Evaluation of a hepatic biomarker of nutritional imbalance in juvenile red drum(Sciaenops ocellatus) fed 60% soybean meal-based diets using NMR-based metabolomics

A 12-week feeding trial with juvenile red drum(Sciaenops ocellatus) fed high-soybean meal(SBM) diets was conducted to investigate a putative biomarker of nutritional imbalance, N-formimino-L-glutamate(FIGLU). Three fishmeal-free, 60% SBM pelleted diets(named B12, Fol, and Met, respectively) were tested to evaluate the effects on growth performance and tissue metabolite profiles of supplementation of vitamin B12(0.012 mg/kg), folate(10 mg/kg), methionine(1 g/kg) respectively, above basal supplementation levels. A fourth SBM-based diet(named B12/Fol/Met) was formulated with a combination of B12, folate, and methionine to attain the above-mentioned target concentrations. A fifth 60% SBM diet(named FWS) with methionine supplementation(1 g/kg above basal supplementation levels), enriched with taurine, lysine and threonine as well as minerals, was also tested. This diet contained formulation targets and additives which have allowed for replacing fishmeal with plant proteins in rainbow trout feeds. Control diets included a fishmeal-based diet(named FM), an unsupplemented basal 60% SBM diet(named SBM60), and a “natural” diet(named N) made up of equal parts of fish(cigar minnows), squid and shrimp as a positive reference for growth performance. Formulated feeds contained approximately 37% total crude protein, approximately 14% total crude lipid and were energetically balanced. Standard growth performance metrics were measured, and tissues(liver, muscle) were collected at week 12 to evaluate diet-induced metabolic changes using nuclear magnetic resonance(NMR)-based metabolomics.Our results show that the FWS diet outperformed all other SBM diets and the FM diet under all performance metrics(P<0.05). FIGLU was not detected in fish fed the N diet but was detected in those fed the SBM diets and the FM diet. BACKGROUND Rectocutaneous fistulae are common.The infection originates within the anal glands and subsequently extends into adjacent regions,ultimately resulting in fistula development.Cellular angiofibroma(CAF),also known as an angiomy ofibroblastoma-like tumor,is a rare benign soft tissue neoplasm predominantly observed in the scrotum,perineum,and inguinal area in males and in the vulva in females.We describe the first documented case CAF that developed within a rectocutaneous fistula and manifested as a perineal mass.CASE SUMMARY In the outpatient setting,a 52-year-old male patient presented with a 2-year history of a growing perineal mass,accompanied by throbbing pain and minor scrotal abrasion.Physical examination revealed a soft,well-defined,non-tender mass at the left buttock that extended towards the perineum,without a visible opening.The initial assessment identified a soft tissue tumor,and the laboratory data were within normal ranges.Abdominal and pelvic computed tomography(CT)revealed swelling of the abscess cavity that was linked to a rectal cutaneous fistula,with a track-like lesion measuring 6 cm×0.7 cm in the left perineal region and attached to the left rectum.Rectoscope examination found no significant inner orifices.A left medial gluteal incision revealed a thick-walled mass,which was excised along with the extending tract,and curettage was performed.Histopathological examination confirmed CAF diagnosis.The patient achieved total resolution during follow-up assessments and did not require additional hospitalization.CONCLUSION CT imaging supports perineal lesion diagnosis and management.Perineal angiofibromas,even with a cutaneous fistula,can be excised transperineally.

BACKGROUND Appendiceal mucinous neoplasms(AMNs),although not classified as rare,are relatively uncommon tumors most often discovered incidentally during colorectal surgery.Accurate identification of AMNs is difficult due to non-specific sym-ptoms,overlapping tumor markers with other conditions,and the potential for misdiagnosis.This underscores the urgent need for precision in diagnosis to pre-vent severe complications.CASE SUMMARY This case report describes the unexpected discovery and treatment of a low-grade AMN(LAMN)in a 74-year-old man undergoing laparoscopic hemicolectomy for transverse colon adenocarcinoma(AC).Preoperatively,non-specific gastroin-testinal symptoms and elevated tumor markers masked the presence of AMN.The tumor,presumed to be an AMN peritoneal cyst intraoperatively,was con-firmed as LAMN through histopathological examination.The neoplasm exhibited mucin accumulation and a distinct immunohistochemical profile:Positive for Ho-meobox protein CDX-2,Cytokeratin 20,special AT-rich sequence-binding protein 2,and Mucin 2 but negative for cytokeratin 7 and Paired box gene 8.This profile aids in distinguishing appendiceal and ovarian mucinous tumors.Postoperative recovery was uncomplicated,and the patient initiated adjuvant chemotherapy for the colon AC.CONCLUSION This case highlights the diagnostic complexity of AMNs,emphasizing the need for vigilant identification to avert potential complications,such as pseudomyxoma peritonei.

BACKGROUND With less than 90 reported cases to date,stercoral perforation of the colon is a rare occurrence.Stercoral ulceration is thought to occur due to ischemic pressure necrosis of the bowel wall,which is caused by the presence of a stercoraceous mass.To underscore this urgent surgical situation concerning clinical presentation,surgical treatment,and results,we present the case of a 66-year-old man with a stercoral perforation.CASE SUMMARY A 66-year-old man with a history of hypertension,hyperlipidemia,and gout presented at the emergency department with lower abdominal pain and a low-grade fever lasting for a few hours.Abdominal computed tomography indicated a suspected bezoar(approximately 7.6 cm)in the dilated cecum,accompanied by pericolic fat stranding,mild proximal dilatation of the ileum,pneumoperitoneum,and minimal ascites.Intraoperatively,feculent peritonitis with isolated cecal perforation were observed.Consequently,a right hemicolectomy with peritoneal lavage was performed.A histopathological examination supported the intraoperative findings.CONCLUSION In stercoral perforations,a diagnosis should be diligently pursued,especially in older adults,and prompt surgical intervention should be implemented.

Increasing phosphorus fertilizer value of recycled iron phosphates by prolonged flooding and organic matter addition

Iron(Fe)minerals are commonly used to remove phosphorus(P)from waste streams,producing P-loaded Fe(Ⅲ)oxides or Fe(Ⅱ)phosphate minerals(e.g.,vivianite).These minerals may be used as fertilizers to enhance P circularity if solubilized in soil.Here,we tested the P fertilizer value of recycled Fe phosphates(FePs)in a pot trial and in an incubation experiment,hypothesizing that P release from FePs is possible under Fe(Ⅲ)-reducing conditions.First,a pot trial was set up with rice(Oryza sativa)in all combinations of soil flooding or not,three P-deficient soils(acid,neutral,and calcareous),and six FePs(three Fe(Ⅲ)Ps and three Fe(Ⅱ)Ps)referenced to triple superphosphate(TSP)or zero amendments.Shoot P uptake responded to TSP application in all treatments but only marginally to FePs.The redox potential did not decrease to-200 mV by flooding for a brief period(13 d)during the pot trial.A longer incubation experiment(60 d)was performed,including a treatment of glutamate addition to stimulate reductive conditions,and P availability was assessed with CaCl_(2)extraction of soils.Glutamate addition and/or longer incubation lowered soil redox potential to<-100 mV.On the longer term,Fe(Ⅲ)minerals released P,and adequate P was reached in the calcareous soil and in the neutral soil amended with Fe(Ⅲ)P-sludge.It can be concluded that prolonged soil flooding and organic matter addition can enhance the P fertilizer efficiency of FePs.Additionally,application of FeP in powder form may enhance P availability.

BACKGROUND Benign rectal strictures can be categorized as primary(disease-related)and secondary(surgical anastomosis-related).Secondary strictures arise from surgical complications,whereas primary strictures have diverse etiologies,including various inflammatory conditions.Benign strictures are usually managed by surgery and endoscopy.We present an unusual etiology of benign rectal stricture caused by the repeated insertion of foreign objects into the rectum for sexual purposes,resulting in rectal injury and subsequent chronic inflammation.CASE SUMMARY A 53-year-old man presented to the outpatient clinic of the Colorectal Surgery Department with symptoms of chronic constipation and bloody stools.The patient previously experienced rectal injury due to foreign object insertion for sexual purposes.Colonoscopy revealed benign circumferential narrowing of the rectum.He underwent treatment by endoscopic argon plasma coagulation and balloon dilation and follow-up as an outpatient for 4 months.A colonoscopy at the end of the follow-up period revealed no evidence of rectal stricture relapse.CONCLUSION A history of rectal injury,followed by chronic inflammation,should be considered in patients with benign rectal strictures.Management with endoscopic argon plasma coagulation and balloon dilation can prevent the need for surgical resection of benign rectal strictures.

Single Fe atom-anchored manganese dioxide for efficient removal of volatile organic compounds in refrigerator

The efficient and rapid removal of volatile organic compounds(VOCs)holds significant importance for ensuring food quality and human health,particularly within the low-temperature confined spaces in refrigerators.However,achieving effective VOCs degradation under such conditions poses challenges in terms of activating inert bonds and facilitating mass transfer.In this study,we propose a novel solution by designing a cleaner module that incorporates 1.07%single Fe atom-anchored manganese dioxide catalysts(FeSAs-MnO_(2)).The combination of single Fe atoms and defect-rich MnO_(2) substrate efficiently activates molecular oxygen,leading to enhanced generation of highly reactive oxygen species(ROS).Non-thermal plasma(NTP)and circulating fan are introduced to facilitate the regeneration of catalytic activity and improve mass transfer.The FeSAs-MnO_(2) cleaner module demonstrates exceptional performance in trimethylamine(TMA)removal,achieving a conversion efficiency of 98.9%for 9 ppm within just 9 min.Furthermore,accelerated aging tests predict an extended service life of up to 45 years for the FeSAs-MnO_(2) cleaner module,surpassing the expected lifespan of refrigerators significantly.

BACKGROUND A new nomenclature consensus has emerged for liver diseases that were previously known as non-alcoholic fatty liver disease(NAFLD)and metabolic dysfunction-associated fatty liver disease(MAFLD).They are now defined as metabolic dysfunction-associated steatotic liver disease(MASLD),which includes cardiometabolic criteria in adults.This condition,extensively studied in obese or overweight patients,constitutes around 30%of the population,with a steady increase worldwide.Lean patients account for approximately 10%-15%of the MASLD population.However,the pathogenesis is complex and is not well understood.AIM To systematically review the literature on the diagnosis,pathogenesis,characteristics,and prognosis in lean MASLD patients and provide an interpretation of these new criteria.METHODS We conducted a comprehensive database search on PubMed and Google Scholar between January 2012 and September 2023,specifically focusing on lean NAFLD,MAFLD,or MASLD patients.We include original articles with patients aged 18 years or older,with a lean body mass index categorized according to the World Health Organization criteria,using a cutoff of 25 kg/m2 for the general population and 23 kg/m2 for the Asian population.RESULTS We include 85 studies in our analysis.Our findings revealed that,for lean NAFLD patients,the prevalence rate varied widely,ranging from 3.8%to 34.1%.The precise pathogenesis mechanism remained elusive,with associations found in genetic variants,epigenetic modifications,and adaptative metabolic response.Common risk factors included metabolic syndrome,hypertension,and type 2 diabetes mellitus,but their prevalence varied based on the comparison group involving lean patients.Regarding non-invasive tools,Fibrosis-4 index outperformed the NAFLD fibrosis score in lean patients.Lifestyle modifications aided in reducing hepatic steatosis and improving cardiometabolic profiles,with some medications showing efficacy to a lesser extent.However,lean NAFLD patients exhibited a worse prognosis compared to the obese

BACKGROUND Colorectal foreign bodies are commonly encountered during surgery.They are frequently observed in men 20 to 90 years of age and have bimodal age distribution.Surgical management is necessary for cases of rectal perforation.However,surgical site infections are the most common complications after colorectal surgery.CASE SUMMARY We discuss a case of rectal perforation in a patient who presented to our hospital 2 d after its occurrence.The perforation occurred as a result of the patient inserting a sex toy in his rectum.Severe peritonitis was attributable to delayed presentation.CONCLUSION Vacuum-assisted closure was performed to treat the wound,which healed well after therapy.No complications were noted.

Identifying Malaria Hotspots Regions in Ghana Using Bayesian Spatial and Spatiotemporal Models

Background:Malaria remains a significant public health concern in Ghana,with varying risk levels across different geographical areas.Malaria affects millions of people each year and imposes a substantial burden on the health care system and population.Accurate risk estimation and mapping are crucial for effectively allocating resources and implementing targeted interventions to identify regions with disease hotspots.This study aimed to identify regions exhibiting elevated malaria risk so that public health interventions can be implemented,and to identify malaria risk predictors that can be controlled as part of public health interventions for malaria control.Methods:The data on laboratory-confirmed malaria cases from 2015 to 2021 were obtained from the Ghana Health service and Ghana Statistical service.We studied the spatial and spatiotemporal patterns of the relative risk of malaria using Bayesian spatial and spatiotemporal models.The malaria risk for each region was mapped to visually identify regions with malaria hotspots.Clustering and heterogeneity of disease risks were established using correlated and uncorrelated structures via the conditional autoregressive and Gaussian models,respectively.Parameter estimates from the marginal posterior distribution were estimated within the Integrated Nested Laplace Approximation using the R software.Results:The spatial model indicated an increased risk of malaria in the North East,Bono East,Ahafo,Central,Upper West,Brong Ahafo,Ashanti,and Eastern regions.The spatiotemporal model results highlighted an elevated malaria risk in the North East,Upper West,Upper East,Savannah,Bono East,Central,Bono,and Ahafo regions.Both spatial and spatiotemporal models identified the North East,Upper West,Bono East,Central,and Ahafo Regions as hotspots for malaria risk.Substantial variations in risk were evident across regions(H=104.9,P<0.001).Although climatic and economic factors influenced malaria infection,statistical significance was not established.Conclusions:Malaria risk was cluste
