Prognostic factors for recurrence of gastrointestinal bleeding due to Dieulafoy's lesion

AIM： To analyze the effectiveness of the endoscopic therapy and to identify prognostic factors for recurrent bleeding. METHODS： Retrospective study of patients with gas- trointestinal bleeding secondary to Dieulafoy＇s lesion （DL） from 2005 to 2011. We analyzed the demographic characteristics of the patients, risk factors for gastro- intestinal bleeding, endoscopic findings, characteristics of the endoscopic treatment, and the recurrence of bleeding. We included cases in which endoscopy de- scribed a lesion compatible with Dieulafoy. We exclud- ed patients who had potentially bleeding lesions such as angiodysplasia in other areas or had undergone other gastrointestinal endoscopic procedures. RESULTS： Twenty-nine patients with DL were identi- fied. Most of them were men with an average age of 71.5 years. Fifty-five percent of the patients received antiaggregatory or anticoagulant therapy. The most common location for DL was the stomach （51.7%）. The main type of bleeding was oozing in 65.5% of cases. In 27.6% of cases, there was arterial （spurting） bleeding, and 6.9% of the patients presented with an adherent clot. A single endoscopic treatment was ap- plied to nine patients （31%）, eight of them with adren- aline and one with argon, while 69% of the patients received combined treatment. Six patients （20.7%） presented with recurrent bleeding at a median of 4 d after endoscopy （interquartile range = 97.75）. Within these six patients, the new endoscopic treatment ob- tained a therapeutic success of 100%. The presence of arterial bleeding at endoscopy was associated with a higher recurrence rate for bleeding （50% vs 33.3% for other type of bleeding） rP = 0.024, odds ratio （OR） = 8.5, 95% CI = 1.13-63.87]. The use of combined en- doscopic treatment prevented the recurrence of bleed- ing （10% vs 44.4% of single treatment）（P = 0.034, OR = 0.14, 95% CI = 0.19-0.99）. Inhibition effect of miR-577 on hepatocellular carcinoma cell growth via targeting β-catenin

Objective:To investigate the expression and the regulation effect of cell growth of microRNA-577 in hepatocellular carcinoma(HCC).Methods:qRT-PCR was applied to detect the relative expression of miR-577 in 70 paired HCC and matched tumor adjacent tissues collecting from resection between March 2011 and March *** chi-square test was used to analyze the relationship between the miR-577 expression and clinical *** miR-577 mimics were transfected into HepG2 cells:cell cycles were detected by flow cytometry,cell proliferation was measured by MTT assay and BrdU incorporation assay,and cell apoptosis was determined by flow cytometry and Caspase 3/7 activity *** expressions ofβ-catenin were measured by *** correlation analysis was used to analyze the relationship between miR-577 and ***-PCR and westernblot were used to detect the expression of p-catenin in transfected HepG2 ***:The relative expressions of miR-577 was significantly lower in HCC tissues compared to the matched normal tumor-adjacent tissues(P<0.05).Low expression of miR-577 was significantly associated with large tumor size(≥5 cm,P<0.05) and advanced tumor node metastasis stage(Ⅲ+Ⅳ,P<0.05).Transfection of miR-577 mimics could inhibit repress cell proliferation,enhance cell apoptosis and block the cell cycles in G_1/G_1 phase(P<0.05).miR-577 in HCC group had a significant negative correlation relationship with the expression of downstream target of β-catenin(P<0.05).Both the mRNA and protein expression in HepG2 cells were down-regulated after transfection(P<0.05).Conclusions:Low expression of miR-577 is related to the malignant clinicopathological features in HCC tissues,and miR-577 may suppress HCC growth through down-regulating p-catenin.

AIM: To evaluate the factors involved in the impairment of health-related quality of life (HRQOL) in patients with celiac ***: A multicenter, cross-sectional prospective study was performed in patients with celiac disease who completed two HRQOL questionnaires: the gastrointestinal quality of life index (GIQLI) and the EuroQol-5D (EQ).RESULTS: Three hundred and forty patients (163 controlled with a gluten-free diet, and 177 newly diagnosed with a normal diet) were included. The GIQLI score was significantly better in patients on a gluten-free diet (GFD) than in non-treated patients on their usual diet, both in terms of the overall score (3.3 vs 2.7, respectively; P < 0.001), as well as on the individual questionnaire dimensions. Both the preference value of the EQ as the visual analogue scale were significantly better in treated than in non-treated patients (0.93 vs 0.72 P < 0.001 and 80 vs 70 P < 0.001, respectively). Variables significantly associated with a worse HRQOL score were female gender, failure to adhere to a GFD, and symptomatic ***: In untreated celiac disease, the most important factors that influence patient perception of health are the presence of symptoms and a normal diet. HRQOL improves to levels similar to those described in the general population in celiac disease patients well controlled with a GFD.

Experiment research of cisplatin implants inhibiting transplantation tumor growth and regulating the expression of KLK7 and E-cad of tumor-bearing mice with gastric cancer

Objective: To study the influence of cisplatin implants on transplantation tumor growth and the expression of tissue kallikrein-7(KLK7) and E-cadherin(E-cad) in tumor-bearing mice with gastric cancer. Methods: BALB/c nude mice were collected as experimental animal and were randomly divided into model control group(Group A), tail intravenous injection of cisplatin group(Group B), intratumor injection of cisplatin group(Group C) and cisplatin implants treatment group(Group D). After the drugs intervening, the weight and volume of transplantation tumors were measured on Day 20, Day 30 and Day 40 and serum and KLK7 and E-cad contents in transplanted tumor tissue were examined. Results: On Day 20, Day 30 and Day 40 after treatment, the weight and volume of transplantation tumors of tumorbearing mice in four groups were different(Group A > Group B > Group C > Group D). The contents of KLK-7 and E-cad in tumor tissue and serum of tumor-bearing mice in four groups were different(Group A > Group B > Group C > Group D in KLK-7) and(Group A < Group B < Group C < Group D in E-cad). The weight and volume, and KLK7 and E-cad contents of transplantation tumors in four groups were significant difference(P<0.05). Classification of rectal cancer according to recurrence types-comparison of Japanese guidelines and Western guidelines

BACKGROUND Rectal cancer is characterized by more local recurrence(LR)and lung metastasis than colon ***,the diagnosis of rectal cancer is not standardized as there is no global consensus on its definition and *** classification of rectal cancer differs between Japanese and Western *** To clarify the characteristics of rectal cancer by comparing the tumor location and characteristics of rectal cancer with those of colon cancer according to each set of *** A total of 958 patients with Stage II and III colorectal cancer were included in the analysis:607 with colon cancer and 351 with rectal *** of rectal cancers was assessed by enema examination and rigid *** to Japan guidelines,rectal cancer is classified as Rb(below the peritoneal inversion),Ra(between the inferior margin of second sacral vertebrae and Rb)or RS(between Ra and sacral promontory).RESULTS There were no significant differences between RS rectal cancer and colon cancer in the rates of liver and lung metastasis or *** metastasis and LR were significantly more common among Rb rectal cancer(in Japan)than in colon cancer(P=0.0043 and P=0.0002,respectively).Lung metastases and LR occurred at significantly higher rates in rectal cancer measuring≤12 cm and≤10 cm than in colon cancers(P=0.0117,P=0.0467,P=0.0036,P=0.0010).Finally,the rates of liver metastasis,lung metastasis,and LR in rectal cancers measuring 11 cm to 15 cm were 6.9%,2.8%,and 5.7%,*** were equivalent to the rates in colon *** High rectal cancer may be treated with the same treatment strategies as colon *** was no difference in the classification of colorectal cancer between Japan and Western countries.

Thymoquinone inhibits proliferation in gastric cancer via the STAT3 pathway in vivo and in vitro

AIM: To elucidate the mechanism of thymoquinone(TQ)-induced apoptosis in human gastric cancer cells in vitro and in ***: HGC27, BGC823, and SGC7901 cells were cultured in vitro and treated with TQ(0, 10, 25, 50, 75, 100, 125 μmol/L) for 12 h, 24 h, and 36 h, and then the proliferation inhibitory rates were detected by methylthiazole tetrazolium assay. Apoptosis was observed after Hoechst staining. The protein expressions of signal transducer and activator of transcription(STAT)3, p-STAT3, STAT5, p-STAT5, phospho-janus-activated kinase 2(JAK2), JAK2, p-Src, Src, glyceraldehyde-3-phosphate dehydrogenase, lamin-A, survivin, Cyclin D, Bcl-2, Bax, peroxisome proliferator activated receptor, and caspase-3,7,9 were detected by western blot. Cell cycle and apoptosis weredetermined with flow cytometry. TQ induced dosedependent apoptotic cell death in HGC27 cells was measured by Annexin V-fluorescein isothiocyanate(FITC)/propidium iodide(PI) analysis and Hoechst 33258. RESULTS: TQ inhibited the phosphorylation of STAT3 but not STAT5. TQ-induced downregulation of STAT3 activation was associated with a reduction in JAK2 and c-Src activity. TQ also downregulated the expression of STAT3-regulated genes, such as Bcl-2, cyclin D, survivin, and vascular endothelial growth factor, and activated caspase-3,7,9. Consistent with the in vitro results, TQ was significantly effective as an antitumor agent in a xenograft tumor mouse model. CONCLUSION: This study provides strong evidence that downregulation of the STAT3 signaling pathway mediates TQ-induced apoptosis in gastric cancer.
