Objective To establish rat models of chronic obstructive pulmonary disease (COPD) and study the pathological characteristics of airflow obstruction Method SO 2 inhalation method was used to establish rat models After exposure to SO 2 for 7 weeks, peak expiratory flow (PEF), peak inspiratory flow (PIF), intratracheal pressure (IP) and IP slope in rat were measured by Maclab data recording and analysis system Experimental rats with PEF less than 80% of the mean of the normal rats were classified as airflow obstructed, while those with PEF greater than 80% of mean of normal rats were non obstructed Pathological changes in airway and lung tissue were compared between these two groups Result In experimental animals, PEF was significantly decreased ( P respiratory bronchioles were more marked in experimental rats with airflow obstruction than those without airflow obstruction ( P respiratory bronchioles ( P <0 001 or P <0 05) Conclusion SO 2 inhalation may cause chronic bronchitis with airflow obstruction, i e COPD in rats COPD was induced in 64% (16 of 25) of the experimental group rats

The effect of theophylline on sleep-disordered breathing in patients with stable chronic congestive heart failure

Objective To determine the prevalence of sleep-disordered breathing in patients with stable, optimally treated chronic congestive heart failure and the effect of short-term oral theophylline therapy on periodic breathing in these patients.Methods Patients with stable, optimally treated chronic congestive heart failure were monitored by polysomnography during nocturnal sleep. The effects of theophylline therapy on periodic breathing associated with stable heart failure were observed before and after treatment.Results Patients were divided into two groups. Group Ⅰ (n =21) consisted of individuals with 15 episodes of apnea and hypopnea [as determined by the apnea-hypopnea index (AMI) ] per hour or less; Group Ⅱ (n =15, 41.7%) individuals had an index of more than 15 episodes per hour. In group Ⅱ , the AHI varied from 16. 8 to 78. 8 (42. 6 ±+15. 5) in which the obstructive AHI was 11.1 ± 8. 4 and the central AHI was 31. 5 ±9.6. Group Ⅱ had significantly more arousals (36. 8 ±21. 3 compared with 19.4 ±11.2 in group Ⅰ) that were directly attributable to episodes of apnea and hypopnea, lower arterial oxyhemoglobin saturation (76. 7% ±4. 6% compared with 86. 5% ±2. 8%) and lower left ventricular ejection fraction (24. 2% ±8. 8% compared with 31. 5%±10. 6%). Thirteen patients with compensated heart failure and periodic breathing received theophylline orally (at an average dose of 4. 3 mg/kg) for five to seven days. After treatment, the mean plasma theophylline concentration was (11. 3±2. 5) μg/ml. Theophylline therapy resulted in significant decreases in the number of AHI (20. 8±13. 2 vs. 42. 6±15. 5; P < 0. 001) and the number of episodes of central apnea-hypopnea per hour (10.1 ±7. 6 vs. 31. 5±9. 6; P<0. 001). Furthermore, the percentage of total sleep time during which arterial oxyhemoglobin saturation (SaO2) was less than 90 percent (8. 8%±8. 6% vs. 23. 4% ±24.1%; P<0. 05) and the arousals per hour (18. 7 ±21. 2 vs. 36. 8 ±21. 3; P<0. 05) were also

Although significant breakthroughs have been achieved in tuberculosis management, we still encounter numerous difficulties in diagnosis and treatment of the disease. Additionally, a new challenge, multidrug-resistant tuberculosis （MDR-TB） with unspecific clinical presentation, often results in delayed diagnosis. In this paper, we reported a case of disseminated tuberculosis with rare presentation of ventricular fibrillation, which proved resistant to both isoniazid and rifampicin. A review of literature showed that ventricular fibrillation or tachycardia in tuberculosis patients with pericarditis or myocarditis has been sporadically reported in the past, but none has been conducted involving patients with MDR-TB infections.

Pulmonary vein thrombosis is a rare disease and is usually represented as a complication of atrial fibrillation, pulmonary tumors, and Iobectomy. Although it is a potentially life threatening condition, the venous disease is easy to misdiagnose because of the non-specific symptoms, In this article, we present a 30-year-old patient who suffered from pulmonary vein thrombosis without any causes. He was diagnosed with other pulmonary disorders till the thrombus within the pulmonary vein extended into the left atrium. Left atrium mass resection and a left lower Iobectomy were undertaken with relative urgency. The postoperative course was uneventful. The patient received a long course of oral anticoagulant therapy.

Pulmonary arterial hypertension (PAH) has become an important topic of basic and clinical research in recent years. Morphologic researches have shown that specific PAH-lesions are located in the lobular small muscular arteries and correlate with hemodynamic measurements. However, it still remains to be shown how pathological changes of the small arteries in the lobule develop to PAH. Based on both fractal properties of pulmonary arterial tree and asynchronous phasic contractions of lobular arterial muscles under the evenness of the pulmonary capillary pressure (PCP) in the lung, the author has constructed an integrated model of pulmonary circulation which has produced a mathematical relationship between the mean pulmonary arterial pressure (MPAP) and the cardiac output (CO). By use of the expression between MPAP and CO, it has been able to explain the pathogenesis of PAH in terms of statistical changes among regional and temporal perfusions in the lung. In order to detect clinically the early stage of PAH, the author has suggested that it is important to establish the pulmonary functional imaging of regional and temporal perfusions.
