RADIOGRAPHIC ANATOMICAL INSIGHTS INTO MECHANISM OF LOWER CALYCEAL STONE EVACUATION FOLLOWING ESWL

This study was aimed to elucidate whether there is significant relationship between the lower calyceal anatomy and stone evacuation following extracorporeal shock wave lithotripsy(ESWL). From April 1995 to November 1999,145 patients with lower caliceal stones underwent ESWL with Storz Modulith SL20 lithotriptor. The pelvicalyecal angle, lower pole infundibular length and width, and lower pole calyceal pattern of the stone bearing calyx were measured on pre ESWL intravenous urograms. Abdominal plain film and ultrasonography taken of all patients at the 3 month followup showed that the overall stone free rate was 65% after 3 months. The stone free rate was significantly correlated to the radiographic anatomical factor, 94% in patients with a pelvicalyceal angle of 90 degrees or greater, 57% in those with less than 90 degrees pelvicalyceal angle; 80% in those with infundibular length of 30 mm or less, 59% in those with infundibular length greater than 30 mm; 73% in those with infundibular width greater than 4 mm, 48% in those with infundibular width of 4 mm or less; 85% in those with a simple calyceal pattern and 41% in those with a complex one. These findings suggest that the 4 radiographic anatomical factors of the lower pole calyx(pelvicalyceal angle, infundibular length and width and calyceal pattern) play an important role in stone evacuation after ESWL. Pelvicalyceal angle of 90 degrees or greater, infundibular length of 30 mm or less and width greater than 4 mm, and simple calyceal pattern are respectively noted to relate to an improved stone free status. In contrast, an acute pelvicalyceal angle, long infundibular length and narrow infundibular width and complex calyceal pattern are individually unfavorable factors for stone emptying following ESWL.

目的:评价患者施行睾丸固定手术时的年龄对其后精子参数的可能影响。方法:47例单侧隐睾史不育患者,按照患者施行单侧睾丸固定术时的年龄分为A组(2-7岁,n=23),B组(8-13岁,n=14)和C组(14-17岁,n=10)。对3组患者的精液作精液分析和精子头-尾膜的完整性检测。结果:3组患者的精液中均可观察到活动精子,精子活动率和精子浓度有很大的个体差异,畸形精子率普遍较高,精子头-尾膜完整率低。少弱畸精子症的发生率在B组(35%)和C组(50%)升高。结论:单侧隐睾史不育患者会有较差的精子参数。单侧隐睾症患者在幼年阶段尽早治疗有利于成年后的精子发生。

STAT1 is involved in signal transduction in the EPOinduced HEL cells

Erythropoietin (EPO) is the ma jor regulator of mammalian erythropoisis, which stimulates the growth and differentiation of hematopoietic cells through interaction with its receptor (EPO-R). Here we use HEL cells (a human erythro-leukemia cell line) as a model to elucidate the pathway of signal transduction in the EPO-induced HEL cells. our data show that the EPOR (EPO receptor) on the surface of HEL cells interacts with the Janus tyrosine protein kinase (Jak2) to transduce intracellular signals through phosphorylation of cytoplasmic proteins in EPO-treated HEL cells. Both STAT1 and STAT5 in this cell line are tyrosine-phosphorylated and translocated to nucleus following the binding of EPO to HEL ***, the binding of both STAT1 and STAT5 proteins to specific DNA elements (SIE and PIE elements) is revealed in an EPO-dependent manner. Our data demonstrate that the pathway of signal transduction following the binding of EPO to HEL cells is similar to immature erythroid cell from the spleen of mice infected with anemia strain of Friend virus.

Introduction & objectives:We investigated the safety and efficacy of a new male circumcision with a circular *** & methods:From 1 March to 30 February 2014,441 cases of stapler circumcision and 438 of conventional circumcision were *** were followed up 1 week,2 weeks,3 weeks,1 month and3 months after *** were collected and *** time,pain score,blood loss,postoperative complications,and treatment costs were compared between the two ***:Operative time and blood loss in stapler group were significantly less than in conventional group(6.8 ±3.1 mins vs 24.2 ± 3.2mins,p<0.01),(1.8 ± 1.8ml vs 9.4 ± 1.5ml,p<0.01);Intraoperative and postoperative pain score in stapler group were significantly lower than conventional group(0.8 ± 0.5 vs 2.4 ± 0.8 p<0.01),(4.0 ± 0.9vs 5.8 ± 1.0,p<0.05);In addition,stapler group had a significantly lower complications than conventional group((2.3%vs 7.5%p<0.01).However,the costs of stapler group were much higher than those of conventional group(2210.1 ± 50.3 Yuan vs 782.5 ± 45.5 Yuan).Meanwhile,42 patients(9.5%) with stapler needed to take stitches due to bleeding or incomplete wound closure during *** patients(388/441,88.0%) undergone stapler circumcision needed removal of residual staple ***:Circular Stapler is a safe,efficient new device for adult male *** has some advantages:short operative time,less pain,less blood loss,less complications and better cosmetic ***,the device still need to be further improved on staple residues.

Nephroureterectomy with removal of a bladder cuff was performed successfully on a 47-year -old man for transitional cell carcinoma of the right renal pelvis using hand-assisted laparoscopy. The results showed that hand-assisted laparoscopic nephroureterectomy is an efficacious alternative to open surgery and provides the urologist new to laparoscopy a shorter learning curve compared with standard laparoscopy.

Comparison of 80 W versus 120 W 532 nm Laser Prostatectomy for BPH

Purpose: Improvements to photoselective vaporization of the prostate (PVP) have been made over time, particularly with the introduction of higher power systems. Few studies however have compared the performance of these systems to their predecessors. The purpose of this study was to compare the clinical and perioperative outcomes of 80 W vs. 120 W PVP. Materials and Methods: A series of 267 and 209 consecutive patients underwent 80 W and 120 W PVP, respectively, from September 2001 to May 2009 at Weill Cornell Medical College (GreenLightTM laser system, American Medical Systems, Inc., Minnetonka, MN). Data were collected on patient demographics, International Prostate Symptom Score (IPSS), prostate-specific antigen (PSA), and perioperative parameters. Maximum flow rate (Qmax), postvoid residual (PVR), and transrectal ultrasound prostate volume were recorded. Statistical analyses were carried out utilizing the Shapiro-Wilk, Mann-Whitney, Wilcoxon and unpaired t tests (SPSS 15.0, SPSS Inc., Chicago, IL, USA). Results: Baseline parameters were similar between the two groups except for greater median age (72.8 vs. 69.2 years, p = 0.01) and lower median PSA (2.0 vs. 3.3, p = 0.01) in the 80 W group. Median laser time was longer in the 80W group (85 vs. 51 minutes, p 0.001) with a higher median energy utilized (253 vs. 210 kJ, p = 0.001). Final IPSS, Qmax, PVR, and PSA were equivalent between the two groups. Conclusions: In our series, PVP was safe and effective. Durable and similar improvements in symptoms and objective parameters were achieved in patients with both 80 W and 120 W laser systems. PVP with the 120 W system, however, provided faster and more efficient vaporization compared to the 80 W system.

目的观察早期糖尿病性膀胱病(DCP)的尿动力学及病理学改变,探讨其发病机制及病理演绎过程。方法建立早期糖尿病大鼠模型(30只),以正常大鼠(30只)作对照,饲养60 d后,进行膀胱静态压、最大膀胱容量、最大膀胱收缩压等尿动力学指标测定。然后行膀胱湿重、膀胱壁厚度及光镜、电镜病理学观察。结果60 d后,糖尿病组膀胱静态压、最大膀胱容量、最大膀胱收缩压、膀胱顺应性、膀胱湿重、膀胱壁厚度分别为(0.95±0.10)kPa、(3.49±0.40)ml、(3.09±0.10)kPa、(1.39±0.11)ml/kPa、(182.0±1.5)mg、(1.41±0.07)mm,而对照组分别为(0.60±0.03)kPa、(1.82±0.12)ml、(4.91±0.30)kPa(0.68±0.07)ml/kPa、(149.0±1.4)mg、(0.93±0.04)mm。两组对应参数相比,差异均有统计学意义(P<0.05或<0.01)。糖尿病组膀胱逼尿肌边缘有空泡样变性、肌细胞固缩、细胞连接纤维化、成纤维细胞变性及胶原纤维排列紊乱。结论DCP大鼠早期就有膀胱的动力学及病理学改变,提示对DCP患者应尽早干预,控制病情的进一步恶化。
