Background Treatment of extensive post-traumatic composite bone and soft tissue defects remains a complicated therapeutic problem and a challenge for surgeons. We investigated the application of local flaps and Ilizarov osteogenesis in the reestablishment of severe combined defects of tibial bone and soft tissue. Methods Sixteen patients with bone and soft tissue defects were included. The mean age of the patients was 31.5 years. The average time from injury to initial surgery was 14.4 weeks. The average soft tissue and bone defect sizes were 92.9 cm2 and 8.7 cm, respectively. Local flaps were created to reconstruct the soft tissue defects. The Ilizarov external fixator or the Orthofix Limb reconstruction System was used to reconstruct bony defects using delayed distraction osteogenesis. Results Two myocutaneous flaps and 14 reverse island flaps were applied. All transferred flaps survived. Fifteen patients healed with equal leg length, and one healed with a residual leg-length discrepancy of 1.5 cm. One patient with an Ilizarov external fixator developed ankle joint stiffening and a pin-track infection that was successfully treated with oral antibiotics. No patient developed pin loosening. All patients walked without assistance. Fifteen patients returned to their work, and one lost her job. The results were evaluated using the Paley bone and functional assessment scores. The bone assessment results were excellent in 14 and good in two patients. Functional assessment scores were excellent in 13, good in two, and fair in one patient. Conclusion The combination of local flaps and sequential distraction osteogenesis can be used for successful reconstruction of defects of incorporated bone and soft tissue.

The study was carried out through summarizing and comparing the percentage of long-term complications of four types breast implants,and analyzing the cause of *** The study reviewed 573 patients with breast implants including textured or smooth silicone gel-filled or saline filled implants which were placed between 1993 and 2002.427 cases were selected to followed-up in which the surgical technique,the plane of placement and the operator were all the *** on outcomes of following-up,we summarized all kinds of long-term complications and the incidence of every type of *** There were 803 of 832 prostheses (94.24%) with good results in 427 cases,while 49 prostheses (5.76%) with varied complications in 42 *** level Ⅲ or Ⅳ contracture occurred in 26 prostheses,reduction of the breast in 24 prostheses,malposition or pain in 10 *** or leakage were detected in repeated surgery in 24 *** more severe capsular contracture happened in cases with silicone gel-filled implant than in cases with saline-filled *** reduction of the breast due to the rupture or leakage of implants almost occurred in cases with saline-filled *** study also implied the complication incidence had the positive correlation with the length of time after *** The data provided by the statistic analysis demonstrated that the distinctly difference of the complication incidence exist only between smooth saline-filled implants and smooth silicone gel-filled *** there were no difference among the *** 30 of 49 prostheses with varied complication of rupture or leakage were detected which could cause the other severe *** the quality of implants is the key to decrease the complication incidence,then the degeneration of prosthese after being implanted becomes the urgent and troublesome problem to solve for all of us.6 refs,1 tab.

Objective To investigate a simple, valuable new method for coverage of the soft tissue defect over the lower one-third of the leg, ankle and the foot. Methods The distally based peroneus brevis muscle fkps were applied for coverage of the soft tissue defects over the lower one-third of the leg,the ankle and the foot. The muscle flaps were covered with split-thickness skin grafts. Results The distally based peroneus brevis muscle flaps were applied for coverage of the soft tissue defects over the lower one-third of the leg, the ankle and the foot in 16 cases. The larges area of the soft tissue defect was 5 cm × 7 cm. The smallest was 3 cm × 4 cm. Primary healing occured in 14 cases undergoing muscle flap construction, second-stage healing occured in 2 cases, no total flap necrosis occured in any cases. Conclusion ThisChina Medical Abstracts(surgery) technigue is a simple and complication are lesser. The successful rates are higher. This muscle flap is suitable to the mudium or small soft tissue

To investigate the relationship between p53 codon 72 polymorphism and susceptibility to keloid in a southern Chinese *** The p53 genotypes were determined by polymerase chain reaction-reverse dot blot (PCR-RDB) and DNA direct sequencing in 45 patients with keloid and 60 unrelated healthy *** The frequency of the p53 Pro allele among keloid patients was significantly higher than that among healthy controls (χ2=4.485,P=0.034).The Pro/Arg and Arg/Arg genotype distribution among keloid patients was not significantly different from that among healthy controls (χ2=0.919,1.346;P=0.330,0.246,respectively).However,the Pro/Pro genotype frequency among keloid patients was significantly higher than that among healthy controls (χ2=4.375,P=0.036).The p53 Pro/Pro genotype significantly increased the risk for developing keloid,compared to the combination of Pro/Arg and Arg/Arg genotypes,with the odds ratio (OR) of 2.400 (95% CI:1.048～5.498).Conclusion Determination of the p53 codon 72 genotype may be used as a stratification marker to predicate high-risk individuals for keloid.11 refs.1 tab.
