OBJECTIVE: To assess the efficacy and safety of chinese patent medicine(CPM) with the principle of tonifying Qi, promoting blood circulation by removing blood stasis, and resolving phlegm(TQ-PBC-RP)in the management of stable chronic obstructive pulmonary disease(COPD).METHODS: A systematic review of randomized controlled trials(RCTs) identified from electronic databases and print was conducted. RCTs testing CPMs with TQ-PBC-RP against any type of controlled intervention in patients with stable COPD and assessing clinically relevant outcomes were included. Methodological quality was evaluated with the risk of bias tool according to systematic review handbook 5.0.2. Quality of evidence was estimatedby the rating approach developed by the Grading of Recommendations, Assessment, Development,and Evaluation Working ***: Thirteen eligible RCTs with 12 oral CPMs were tested. Significant differences between groups in favor of CPMs were not reported in all trials. Most trials included were deemed to be of low methodological quality with poor evidence *** of large data heterogeneity, statistical pooling was not performed for all ***: The effectiveness of CPM in the treatment of stable COPD is not supported by evidence. Currently, evidence from RCTs is scarce and methodologically weak. Considering the popularity of CPMs among patients undergoing COPD, rigorously designed trials are warranted.

Cohort study on the treatment of BRAF V600E mutant metastatic colorectal cancer with integrated chinese and western medicine

BACKGROUND Patients with BRAF V600E mutant metastatic colorectal cancer(mCRC)have a low incidence rate,poor biological activity,suboptimal response to conventional treatments,and a poor *** the previous cohort study on mCRC conducted by our team,it was observed that integrated chinese and western medicine treatment could significantly prolong the overall survival(OS)of patients with colorectal ***,we further explored the survival benefits in the population with BRAF V600E mutant *** To evaluate the efficacy of integrated chinese and western medicine in the treatment of BRAF V600E mutant metastatic colorectal *** A cohort study was conducted on patients with BRAF V600E mutant metastatic colorectal cancer admitted to Xiyuan Hospital of China Academy of chinese Medical Sciences and traditional chinese medicine Hospital of Xinjiang Uygur Autonomous Region from January 2016 to December *** patients were divided into two *** A total of 34 cases were included,with 23 in chinese-western medicine cohort(cohort A)and 11 in western medicine cohort(cohort B).The median overall survival was 19.9 months in cohort A and 14.2 months in cohort B,with a statistically significant difference(P=0.038,hazard ratio=0.46).The 1-3-year survival rates were 95.65%(22/23),39.13%(9/23),and 26.09%(6/23)in cohort A,and 63.64%(7/11),18.18%(2/11),and 9.09%(1/11)in cohort B,*** analysis showed statistically significant differences in median OS between the two cohorts in the right colon,liver metastasis,chemotherapy,and first-line treatment subgroups(Pintegrated chinese and western medicine can prolong the survival and reduce the risk of death in patients with BRAF V600E mutant metastatic colorectal cancer,with more pronounced benefits observed in patients with right colon involvement,liver metastasis,combined chemotherapy,and first-line treatment.

Inflammatory microenvironment and expression of chemokines in hepatocellular carcinoma

AIM:To study the inflammatory microenvironment and expression of chemokines in hepatocellular carcinoma(HCC) in nude ***:CBRH-7919 HCC cells were injected into the subcutaneous region of nude *** two weeks after the challenge,tumor growth was measured every week for six *** stromal microenvironment and inflammatory cell infiltration was assessed by immunohistochemistry in paired tumor and adjacent peritumoral samples,and macrophage phenotype was assessed using double-stain immunohistochemistry incorporating expression of an intracellular enzyme.A chemokine PCR array,comprised of 98 genes,was used to screen differential gene expressions,which were validated by western ***,expression of identified chemokines was knocked-down by RNA interference,and the effect on tumor growth was ***:Inflammatory cell infiltrates are a key feature of adjacent peritumoral tissues with increased macrophage,neutrophil,and T cell(specifically helperand activated subsets)*** within adjacent peritumoral tissues express inducible nitric oxide synthase,suggestive of a proinflammatory ***-one genes were identified in tumor tissues during the progression period,including 50that were overexpressed(including CXCL1,CXCL2 and CXCL3)and three that were underexpressed(CXCR1,Ifg and Actb).RNA interference of CXCL1 in the CBRH-7919 cells decreased the growth of tumors in nude mice and inhibited expression of CXCL2,CXCL3and interleukin-1β***:These findings suggest that CXCL1plays a critical role in tumor growth and may serve as a potential molecular target for use in HCC therapy.

Effect of Artemisia annua Extract on Treating Active Rheumatoid Arthritis:A Randomized Controlled Trial

Objective：To investigate the effect and safety of the complementary use of the extract of Artemisia annua L.（EAA） on treating active rheumatoid arthritis（RA）. Methods：A randomized controlled clinical trial was performed. All the 159 participates with active RA were randomly assigned to the control group（80 cases） and EAA group（79 cases） using concealed random allocation method. In the control group, patients were medicated with leflunomide and methotrexate for 48 weeks; and patients in the EAA group were administrated with leflunomide, methotrexate plus EAA（30 g/d）. At the time points of 0, 12, 24 and 48 weeks, the clinical outcome measures, including objective pain score, tenderness score, number of painful joints, number of swollen joints, health assessment questionnaire（HAQ） score for quality of life, levels of serum rheumatoid factor（RF）, anti-cyclic citrullinated protein antibodies（CCP-Ab）, erythrocyte sedimentation rate（ESR）, C reactive protein（CRP）, visual analogue score for pain（VAS）, and the overall efficacy were detected and recorded. Results：The objective pain score, number of painful joints and ESR at 12 weeks, tenderness score and HAQ at 24 weeks, and the tenderness score, number of painfull joints, number of swollen joints, HAQ, CRP, RF and CCP-Ab at 48 weeks were significantly improved in the EAA group compared with the control group（P〈0.01 or P〈0.05）. At 24 and 48 weeks, the overall efficacy of the EAA group was significantly higher than the control group（P〈0.01）. There were significantly higher withdrawal rate of corticosteroids within 12 weeks post-treatment and lower incidence rate of adverse effects in the EAA group compared with the control group（P〈0.01 or P〈0.05）. Conclusion：EAA plus methotrexate and leflunomide were more effective and safer than the routine use of methotrexate and leflunomide in the treatment of active RA.

Effect of adjuvant therapy with electroacupuncture on bone turnover markers and interleukin 17 in patients with rheumatoid arthritis

OBJECTIVE: To evaluate the effect of electroacupuncture as an adjuvant treatment with first-line medications on bone metabolism biomarkers and interleukin-17(IL-17) in the peripheral blood of patients with rheumatoid arthritis(RA).METHODS: Sixty RA patients were randomized into three groups. The control group was treated with methotrexate plus leflunomide(MTX + LEF),the acupuncture group was treated with simple needling plus MTX + LEF, and the patients in the electroacupuncture(EA) group were treated with EA plus MTX + LEF. EA or acupuncture was applied every other day for a total of 10 times over a treatment period of 8 ***: In all three treatment groups, serum levels of the bone metabolism markers PICP, N-MID,and B-ALP were elevated and the concentrations of the inflammatory markers β-CTx, IL-17, CRP, and TRACP-5b were reduced after treatment. These differences were significant for the EA group but not the other groups(P < 0.05).CONCLUSION: EA could effectively reduce the suffering and improve the quality of life of RA patients. It is a promising adjuvant therapy for enhancing the effectiveness of clinical therapeutics.
