BACKGROUND Monoclonal antibodies(mAbs)have shown clinical benefits against coronavirus disease 2019(COVID-19)caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2).Several studies have reported the use of bamlanivimab as a promising treatment option for COVID-19.AIM To synthesize the latest evidence for the efficacy and safety of bamlanivimab alone in the treatment of adult patients with COVID-19.METHODS A literature search was conducted in PubMed,Cochrane Library,Web of Science,medRxiv,and Google Scholar using“SARS-CoV-2”,“COVID-19”,“LY-CoV555”,and“Bamlanivimab”keywords up to January 25,2023.The quality of included studies was assessed using the Cochrane bias tools.The Comprehensive Meta-Analysis software version 3.0 was used to analyze the data.RESULTS A total of 30 studies involving 47368 patients were included.A significant difference was observed between the bamlanivimab and standard of care/placebo groups in terms of mortality rate[risk ratio(RR)=50,95%confidence interval(CI):0.36-0.70],hospitalization rate(RR=0.51;95%CI:0.39-0.68),and emergency department(ED)visits(RR=0.69;95%CI:0.47-0.99);while the two groups exhibited no significant difference in terms of intensive care unit(ICU)admission(P>0.05).Compared to other mAbs,bamlanivimab was associated with a higher rate of hospitalization(RR=1.44;95%CI:1.07-1.94).However,no significant difference was detected between the bamlanivimab and other mAbs groups in terms of mortality rate,ICU admission,and ED(P>0.05).The incidence of any adverse events was similar between the bamlanivimab and control groups(P>0.05).CONCLUSION Although the results suggest the efficacy and safety of bamlanivimab in COVID-19 patients,further research is required to confirm the efficacy of this drug for the current circulating SARS-CoV-2 variants.

Insomnia,as one of the emotional diseases,has been increasing in recent years,which has a great impact on people's life and work.Therefore,researchers are eager to find a more perfect treatment.The microbiome-gut-brain axis is a new theory that has gradually become popular abroad in recent years and has a profound impact in the field of insomnia.In recent years,traditional Chinese medicine(TCM)has played an increasingly important role in the treatment of insomnia,especially acupuncture and Chinese herbal medicine.It is the main method of TCM in the treatment of insomnia.This paper mainly reviews the combination degree of"microorganism-gut-brain axis"theory with TCM and acupuncture under the system of TCM.To explore the mechanism of TCM and acupuncture in the treatment of insomnia under the guidance of"microorganismgut-brain axis"theory,in order to provide a new idea for the diagnosis and treatment of insomnia.

First report on natural Leishmania infection of Phlebotomus sergenti due Leishmania tropica by high resolution melting curve method in Southeastern Iran

Objective:To identify the Leishmania species in infected sand flies by Real-time PCR coupled with HRM analysis.Methods:Real-time PCR coupled with HRM analysis targeting the first internal transcribed spacer(ITS1)of nuclear ribosomal DNA as the genetic marker was used to identify and distinguish Lelthmania species in sand flies specimens.Results:Three out of 115females of Phlebotomus sergenti(P.sergenti)(2.6%)were positive to Leishmania tropica(L.tropica).Conclusions:This is the first report on P.sergenti as the main and proven vector of anthroponitic cutaneous leishmaniasis in Dehbakri County using Real-time PCR coupled with HUM analysis.This method is rapid,sensitive and specific for diagnosing of parasites in infected Sand flies and ideal for large scale genotyping projects.

Objective:To make evidence-based decisions based on broad mortality trends for Razavi Khorasan province,Iran.Methods:In order to determine the baseline number of deaths,we used univariate time series analyses for monthly data from the monthly vital statistics reports(From April 2015 to March 2022).For excess mortalities,these baselines were subtracted from reported deaths with a 95%prediction interval.To compare time and causes,a P-score was calculated.Results:From March 2020 to March 2022,there were 61949 registered deaths,and the estimated deaths with a 95%confidence interval(CI)were 43246.16(35718.28,50774.05).So,in 2020-2021 and 2021-2022,the death counts were 35.15%and 51.33%higher than projected.A total of 18666 cardiovascular diseases were reported and a total of 15704.46(12006.95,19401.96)was estimated.The P-score for this duration was 14.49%and 23.23%higher than expected.Infectious and parasitic diseases plus COVID-19 were 16633 and estimated to be 1044.87(456.77,1632.96).A total of 4420 diseases of the respiratory system were reported,and 4564.94 deaths were predicted(2277.43,6852.43).In the first year of the pandemic,the P-score dropped to-35.28%and in the second year,it jumped sharply to 22.38%.Conclusions:Excess mortality,along with cause-specific mortality,can be helpful for monitoring trends and developing public health policies at the local,national,and international levels.

Extensive right coronary artery thrombosis in a patient with COVID-19:A case report

BACKGROUND Occurring in approximately 30%of hospitalized patients,cardiovascular complications that take place during the course of coronavirus disease 2019(COVID-19)have been shown to cause morbidity and mortality.This case is the first report of extensive right coronary artery(RCA)thrombosis that was evaluated by intracoronary imaging and intracoronary invasive physiology in a patient with COVID-19.CASE SUMMARY A 62-year-old woman presented with flu-like symptoms;ten days later,she presented with inferior ST-segment elevations,chest pain,dyspnea,nausea and vomiting.The patient was diagnosed with COVID-19 following a positive test result.Emergency angiography of the RCA and its branches indicated intraluminal filling defects,suggesting a thrombus.Intravascular ultrasound confirmed a subacute thrombus in the RCA,the right posterior descending branch and the right posterior ventricular(RPV)branch.There was also an acute thrombus in the RPV branch and atherosclerosis in the RCA.Dual antiplatelet/anticoagulation therapy was administered.After 7 d,angiography revealed complete disappearance of the thrombi.Optical coherence tomography confirmed this with the exception of a small thrombus in the RPV branch and atherosclerotic plaque in the RCA.The atherosclerotic RCA was measured using the resting fullcycle ratio,indicating no impairment to coronary physiology.The patient was discharged on the 11^(th) day of hospitalization and remained asymptomatic through the 6-mo follow-up.CONCLUSION This was the first report of RCA thrombosis in a patient with COVID-19.Dual antiplatelet/anticoagulation therapy was successful.

Objective:To determine some bio-ecological aspects of malaria vectors in Jask County,where is targeted for malaria elimination in the national program.Methods:Mosquitoes were collected monthly during 2013-2014 using different collection methods.Subsequently,ELISA test was used to detect the human blood index of mosquitoes.The susceptibility status of Anopheles stephensi was evaluated against the diagnostic dosages of seven WHO recommended insecticides.Results:A total of 3 650 female and 4 736 Anopheles larvae were collected including Anopheles stephensi,Anopheles culicifacies s.l.,Anopheles dthali.Anopheles fluviatilis s.l.,Anopheles moghulensis and Anopheles turkhodi species.Anopheles stephensi was the dominant collected species on human baits and indoors with high rate of unfed and gravid specimens in internal and external window traps.Human blood index was calculated as 14.3%for this species.It was also found to be resistant to DDT and Dieldrin.Conclusions:The collected species had a wide range of habitats,and resting behaviors.With regarding to the presence of most important malaria vectors in Jask.control of the disease may be so complicated;as based on the weather condition it can be transmitted during the whole year,except for cold months.With this strong potential of transmission,existing population movements in the area may lead to imported cases of malaria and local outbreak(s).So,more specific studies on malaria vectors in high risk areas of Jask County are recommended.

Noise robustness of an operational modal-based structural damage-detection scheme using impact-synchronous modal analysis

Data-driven damage-detection schemes are usually unsupervised machine-learning models in practice,as these do not require any training.Vibration-based features are commonly used in these schemes but often require several other parameters to accurately correlate with damage,as they may not globally represent the model,making them less sensitive to damage.Modal data,such as frequency response functions(FRFs)and principal component analysis(PCA)reduced FRFs(PCA-FRFs),inherits the dynamic characteristics of the structure,and it changes when damage occurs,thus showing sensitivity to damage.However,noise from the environment or external sources such as wind,operating machines,or the in-service system itself,can reduce the modal data's sensitivity to damage if not handled properly,which affects damage-detection accuracy.This study proposes a noise-robust operational modal-based structural damage-detection scheme that uses impact-synchronous modal analysis(ISMA)to generate clean,static-like FRFs for damage diagnosis.ISMA allows modal data collection without requiring shutdown conditions,and its denoising feature aids in generating clean,static-like FRFs for damage diagnosis.Our results showed that the FRFs obtained through ISMA under noise conditions have frequency response assurance criterion(FRAC)and cross signature assurance criterion(CSAC)scores greater than 0.9 when compared with FRFs obtained through experimental modal analysis(EMA)under static conditions;this validates the denoising feature of ISMA.When the denoised FRFs are reduced to PCA-FRFs and used in an unsupervised learning-based damage-detection scheme,zero false alarms occur.
