THE INFLUENCE OF APLIPOPROTEIN B AND GENE POLYMORPHISMS ON THE RESPONSE TO SIMVASTATIN THERAPY IN PATIENTS WITH HYPERLIPIDEMIA

Objective:To investigate the effect of apolipoprotein B(apoB) and E(apoE) genetic variations on lipid pro-file at baseline (before treatment),and also on the subsequent response to simvastatin ***:Eighty-eight patients with hyperlipidemia were treated with simvastatin 5mg *** plasma lev-els of total cholesterol(TC) and low-density lipoprotein cholesterol(LDL-C),triglyceride(TG) and apoB were measured pre-treatment and at the end of the 4th,8th and 12th post-treatment *** of apoB at XbaI locus and apoE were determined by restriction fragment length polymorphism(RFLP).Results:In all patients ,relative frequencies of X-allele and X+allele were 0.943 and 0.057 for apoB gene *** apoE gene the relative frequency of ε2allele was determined as 0.182,ε3 as 0.580 and ε4as *** reduction in TC level was more prominent in patients carrying X-allele than in those with X+ allele following treatment(-23.9% vs.-13.6%,P<0.05).Compared with patients carrying ε3 or ε4 allele,those with ε 2 allele showed a significantly higher percentage in reduction of apoB level after treatment(P<0.05).Conclusion:The relative frequency of apoB X+allele is high in patients with hyperlipidemia,in whom the TC-lowering efficacy is decreased following treatment of *** relative frequencies of ε 2 and ε 4are also high in hyperlipidemic patients,and the ε2 allele is associated with reduction in apoB level during lipid-regulating therapy.

Background Ageing is associated with increased incidence of dyslipidemia. Toinvestigate potential molecular mechanisms, the effects of age and fibrate administration onperoxisome proliferator-activated receptor α (PPARα) expression in livers of young and old ratswere studied. Methods A total of 16 young (2-month-old) and 16 old rats (24-month-old) were randomlyassigned to a control group and fenofibrate group (fenofibrate in a total therapeutic dosage of 0.5% in ratio to each treated rat weight in 14 days). RT-PCR was applied to evaluate hepatic mRNAexpression of PPARα and its target genes. Western blotting was used to determine PPARα proteinlevel in liver tissue. Results When compared with 2-month-old rats, the liver tissue from24-month-old rats showed reduced expression of PPARα mRNA (52% , P <0. 05) and protein (109% , P <0. 01). Consequently, the mRNA levels of PPAR target genes, LPL, ACO, ACS and CPT-1 were markedlylowered by 19% , 8% , 13% and 9% respectively, and apoCⅢ increased by 24% in livers from24-month-old rats, compared with values obtained from 2-month-old rats ( P < 0. 05 ) . Fenofibratetherapy significantly lowered plasma triglyceride and total cholesterol levels in old rats,accompanied with improvement in hepatic expression of genes, including LPL, ACO, ACS, CPT-1 andapoCⅢ, but no change was found in PPARα expression in livers from either 24-month or 2-month-oldrats. Conclusions The decrease in the hepatic PPARα expression is probably directly related to thelipid metabolic disturbances observed in old animals. The beneficial effects of fenofibrateadministration in old rats suggests that fibrates may be useful for treating lipid disturbances inold people.

EFFECT OF PEROXISOME PROLIFERATOR-ACTIVATED RECEPTOR ACTIVATORS ON TUMOR NECROSIS FACTOR-αEXPRESSION IN NEONATAL RAT CARDIAC MYOCYTES

Objective To investigate the effect of peroxisome proliferator-activated receptor-α(PPARα) and PPARγactivators on tumor necrosis factor-α(TNFα) expression in neonatal rat cardiac myocytes. Methods Primary cultures of cardiac myocytes from 1- to 3-day-old Wistar rats were prepared, and myocytes were ex-posed to lipopolysaccharide (LPS) and varying concentrations of PPARαor PPARγactivator (fenofibrate or pioglitazone).RT-PCR and ELISA were used to measure TNFα, PPARα, and PPARγexpression in cultured cardiac myocytes. Transient tr-ansfection of TNFαpromoter with or without nuclear factor-kappaB (NF-κB) binding site to cardiac myocytes was performed. Results Pretreatment of cardiac myocytes with fenofibrate or pioglitazone inhibited LPS-induced TNFαmRNA and protein expression in a dose-dependent manner. However, no significant changes were observed on PPARαor PPARγmRNA expression when cardiac myocytes were pretreated with fenofibrate or pioglitazone. Proportional suppression of TNFαpromoter activity was observed when myocytes was transiently transfected with whole length of TNFαpromoter (-721/+17) after being stimulated with LPS and fenofibrate or pioglitazone, whereas no change of promoter activity was observed with transfection of TNFαreporter construct in deletion of NF-κB binding site (-182/+17). Conclusions PPARαand PPARγactivators may inhibit cardiac TNFαexpression but not accompanied by change of PPARαor PPARγmRNA expression. Therefore PPARαand PPARγactivators appear to play a role in anti-inflammation. The mechanism may partly be involved in suppression of the NF-κB pathway.
