High Blood Pressure and Pregnancy: Epidemiological, Diagnostic, Therapeutic and Prognostic Aspects at the Maternity Unit of the institute of social hygiene of dakar (Senegal)

Objectives: To specify the epidemiological and clinical profile of the patients, to determine the methods of management and to evaluate the maternal and perinatal prognosis in the association of arterial hypertension and pregnancy (HTA) at the institute of social hygiene of dakar. Material and Methods: This was a retrospective, descriptive and analytical study on the management of the association of hypertension and pregnancy in the maternity ward of the institute of social hygiene of dakar between January 1st, 2019 and on December 31st, 2020. Results: During the study period, we recorded 326 cases of hypertension associated with pregnancy among the 4290 pregnant women, i.e. a frequency of 7.6%. The epidemiological profile of the patients was that of a woman with an average age of 29, nulliparous (48.2%), married (99.1%), at a low socioeconomic level (61.3%) and with a personal history preeclampsia (14.4%). Most patients (83.1%) were evacuated or referred with pre-ec- lampsia as the main reason (40.8%). The average gestational age was 37 weeks of amenorrhea (SA) with extremes of 20 SA 6 days and 41 SA 5 days. Term pregnancies were the most represented (76.7%). Prenatal follow-up (CPN) was carried out in 319 patients (97.8%) with an average number of CPN of 3 and extremes of 1 and 9. Severe diastolic and severe systolic hypertension concerned respectively 19.3% and 16, 9% of patients. One hundred and eighty patients (55.2%) presented with oliguria. Strip albuminuria was performed in 235 patients (72.1%). It found 3 or more crosses in 38.3% of cases. We recorded 53 cases of anemia (16.3%), 16 cases of significant proteinuria (5%), hepatic cytolysis in 22 patients (6.7%), thrombocytopenia in 37 patients (11.3%), and 43 cases of hyperuricaemia (13.2%). Sanitary Evacuations at the Maternity Unit of the social hygiene institute (IHS) in dakar in 2020

Objective: Determine the frequency of evacuations, specify the epidemiological and clinical characteristics of the evacuees, evaluate the data of the evacuation, the management and the maternal-fetal prognosis. Methods: Prospective and descriptive retrospective study concerning obstetrical evacuations received at the maternity ward of the Hospital of the institute of social hygiene in dakar between January 1 and December 31, 2020, i.e. a period of 12 months. Results: During the study period, we collected 1156 evacuees out of a total of 3507 patients treated in the Service, i.e. a frequency of 33%. The average age of the patients was 27.07 years with extremes of 14 and 46 years. Patients aged between 20 and 29 were the most represented (51.73%). The average parity was 1.6 with extremes of 0 and 10 pares. The nulliparous (46.37%) were the majority. The majority of evacuated patients (99.6%) resided in the dakar region, including 58% in the suburbs and 42% in the city center. The patients received had performed an average of 3 prenatal consultations with extremes ranging from 0 to 9 CPN. They most often came from health centers (55.05%) or hospitals (29.09%). The reasons for evacuations were dominated by dystocia (21.54%) followed by premature rupture of membranes (17.21%) and premature deliveries (16.35%). On admission, only 176 patients (15.2%) had an evacuation sheet. Patients transited on average through two health structures (extremes ranging from 0 to 7 structures) before reaching the reception structure. The evacuation was most often done with a private vehicle on the patient’s own means (91.96%). The outcome of the evacuees was most often vaginal delivery or hospitalization (72.79%). The majority of patients (99.4%) had evolved favorably but we deplore one maternal death (0.09%) linked to a late puerperal infection. Introduction: The human immunodeficiency virus (HIV) is a pandemic of concern to the World Health Organization. It is all the more worrisome because if not properly managed, it can be responsible for several complications, including abnormal blood counts. These abnormalities may also be related to treatment. Objectives: The main objective of this study is to show that adequate antiretroviral therapy can correct various abnormalities exerted by HIV on patients’ blood counts. The specific objective is to determine all the abnormalities we can see in blood count before and during HIV treatment. Materials and Methods: Our study was conducted in the hematology laboratory and the dermatology departments of the Aristide Le Dantec Hospital and the institute of social hygiene. It took place from December 2009 to October 2011. It is a retrospective descriptive and analytical study involving HIV-positive patients (HIV 1 and 2) and under antiretroviral treatment (at least six months of treatment). We included 110 patients in the study. Blood counts were performed at the hematology laboratory of the Aristide le Dantec hospital using a KX21 automaton. Each patient received three blood counts during the first six months of treatment (M0, M3 and M6). A univariate analysis was performed to determine the profile of the abnormalities of the blood count and the chi 2 test was used and a threshold of p 0.05 was considered significant. Results: The mean age of the patients was 38.6 ± 8.6 years (extremes of 18 and 64 years) with a sex ratio of 0.42 (77 females and 33 males). The prevalence of anemia was 80% (n = 88) at M0, 53, 63% at M3 (n = 59), and 38.20% at M6 (n = 42) (p = 0.02). We noted a favorable evolution with treatment. For the other abnormalities, the evolution was favorable for leukopenia (p = 0.011) and thrombocytopenia (p = 0.007). Conclusion: Our study showed a correction of blood count abnormalities in PLHIV with antiretroviral treatment.

Evaluation of Malnutrition in Infants Aged 0 - 59 Months in the Suburbs of dakar

Introduction: Child malnutrition is a major public health concern worldwide, particularly in low- and middle-income countries. The latest report from Senegal’s Continuous Demographic and Health Survey revealed that 18% of children in Senegal were stunted (chronic malnutrition), 8% were wasted (acute malnutrition) and 14% were underweight. Thus, this study aimed to assess the characteristics associated with malnutrition in children according to their nutritional status. Methods: This descriptive transverse study was conducted at the Pediatric social institute of Pikine/Guediawaye and the National Hospital Center of Pikine, in Senegal between October and December 2019. A total of 94 children were recruited, with the consent of their legal tutors. Descriptive and multivariate analyses of the factors associated with malnutrition were performed. The z-scores for the indices of nutrition were determined with WHO Anthro® software version 3.2.2. All the data analyses were performed using R software version 4.2.2. Result: From the 94 children recruited, 51.06% were female, with a sex ratio (male/female) of 0.96. Acute malnutrition was recorded in 62.77% of cases, chronic malnutrition in41.49%, and underweight in 71.27%. Linear regression analysis showed that many factors, such as the female gender [OR = 1.82 CI (1.02 - 3.3), P-value = 0.04], consanguinity [OR = 2.84 CI (1.14 - 7.65), P-value = 0.03], low birth weight [OR = 4.83 CI (2.15 - 12.89), P-value = 0.0004], were associated with acute malnutrition. Low birth weight (<2.5 kg) [OR = 10.66 CI (3.82 - 44.39) P-value < 0.0001], non-exclusive breastfeeding [OR = 3 CI (1.40 - 7.13)] P-value = 0.007], dietary diversification before six months [OR = 2.04 CI (1.23 - 3.51), P-value = 0.007] and others factors were associated with underweight. Simulating optimal vaccination times during cholera outbreaks

The use of cholera vaccines has been increasingly recognized as an effective control measure in cholera endemic countries. However, guidelines for using vaccination during cholera outbreaks are still to be established, and it remains an open question as to how and when the vaccines should be deployed to best control ongoing cholera outbreaks. Here we formulate a new optimal control model to assess the value of cholera vaccines in epidemic settings and cost-effective optimal times to deploy a vaccine. Our results suggest that as long as the vaccine prices are sufficiently low, vaccination should always start from or immediately after the onset of a cholera outbreak.

Outcome after Discharge from Hospital of Children with Complicated Severe Acute Malnutrition and Predictors Factors of Non-Response during Outpatient Treatment, in Senegal

Introduction: Malnutrition is a public health problem. It is responsible for high morbidity and mortality in children aged 6 - 59 months. The aim of this study was to determine the outcome of children with complicated severe acute malnutrition after hospital discharge. Methods: This is a descriptive and analytical cross-sectional study conducted from March 1 to November 31, 2021 at the Albert Royer Children’s Hospital. Children discharged from hospital for complicated severe acute malnutrition were included. Results: A total of 103 children were included. The mean age of children at hospitalization was 16.41 ± 10.11 months and mean duration of follow-up after hospital discharge was 29.63 ± 8.59 months. Three children (2.91%) died after hospital discharge. The mean z-score of the weight-for-current height was -1.34 ± 1.08. Almost a quarter (24.27%) of the children remain acute malnourished with 3.88% of severe acute malnutrition (SAM). The predictors’ factors associated with non-response were weaning before 2 years of age (ORaj: 12.21;95% CI [6.82 - 18.44];p = 0.04) and tuberculosis (ORaj: 21.06;95% CI [12.54 - 41.09];p = 0.03). Conclusion: The rate of recovery of nutritional status in children with complicated severe acute malnutrition is satisfactory. Ablactation before the age of two and the existence of tuberculosis are significantly associated with non-recovery of nutritional status.

Incidence and economic impact of ophthalmological occupational accidents in Greece

Scope: This article seeks to assess the eco-nomic impact of industrial accidents with ref-erence to the incidence of opthalmological in-dustrial injuries occurring in the Thriacion Plain industrial zone in Greece during the period 1991-2000. Material and Methods: Data provided by 53 industrial units have been collected and classified. During the study period, 2,011 adults and juveniles out of the 15,600 people employed in the area (a 13% aggregate) suffered opthal-mological injuries occurring at the workplace. Results: The highest percentage of injuries were due to trauma (61% of adults, 67% of juveniles), followed by chemical burns. In the majority of cases (75% of adults and 50% of juveniles) treatment was provided at the local IKA (social Security Fund) health care unit, in the town of Elefsis. It was estimated that a total of 10,192 working days were lost, with compensation costs (benefit payments) alone amounting to approximately € 118,000.00, not allowing for in-direct employer costs such as loss of produc-tivity, substitution and replacement costs. Con-clusions: The high incidence of industrial acci-dents in combination with the high percentage of child labor in the area of Thriacion Industrial Plain were striking and contributed significantly to high- er rates of work-related injuries of ocular interest in Greece. The latter were shown to result in a sig- nificant deterioration of the employees’ health status, with the majority of the accidents being due to the lack of safety precautions at the work-place at a considerable financial burden on the social security system in direct benefit payments. The social security system needs to press for tighter work safety regulations and their proper implementation if to minimize the burden these accidents impose on its budget.

Introduction: Many health problems in pregnant women result in part from inadequate nutrition. Much progress has been made in improving the nutritional situation in Dakar, but it remains a concern to this day. Promoting dietary diversity is an effective way to combat any kind of nutrient deficiency. This study aims to investigate the determinants of dietary diversity in pregnant women in the Dakar region. Method: It was a cross-sectional, descriptive, analytical study with a three-stage survey. The calculated sample was 292 pregnant women. Socio-demographic, economic, pregnancy-related data, related to practices and knowledge on nutrition were collected using a questionnaire and an observation grid. The collection was conducted from November 01 to December 15, 2017 among pregnant women in the Dakar region. Data were entered using epi info software and analyses were done with R. Simple linear regression was used to find the explanatory factors of dietary diversification. The fit of the regression model was done by the Hosmer Lemeshow test. The odds ratios were measured with a 5% alpha risk. Results: A total of 300 pregnant women were surveyed in two different health districts. Purpose: In Sub-Saharan Africa, an important proportion of incident HIV cases occur among heterosexual serodiscordant couples (HSDC) but the majority of HIV negative partners can remain seronegative. These are called HIV-exposed seronegative (HESN). We aimed to compare immune activation (IA) levels between HESN, their HIV infected counterparts (HIV+ partners) and HIV unexposed uninfected individuals (HIV-neg Controls) and to evaluate the association between sexual exposure to HIV (SEHIV) and IA. Methods: We conducted a cross-sectional study in dakar, Senegal on 148 participants recruited between November 2013 and February 2014: 40 HIV-neg Controls, 54 HESN and 54 HIV+ Partners. SEHIV was evaluated individually using questionnaires. IA level was measured by plasma level of β2-microglobulin (β2m). Logistic regression was used to estimate odds ratios (OR) and 95% confidence intervals (CI) for the different associations. Results: The median levels of β2m were 1.57 mg/l (IQR: 1.37 - 1.77), 2.14 mg/l (IQR: 1.76 - 2.43) and 2.24 mg/l (IQR: 1.80 - 3.17) for HIV-neg Controls, HESN and HIV+ partners, respectively. After adjustment, HESN had similar levels of IA with HIV+ partners but significantly higher than that of HIV-neg Controls (adjusted OR = 6.28;95% CI: [2.19 - 18.00]). The association between IA and SEHIV was evaluated in the HIV negative individuals. High frequency of SEHIV was associated with a β2m > 2.2 mg/l (OR = 6.56;95% CI: [1.71 - 25.21]);significantly more than median cut off value of >1.81 mg/l. Conclusions: Our study shows that, despite being uninfected with HIV, HESN individuals show a high level of IA, which was depended on the level of SEHIV.
