Effect of Abnormal Posture on Quality of Life in Patients with Parkinson’s Disease

Background: Relationship between abnormal posture and QOL of Parkinson's disease is not clear. Objectives: To clarify the association between quality of life (QOL) and abnormal posture in patients with Parkinson’s disease (PD) and examine which QOL dimensions are involved. Methods: This study retrospectively examined PD patients (n = 57) who had undergone outpatient rehabilitation between January and March 2014. Evaluation items were Modified Hoehn Yahr (H-Y) stage, Unified Parkinson’s Disease Rating Scale (UPDRS)-Part III score, Timed Up and Go test (TUG, 3-m walking) result, Parkinson’s Disease Questionnaire (PDQ)-39 score, and the angle of forward and lateral trunk flexion in the standing position. Statistical analysis was performed to reveal the relationship between the total PDQ-39 score and individual evaluation items and which QOL dimensions were affected by abnormal posture due to forward or lateral flexion. Results: A total of 38 PD patients (17 men, 21 women;mean age, 73.2 ± 8.6 years) were enrolled. Among the evaluation items, lateral flexion angle showed a significant correlation with the total PDQ-39 score (r = 0.422, P = 0.008). PDQ-39 dimensions showing an association with forward flexion were activities of daily living (ADL) (P = 0.002) and communication (P = 0.007), whereas ADL (P = 0.004), communication (P = 0.021), and social support (P = 0.029) were associated with lateral flexion. Mobility was not associated with forward or lateral flexion. Conclusion: The present findings revealed a correlation between lateral flexion and QOL in PD patients. Among the PDQ-39 dimensions, ADL and communication were associated with abnormal posture.

Unilateral mimicking bilateral BPPV- a forgotten entity? Characteristics of a largecohort of patients, comparison with posterior canal BPPV and clinical implications

Objective: Unilateral mimicking bilateral benign paroxysmal positional vertigo (umb-BPPV) was attributed to inappropriate head positioning during testing of the posterior canal. Despite its inclusion in theDiagnostic criteria for the classification of vestibular disorders of the Barany Society, the clinical characteristics and treatment responsiveness of this BPPV subtype have not been intensively ***: Records of patients with BPPV seen at a single outpatient dizziness clinic during the years 2000e2020 were reviewed. Eighty seven patients with umb-BPPV and 86 random patients with posteriorcanal BPPV (p-BPPV) were retrieved. Their demographics and BPPV characteristics were ***: Patients' and BPPV characteristics were similar in umb- and p-BPPV except for the prevalence ofmales in the umb-BPPV group. No differences were found between treatment responsiveness and recurrences in both groups. The recurrence rate of umb-BPPV was not influenced by age, gender, BPPV side,duration of symptoms or treatment responsiveness during the first ***: In accordance with our hypothesis about mixed canalo- and cupulolithiasis as the underlying mechanism of umb-BPPV, patients did not differ in characteristics and treatment responsivenessfrom p-BPPV patients. Recognition of umb-BPPV is important since inappropriate treatment can cause anunnecessary delay in therapy success.

Background The initiation and expansion of China＇s national free antiretroviral therapy program has led to significant improvement of survival among its participants. Success of further scaling up treatment coverage rests upon intensifying HIV screening and efficient linkage of care. Timely CD4 cell count testing after HIV diagnosis is necessary to determine whether a patient meets criteria for antiretroviral treatment, and represents a crucial link to engage HIV-infected patients in appropriate care, which has not been evaluated in *** We evaluated all patients ≥16 years who tested HIV positive from 2005 to 2009 in Yunnan and *** Logistic regression models were applied to identify factors associated with lack of CD4 cell count testing within 6 months after HIV *** A total of 83 556 patients were included. Over the study period, 30 635 （37%） of subjects received a CD4 cell count within 6 months of receiving the HIV diagnosis. The rate of CD4 cell count testing within 6 months of HIV diagnosis increased significantly from 7% in 2005 to 62% in 2009. Besides the earlier years of HIV diagnosis, negative predictors for CD4 cell count testing in multivariate analyses included older age, not married or unclear marriage status,incarceration, diagnosis at sexual transmitted disease clinics, mode of HIV transmission classified as men who have sex with men, intravenous drug users or transmission route unclear, while minority ethnicity, receipt of high school or higher education, diagnosis at voluntary counseling and testing clinics, and having HIV positive parents were *** Significant progress has been made in increasing CD4 testing among newly diagnosed HIV positive patients in Yunnan and Guangxi from 2005-2009. The Self-Administered International Prostate Symptoms Score (IPSS) Questionnaire of Kosovo Men with Benign Prostatic Hyperplasia

Background: Benign prostatic hyperplasia (BPH) has been a major health problem for aging males because of its related symptoms and complications. Although it is not a life-threatening condition, BPH has an adverse effect on a patient’s quality of life, as manifested in community and clinical trials. Aim: The aim of the study was the perception of patients in their symptomatology by comparing the place of residence and quality of life in living with HBP using the IPSS questionnaire. Materials and Methods: This was a prospectively designed study conducted at University clinical Centre of Kosovo—Urology clinic. The study was conducted from October 2020 to January 2021. The study in criteria was as follows: all patients who presented to the urology outpatient clinic with BPH, Lower Urinary Tract Symptoms (LUTS) and aged > 40 years. The exclusion criteria included the following cases: patients with uncontrolled diabetes (prostate-specific antigen > 4 ng/dL);patients with a history of transurethral resection of the prostate. Results: This study was carried out on 100 consecutive patients with HBP and LUTS attending. The largest proportion of patients 51% was in the 46 - 55 age group. Most patients were between 45 - 65 years of 51%. The largest number of them (52%) was with severe symptoms and 41% of them were with moderate symptoms. Comparing the three levels of IPSS and the quality of life of patients with HBP and LUTS, when asked how they would feel if they still had problems with urination, 22% said they felt upset, analyzing the categorization of patients according to IPSS: mild symptoms were Mean/SD (1.71. ± 1.113), moderate symptoms (2.9 ± 1.49) and severe symptoms (4.31 ± 1.27) p Conclusion: The

Pain syndromes are often associated with depression. In a prospective study we analysed if determinants of depression differ among patients with different primary headaches and between headaches and non-headache pain. During a 2-year period between 1 February 2002 and 31 January 2004, 635 subjects (migraine n=231; tension-type headache n=176; cluster headache n=11; patients with low back pain n=103; and healthy subjects n=114) seen by two neurologists filled in a questionnaire on pain characteristics, the MIDAS questionnaire and the Beck Depression Inventory. A multivariate general regression model was used to identify independent predictors of the severity of depressive symptoms. Pain was most frequent in chronic tension-type headache and most intense in the cluster subgroup (P < 0.001, Kruskal-Wallis ANOVA). In univariate tests gender, age, pain frequency, pain intensity and disability were all significantly associated with the severity of depressive symptoms. In the multivariate model disability was the most important independent determinant of the severity of depressive symptoms in the pooled headache group as well as in the migraine and tension-type headache subgroups. In contrast to patients with headache, pain frequency and pain intensity were the significant independent predictors of the severity of depressive symptoms in patients with low back pain. In a multivariate model, after controlling for other factors, determinants of the severity of depressive symptoms were different in headache and non-headache pain subjects, suggesting a different mechanism for developing depression in primary headaches and in other pain syndromes.

目的本研究主要探讨吸烟是否将影响慢性疼痛患者在为期3周的疼痛康复项目中阿片类药物的减量，同时，探讨吸烟状态、住院期间阿片类药物的使用及疼痛的严重程度之间的关系。方法作者使用了回顾性、重复测试的试验设计以评估患者在治疗前后的一些指标。评估的对象是在2003年9月至2007年2月之间接受一项为期3周的门诊疼痛治疗项目的全部患者。评估指标包括阿片类药物的成功戒除率、多学科疼痛调查的疼痛程度量表和治疗项目完成情况。结果共有1241例患者纳入本研究（女性928例）；其中，313例（25％）是吸烟者，294例（24％）是前烟民，其余634例（51％）未曾吸烟。入组时，相较于前烟民及不吸烟者，有更多的吸烟者住院期间使用阿片类药物（P〈0．001）。吸烟者使用的平均等效吗啡剂量（P=0．013）和疼痛评分（P〈0．001）明显高于前烟民和不吸烟者。阿片类药物的成功戒除率与患者是否吸烟无关，治疗项目结束时，各组患者的疼痛均有明显缓解（P〈0．001）。但是，有较大比例的吸烟者未能完成3周的治疗疗程（P〈0．001）。结论大部分完成全部疼痛康复计划的患者能够成功戒除阿片类药物，这与患者是否吸烟无关。但值得注意的是，有更多的吸烟者未能完成该治疗项目，其中最常见的原因包括对治疗的期望值存在差异，突发急性疾病，以及心理社会的应激。
