Background:Chimeric antigen receptor T(car-T)cell therapy has achieved marked therapeutic success in ameliorating hematological ***,there is an extant void in the clinical guidelines concerning the most effective chemotherapy regimen prior to chimeric antigen receptor T(car-T)cell therapy,as well as the optimal timing for car-T cell infusion *** and Methods:We employed cell-derived tumor xenograft(CDX)murine models to delineate the optimal pre-conditioning chemotherapy regimen and timing for car-T cell ***,transcriptome sequencing was implemented to identify the therapeutic targets and elucidate the underlying mechanisms governing the treatment ***:Our preclinical in vivo evaluation determined that a combination of cyclophosphamide and fludarabine,followed by the infusion of CD19 car-T cells five days subsequent to the chemotherapy,exerts the most efficacious therapeutic effect in B-cell hematological ***,RNA-seq data indicated that the therapeutic efficacy predominantly perturbs tumor cell metabolism,primarily through the inhibition of key mitochondrial targets,such as C-Jun Kinase enzyme(C-JUN).Conclusion:In summary,the present study offers critical clinical guidance and serves as an authoritative reference for the deployment of CD19 car-T cell therapy in the treatment of B-cell hematological malignancies.

Although the incidence of pediatric heart failure is low, the mortality is relatively high, with severe clinical symptoms requiring repeated hospitalization or intensive care treatment in the surviving patients. cardiac biopsy specimens have revealed a higher number of resident human cardiac progenitor cells, with greater proliferation and differentiation capacity, in the neonatal period as compared with adults, demonstrating the regeneration potential of the young heart, with rising interest in cardiac regeneration therapy in critically ill pediatric patients. We review here the available literature data, searching the MEDLINE, Google Scholar and EMBASE database for completed, and *** homepage for ongoing studies involving pediatric cardiac regeneration reports. Because of difficulties conducting randomized blinded clinical trials in pediatric patients, mostly case reports or cohort studies with a limited number of individuals have been published in the field of pediatric regenerative cardiology. The majority of pediatric autologous cell transplantations into the cardiac tissue have been performed in critically ill children with severe or terminal heart failure. Congenital heart disease, myocarditis, and idiopathic hypertrophic or dilated cardiomyopathy leading to congestive heart failure are some possible areas of interest for pediatric cardiac regeneration therapy. Autologous bone marrow mononuclear cells, progenitor cells, or cardiospheres have been applied either intracoronary or percutaneously intramyocardially in severely ill children, leading to a reported clinical benefit of cell-based cardiac therapies. In conclusion, compassionate use of autologous stem cell administration has led to at least short-term improvement in heart function and clinical stability in the majority of the critically ill pediatric patients.

We describe a patient suffering from late stent thrombosis in a paclitaxel-eluting stent which had an underexpanded ring due to the three-hundred-sixty-degree circumferential calcified plaque. Intravascular ultrasound (IVUS) revealed rotational atherectomy could success-fully ablate both the metallic ring and the calcified ring. The ablated segment was scaffolded with a new paclitaxel-eluting stent, well ex-panded and documented by IVUS. To our knowledge, this is the first case report of stent ablation for an unexpanded paclitaxel-eluting stent. From the Medline index, there were only six case reports of stent ablation. We review and summarize the operation details of stent ablation from these reports.

Syncope and near syncope are great diagnostic challenges in medicine. On the one hand, the symptom may result from a benign condition and pose little or no t hreat to health other than that related to falling. On the other hand, syncope o r near syncope can be the manifestation of a serious underlying condition that poses an imminent threat to life. Patients with a cardiac cause of syncope are a t far greater risk of dying in the first year after an episode of syncope or nea r syncope than individuals with a noncardiac cause. A cardiac cause of syncope should be considered in every patient with syncope or near syn cope, but it is particularly common in older patients or in patients with known structural hear t disease, arrhythmia, or certain electrocardiographic abnormalities. Although m any diagnostic tests may be helpful in the evaluation of syncope and near synco pe, the history, physical examination, and electrocardiogram pinpoint the cause in many circumstances. Syncope after exercise may be due to left ventricular out flow tract obstruction from aortic stenosis or hypertrophic obstructive cardiomy opathy but can also suggest the diagnosis of postexercise hypotension in which a n abnormality in autonomic regulation of vascular tone or heart rate results in vasodilation or bradycardia after moderate intensity aerobic activity. The pati ent discussed in this case highlights the importance of the clinical history in the evaluation of this condition, since the diagnosis was revealed as the patien ts story was described and eventually acted out.

AIM To test the safety and effectiveness of hypertonic saline solution(HSS + F) as a strategy for weight loss andprevention of further deterioration of renal *** Patients admitted with acute decompensated heart failure(ADHF) who received HSS + F were included in the study. After a period of a standard ADHF treatment, our patients received an intravenous infusion of furosemide(250 mg) combined with HSS(150 mL of 3% NaCl) twice a day for a mean duration of 2.3 d. Our primary outcomes were weight loss and a change in serum creatinine per day of treatment. The parameters of the period prior to treatment with HSS + F were compared with those of the period with HSS + F. RESULTS A total of 47 patients were included. The mean creatinine on admission was 155 μmol/L ± 65 μmol/L, the ejection fraction was 40% ± 17%. The experimental treatment(HSS + F) resulted in greater weight loss per day of treatment than the standard treatment(-1.4 kg/d ± 1.4 kg/d vs-0.4 kg/d ± 1.0 kg/d, P = 0.0168). Importantly, the change in creatinine was not significantly *** This study supports the effectiveness of HSS + F on weight loss in patients with ADHF. The safety profile, particularly with regard to renal function, leads us to believe that HSS + F may be a valuable option for those patients presenting with ADHF who do not respond to conventional treatment with intravenous furosemide alone.

Tracheostomy manipulations: Impact on tracheostomy safety

Importance: Tracheotomy is one of the riskiest procedures for composite morbidity within pediatric otolaryngology. During the postoperative period, each time the tracheostomy tube is manipulated, there is opportunity for morbidity (e.g. a patient is vulnerable to accidental decannulation and airway loss). Objective: To identify areas of improvement in caring for 'fresh tracheostomy' patients by determining the number of times a tracheostomy tube is manipulated from placement until discharge. The hypothesis is that the more a tracheostomy is manipulated, the higher probability of morbidity. Methods: A quality improvement initiative was conducted to map the care of patients who underwent tracheostomy placement over 12 months. Tracheostomy care and manipulation by all providers were reviewed. Complications, wound care, and respiratory treatments were also evaluated. Results: Patients were hospitalized for an average of 39 days (7–140) following tracheostomy. The first tracheostomy tube change occurred on average 6 days (5–10) following placement. Tracheostomy tubes were manipulated an average of 6 (2.5–11.9) times a day to amount to 216 (51–1091) times between placement and discharge. Bedside nurses and respiratory therapists were responsible for 95% of these actions;physicians accounted for 4%. There were 6 tracheostomy related complications. Three were accidental decannulations resulting in cardiopulmonary arrest. One of these caused long term patient morbidity. Patients with more than 4 manipulations per day during the 2 weeks following tracheostomy tube placement, were more likely to have a tracheostomy related complication than those with less than 4 (OR: 12.5;95% CI: 1.2–130.6;P = 0.0349). Interpretation: While uncommon, complications related to tracheostomy can have serious long term effects and at best prolongs length of stay for patients. Adoptive cellular immunotherapy with chimeric antigen receptor(car)T cells has emerged as a novel modality for treating relapsed and/or refractory B-cell non-Hodgkin lymphoma(B-NHL).With increasing approval of car T-cell products and advances in car T cell therapy,car T cells are expected to be used in a growing number of ***,car T-cell-associated toxicities can be severe or even fatal,thus compromising the survival benefit from this *** and studying the clinical management of these toxicities are *** contrast to other hematological malignancies,such as acute lymphoblastic leukemia and multiple myeloma,anti-CD19 car T-cell-associated toxicities in B-NHL have several distinctive features,most notably local cytokine-release syndrome(CRS).However,previously published guidelines have provided few specific recommendations for the grading and management of toxicities associated with car T-cell treatment for ***,we developed this consensus for the prevention,recognition,and management of these toxicities,on the basis of published literature regarding the management of anti-CD19 car T-cell-associated toxicities and the clinical experience of multiple Chinese *** consensus refines a grading system and classification of CRS in B-NHL and corresponding measures for CRS management,and delineates comprehensive principles and exploratory recommendations for managing anti-CD19 car T-cell-associated toxicities in addition to CRS.

Objective: To describe the quantity and type of school health services provide d by nurses in Boston Public Schools. Setting: In 2001, the 63024 students enrol led predominantly belonged to minority groups (48%black, 28%Hispanic, and 9%A sian)-and were eligible for a free or reduced-price lunch (71%). Main Outcome Measure: Analysis of the 2001-2002 Boston Public Schools Health Services datab ase. Results: A total of 63 024 students generated 721291 individual encounters with 93.5 full-time equivalent school nurses, including episodic care (57.8% ), medication administration (31.5%), procedures (6.2%), and screening (5.1 %). A total of 2420 students had an individual health care plan for the adminis tration of medications and procedures during the school day. Students with indiv idual health care plans averaged 117.9 encounters per year with school nurses, and students without averaged 7.2 encounters per year. Outcomes of encounters i ncluded school dismissal (3.0%); verbal communication with parents (10.6%), school staff (3.9%), and community agencies or health care providers (1.1%); and referral to a primary care provider (4.6%) or emergency services ( car e from school nurses in Boston Public Schools. We estimate that these children a re 8 times more likely to see a school nurse than a pediatric health care provid er. These school nurses were involved in the care of children with mental health and chronic he alth care needs. Despite the extensive amount of care, little is known about the efficacy of the care provided or the effect of these services on health care us e.
