Farnesoid X receptor(FXR)is a ligand-activated nuclear receptor and plays important roles in the regulation of metabolism and homeostasis of several important physiological substances,such as bile acids,glucose,and *** such,FXR has become a promising therapeutic target for the treatment of several metabolic diseases and liver ***,fargesone A(FA),a natural product from Magnolia fargesii was identified to be a novel,potent FXR agonist that demonstrated good in vitro and in vivo ***,the detailed interaction mechanism of FA with FXR remains *** this study,we employed multiple computational approaches including molecular docking,molecular dynamics simulation,and binding free energy calculation to address the *** comparisons of the structural dynamics and binding free energies,an optimal binding mode was identified,in which FA interacts with FXR via a direct hydrogen bond with His447 and hydrophobic interactions with multiple residues,such as Leu287,Met290,Met328,Ile352,and *** mutants,namely,H447F and L287N,were further constructed to validate the importance of the identified *** anticipate that these findings could be helpful for future rational design of new FA analogues targeting FXR.

Reperfusion following cerebral ischemia causes both structural and functional damage to brain tissue and could aggravate a patient's condition;this phenomenon is known as cerebral ischemia-reperfusion *** studies have elucidated the neuroprotective role of the sirtuin protein family(Sirtuins)in modulating cerebral ischemia-reperfusion ***,the potential of utilizing it as a novel intervention target to influence the prognosis of cerebral ischemia-reperfusion injury requires additional *** this review,the origin and research progress of Sirtuins are summarized,suggesting the involvement of Sirtuins in diverse mechanisms that affect cerebral ischemia-reperfusion injury,including inflammation,oxidative stress,blood-brain barrier damage,apoptosis,pyroptosis,and *** therapeutic avenues related to Sirtuins that may improve the prognosis of cerebral ischemia-reperfusion injury were also investigated by modulating Sirtuins expression and affecting representative pathways,such as nuclear factor-kappa B signaling,oxidative stress mediated by adenosine monophosphate-activated protein kinase,and the forkhead box *** review also summarizes the potential of endogenous substances,such as RNA and hormones,drugs,dietary supplements,and emerging therapies that regulate Sirtuins *** review also reveals that regulating Sirtuins mitigates cerebral ischemia-reperfusion injury when combined with other risk *** Sirtuins show promise as a potential target for the treatment of cerebral ischemiareperfusion injury,most recent studies are based on rodent models with circadian rhythms that are distinct from those of humans,potentially influencing the efficacy of Sirtuinstargeting drug ***,this review provides new insights into the role of Sirtuins in the pathology and treatment of cerebral ischemia-reperfusion injury.

BACKGROUND The coronavirus disease 2019(COVID-19)pandemic has made it more challenging for patients to undergo yttrium-90(Y-90)radioembolization(RE).Same day Y-90 RE provides an opportunity to minimize logistical challenges and infection risk associated with COVID-19,thus improving patient *** To describe the use of same day Y-90 RE with routine single photon emission computed tomography/computed tomography(SPECT/CT)in order to optimize *** All patients were selected for Y-90 RE through a multidisciplinary tumor board,and were screened and tested for COVID-19 infection per institutional protocol.A same day procedure was developed,consisting of angiography,imaging,and Y-90 resin particle *** SPECT/CT after technetium-99m macroaggregated albumin(Tc-99m MAA)administration was performed for assessment of arterial supply,personalized dosimetry,and extrahepatic ***-treatment Y-90 bremsstrahlung SPECT/CT was performed for confirmation of particle delivery,by utilization of energy windowing to limit signal from previously administered Tc-99m MAA *** A total of 14 patients underwent same day Y-90 RE between March and June *** lung shunt fraction was 6.13%(range 3.5%-13.1%).Y-90 RE was performed for a single lesion in 7 patients,while the remaining 7 patients had treatment of multifocal *** largest lesion measured 8.3 *** patients tolerated the procedure well and were discharged the same *** Same day Y-90 RE with resin-based microspheres is feasible,and provides an opportunity to mitigate infection risk and logistical challenges associated with the COVID-19 pandemic and *** recommend consideration of SPECT/CT,especially among patients with complex malignancies,for the potential to improve outcomes and eligibility of patients to undergo same day Y-90 RE.

Viral epidemics pose major threats to global health and economies. A hallmark of viral infection is the reshaping of host cell membranes and cytoskeletons to form organelle-like structures, known as viral factories, which support viral genome replication. Viral infection in many cases induces the cytoskeletal network to form cage-like structures around viral factories, including actin rings, microtubule cages,and intermediate filament cages. Viruses hijack various organelles to create these replication factories,such as viroplasms, spherules, double-membrane vesicles, tubes, and nuclear viral factories. This review specifically examines the roles of cytoskeletal elements and the endomembrane system in material transport, structural support, and biochemical regulation during viral factory formation. Furthermore, we discuss the broader implications of these interactions for viral replication and highlight potential future research directions.

The most common treatment of spontaneous tumor rupture hemorrhage(STRH)is transcatheter arterial embolization(TAE)followed by liver resection,and surgical navigation using near-infrared fluorescence is effective method for detecting hidden lesions and ill-defined tumor ***,due to the blockage of the tumor-supplying artery after effective TAE treatment,it is difficult to deliver sufficient fluorescent probes to the tumor *** this study,we report on the successful application of superstable homogeneous intermixed formulation technology(SHIFT)in precise conversion hepatectomy for ruptured hepatocellular carcinoma(HCC).A homogeneous lipiodol-ICG formulation obtained by SHIFT(SHIFT-ICG)was developed for clinical practice for STRH.A ruptured HCC patient received the combined protocol for embolization and fluorescence surgical navigation and exhibited excellent hemostatic *** and ICG were both effectively deposited in the primary lesion,including a small metastatic *** follow-up laparoscopic hepatectomy,SHIFT-ICG could clearly and precisely image the full tumor regions and boundaries in real time,and even indistinguishable satellite lesions still expressed a remarkable fluorescence *** conclusion,the simple and green SHIFT-ICG formulation can be effectively used in emergency embolization hemostasis and later precise fluorescence navigation hepatectomy in patients with ruptured HCC bleeding and has high clinical application value.

Portal vein thrombosis(PVT)secondary to blunt abdominal trauma associated with liver injury is extremely rare in healthy individuals as well as in minor liver injury,and it carries a high rate of morbidity and ***,acute asymptomatic PVT is difficult to *** present a young trauma patient with isolated minor liver injury associated with acute PVT.A 27-year-old man presented to the emergency department after a motor vehicle *** primary survey findings were *** secondary survey showed a large contusion(7×7 cm2)at the epigastrium with marked tenderness and localized *** CT angiography of the whole abdomen revealed liver injury grade 3 in hepatic segments 2/3 and 4b(according to the American Association for the Surgery of Trauma classification)extending near the porta hepatis with patent hepatic and portal veins and without other solid organ *** follow-up CT of the whole abdomen on post-injury day 7 showed a 1.8-cm thrombus in the left portal vein with patent right portal and hepatic veins,and a decreased size of the hepatic lacerations.A liver function test was repeated on post-injury day 4,and it revealed improved *** patient received intravenous anticoagulant therapy with low-molecular-weight heparin according to weight-based dosing for *** CT of the whole abdomen performed 2 weeks after anticoagulant therapy showed small residual thrombosis in the left portal *** patient received intravenous anticoagulant therapy for a total of 3 *** the follow-up visits at 1 month,2 months,6 months,and 1 year after the injury,the patients did not have any detectable abnormal *** post-blunt minor liver injury is an extremely rare *** the thrombosis is left untreated,serious morbidity and mortality can ***,its diagnosis in asymptomatic patients is still *** imaging is necessary for highly suspected PVT,especially in liver injury with laceratio

Bone formation and remodeling occurs throughout life and requires the sustained activity of osteoblasts and osteoclasts, particularly during periods of rapid bone growth. Despite increasing evidence linking bone cell activity to global energy homeostasis, little is known about the relative energy requirements or substrate utilization of bone cells. In these studies, we measured the uptake and distribution of glucose in the skeleton in vivo using positron-emitting 18F-fluorodeoxyglucose （[lSF]-FDG） and non-invasive, high-resolution positron emission tomography/computed tomography （PE~/CT） imaging and ex vivo autoradiography. Assessment of [~SF]-FDG uptake demonstrated that relative to other tissues bone accumulated a significant fraction of the total dose of the glucose analog..Skeletal accumulation was greatest in young mice undergoing the rapid bone formation that characterizes early development. PET/CT imaging revealed that [lSF]-FDG uptake was greatest in the epiphyseal and metaphyseal regions of long bones, which accords with the increased osteoblast numbers and activity at this skeletal site. Insulin administration significantly increased skeletal accumulation of [lSF]-FDG, while uptake was reduced in mice lacking the insulin receptor specifically in osteoblasts or fed a high-fat diet. Our results indicated that the skeleton is a site of significant glucose uptake and that its consumption by bone cells is subject to regulation by insulin and disturbances in whole-body metabolism.
