Running Temperature and Mechanical Stability of Grease as Maintenance Parameters of Railway Bearings

Spherical roller bearings in railway car wheels are critical components whose failure may have catastrophic consequences. The present study aims to analyse the mechanical stability of greases and temperature of bearings as indicators for condition-based bearing maintenance. The performed case study includes nine fully-formulated commercial greases examined in the wheel bearings of five ore cars operated in northern Scandinavia. The studied ore cars travelled a distance of about 300 000 km during a period of three years. Small samples of the greases were taken on eight occasions to test their mechanical stability. In addition, the temperatures of the bearings were continuously recorded. After the test period, the wear, electrical damage, and corrosion of the bearings were examined. One of the findings is that the shear stress of the grease at a certain shear velocity （the certain yieldstress （CEY） value） is a good maintenance indicator and is highly dependent on the grease type. The bearing＇s wear, electrical damage and corrosion also depend on the grease type. However, no oxidation of the greases was identified. The paper also outlines a systematic methodology to determine an overall maintenance indicator for railway roller bearings which is based on the field measurements.

AIM:To analyze the outcome of hepatocellular car-cinoma(HCC)resection in cirrhosis patients,related to presence of portal hypertension(PH)and extent of ***:A retrospective analysis of 135 patients with HCC on a background of cirrhosis was submitted to curative liver ***:PH was present in 44(32.5%)*** mortality and morbidity were 2.2% and 33.7%,*** survival time in patients with or without PH was 31.6 and 65.1 mo,respectively(P=0.047);in the subgroup with Child-Pugh class A cirrhosis,median survival was 65.1 mo and 60.5 mo,respectively(P=0.257).Survival for patients submitted to limited liver resection was not significantly different in presence or absence of ***,median survival for patients after resection of 2 or more segments with or without PH was 64.4 mo and 163.9 mo,respectively(P=0.035).CONCLUSION:PH is not an absolute contraindication to liver resection in Child-Pugh class A cirrhotic patients,but resection of 2 or more segments should not be recommended in patients with PH.

Driver identification in intelligent transport systems has immense demand,considering the safety and convenience of traveling in a *** rapid growth of driver assistance systems(DAS)and driver identification system propels the need for understanding the root causes of automobile ***,in the case of insurance,it is necessary to track the number of drivers who commonly drive a car in terms of insurance *** is observed that drivers with frequent records of paying“fines”are compelled to pay higher insurance payments than drivers without any penalty *** driver identification act as an important information source for the intelligent transport *** study focuses on a similar objective to implement a machine learning-based approach for driver *** data is collected from in-vehicle sensors using the controller area network(CAN)and then converted to binary form using a one-hot encoding ***,the transformed data is dimensionally reduced using the Principal Component Analysis(PCA)technique,and further optimal parameters from the dataset are selected using Whale Optimization Algorithm(WOA).The most relevant features are selected and then fed into a Convolutional Neural Network(CNN)*** proposed model is evaluated against four different use cases of driver *** results show that the best prediction accuracy is achieved in the case of drivers without *** proposed model yielded optimal accuracy when evaluated against the K-Nearest Neighbors(KNN)and Support Vector Machines(SVM)models with and without using dimensionality reduction approaches.

The development of immunotherapies for lymphoma has undergone a revolutionary evolution over the past decades. Since the advent of rituximab as the first successful immunotherapy for B-cell non-Hodgkin lymphoma over two decades ago, a plethora of new immunotherapeutic approaches to treat lymphoma has ensued. Four of the most exciting classes of immunotherapies include:chimeric antigen receptor T-cells, bispecific antibodies, immune checkpoint inhibitors, and vaccines. However, with addition of these novel therapies the appropriate timing of treatment, optimal patient population, duration of therapy, toxicity, and cost must be considered. In this review, we describe the most-promising immunotherapeutic approaches for the treatment of lymphoma in clinical development, specifically focusing on clinical trials performed to date and strategies for improvement.

BACKGROUND： Acute pancreatitis is a relatively rare but po- tentially lethal complication after transarterial chemotherapy. This study aimed to review the complications such as acute pancreatitis after transarterial chemotherapy with or without embolization for hepatocellular carcinoma. METHODS： A total of 1632 patients with hepatocellular car- cinoma who had undergone transarterial chemoembolization from ]anuary 2000 to February 2014 in a single-center were reviewed retrospectively. We investigated the potential com- plications of transarterial chemoembolization, such as acute pancreatitis and acute pancreatitis-related complications. RESULTS： Of the 1632 patients with hepatocellular carcinoma who had undergone 5434 transarterial chemoembolizations, 1328 were male and 304 female. The median age of these pa- tients was 61 years. Most （79.6%） of the patients suffered from HBV-related hepatoceUular carcinoma. The median tumor size was 5.2 cm. Of the 1632 patients, 145 patients underwent transarterial chemoembolization with doxorubicin elut- ing bead, making up a total of 538 episodes. The remaining patients underwent transarterial chemoembolization with cisplatin. Seven （0.4%） patients suffered from acute pancre- atitis post-chemoembolization. Six patients had chemoembo- lization with doxorubicin and one had chemoembolization with cisplatin. Patients who received doxornbicin eluting bead had a higher risk of acute pancreatitis [6/145 （4.1%） vs 1/1487 （0.1%）, P〈0.0001]. Two patients had anatomical arterial variations. Four patients developed acute pancreatitis- related complications including necrotizing pancreatitis （n=3） and pseudocyst formation （n=1）. All of the 4 patients resolved after the use of antibiotics and other conservative treatment. Three patients had further transarterial chemoembolization without any complication. Minimal hepatic encephalopathy is a neuro-cognitive dysfunction which occurs in an epidemic proportion of cirrhotic patients,estimated as high as 80% of the population tested. It is characterized by a specific,complex cognitive dysfunction which is independent of sleep dysfunction or problems with overall intelligence. Although named "minimal",minimal hepatic encephalopathy(MHE) can have a far-reaching impact on quality of life,ability to function in daily life and progression to overt hepatic encephalopathy. Importantly,MHE has a profound negative impact on the ability to drive a car and may be a significant factor behind motor vehicle accidents. A crucial aspect of the clinical care of MHE patients is their driving history,which is often ignored in routine care and can add a vital dimension to the overall disease assessment. Driving history should be an integral part of care in patients with MHE. The lack of specific signs and symptoms,the preserved communication skills and lack of insight make MHE a difficult condition to diagnose. Diagnostic strategies for MHE abound,but are usually limited by financial,normative or time constraints. Recent studies into the inhibitory control and critical flicker frequency tests are encouraging since these tests can increase the rates of MHE diagnosis without requiring a psychologist. Although testing for MHE and subsequent therapy is not standard of care at this time,it is important to consider this in cirrhotics in order to improve their ability to live their life to the fullest.

Kounis syndrome seems to be not a rare disease but a rarely diagnosed disorder. Multiple causes can join forc-es and trigger the development of this syndrome. We report the first case of Kounis syndrome manifesting as myocardial infarction with cardiovascular collapse that occurred in the dialysis room following an allergic reac-tion. The dialysis apparatus material of polyurethane, polyamide, polycarbonate, silicon rubber and polypro-pylene were incriminated causes. Physicians should be aware of the causality and existence of this disorder in order to achieve early and correct diagnosis and apply the appropriate therapeutic measures.
