Obesity and its influence on liver dysfunction,morbidity and mortality after liver resection

Background:Obesity and associated steatosis is an increasing health problem *** influence on post-hepatectomy liver failure(PHLF)and after liver resection(LR)is still ***:Patients who underwent LR were investigated and divided into three groups[normal weight:body mass index(BMI)18.5-24.9 kg/m2,overweight:BMI 25.0-29.9 kg/m2,obese:BMI≥30 kg/m2]in this retrospective *** aim of this study was to assess the influence of BMI and nonalcoholic steatohepatitis(NASH)on PHLF and ***:Of 888 included patients,361(40.7%)had normal weight,360(40.5%)were overweight,167(18.8%)were *** age was 62.5 years(IQR,54-69 years).The primary indication for LR was colorectal liver metastases(CLM)(n=366,41.2%).NASH was present in 58(16.1%)of normal weight,84(23.3%)of overweight and 69(41.3%)of obese patients(P<0.001).PHLF occurred in 16.3%in normal weight,15.3%in overweight and 11.4%in obese patients(P=0.32).NASH was not associated with *** was no association between patients’weight and the occurrence of postoperative complications(P=0.45).At multivariable analysis,solely major LR[odds ratio(OR):2.7,95%confidence interval(CI):1.83-4.04;P<0.001]remained a significant predictor for ***:Postoperative complications and PHLF are comparable in normal weight,overweight and obese patients and LRs using modern techniques can be safely performed in these patients.

Comparison of a five-year clinical outcome between Chinese women and men with de novo coronary disease treated with implantation of a drug-eluting stent： a three-center, prospective, registry study

Background The gender-based differences in adverse events after drug-eluting stent （DES） implantation between Chinese women and men have not been fully studied. The present study aimed to compare the 5-year clinical outcome after DES implantation in Chinese women and men. Methods Chinese women （n=298） and men （n=698） with newly diagnosed de novo coronary lesions were studied after DES implantation. The primary endpoint was the occurrence of major adverse cardiac events （MACEs） over a 5-year follow-up, including myocardial infarction （MI）, cardiac death, and target vessel revascularization （TVR）. Propensity score matching （PSM） was used to compare the adjusted MACE rates between sexes. Results Women differed in body habitus and had increased fasting cholesterol. Fewer women presented with MI, and they had better cardiac function with less complex disease. The unadjusted rate of MI at 3 years （2.1%） and 5 years （5.0%） and MACE （25.2%） at 5 years in men was significantly higher than that of women （0.3%, 1.0% and 17.8%, P=0.050, P=0.032, and P=0.011, respectively）. After PSM, the adjusted adverse events between sexes were similar. The stent thrombosis rate rapidly increased after 2 years in men. Conclusions There were significant gender-based differences in baseline characteristics. Chinese men had equivalent outcomes to women after DES after adjustment by PSM. The increased rate of MI in men was attributed to an increased unadjusted rate of MACE.

Micro/-nanoscaled topography-coupled-mechanical action into functional biointerface

The micro/-nanoscaled functional biointerfaces aroused much interest of their key unit element to present unique functions. So far it is still difficult to describe the whole picture of this process. More and more evidences are beginning to support the theory of ‘‘mirco/-nanotopography-coupled-mechanical(TCM) action'' into functional ***, we aim to highlight TCM action on varied micro/-nanoscaled functional biointerfaces, namely to achieve better understanding of micro/-nanoscaled structures by introducing interfacial mechanical behaviors. In this article, recent progressions on ‘‘TCM on air/liquid/solid-phase biointerfaces in nature'', ‘‘in vivo TCM behaviors at micro/-nanoscales' ',‘‘TCM in micro/-nanoscaled artificial surface with living Cells' ' and ‘‘topography, interfacial curvature, mechanics' 'are reviewed. Certain new concepts of ‘‘TCM action based on spatial curvature'', ‘‘medically functional biointerfaces' ' and‘‘biomechanopharmacology'' are also proposed.

Cardiovascular risk prediction in the elderly

Heart failure(HF)in the elderly,besides being a leading cause of mortality and morbidity,is rapidly increasing in prevalence with patients aged 65 and older accounting for more than 75%of heart failure *** patients have historically been unrepresented in clinical HF trials and often present with multiple comorbidities,including frailty,depression,nutritional,functional and cognitive ***,pharmacologic challenges such as adherence to therapy,polypharmacy,altered drug pharmacokinetics and/or renal derangements make them less likely to receive guideline-directed medical therapies for *** of these various interrelated domains is key and should prompt a multidisciplinary,holistic management approach so as to optimize prognosis in this vulnerable subset of the population.

Dietary hypercholesterolemia aggravates contrast media-induced nephropathy

Background Contrast media adinistration can result in severe nephrotoxicity under pothological conditions such as diabetic nephropathy, congestive heart failure, dehydration, et al. The purpose of this study was to evaluate the effects of dietary hypercholesterolemia on contrast media-induced changes in renal function, blood flow, and *** Rats were fed either on a normal rodent diet (group N) or a high-cholesterol supplemented diet (group H; 4% cholesterol and 1% cholic acid) for 8 weeks. Half of the animals (n=6) from each diet group were then given a tail vein injection of 60% diatrizoate (6 ml/kg; group NC and group HC) and the other half were administered saline. Total serum cholesterol, triglyceride, serum creatinine, creatinine clearance rate, fractional excretion of sodium and potassium, and cortical nitric oxide production were determined one day following contrast media administration. Renal blood flow was determined by color Doppler flow imaging and pulsed-mode Doppler. Renal histopathology was observed by light *** Total serum cholesterol and resistance indices of renal blood vessels increased significantly, while creatinine clearance rate and production of nitric oxide in the renal cortex decreased markedly in group HC and group H when compared to group N and group NC. The creatinine clearance rate decreased significantly in group HC compared to group H. Serum creatinine levels and fractional excretion of sodium and potassium in group HC were significantly higher than those in the other three groups. Severe tubular degeneration and necrosis, protein cast accumulation, and medullary congestion were found in group HC. Conclusion Hypercholesterolemia is a risk factor for contrast media-induced nephropathy. Hypercholesterolemia aggravates contrast media-induced nephrotoxicity through the reduced production of nitric oxide.

BACKGROUND As a result of improved and novel treatment strategies,the spectrum of patients with cardiovascular disease is consistently ***,those patients are typically older and characterized by increased burden with *** data on the prognostic impact of age in cardiogenic shock(CS)is ***,this study investigates the prognostic impact of age in patients with *** From 2019 to 2021,consecutive patients with CS of any cause were *** prognostic value of age(i.e.,60-80 years and>80 years)was investigated for 30-day all-cause ***’s correlations,Kaplan-Meier analyses,as well as multivariable Cox proportional regression analyses were performed for *** risk assessment was performed based on the presence or absence of CS related to acute myocardial infarction(AMI).RESULTS 223 CS patients were included with a median age of 77 years(interquartile range:69-82 years).No significant difference in 30-day all-cause mortality was observed for both age-groups(54.6%vs.63.4%,log-rank P=0.169;HR=1.273,95%CI:0.886-1.831,P=0.192).In contrast,when analyzing subgroups stratified by CS-etiology,AMI-related CS patients of the group>80 years showed an increased risk of 30-day all-cause mortality(78.1%vs.60.0%,log-rank P=0.032;HR=1.635,95%CI:1.000-2.673,P=0.050),which was still evident after multivariable adjustment(HR=2.072,95%CI:1.174-3.656,P=0.012).CONCLUSIONS Age was not associated with 30-day all-cause mortality in patients with CS of mixed ***,increasing age was shown to be a significant predictor of increased mortality-risk in the subgroup of patients presenting with AMI-CS.

Intravascular ultrasound assessment of very late bare-metal stent thrombosis： a case report

Very late stent thrombosis （VLST） is increasingly being regarded as a complication of drug-eluting stents （DES）, and delayed endothelization, local hypersensitivity reactions, and late stent malapposition due to excessive positive remodeling have been postulated as mechanisms. Considering that stent endothelialization seems to be completed within 4 weeks following bare-metal stent （BMS） placement and that BMS do not possess antiproliferative coating, the mechanism of VLST may differ between patients with DES and those with BMS. We report a case of VLST 9 years after BMS implantation, in which thrombus from the ruptured neointima was confirmed by intravascular ultrasound. This finding suggests that de novo plaque rupture at the neointimal layer within the stent may be one of the explanations for VLST.
