Surgical resection remains the only definitive treatment for colorectal liver metastasis(CRLM). However, only a minority of cases are deemed resectable at the time of diagnosis. Systemic chemotherapy along with hepatic artery infusion(HAI) is an effective and safe regional chemotherapy modality for the downstaging of patients with isolated unresectable CRLM [1].

A 52-year-old woman has undergone liver transplantation(LT)owing to fulminant viral hepatitis,MELD 42.The first cadaveric graft allocated was a large-for-size graft.LT was performed with a piggyback side-to-side cavocaval anastomosis.At the end of the procedure,the removal of the abdominal retractor produced,due to a narrow ribcage,a caval compression with liver congestion because of a reduced caval outflow.

AIM To understand the influence of frailty on postoperative outcomes for laparoscopic and open colectomy. METHODS Data were obtained from the National Surgical Quality Improvement Program(2005-2012) for patients undergoing colon resection [open colectomy(OC) and laparoscopic colectomy(LC)]. Patients were classified as non-frail(0 points), low frailty(1 point), moderate frailty(2 points), and severe frailty(≥ 3) using the Modified Frailty Index. 30-d mortality and complications were used as the primary end point and analyzed for the overall population. Complications were grouped into major and minor. Subset analysis was performed for patients undergoing colectomy(total colectomy, partial colectomy and sigmoid colectomy) and separately for patients undergoing rectal surgery(abdominoperineal resection, low anterior resection, and proctocolectomy). We analyzed the data using SAS Platform JMP Pro version 10.0.0(SAS Institute Inc., Cary, NC, United States). RESULTS A total of 94811 patients were identified; the majority underwent OC(58.7%), were white(76.9%), andnon-frail(44.8%). The median age was 61.3 years. Prolonged length of stay(LOS) occurred in 4.7%, and 30-d mortality was 2.28%. Patients undergoing OC were older(61.89 ± 15.31 vs 60.55 ± 14.93) and had a higher ASA score(48.3% ASA3 vs 57.7% ASA2 in the LC group)(P < 0.0001). Most patients were non-frail(42.5% OC vs 48% LC, P < 0.0001). Complications, prolonged LOS, and mortality were significantly more common in patients undergoing OC(P < 0.0001). OC had a higher risk of death and complications compared to LC for all frailty scores(non-frail: OR = 4.7, and OR = 4.67; mildly frail: OR = 2.51, and OR = 2.47; moderately frail: OR = 2.94, and OR = 2.02, severely frail: OR = 2.37, and OR = 2.34, P < 0.05) and an increase in absolute mortality with increasing frailty(non-frail 0.68% OC, mildly frail 1.39%, moderately frail 3.44%, and severely frail 5.83%, P < 0.0001). Pancreatic duct adenocarcinoma is one of the most fatal malignancies, with R0 resection remaining the most important part of treatment of this malignancy. However, pancreatectomy is believed to be one of the most challenging procedures and R0 resection remains the only chance for patients with pancreatic cancer to have a good prognosis. Some surgeons have tried minimally invasive pancreatic surgery, but the shortand long-term outcomes of pancreatic malignancy remain controversial between open and minimally invasive procedures. We collected comparative data about minimally invasive and open pancreatic surgery. The available evidence suggests that minimally invasive pancreaticoduodenectomy(MIPD) is as safe and feasible as open PD(OPD), and shows some benefit, such as less intraoperative blood loss and shorter postoperative hospital stay. Despite the limited evidence for MIPD in pancreatic cancer, most of the available data show that the short-term oncological adequacy is similar between MIPD and OPD. Some surgical techniques, including superior mesenteric artery-first approach and laparoscopic pancreatoduodenectomy with major vein resection, are believed to improve the rate of R0 resection. Laparoscopic distal pancreatectomy is less technically demanding and is accepted in more pancreatic centers. It is technically safe and feasible and has similar short-term oncological prognosis compared with open distal pancreatectomy.

The laparoscopic approach to the surgical treatment of colorectal liver metastases(CRLMs)is supported nowadays by high evidence(1).In the hand of expert surgeons,patients undergoing laparoscopic liver surgery(LLS)for CLRMs benefit from a number of advantages,including a shorter postoperative in-hospital stay,less pain,fewer complications and a faster recovery(1).More importantly,all these profits are achieved while preserving the oncological outcomes,which are not-inferior to those achieved by open procedures(1).Recently published long-term results of randomized trials have shown similar patient survival after laparoscopic and open surgery for CRLM(2).

Prolapse of one or more pelvic organs through the vagina,such as the uterus,bladder,or rectum,is an increasingly common occurrence in the aging female population,with rates approaching 60%of parous women.1 Its associated symptoms of vaginal bulge or bladder,bowel,or sexual dysfunction can drastically affect a woman's quality of life.Multiple surgical options for the treatment pelvic organ prolapse exist and may involve autologous tissue repair or the use of biological or synthetic grafts.Abdominal sacrocolpopexy,first described in 1957 by Arthure and Savage,involves using the sacrum as a point of support for the vaginal apex.2 The technique was further refined by Huguier and Scali by the incorporation of a graft to improve tissue strength and to improve postoperative anatomical cure rates.3 For many surgeons,abdominal sacrocolpopexy is the preferred surgical technique as it has demonstrated superior outcomes with higher postoperative success rates through restoration of normal anatomy and lower rates of prolapse recurrence and postoperative dyspareunia.

Gastric bezoars are uncommon in the bariatric surgery population. Though popular in earlier decades, the Vertical Banded Gastroplasty (VBG) is no longer a staple procedure in the United States. It has been supplanted by the Roux-en-Y gastric bypass (RYGBP) and the laparoscopic adjustable gastric band (LAGB) as the most commonly performed bariatric procedures. However, there are many patients who have previously undergone VBGs, and may present with associated complications. We present a case of a gastric obstruction caused by a bezoar in a patient who had a VBG fifteen years prior to presentation.

Transoral Endoscopic Thyroidectomy Vestibular Approach(TOETVA)is a recently described surgical technique where the thyroid and central neck can be accessed without a cutaneous incision.To date nearly 700 cases have been described within the English literature demonstrating the feasibility,safety,and efficacy of the technique.As more institutions begin to adopt the surgical approach,it is important that surgeons pay close attention to appropriate patient selection and surgical optimization to prevent experience-related complications.A valuable tool to facilitate these considerations is the utilization of surgeon-performed ultrasonography(US).While the merits of surgeon-performed US are well-documented within head&neck endocrine surgery as a whole,its value may be of even greater importance when implementing a novel surgical technique such as TOETVA.Here we highlight and summarize the role of surgeon-performed US within head&neck endocrine surgery,focusing on how it may influence patient selection and surgical planning with TOETVA.

Recently,several studies have reported local full-thickness resection techniques using flexible endoscopy for gastric tumors,such as gastrointestinal stromal tumors,gastric carcinoid tumors,and early gastric cancer(EGC). These techniques have the advantage of allowing precise resection lines to be determined using intraluminal endoscopy. Thus,it is possible to minimize the resection area and subsequent deformity. Some of these methods include:(1) classical laparoscopic and endoscopic cooperative surgery(LECS);(2) inverted LECS;(3) combination of laparoscopic and endoscopic approaches to neoplasia with non-exposure technique; and(4) non-exposed endoscopic wall-inversion surgery. Furthermore,a recent prospective multicenter trial of the sentinel node navigation surgery(SNNS) for EGC has shown acceptable results in terms of sentinel node detection rate and the accuracy of nodal metastasis. Endoscopic full-thickness resection with SNNS is expected to become a treatment option that bridges the gap between endoscopic submucosal dissection and standard surgery for EGC. In the future,the indications for these procedures for gastric tumors could be expanded.
