)or non-alcoholic steatohepatitis(NASH)represents a tipping point leading to liver injury and subsequent hepatic complications in the natural progression of what is now termed metabolic dysfunction-associated steatotic liver diseases(MASLD),formerly known as non-alcoholic fatty liver disease(NAFLD).With no pharmacological treat-ment currently available for MASH/NASH,the race is on to develop drugs targeting multiple facets of hepatic metabolism,inflammation,and pro-fibrotic events,which are major drivers of MASH.Nuclear receptors(NRs)regulate genomic transcription upon binding to lipophilic ligands and govern multiple aspects of liver metabolism and inflammation.Ligands of NRs may include hormones,lipids,bile acids,and synthetic ligands,which upon binding to NRs regulate the transcriptional activities of target genes.NR ligands are presently the most promising drug candidates expected to receive approval from the United States Food and Drug Administration as a pharmacological treatment for MASH.This review aims to cover the current understanding of NRs,including nuclear hormone receptors,non-steroid hormone receptors,circadian NRs,and orphan NRs,which are currently undergoing clinical trials for MASH treatment,along with NRs that have shown promising results in preclinical studies.

Solute transport and geochemical modeling of the coastal quaternary aquifer, Delta Dahab Basin, South Sinai, Egypt

The wadi dahab delta is in a dry, arid coastal zone within Egypt’s south Sinai Peninsula’s eastern portion. The primary water source is the Quaternary coastal alluvial aquifer. The groundwater salinity varies from 890to 8213 mg/L, with a mean value of 3417 mg/L. The dissolved major ions have been used to calculate the seawater mixing index(SWMI) using a linear equation that discriminates the groundwater mostly affected by water–rock interaction(SWMI 1>) and other samples mixed with Seawater(SWMI < 1). The isotopic composition of groundwater for specifically chosen groundwater samples ranges from-0.645‰ to +5.212‰ for δ^(18)O and from-9.582‰ to + 22.778‰ for δ^(2)H, where the seawater represented by a Red Sea water sample(δ^(18)O + 1.64‰-δ^(2)H + 9.80‰) and reject brine water are considerably enriched the isotopic groundwater values. The geochemical NETPATH model constrained by the dissolved significant ions, isotopes, and the rock aquifer forming minerals as phases indicate the mixing percent with the seawater ranges from 9% to 97% of seawater from 91% to 3% of original recharge water. According to the SEAWAT 3-D flow models, seawater has penetrated the Northeastern Dahab delta aquifer, with the intrusion zone extending1500 m inland. The salt dissolution, upwelling of saline water, recharge from the upstream mountain block, and seawater encroachment are the primary aspects contributing to the deterioration of groundwater quality. These findings may have significance for effective groundwater withdrawal management in arid locations worldwide with similar hydrogeological systems.

Performance assessment of prefabricated vertical drains in mitigating soil reliquefaction subjected to repeated seismic events using shaking table experiments

The use of prefabricated vertical drains(PVD)in liquefiable deposits is gaining attention due to enhanced drainage.However,investigations on PVD in mitigating re-liquefaction during repeated shaking events are not available.This study performed a series of shaking table experiments on untreated and PVD-treated specimens prepared with 40%and 60%relative density.Repeated sinusoidal loading was applied with an incremental peak acceleration of 0.1g,0.2g,0.3g,and 0.4g,at 5 Hz shaking frequency with 40 s duration.The performance of treated ground was evaluated based on the generation and dissipation of excess pore water pressure(EPWP),induced sand densification,subsidence,and cyclic stress ratio.In addition,the strain accumulated in fresh and exhumed PVD was investigated using geotextile tensile testing apparatus aided with digital image correlation.No evidence of pore pressure was reported up to 0.2g peak acceleration for 40%and 60%relative density specimens.The continuous occurrence of soil densification and drainage medium restrained and delayed the generation of EPWP and expedited the dissipation process.This study demonstrates PVD can mitigate re-liquefaction,without suffering from deterioration,when subjected to medium to high intense repeated shaking events.

Application of CFD and FEA Coupling to Predict Structural Dynamic Responses of A Trimaran in Uni-and Bi-Directional Waves

To predict the wave loads of a flexible trimaran in different wave fields,a one-way interaction numerical simulation method is proposed by integrating the fluid solver(Star-CCM+)and structural solver(Abaqus).Differing from the existing coupled CFD-FEA method for monohull ships in head waves,the presented method equates the mass and stiffness of the whole ship to the hull shell so that any transverse and longitudinal section stress of the hull in oblique waves can be obtained.Firstly,verification study and sensitivity analysis are carried out by comparing the trimaran motions using different mesh sizes and time step schemes.Discussion on the wave elevation of uni-and bi-directional waves is also carried out.Then a comprehensive analysis on the structural responses of the trimaran in different uni-directional regular wave and bi-directional cross sea conditions is carried out,respectively.Finally,the differences in structural response characteristics of trimaran in different wave fields are studied.The results show that the present method can reduce the computational burden of the two-way fluid-structure interaction simulations.

Machine learning algorithm partially reconfigured on FPGA for an image edge detection system

Unmanned aerial vehicles(UAVs)have been widely used in military,medical,wireless communications,aerial surveillance,etc.One key topic involving UAVs is pose estimation in autonomous navigation.A standard procedure for this process is to combine inertial navigation system sensor information with the global navigation satellite system(GNSS)signal.However,some factors can interfere with the GNSS signal,such as ionospheric scintillation,jamming,or spoofing.One alternative method to avoid using the GNSS signal is to apply an image processing approach by matching UAV images with georeferenced images.But a high effort is required for image edge extraction.Here a support vector regression(SVR)model is proposed to reduce this computational load and processing time.The dynamic partial reconfiguration(DPR)of part of the SVR datapath is implemented to accelerate the process,reduce the area,and analyze its granularity by increasing the grain size of the reconfigurable region.Results show that the implementation in hardware is 68 times faster than that in software.This architecture with DPR also facilitates the low power consumption of 4 mW,leading to a reduction of 57%than that without DPR.This is also the lowest power consumption in current machine learning hardware implementations.Besides,the circuitry area is 41 times smaller.SVR with Gaussian kernel shows a success rate of 99.18%and minimum square error of 0.0146 for testing with the planning trajectory.This system is useful for adaptive applications where the user/designer can modify/reconfigure the hardware layout during its application,thus contributing to lower power consumption,smaller hardware area,and shorter execution time.

Metabolomics for the diagnosis of bladder cancer: A systematic review

Objective: Metabolomics has been extensively utilized in bladder cancer (BCa) research, employing mass spectrometry and nuclear magnetic resonance spectroscopy to compare various variables (tissues, serum, blood, and urine). This study aimed to identify potential biomarkers for early BCa diagnosis.Methods: A search strategy was designed to identify clinical trials, descriptive and analytical observational studies from databases such as Medline, Embase, Cochrane Central Register of Controlled Trials, and Latin American and Caribbean Literature in Health Sciences. Inclusion criteria comprised studies involving BCa tissue, serum, blood, or urine profiling using widely adopted metabolomics techniques like mass spectrometry and nuclear magnetic resonance. Primary outcomes included description of metabolites and metabolomics profiling in BCa patients and the association of metabolites and metabolomics profiling with BCa diagnosis compared to control patients. The risk of bias was assessed using the Quality Assessment of studies of Diagnostic Accuracy.Results: The search strategy yielded 2832 studies, of which 30 case-control studies were included. Urine was predominantly used as the primary sample for metabolite identification. Risk of bias was often unclear inpatient selection, blinding of the index test, and reference standard assessment, but no applicability concerns were observed. Metabolites and metabolomics profiles associated with BCa diagnosis were identified in glucose, amino acids, nucleotides, lipids, and aldehydes metabolism.Conclusion: The identified metabolites in urine included citric acid, valine, tryptophan, taurine, aspartic acid, uridine, ribose, phosphocholine, and carnitine. Tissue samples exhibited elevated levels of lactic acid, amino acids, and lipids. Consistent findings across tissue, urine, and serum samples revealed downregulation of citric acid and upregulation of lactic acid, valine, tryptophan, taurine, glutamine, aspartic acid, uridine, ribose, and phosphocholine.

In this paper, we offer a review of type-3 fuzzy logic systems and their applications in control. The main objective of this work is to observe and analyze in detail the applications in the control area using type-3 fuzzy logic systems. In this case, we review their most important applications in control and other related topics with type-3 fuzzy systems. Intelligent algorithms have been receiving increasing attention in control and for this reason a review in this area is important. This paper reviews the main applications that make use of Intelligent Computing methods. Specifically, type-3 fuzzy logic systems. The aim of this research is to be able to appreciate, in detail, the applications in control systems and to point out the scientific trends in the use of Intelligent Computing techniques. This is done with the construction and visualization of bibliometric networks, developed with Vos Viewer Software, which it is a free Java-based program,mainly intended to be used for analyzing and visualizing bibliometric networks. With this tool, we can create maps of publications, authors, or journals based on a co-citation network or construct maps of keywords, countries based on a co-occurrence networks, research groups, etc.

Discovering hidden patterns:Association rules for cardiovascular diseases in type 2 diabetes mellitus

BACKGROUND It is increasingly common to find patients affected by a combination of type 2 diabetes mellitus(T2DM)and coronary artery disease(CAD),and studies are able to correlate their relationships with available biological and clinical evidence.The aim of the current study was to apply association rule mining(ARM)to discover whether there are consistent patterns of clinical features relevant to these diseases.ARM leverages clinical and laboratory data to the meaningful patterns for diabetic CAD by harnessing the power help of data-driven algorithms to optimise the decision-making in patient care.AIM To reinforce the evidence of the T2DM-CAD interplay and demonstrate the ability of ARM to provide new insights into multivariate pattern discovery.METHODS This cross-sectional study was conducted at the Department of Biochemistry in a specialized tertiary care centre in Delhi,involving a total of 300 consented subjects categorized into three groups:CAD with diabetes,CAD without diabetes,and healthy controls,with 100 subjects in each group.The participants were enrolled from the Cardiology IPD&OPD for the sample collection.The study employed ARM technique to extract the meaningful patterns and relationships from the clinical data with its original value.RESULTS The clinical dataset comprised 35 attributes from enrolled subjects.The analysis produced rules with a maximum branching factor of 4 and a rule length of 5,necessitating a 1%probability increase for enhancement.Prominent patterns emerged,highlighting strong links between health indicators and diabetes likelihood,particularly elevated HbA1C and random blood sugar levels.The ARM technique identified individuals with a random blood sugar level>175 and HbA1C>6.6 are likely in the“CAD-with-diabetes”group,offering valuable insights into health indicators and influencing factors on disease outcomes.CONCLUSION The application of this method holds promise for healthcare practitioners to offer valuable insights for enhancing patient treatment targeting specific subtype
