Post-transplantation hepatocellular carcinoma recurrence: Patterns and relation between vascularity and differentiation degree

AIM: To evaluate the relationship between hepatocellular carcinoma(HCC) vascularity and grade; to describe patterns and vascular/histopathological variations of post-transplantation ***: This retrospective study included 165 patients(143 men, 22 women; median age 56.8 years, range 28-70.4 years) transplanted for HCC who had a follow-up period longer than 2 mo. Pre-transplantation dynamic computed tomography or magnetic resonance examinations were retrospectively reviewed, classifying HCC imaging enhancement pattern into hypervascular and hypovascular based on presence of wash-in during arterial phase. All pathologic reports of the explanted livers were reviewed, collecting data about HCC differentiation degree. The association between imaging vascular pattern and pathological grade was estimated using the Fisher exact test. All follow-up clinical and imaging data were reviewed for evidence of recurrence. Recurrence rate was calculated and imaging features of recurrent tumor were collected, classifying early and late recurrences based on timing(differentiation degree was collected where available. The presence of variations in imaging enhancement pattern and pathological grade between the primary tumor and the intrahepatic recurrence was evaluated and the association between imaging and histopatholgical variations was estimated by using the χ2 test. RESULTS: Of the 163 patients with imaging evidence of viable tumor, 156(95.7%) had hypervascular and 7(4.3%) hypovascular HCC. Among the 125 patients with evidence of viable tumor in the explanted liver, 19(15.2%) had grade 1, 56(44.8%) grade 2, 40(32%) grade 3 and 4(3.2%) grade 4 HCC, while the differentiation degree was not assessable for 6 patients(4.8%). On the Large-Scale Structure of the Universe as given by the Voronoi diagrams

The size distributions of 2D and 3D Voronoi cells and of cells of Vp（2, 3）,--2D cut of 3D Voronoi diagram--are explored, with the slngle-parameter （re-scaled） gamma distribution playing a central role in the analytical fitting. Observational evidence for a cellular universe is briefly reviewed. A simulated Vp（2, 3） map with galaxies lying on the cell boundaries is constructed to compare, as regards general appearance, with the observed CfA map of galaxies and voids, the parameters of the simulation being so chosen as to reproduce the largest observed void size.

Effect of Heat Loss on Laminar Flamelet Species Concentration

The effects of heat loss on the structure of laminar flamelets,which are the constitutive elements of turbulent flames under the most common operating conditions,are investigated for typical aeronautical gas-turbine operating conditions at *** magnitude of heat loss is quantified via the "enthalpy defect" measured with respect to an adiabatic flame.A procedure to generate laminar flamelets with an assigned enthalpy defect at the boundaries is devised and applied to nonpremixed propane/air flames,as propane reproduces the essential features of higher hydrocarbon *** is found,contrary to commonly held beliefs,that the enthalpy defect has a significant effect on the concentration not only of minor species,but also of main reaction *** effects are found in general to be more pronounced for fuel-rich *** impact is anticipated on the formation rate of nitric *** effects of scalar dissipation rate are also discussed.

An Experimental Investigation of the dissipation Mechanisms in the Suction Side Boundary Layer of a Turbine Blade

The present work is part of an extensive experimental activity carried out by the authors in recent years aimed at investigating the boundary layer transition phenomenon in turbine blades. The large scale of the cascade and the use of advanced LDV instrumentation and precision probe traversing mechanism resulted in high degree of spatial resolution and high accuracy of measurements. The main dissipation mechanism determining the profile losses in turbomachinery blades is the work of deformation of the mean motion within the boundary layer operated by both viscous and turbulent shear stresses. In the present paper, the local viscous and turbulent deformation works have been directly evaluated from the detailed measurements of boundary layer mean velocity and Reynolds shear stress. The results show the distributions and the relative importance of the viscous and turbulent contributions to the loss production, in relation with the boundary layer states occurring along the turbine profile.

Metabolic syndrome in hypertensive patients:An unholy alliance

For many years, it has been recognized that hypertension tends to cluster with various anthropometric and metabolic abnormalities including abdominal obesity, elevated triglycerides, reduced high-density lipoprotein cholesterol, glucose intolerance, insulin resistance and hyperuricemia. This constellation of various conditions has been transformed from a pathophysiological concept to a clinical entity, which has been defined metabolic syndrome(MetS). The consequences of the MetS have been difficult to assess without commonly accepted criteria to diagnose it. For this reason, on 2009 the International diabetes Federation, the American Heart Association and other scientific organizations proposed a unified MetS definition. The incidence of the MetS has been increasing worldwide in parallel with an increase in overweight and obesity. The epidemic proportion reached by the MetS represents a major public health challenge, because several lines of evidence showed that the MetS, even without type 2 diabetes, confers an increased risk of cardiovascular morbidity and mortality in different populations including also hypertensive patients. It is likely that the enhanced cardiovascular risk associated with MetS in patients with high blood pressure may be largely mediated through an increased prevalence of preclinical cardiovascular and renal changes, such as left ventricular hypertrophy, early carotid atherosclerosis, impaired aortic elasticity, hypertensive retinopathy and microalbuminuria. Indeed, many reports support this notion, showing that hypertensive patients with MetS exhibit, more often than those without it, these early signs of end organ damage, most of which are recognized as significant independent predictors of adverse cardiovascular outcomes.

Is maturity index an efficient tool to assess the effects of the physical disturbance on the marine nematode assemblages?—A critical interpretation of disturbance-induced maturity successions in some study cases in Maldives

Maturity index（MI）, based on nematode life strategies, has been proposed in 1990 to assess the possible variations of the terrestrial and freshwater nematode assemblages induced by anthropogenic activities. It was subsequently applied also to marine ecosystems and, even if comparatively not yet very popular, it offers a good method to assess the ecological quality in relation to a wide range of anthropogenic drivers. However, few data are available on its response to physical stress, a key factor especially in the coastal areas. In this study, marine nematode genera from two study cases carried out in Maldives are used to test both MI and life strategy traits（i.e.,c-p classes） for detecting the effects of physical disturbance. The results confirm that nematodes are well adapted to physical stress showing a general high rate of recovery. C-p scaling and MI did not seem to be appropriate for revealing this disturbance type probably because there are no empirical evidences on the life strategy of several genera, and a possible differential response to various disturbance types may be hypothesized.

Synthesis of Water-soluble, Polyester-based Dendrimer Prodrugs for Exploiting Therapeutic Properties of Two Triterpenoid Acids

Dendrimers are macromolecules characterized by high controlled size, shape and architecture, presence of inner cavities able to accommodate small molecules and many peripheral functional groups to bind target entities. They are of eminent interest for biomedical applications, including gene transfection, tissue engineering, imaging, and drug delivery. The well-known pharmacological activities of ursolic and oleanolic acids are limited by their small water solubility, non-specific cell distribution, low bioavailability, poor pharmacokinetics, and their direct administration could result in the release of thrombi. To overcome such problems, in this paper we described their physical incorporation inside amino acids-modified polyester-based dendrimers which made them highly water-soluble. IR, NMR, zeta potential, mean size of particles, buffer capacity and drug release profiles of prepared materials were reported. The achieved water-soluble complexes harmonize a polycationic character and a buffer capacity which presuppose efficient cell penetration and increased residence time with a biodegradable cell respectful scaffold, thus appearing as a promising team of not toxic prodrugs for safe administration of ursolic and oleanolic acids.
