背景在此项研究中，我们评估了应用苯妥英钠预防癫痫的患儿行开颅手术中输注丙泊酚对其肝、胰酶和酸碱平衡的影响。方法在这项前瞻性的临床研究中，我们测定了30例4—12岁患儿的血清谷草转氨酶（AST）、谷丙转氨酶（ALT）、γ-谷氨酰转肽酶（GGT）、碱性磷酸酶（ALP）、胰淀粉酶、胰脂肪酶及三酰甘油水平。患儿接受丙泊酚麻醉并应用苯妥英钠预防癫痫。以前已用苯妥英钠的患儿则继续用药。未曾用药者口服苯妥英钠5mg·kg^-1·d^-1。经医院批准于术前1天，术后第1、3、5、7天测定患儿血清中AST、ALT、GGT、ALP、胆红素、胰淀粉酶、胰脂肪酶、三酰甘油水平。分别于气管插管后、术中2小时和术中4小时、拔管以及拔管后1、2、6、12小时采集动脉血气样本。结果术后血清AST、ALT、GGT、ALP、胰淀粉酶、胰脂肪酶、三酰甘油水平显著高于术前基础值，且于术后第1天达峰值，并在1周内恢复正常。拔管后碱剩余与基础值相比显著降低，但仍在正常范围内，并于术后6小时恢复到基础值水平。患儿并未表现出肝炎或胰腺炎的临床症状或体征。胆红素水平正常。患儿在术后4—6个月内未发生肝胰相关的并发症。结论尽管开颅术患儿肝、胰酶在术后轻微上升，但丙泊酚全麻维持未对其酸碱平衡或肝、胰酶产生临床意义的显著影响。

背景手术后肠梗阻是腹主动脉手术主要的胃肠并发症，可增加患者发病率和死亡率，延长住院时间，增加医疗费用。本项研究中，我们评价了硬膜外给予新斯的明对腹主动脉手术术后肠梗阻的影响。方法选择我院45例行择期腹主动脉手术的患者。所有患者均接受相同的全麻联合硬膜外麻醉。全麻诱导前，于T7～T8间隙放置硬膜外导管，给予布比卡因20ml（Q5％）（推注时间〉15分钟）。将患者随机分成两组，术毕和术后8小时分别通过硬膜外导管给予生理盐水稀释的1μg／kg新斯的明5ml（Ⅰ组），或给予生理盐水5ml（Ⅱ组）。在重症监护病房记录手术后出现肠鸣音的时间，同时记录每天肛门排气与排便的时间。结果Ⅰ组患者第1次出现肠鸣音时间和肛门排气时间均明显短于Ⅱ组（分别为11．6±11．2小时vs 22．6±12．8小时；21．8±15．6小时vs 36．6±19．1小时，P〈0．05）。两组第1次排便时间相近（P〉0．05）。Ⅱ组手术后恶心的发生明显高于Ⅰ组（P〈0.05）。两组手术后并发症发生率相近（P〉0.05）。结论胸段硬膜外给予新斯的明能够使腹主动脉手术后肠鸣音恢复更快，手术后肠梗阻时间缩短。

Code blue applications as an indicator of clinical quality

目的发布欧洲重症医学学会(European Society of Intensive Care Medicine, ESICM)关于神经重症监护患者液体治疗的专家共识和临床实践推荐意见。设计在2016年10月召开的ESICM LIVE 2016会议上召集了一个由22名国际专家组成的共识委员会,随后委员会成员通过电话会议和在线讨论制定本专家共识。方法回顾人群、干预、对比和结局(population, intervention,comparison, and outcomes, PICO)问题,根据需要进行更新并得到证据概要。本共识关注3个主要议题:(1)神经重症监护患者的液体复苏和维持;(2)高渗液体在颅内压控制中的应用;(3)蛛网膜下腔出血后迟发性脑缺血的液体管理。经过广泛的文献检索,应用推荐分级的评估、制订与评价(Gradingof Recommendations Assessment, Development and Evaluation, GRADE)系统原则评估证据质量(从高到极低),制定治疗推荐(强或弱),并在适用时发布最佳实践声明。基于文献和专家意见提供的综合证据,应用改良德尔菲方法(使用序贯方法以避免偏倚和误解)产生最终共识声明。结果最终共识包含总共32条声明,其中13条为强推荐,17条为弱推荐,另外2条声明未提供推荐意见。结论我们发布了一份关于神经重症监护患者液体治疗的共识声明和临床实践推荐。

脓毒症是一种由感染引起的异质性疾病,感染触发了一系列复杂的局部或者全身的免疫炎症反应,引起多器官功能衰竭,发病率和病死率显著升高。由于至今仍然没有诊断脓毒症的金标准,所以脓毒症的临床诊断仍是一个难题。因此,脓毒症的临床诊断需要不断改变来满足临床和研究的要求。然而,尽管有许多新型的生物标记和筛选工具去预测脓毒症发生的风险,但是这些措施的诊断价值和有效性不足以让人满意,并且没有充分的证据去建议临床使用这些新技术。因此,脓毒症的临床诊断标准需要定期更新去适应不断产生的新证据。这篇综述旨在呈现当前脓毒症的诊断和早期识别方面的最新研究证据。临床运用不同的诊断方法的推荐意见依赖于推荐、评价、发展和评估分级体系(Grades of Recommendation Assessment,Development and Evaluation,GRADE),因为大部分的研究是观察性研究,并没有对这些方法进行可靠评估,采用的是两步推理方法。未来需要更多研究来确认或者反驳某一特殊的指标检测,同时应该直接采用相关病人的结果数据。
