In spite of tremendous progresses in surgical and chemo-radiotherapeutic regimens, rectal cancer still suffers from high relapse and mortality rates, and metastatic disease is incurable. Here we assess some of the most recent and validated biomarkers and potential targets studied in rectal cancer, and provide comments to a recent monographic topic covering several aspects of colorectal cancer, published in Current Cancer Drug Targets .

Gastrointestinal stromal tumours(GISTs) are mesen-chymal neoplasms originating in the gastrointestinal tract, usually in the stomach or the small intestine, and rarely elsewhere in the abdomen. The malignant potential of GISTs is variable ranging from small lesions with a benign behaviour to fatal sarcomas. The majo-rity of the tumours stain positively for the CD-117(KIT) and discovered on GIST-1(DOG-1 or anoctamin 1) expression, and they are characterized by the presence of a driver kinase-activating mutation in either KIT or platelet-derived growth factor receptor α. Although surgery is the primary modality of treatment, almost half of the patients have disease recurrence following surgery, which highlights the need for an effective adjuvant therapy. Traditionally, GISTs are considered chemotherapy and radiotherapy resistant. With the advent of targeted therapy(tyrosine kinase inhibitors), there has been a paradigm shift in the management of GISTs in the last decade. We present a comprehensive review of targeted therapy in the management of GISTs.

Although most natural proteins are composed exclusively of L-configuration amino acids, more and more emerging evidences showed that D-amino acids play crucial roles in sustaining the functions of proteins or peptides, as well as in the development of diseases. Peptidyl-aminoacyl-L/D-isomerase is a key enzyme to catalyze the transition from L-amino acids to D-amino acids. However, the molecular characterization and functional analysis of the enzyme in amphibian species of Bombina maxima were unclear. In this study, the enzyme was purified from skin secretions of B. maxima. The apparent molecular weight of the purified enzyme was determined to be 35 kDa. Interestingly, no internal or interchain disulfide bonds was found for this enzyme. Functional study revealed that the isomerase has a strong ability to catalyzes the isomerization of an L-Ile in position 2 of a model peptide(IIGPVLGC) to D-allo-Ile. Mutation study demonstrated that the lysine in position122 of the enzyme was essential for its function. Interestingly, a D-amino acids containing peptide named maximin H was identified from B. maxima skin secretions and displayed a strong antimicrobial activity. However, the activity of maximin H was significant decreased once the its D-Ile changed to L-Ile. These findings suggested that peptidyl-aminoacyl-L/D-isomerase was vital for sustaining the functions of frog-derived bio-active peptides and the survival of frogs.

Current evidence of pathophysiology of diabetic macular edema: A review

Janus kinase 3(JAK3) is a member of Janus kinase(JAK) family, and it represents a promising target for the treatment of immune diseases and cancers. However, no highly selective inhibitors of JAK3 have been developed. For discovering the binding mechanism of JAK3 and these inhibitors, a molecular modeling study combining molecular docking, three-dimensional quantitative structure-activity relationships(3D-QSAR), molecular dynamics and binding free energy calculations was performed on a series of pyrimidine-based compounds which could bind with the unique residue Cys909 of JAK3 kinase as the selective inhibitors of JAK3 in this work. The optimum Co MFA and Co MSIA models were generated based on the conformations obtained by molecular docking. The results showed that the models have satisfactory predicted capacity in both internal and external validation. Furthermore, a 50 ns molecular dynamics simulation was carried out to determine the detailed binding process of inhibitors with different activities. It was demonstrated that hydrogen bond interactions with Leu828, Glu903, Tyr904, Leu905 and Leu956 of JAK3 are significant for activity increase, and the Van der Waals interaction is mainly responsible for stable complex.

Diabetic macular edema(DME) is an important cause of vision loss in patients with diabetes mellitus. The pathophysiology of DME can be described as a process whereby hyperglycaemia leads to overlapping and inter-related pathways that play a role not only in the initial vascular events, but also in the events that cause the edema to become chronic. On a macrocellular level, DME is believed to be in part caused by alterations in hydrostatic and oncotic pressures and shear stress. Angiogenic factor expression, inflammation and oxidative stress constitute the key components of microvascular pathways. The interactions, signalling events and feedback loops between the various molecules are complicated and are not completely understood. These molecular mediators, acting in conjunction with macrocellular factors, which are all stimulated in part by the hyperglycaemia and hypoxia, can have a direct endothelial effect leading to hyperpermeability, disruption of vascular endothelial cell junctions, and leukostasis. Macular edema is thought to be caused as a result of these consequences.
