Prevalence and Characterization of Pathogens Responsible for Enteric Fever and Assessment of Their Antibiotic Susceptibility Pattern in Bangladesh

In Bangladesh, Enteric fever is a persistent health issue throughout the year, occasionally reaching epidemic levels. The growing antibiotic resistance makes treatment increasingly challenging. Therefore, studying the antibiotic resistance patterns within the population is crucial to ensure a more effective treatment strategy. This study was designed to determine the prevalence, antimicrobial susceptibility profile, and factors associated with Salmonella spp. infections among clinically suspected Enteric fever patients in Bangladesh. This study also aimed to investigate whether there has been a re-emergence in the susceptibility of bacterial strains to conventional drugs. Data were collected from February 2024 to July 2024, from patients suspected Enteric fever (fever less than seven days duration) in a Private Diagnostic Center of Bangladesh. A total of 195 blood samples were cultured, where 53.85% came out positive, among which 79.05% Salmonella typhi and 20.95% Salmonella paratyphi A were found. Prevalence of Typhoid fever was observed high among the school-going age group (0 - 15 years) patients. Both these organisms were susceptible to ceftazidime, cefixime, ceftriaxone and cefepime but resistant to nalidixic acid, ciprofloxacin and levofloxacin. Nalidixic Acid is resistant to all S. paratyphi A and sensitive to few S. typhi. Ciprofloxacin and levofloxacin showed delayed response (36.14% and 22.72% sensitive to S. typhi and S. paratyphi A, respectively) and resistance (63.85% and 77.27% resistant to S. typhi and S. paratyphi A, respectively). In case of S. typhi, the resistance was found against ampicillin (32.53%), chloramphenicol (27.71%), cotrimoxazole (24.09%) and the resistance of S. paratyphi A found against ampicillin (4.54%), chloramphenicol (0%) and cotrimoxazole (0%). This study will provide clinicians with alternative drug options and facilitate the effective treatment of Enteric fever.

12 Years Analysis of Bacterial Isolates and Their Antibiotics Sensitivity in a Tertiary Care Burns Unit

The aim of this study was to review the changes in distribution of bacterial populations and their antibiotic sensitivity over 12 years in a tertiary care burn unit. Understanding the periodic variation of isolated microorganisms and their antibiotic sensitivity helps in selecting the appropriate antimicrobial therapy before culture and sensitivity are reported. It also aids the design of antibiotics protocols. The study was retrospective. The data were obtained from the computerised hospital medical record system and the burn unit records. Overall, Pseudomonas aeruginosa was the most commonly isolated microorganism followed by Staphylococcus aureus, Meticillin-resistant Staphylococcus aureus (MRSA), and the genus Acinetobacter. Acinetobacter isolation rose rapidly and became more prevalent than P. aeruginosa over the last three years. Other organisms became isolated more frequently, such as Klebsiella pneumoniae, but their overall prevalence was low. Pseudomonas species frequency of isolation declined. P. aeruginosa, MRSA, and other microorganisms showed increasing sensitivity to a number of antibiotics. MRSA remained highly sensitive to vancomycin, and Acinetobacter showed high resistance to all antibiotics tested except colistin. K. pneumoniae was highly resistant to most of the antibiotics tested except the carbapenems, but the resistance to carbapenems increased over time.

Diabetic macular edema(DME) is the most common cause of vision loss in diabetic retinopathy,affecting 1 in 15 patients with diabetes mellitus(DM).The disruption of the inner blood-retina barrier(BRB) has been largely investigated and attributed the primary role in the pathogenesis and progression in DME, but there is increasing evidence regarding the role of outer BRB, separating the RPE from the underlying choriocapillaris,in the occurrence and evolution of *** development of novel imaging technologies has led to major improvement in the field of in vivo structural analysis of the macula allowing us to delve deeper into the pathogenesis of DME and expanding our vision regarding this *** this review we gathered the results of studies that investigated specific outer BRB optical coherence tomography parameters in patients with DM with the aim to outline the current status of its role in the pathogenesis and progression of DME and identify new research pathways contributing to the advancement of knowledge in the understanding of this condition.

The antibacterial activity of selected plants towards resistant bacteria isolated from clinical specimens

Objective:To evaluate the antibacterial activity of eight plants against methicillinresistant Staphylococcus aureus(MRSA),extended spectrum beta-lactamase and carbapenemase-resistant Enterobacteriaceae,which are the most prevalent causes of infections in ***:The antibacterial activity was calculated based on the minimum inhibitory concentration using Mueller-Hinton broth in a microdilution ***:The best antibacterial activity,calculated as minimum inhibitory concentration values,against MRSA was shown by the Kaempferia pandurata(Roxb)(***)extract(256 μg/mL) and the Senna alata(***) extract(512 μg/mL).Phytochemical screening of dried *** leaf and its extract showed the presence of flavonoids,alkaloids,saponins,quinones,tannins and sterols,while dried *** and its extract only showed the presence of flavonoids and sterols/***:*** and *** have the potential to be developed as antibacterial agents,especially against MRSA strain,but further in vivo research and discovery of the mode of its action are still needed to shed light on the effects.

Determinants of internet addiction among medical students in Casablanca: a cross-sectional study

Objective:This study aimed to estimate the prevalence and determinants of Internet addiction among medical students at the Faculty of Medicine and pharmacy of Casablanca,***:This was a cross-sectional study conducted among students at the Faculty of Medicine and pharmacy in Casablanca between October and March *** online questionnaire was administered to students to collect data and internet addiction was assessed by the Young questionnaire.A score threshold≥50 was adopted to define *** and multivariate logistic regression analyses were used to identify factors associated with internet ***:Out of a total of 4093 FMPC students enrolled in the 2020-2021 academic year,506 agreed to participate in this study,including 303 females and 203 *** mean addiction score assessed on the Young scale was(49.08±16.11).The prevalence of Internet addiction was 44.5%(225/506,95% CI:40% to 49%).Multiple regression analysis showed that being older than 20 years(OR=0.17,95% CI:0.40 to 0.64),being female(OR=1.70,95% CI:1.04 to 2.78),being in the dissertation year(6th year)(OR=5.17,95% CI:2.23 to 11.44),having a history of psychiatric consultation(OR=2.64,95% CI:1.34 to 5.21),having divorced parents(OR=2.64,95% CI:1.05 to 5.87),use of sleeping medication(OR=2.9,95% CI:1.05 to 3.70),sleep disorders(OR=2.06,95% CI:1.25 to 3.79),sleep deprivation(OR=2.26,95% CI:1.39 to 3.65),excessive daytime sleepiness(OR=5.39,95% CI:2.19 to 13.24),anxiety disorders(OR=1.47,95% CI:1.18 to 2.30),duration of internet connection(>4 h)(OR=11.43,95% CI:4.85 to 27.66),and having frequent conflicts with parents(OR=2.37,95% CI:1.49 to 3.79)and friends(OR=0.26,95% CI:0.11 to 0.65)were independently associated with internet ***:The prevalence of Internet addiction among medical students in Casablanca remains *** action on the determinants would be of great value in prevention.

Synthesis of N-(phenylcarbamothioyl)-benzamide derivatives and their cytotoxic activity against MCF-7 cells

Cancer is one of the leading causes of death both in developing countries and across the *** Indonesia,cancer ranks as the fifth primary cause of death following heart disease,stroke,respiratory tract and ***,studies on thiourea derivative compounds as anticancer agents have been profoundly conducted but still require further continuous *** the present study,we aimed to synthesize new anticancer compounds of N-(phenylcarbamothioyl)-benzamide derivatives,namely N-(phenylcarbamothioyl)-4-bromobenzamide and N-(phenylcarbamothioyl)-4-fluorobenzamide compounds and assess their activities against MCF-7 breast cancer *** initial step was to predict the drug-receptor activity through docking between the tested compounds using epidermal growth factor receptor(EGFR)(PDB code: 1 M17).The compounds were futher synthesized from the reactions between benzoyl chloride derivatives and *** structures of the new compounds were identified using FTIR,~1 H NMR,13 C NMR and mass *** cytotoxic activities(IC_(50)) to breast cancer cells of MCF-7 N-(phenylcarbamothioyl)-4-bromobenzamide compound and N-(phenylcarbamothioyl)-4-fluorobenzamide were 0.27 mM and 0.31 mM,*** two new compounds had better cytotoxic activities than those of the current hydroxyurea-based anticancer drugs(the reference compound) with an IC_(50) value of 9.76 ***,these two new compounds were not toxic to Vero normal ***,they possessed tremendous potentials as the candidates for new drugs against breast cancer.

Synthesis and anticancer evaluation of N-benzoyl-N’-phenyltiourea derivatives against human breast cancer cells(T47D)

New anti-breast cancer compounds have been found and may prove to have stronger *** predict the activities of N-benzoyl-N’-phenylthiourea(BPTU)derivatives,namely N-(3-chloro)benzoyl-N’-phenylthiourea(3-Cl-BPTU)and N-(3,4-dichloro)benzoyl-N’-phenylthiourea(3,4-2 Cl-BPTU)with Sirtuin-1 receptor(PDB code:4 I5 I),molecular docking was conducted at the beginning of this *** compounds were then synthesized from benzoyl chloride derivatives and *** structure was confirmed using FTIR,1 H NMR,13 C NMR and Mass Spectra,while the anticancer activity was tested in vitro against human breast cancer cells(T47 D)using MTT *** results indicated that the anti-cancer activities of the test compounds were better than those of the hydroxyurea as the reference compound,evidenced by the Rerank Score(RS).Furthermore,cytotoxic effect of 3-Cl-BPTU(IC50:0.43 m M)and 3,4-dichloro-BPTU(IC50:0.85 m M)showed better result compared with hydroxyurea(IC50:4.58 m M).Therefore,we concluded that these compounds could possess termendous potential as the candidate for a new anticancer drug.

Development and Validation of a Liquid Chromatography-Tandem Mass Spectrometry Method for Determination of Artemisinin in Rat Plasma

Artemisinin is a potent anti-malarial drug isolated from traditional Chinese medicinal herb, Artemisia annua. The objective of this study was to develop and validate a sensitive and specific LC-MS/MS method for the determination of artemisinin in rat plasma using amlodipine as Internal Standard. The method consist of a simple liquid-liquid extraction with methyl tertiary butyl ether （MTBE） with subsequent evaporation of the supernatant to dryness followed by the analysis of the reconstituted sample by LC-MS/？vIS with a Z-spray atmospheric pressure ionization （API） interface in the positive ion-multiple reaction monitoring mode to monitor precursor--〉product ions of m/z 282.70--〉m/z 209.0 for artemisinin and m/z 408.9--〉m/z 237.0 for amlodipine respectively. The method was linear （0.999） over the concentration range of 7.8-2000 ng/mL in rat plasma. The intra and inter-day accuracy were measured to be within 94-104.2% and precision （CV） were all less than 5%. The extraction recovery means for internal standard and all the artemisinin concentrations used were between 82-85%.

Assessing Drug Use Indicators in Health Insurance Facilities, Gezira State, Sudan, 2017-2018

Background: Inappropriate use of medicines is a global concern with serious consequences related to prescribing, dispensing, and use. WHO estimated that 50% of medicines are not used correctly on their journey from the facility to home. Objective: To assess medicines use using WHO drug core indicators regarding prescribing, patient, and facilities. Setting: Outpatients, Health centers in Wadmadani locality (Urban area) in Gezira State, Sudan. Method: A cross-sectional, prospective, analytical study was conducted in 30 health centers and 60 patients from each center were selected using a simple random sampling technique. WHO indicators form was used to collect data containing different variables. T-test at a level of confidence of 95% was used to test differences between indicators. Statistical Package for Social Science (SPSS) was used for data analysis. Results: The main prescribing indicators were 2.5 ± 0.6 for drugs per encounter, 44.1% ± 14.2%. Generic 54 ± 18.0 antibiotics, 12.0% ± 9.3% injectable, and 95.2% ± 11.5% of drugs were prescribed according to the NHIF-EML. The main patient’s indicators were, 2.9 ± 0.8 minutes for consultation time, 99.5 ± 36.8 seconds for dispensing time, and 72.5% ± 16.0% for medicines actually dispensed, 49.0% ± 18.0% for medicines adequately labeled, and 22.5% ± 7.3% of the patient
