Complete remission of primary hepatic lymphoma in a patient with human immunodeficiency virus

Diffuse large B cell primary hepatic lymphoma is a rare disease with limited available information regarding treatment strategy. Although the liver contains lymphoid tissue and is an important site for lymphocytes activation, primary hepatic lymphoma is rare. Host factors make the liver a poor environment for malignant lymphoma development. Its coexistence with human immunodeficiency virus(hiv) infection increases morbidity and mortality risks. Additionally, jaundice increases chances of developing adverse effects from chemotherapy. Here, we report a case of diffuse large B cell primary hepatic lymphoma in a 32-year-old hiv positive man. Due to elevated liver enzyme levels and jaundice, the patient was initially treated with an R-DHAP regimen, which was replaced with an R-CHOP regimen. Restaging images with a positron emission tomography scan after the latest chemotherapy cycle confirmed remission. This is the first report of complete remission of primary hepatic diffuse large B cell lymphoma in an hiv positive patient in the English literature.

Convalescent plasma(CP)transfusion has been indicated as a promising therapy in the treatment for other emerging viral ***,the quality control of CP and individual variation in patients in different studies make it rather difficult to evaluate the efficacy and risk of CP therapy for coronavirus disease 2019(COVID-19).We aimed to explore the potential efficacy of CP therapy,and to assess the possible factors associated with its *** enrolled eight critical or severe COVID-19 patients from four *** patient was transfused with 200–400mL of CP from seven recovered *** primary indicators for clinical efficacy assessment were the changes of clinical symptoms,laboratory parameters,and radiological image after CP *** donors had a wide range of antibody levels measured by serology tests which were to some degree correlated with the neutralizing antibody(NAb)*** adverse events were observed during and after CP *** CP transfusion,six out of eight patients showed improved oxygen support status;chest CT indicated varying degrees of absorption of pulmonary lesions in six patients within 8 days;the viral load was decreased to a negative level in five patients who had the previous viremia;other laboratory parameters also tended to improve,including increased lymphocyte counts,decreased C-reactive protein,procalcitonin,and indicators for liver *** clinical efficacy might be associated with CP transfusion time,transfused dose,and the NAb levels of *** study indicated that CP might be a potential therapy for severe patients with COVID-19.

After more than 30 years of battling a global epidemic, the prospect of eliminating human immunodeficiency virus （hiv） as the most challenging infectious disease of the modern era is within our reach. Major scientific discoveries about the virus responsible for this immunodeficiency disease state, including its pathogenesis, transmission patterns and clinical course, have led to the development of potent antiretroviral drugs that offer great hopes in hiv treatment and prevention. Although these agents and many others still in development and testing are capable of effectively suppressing viral replication and survival, the medical management of hiv infection at the individual and the population levels remains challenging. Timely initiation of antiretroviral drugs, adherence to the appropriate therapeutic regimens, effective use of these agents in the pre and post-exposure prophylaxis contexts, treatment of comorbid conditions and addressin~ social and nsvcholo2ical factors involved in the care of individuals continue to be important considerations.

A distance learning Continuing Medical Education （CME） project based on live interactive presentations and a group participatory model demonstrated that important strides can be made in the quality of CME available to health care professionals in African rural settings. Implementers choose a communication model consistent with the fundamental orality of Kenyan and other sub-Saharan African countries. The project involved four hospitals and one training institution in rural Kenya. The testing of learners and focus group discussions with learners, facilitators and presenters indicated that the project＇s methodologies, that strove to be culturally and work place friendly, contributed to gains in knowledge, competencies including case management, the continuity of patient care, team work, staff morale and other issues of expressed importance to the hospital healthcare work force and hospital administrators. The learning system, known as Advancing Continuing Medical Education through Information Technology （ACME-IT）, was implemented by the Kenyan Ministry of Medical Services, EC Associates and the US-funded Aphia2 Western initiative implemented by PATH. The findings of this pilot suggests that the ACME-IT methodology that set up learning centers at the participating institutions is a promising viable alternative to the traditional and relatively expensive workshop training that characterizes much of CME in lower and middle income countries.

Antibody-dependent cellular cytotoxicity(ADCC)responses to viral infection are a form of antibody regulated immune responses mediated through the Fc *** severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)triggered ADCC responses contributes to COVID-19 disease development is currently not well *** understand the potential correlation between ADCC responses and COVID-19 disease development,we analyzed the ADCC activity and neutralizing antibody response in 255 individuals ranging from asymptomatic to fatal infections over 1 year post *** was elicited by 10 days post-infection,peaked by 11-20 days,and remained detectable until 400 days *** general,patients with severe disease had higher ADCC ***,patients who had severe disease and recovered had higher ADCC activities than patients who had severe disease and ***,ADCC activities were mediated by a diversity of epitopes in SARS-COV-2-infected mice and induced to comparable levels against SARS-CoV-2 variants of concern(VOCs)(B.1.1.7,B.1.351,and P.1)as that against the D614G mutant in human patients and vaccinated *** study indicates anti-SARS-CoV-2 ADCC as a major trait of COVID-19 patients with various conditions,which can be applied to estimate the extra-neutralization level against COVID-19,especially lethal COVID-19.

Power Relation and Condom Use in Commercial Sex Behaviors

To explore whether condom use is influenced by power relation in commercial sex behaviors. Methods Variables were designed to measure the power relation in commercial sex behaviors based on the theory of gender and power relation and data were collected from male sexually transmitted diseases （STD） patients and female commercial sex workers （FSWs） working at recreation centers or being detained in a women education center to identify the relationship between condom use and power relation in male and female respondents using bivariate and multiple regression analysis. Results A significant relationship was identified between power relation and female condom use, the higher the score of power relations, the higher frequency the condom use, but no similar result was found in males. Females got a higher score of power relation than males. Conclusions Power relation is one of the factors that influence condom use, which should be considered when relevant theories are used to study the rate of condom use. It is worthwhile exploring the relationship between safe sex and power relation in spouses and regular sex partners when interventions are adopted to prevent hiv/AIDS spreading from high risk groups to the general population.
