Primary hypertension in a postoperative paraganglioma patient:A case report

Nursing care in old patients with heart failure： current status and future perspectives

The prognosis of heart failure （HF） patients has significantly improved in the last 20 years, given the advent of therapies such as angiotensin-converting enzyme （ACE） inhibitors/angiotensin receptor blockers （ARBs）, beta-block- ers, and implantable cardioverter-defibrillator （ICD）/cardiac resynchronization therapy devices （CRT）. In addition to these promising therapies, proper nursing-care is also important. Here we summarize recent progress of nursing-care strategies in older HF patients, including routine nursing care, transitional care model, self-care, and role of exercise training in old patients with HF.

Recent studies have showed that thrombosis is closely related to leucocytes involved in *** with the binding of leukocyte integrin Mac-1 and platelet GPIbαcan inhibit thrombosis without affecting physiological ***-1-GPIbαis proposed as a potential safety target for antithrombotic *** tablet(GXNT)is an oral Chinese patent medicine used for the treatment of angina pectoris,which contains phenolic acid active ingredients,such as salvianolic acids,ferulic acid,chlorogenic acid,caffeic acid,rosmarinic acid,tanshinol,and *** previous studies demonstrated that GXN exhibited significant antithrombotic effects,and clinical studies suggested that it did not increase bleeding *** addition,GXN exerted a significantly regulatory effect on immune *** the current study,we intended to evaluate the effects of GXN on bleeding events and explore the safety antithrombotic mechanism of GXN based on leukocyte-platelet ***,we established a gastric ulcer model induced by acetic acid in rats and found that GXN not only did not increase the degree of gastrointestinal bleeding when gastric ulcer occurred,but also had a certain promoting effect on the healing of gastric ***,in vitroexperiments showed that after pretreatment with GXN and activation by phorbol 12-myristate-13-acetate(PMA),the adhesion and aggregation of leukocytes with human platelets were *** was also found that GXN reduced the expression and activation of Mac-1 in leucocytes,and inhibited platelet activation due to leukocyte engagement via ***,the results suggest that GXN may be a safe antithrombotic agent,and its low bleeding risk mechanism is probably related to inhibited leukocyte-platelet aggregation and its interaction target Mac-1-GPIbα.

Electroversion in treatment of arrhythmia in a patient with Wolff-Parkinson-White syndrome and cervical spinal cord injury

We report electroversion in treatment of atrial fibrillation （AF） and atrioventricular nodal reentry ta- chycardia （AVNRT） in a patient with Wolff-Parkinson-White syndrome and cervical spinal cord injury. At first, the pa- tient sustained respiratory failure and weak cough reflex, thereafter repeated bronchoscopy was used to aspirate the sputum as well as control the pneumonia, which resulted in arrhythmia （AF and AVNRT）. Two doses of intravenousamiodarone failed to correct the arrhythmia. After restora- tion of sinus rhythm by electroversion, he was successfully weaned from mechanical ventilation and discharged from the intensive care unit without recurrent arrhythmia.

Effect of acupuncture at Neiguan (PC 6) on cardiac function using echocardiography in myocardial ischemia rats induced by isoproterenol

OBJECTIVE: To investigate the effect of acupuncture at Neiguan(PC 6) on cardiac function using echocardiography in rat models of myocardial ischemia(MI) induced by isoproterenol(ISO).METHODS: Twenty-seven Sprague-Dawley rats were randomly assigned to normal, model, and acupuncture groups. The model and acupuncture groups were given injections of ISO(85 mg/kg) to establish the MI model. After model establishment,the acupuncture group was treated with acupuncture at Neiguan(PC 6) for 30 min. Echocardiography was used to monitor diastolic and systolic function for 30 min starting from the time after the acupuncture needles were removed. Changes in the length of left ventricular internal diameter at end-diastole(LVIDd), length of left ventricular internal diameter at end-systole(LVIDs), the ratio of mitral peak velocity at early diastole and atrial contraction(E/A), ejection fraction(EF), fractional shortening(FS), and stroke volume(SV) were ***: Compared with the model group at 0and 15 min after needles were removed, the means of LVIDd and LVIDs were significantly lower(P <0.01) and E/A, EF, FS, and SV significantly higher(P < 0.01) in the acupuncture group. In the acupuncture group, the means of LVIDd and LVIDs15 min after the needles were removed were significantly higher(P < 0.01) than those at 0 min. The means of E/A, EF, FS, and SV significantly decreased(P < 0.01) from 0 to 15 min in the acupuncture ***: These findings indicate that acupuncture at Neiguan(PC 6) can affect cardiac function by increasing left ventricular diastolic and systolic function in MI rat models, but the effect only lasts for 15 min.

Histone modifications have been implicated in learning and memory. Our previous transcriptome data showed that expression of sirtuins 6(SIRT6), a member of Histone deacetylases(HDACs) family in the hippocampal CA1 was decreased after contextual fear conditioning. However, the role of SIRT6 in the formation of memory is still elusive. In the present study, we found that contextual fear conditioning inhibited translational expression of SIRT6 in the CA1. Microinfusion of lentiviral vector-expressing SIRT6 into the CA1 region selectively enhanced the expression of SIRT6 and impaired the formation of long-term contextual fear memory without affecting short-term fear memory. The overexpression of SIRT6 in the CA1 had no effect on anxiety-like behaviors or locomotor activity. Also, we also found that SIRT6 overexpression significantly inhibited the expression of insulin-like factor 2(IGF2) and amounts of proteins and/or phosphoproteins(e.g. Akt, p Akt, m TOR and p-m TOR) related to the IGF2 signal pathway in the CA1. These results demonstrate that the overexpression of SIRT6 in the CA1 impaired the formation of long-term fear memory, and SIRT6 in the CA1 may negatively modulate the formation of contextual fear memory via inhibiting the IGF signaling pathway.

A61-year-old woman was admitted to the department of vasculocardiology,First Affiliated Hospital of Zhengzhou University presented with recurrent respiratory distress for 2 weeks without fever,cough,phlegm,chest pain or *** symptom had generally worsened at night when she lying supine,and that could be partly alleviated when she had a seat or stand for a few *** weeks ago,the patient was admitted to the local hospital with diagnosis of ＂coronary disease;chronic hepatitis B;stage Ⅱ hypertension＂.After administered with conservative treatment,her symptoms were not significantly *** patient was then referred to our hospital in September 2012 for further evaluation and *** patient had a past medical history of hypertension with regular medications of compound kendir lenves （one tablet daily） and reserpine （one tablet daily）.Her family history was negative.
