To present the clinical characteristics and the misdiagnosis rate of acute coronary syndrome manifested primarily as throat discomfort,we conducted a multicentric and retrospective study in the cardiology and otorhinolaryngology departments.Records of patients with primary complaint of throat discomfort,absence of chest pain at onset,and an ultimate diagnosis of acute coronary syndrome,as well as patients with pharyngitis(as controls)were collected from May 2015 to April 2016.The patients’main manifestations were compared.Logistic regression results showed that chest tightness,dyspnea,perspiring,and exertional throat symptoms were significantly associated with acute coronary syndrome,with odds ratios of 8.3(95%CI 2.2–31.5),10.9(95%CI 1.8–66.9),25.4(95%CI 3.6–179.9),and 81.2(95%CI 13.0–506.7).A total of 25(56.82%)out of 44 acute coronary syndrome patients,who were first admitted to the otorhinolaryngology department,were misdiagnosed,with a 12%(3/25)mortality rate.Throat discomfort can be the principal manifestation of acute coronary syndrome.Such patients exhibit high misdiagnosis and mortality rates.Exertional throat symptoms,chest tightness,perspiring,and dyspnea were important indicators of acute coronary syndrome in patients whose main complaint was throat discomfort.The awareness of this condition will result in prompt diagnosis and reduce morbidity and mortality.

Post Blast Tympanic Perforations, Clinical and Paraclinical Study at Six Yaounde Hospitals

Introduction: Post-blast tympanic perforations represent an entity of traumatic perforations;various mechanisms are at the origin of the latter, and the increase in conflicts and acts of violence are increasing the studies on this subject, but few data are available in our context. Objective: This paper aims to study post-blast tympanic perforations in Yaounde, specifically the epidemiological, clinical, and paraclinical aspects. Methodology: We conducted a descriptive cross-sectional study with retrospective and prospective data collection in six Yaounde hospitals over 63 months from January 1, 2018, to March 31, 2023. All patients with at least one post-blast tympanic perforation were included. Socio-demographic, clinical and paraclinical data were collected on a questionnaire and analysed using SPSS 28 software. Results: We included 124 patients. The prevalence of post-blast tympanic perforations was 0.1% of the consultations in the departments. 71 The average age was 28.6 ± 9.1 years, with extremes ranging from 7 to 49 years. The median consultation time was five days. The most frequent injury circumstances were physical aggression (81.45%) and armed conflict (10.5%). The injury mechanisms were slaps (75%), punches (21.74%) and grenades (4.8%). The main symptoms on admission were hearing loss (63.7%), tinnitus (58.1%) and otalgia (57.3%). The tympanic perforations were unilateral in all cases, the anterior-inferior location was more characteristic, and the deafness was conductive in 58.3% of cases, followed by mixed deafness in 23.3% of cases. Conclusion: Post-blast tympanic perforations are rare in consultation. The population is primarily male and from the second decade of life. The main circumstance is aggression.

The GJB2 gene(connexin 26) has been shown to be responsible for DFNB1 and DFNA3. We screened the GJB2 gene in 488 patients with prelingual deafness(Group 1), 124 with postlingual deafness(Group 2), and 117 normal hearing subjects(Group 3). We found that, in Group 1, 65 patients(13.32%) were homozygotes or compound heterozygotes and 51 patients (10.45%) carried a single pathogenic mutation. The 235delC mutation was the most frequent mutation, accounting for 73.22% of the known pathogenic alleles in Group 1. No homozygotes or compound heterozygotes were detected in Group 2 or Group 3. Some postlingual deaf patients (2.42%) and normal hearing subjects(4.27%) were 235delC carriers. Our preliminary data indicate that 235delC, the most frequent mutation identified in this study, is a major cause for prelingual deafness.

To investigate the correlation between environmental-meteorological factors and daily visits for acute otitis media(AOM)in Lanzhou,China.Methods:Data were collected in 2014e2016 by the departments of otolaryngology-Head and Neck Surgery at two hospitals in Lanzhou.Relevant information,including age,sex and visiting time,was collected.Environmental data included air quality index,PM10,PM2.5,O3,CO,NO2 and SO2,and meteorological data included daily average temperature(T,C),daily mean atmospheric pressure(AP,hPa),daily average relative humidity(RH,%)and daily mean wind speed(W,m/s).The SPSS22.0 software was used to generate Spearman correlation coefficients in descriptive statistical analysis,and the R3.5.0 software was used to calculate relative risk(RR)and to obtain exposure-response curves.The relationship between meteorological-environmental parameters and daily AOM visits was summarized.Results:Correlations were identified between daily AOM visits and CO,O3,SO2,CO,NO2,PM2.5 and PM10 levels.NO2,SO2,CO,AP,RH and T levels significantly correlated with daily AOM visits with a lag exposure-response pattern.The effects of CO,NO2,SO2 and AP on daily AOM visits were significantly stronger compared to other factors(P<0.01).O3,W,T and RH were negatively correlated with daily AOM visits.The highest RR lagged by 3e4 days.Conclusions:The number of daily AOM visits appeared to be correlated with short-term exposure to mixed air pollutants and meteorological factors from 2014 through 2016 in Lanzhou.

As the most commonly occurring cancer in women worldwide,breast cancer poses a formidable public health challenge on a global scale.Breast cancer consists of a group of biologically and molecularly heterogeneous diseases originated from the breast.While the risk factors associated with this cancer varies with respect to other cancers,genetic predisposition,most notably mutations in BRCA1 or BRCA2 gene,is an important causative factor for this malignancy.Breast cancers can begin in different areas of the breast,such as the ducts,the lobules,or the tissue in between.Within the large group of diverse breast carcinomas,there are various denoted types of breast cancer based on their invasiveness relative to the primary tumor sites.It is important to distinguish between the various subtypes because they have different prognoses and treatment implications.As there are remarkable parallels between normal development and breast cancer progression at the molecular level,it has been postulated that breast cancer may be derived from mammary cancer stem cells.Normal breast development and mammary stem cells are regulated by several signaling pathways,such as estrogen receptors(ERs),HER2,and Wnt/b-catenin signaling pathways,which control stem cell proliferation,cell death,cell differentiation,and cell motility.Furthermore,emerging evidence indicates that epigenetic regulations and noncoding RNAs may play important roles in breast cancer development and may contribute to the heterogeneity and metastatic aspects of breast cancer,especially for triple-negative breast cancer.This review provides a comprehensive survey of the molecular,cellular and genetic aspects of breast cancer.

Background:The current World Health Organization(WHO) classification of nasopharyngeal carcinoma(NPC) con?veys little prognostic information.This study aimed to propose an NPC histopathologic classification that can poten?tially be used to predict prognosis and treatment response.Methods:We initially developed a histopathologic classification based on the morphologic traits and cell differentia?tion of tumors of 2716 NPC patients who were identified at Sun Yat?sen University Cancer Center(SYSUCC)(training cohort).Then,the proposed classification was applied to 1702 patients(retrospective validation cohort) from hospitals outside SYSUCC and 1613 patients(prospective validation cohort) from SYSUCC.The efficacy of radiochemotherapy and radiotherapy modalities was compared between the proposed subtypes.We used Cox proportional hazards models to estimate hazard ratios(HRs) with 95% confidence intervals(CI) for overall survival(OS).Results:The 5?year OS rates for all NPC patients who were diagnosed with epithelial carcinoma(EC;3708 patients),mixed sarcomatoid?epithelial carcinoma(MSEC;1247 patients),sarcomatoid carcinoma(SC;823 patients),and squamous cell carcinoma(SCC;253 patients) were 79.4%,70.5%,59.6%,and 42.6%,respectively(P < 0.001).In mul?tivariate models,patients with MSEC had a shorter OS than patients with EC(HR = 1.44,95% CI = 1.27–1.62),SC(HR = 2.00,95% CI = 1.76–2.28),or SCC(HR = 4.23,95% CI = 3.34–5.38).Radiochemotherapy significantly improved survival compared with radiotherapy alone for patients with EC(HR 49–0.75),and possibly for those with SCC(HR = 0.67,95% CI = 0.56–0.80),MSEC(HR = 0.58,95% CI = 0..74–1.28).= 0.63;95% CI = 0.40–0.98),but not for patients with SC(HR = 0.97,95% CI = 0Conclusions:The proposed classification offers more information for the prediction of NPC prognosis compared with the WHO classification and might be a valuable tool to guide treatment decisions for subtypes that are associ?ated with a poor prognosis.

自闭症个体加工言语声音有障碍,而音乐家对于声音则具有高于普通人的敏感性。声音加工的神经机制是否具有可塑性的呢?我们采集了音乐家与非音乐家在只有一个言语者或者多个言语者时对音节/da/的frequency following responses(FFRs)。并且,言语者的变化还发生在不同的语言条件下:单个音节/da/或者许多不同的音节但包含/da/。相比较于非音乐家,音乐家对于声音表现出更稳定的皮层下反应。音乐家能够利用谐波的线索在两种不同的语言条件下以皮层下的神经反应来表征声音,而非音乐家对相应声音的皮层下反应却只出现在多音节的语言条件下。除外谐波,音乐家还可以利用基频进行皮层下的声音表征。更重要的是,以基频进行皮层下的声音表征的程度与音乐经验呈正相关。我们的研究结果表明,音乐经验调节皮层下的声音加工。此研究首次为本领域提供了有关声音加工可塑性的电生理证据,对于诸如自闭症等发展性疾病的干预有重要的启示。

Psychosocial Experience in Goiter Patients

Introduction: Goiter often poses aesthetic disgrace problem. The psychosocial impacts of goiter, in particular in woman may be significant. Objective: studying the psychological and social impacts of the disease in goiter patients within the African context and their consequences on the treatment. Methods: It was a transversal study that took place from May 1to July 31, 2011 in the Collines and Donga departments situated in the center and the north of Benin. The study consisted of a survey conducted through an individual discussion based on questionnaire submitted to goiter patients who accepted to provide their answers. Results: This survey involved 86 patients of which 83 women and 3 men. Their average age was 43.7 ± 13.41. Goiter was noticeable in the totality of the patients. 68.6% of patients declared that they experienced on daily basis the shame caused by the disease. 66% thought that it was a natural disease;however, 27.9% believed that it was caused by bewitchment or sorcery. 46.5% and 37.2% resorted to modern and indigenous medicine respectively;16.3% made no therapeutic move. Despite the psychosocial impacts of the disease, 50% of the patients rejected surgical intervention possibility. The core motives they raised were lack of financial means (34.8%), disease recurrence fright (23.3%), anesthesia fright (16.3%), and surgical fright (14%). Conclusion: Despite the psychological impacts of the disease, strong resistance exists probably more on cultural aspect than the motives mentioned by thyroidectomy patients.
