Burden of multiple myeloma in China:an analysis of the Global Burden of Disease,Injuries,and Risk Factors Study 2019

Background:There is limited data to comprehensively evaluate the epidemiological characteristics of multiple myeloma(MM)in China;therefore,this study determined the characteristics of the disease burden of MM at national and provincial levels in ***:The burden of MM,including incidence,mortality,prevalence,and disability-adjusted life years(DALYs),with a 95%uncertainty interval(UI),was determined in China following the general analytical strategy used in the Global Burden of Disease,Injuries,and Risk Factors Study *** trends in the burden of MM from 1990 to 2019 were also ***:There were an estimated 347.45 thousand DALYs with an age-standardized DALY rate of 17.05(95%UI,12.31-20.77)per 100,000 in *** estimated number of incident case and deaths of MM were 18,793 and 13,421,with age-standardized incidence and mortality rates of 0.93(95%UI,0.67-1.15)and 0.67(95%UI,0.50-0.82)per 100,000,*** age-specific DALY rates per 100,000 increased to more than 10.00 in the 40 to 44 years age group reaching a peak(93.82)in the 70 to 74 years age *** had a higher burden than females,with approximately 1.5-to 2.0-fold sex difference in age-specific DALY rates in all age *** 1990 to 2019,the DALYs of MM increased 134%,from 148,479 in 1990 to 347,453 in ***:The burden of MM has doubled over the last three decades,which highlights the need to establish effective disease prevention and control strategies at both the national and provincial levels.

The evidence for the effects of environmental factors on COVID-19 case fatality remains controversial,and it is crucial to understand the role of preventable environmental factors in driving COVID-19 *** thus conducted a nationwide cohort study to estimate the effects of environmental factors(temperature,particulate matter[PM2.5,PM10],sulfur dioxide[SO2],nitrogen dioxide[NO2],and ozone[O3])on COVID-19 case fatality.A total of 71,808 confirmed COVID-19 cases were identified and followed up for their vital status through April 25,*** to ambient air pollution and temperature were estimated by linking the city-and county-level monitoring data to the residential community of each *** each participant,two windows were defined:the period from symptom onset to diagnosis(exposure window I)and the period from diagnosis date to date of death/recovery or end of the study period(exposure window II).Cox proportional hazards models were used to estimate the associations between these environmental factors and COVID-19 case ***-19 case fatality increased in association with environmental factors for the two exposure *** example,each 10 mg/m^(3) increase in PM2.5,PM10,O3,and NO2 in window I was associated with a hazard ratio of 1.11(95%CI 1.09,1.13),1.10(95%CI 1.08,1.13),1.09(95 CI 1.03,1.14),and 1.27(95%CI 1.19,1.35)for COVID-19 fatality,respectively.A significant effect was also observed for low temperature,with a hazard ratio of 1.03(95%CI 1.01,1.04)for COVID-19 case fatality per 1C *** analysis indicated that these effects were stronger in the elderly,as well as in those with mild symptoms and living in Wuhan or ***,the sensitivity analyses also yielded consistent ***-term exposure to ambient air pollution and low temperature during the illness would play a nonnegligible part in causing case fatality due to *** exposures to high concentrations of PM2.5,PM10,O3,SO2,and NO2 and low

The coronavirus disease 2019(COVID-19)has currently caused the mortality of millions of people around the *** from the direct mortality from the COVID-19,the indirect effects of the pandemic have also led to an increase in the mortality rate of other non-COVID *** indicates that novel COVID-19 pandemic has caused an inflation in acute cardiovascular mortality,which did not relate to COVID-19 *** has in fact increased the risk of death in cardiovascular disease(CVD)*** this purpose,it is dramatically inevitable to monitor CVD patients’vital signs and to detect abnormal events before the occurrence of any critical conditions resulted in *** of things(IoT)and health monitoring sensors have improved the medical care systems by enabling latency-sensitive surveillance and computing of large amounts of patients’*** major challenge being faced currently in this problem is its limited scalability and late detection of cardiovascular events in IoT-based computing *** this end,this paper proposes a novel framework to early detection of cardiovascular events based on a deep learning architecture in IoT *** results showed that the proposed method was able to detect cardiovascular events with better performance(95.30%average sensitivity and 95.94%mean prediction values).

Introduction:Accurately filling out death certificates is essential for death ***,manually determining the underlying cause of death is often *** this study,we investigate the Wide and Deep framework as a method to improve the accuracy and reliability of inferring the underlying cause of ***:death report data from national-level cause of death surveillance sites in Fujian Province from 2016 to 2022,involving 403,547 deaths,were *** Wide and Deep embedded with Convolutional Neural Networks(CNN)was *** performance was assessed using weighted accuracy,weighted precision,weighted recall,and weighted area under the curve(AUC).A comparison was made with XGBoost,CNN,Gated Recurrent Unit(GRU),Transformer,and GRU with ***:The Wide and Deep achieved strong performance metrics on the test set:precision of 95.75%,recall of 92.08%,F1 Score of 93.78%,and an AUC of 95.99%.The model also displayed specific F1 Scores for different cause-of-death chain lengths:97.13%for single causes,95.08%for double causes,91.24%for triple causes,and 79.50%for quadruple ***:The Wide and Deep significantly enhances the ability to determine the root causes of death,providing a valuable tool for improving causeof-death surveillance *** artificial intelligence(AI)in this field is anticipated to streamline death registration and reporting procedures,thereby boosting the precision of public health data.

Early intervention to prevent premature mortality is vital for adults with congenital heart defects(CHD).Anatomic complexity and comorbid conditions are thought to contribute to CHD *** hypertension(HTN)and heart failure(HF)are the comorbid conditions among the most prevalent causes of death in the United States,and commonly accompany CHD,it is crucial to evaluate whether they are reliable predictors of mortality for adults with CHD(ACHD)independent of anatomic CHD complexity.A retrospective cross-sectional analysis of ACHD,aged 18–64,with concomitant HTN and/or HF and at least one health care encounter during 2008–2010 were *** 5,397 ACHD patients(18.3%HTN without HF,4.4%HF without HTN,8.3%with both),3.0%died(n=163)during the study ***,the sample was 45.1%white,61.4%female,and 29.0%had a complex *** those who died,23.3%had HTN without HF,17.2%had HF without HTN,and 42.3%had *** analyses revealed that older age,male gender,black race,and having public health insurance were associated with increased mortality during the three-year study period compared to ACHD patients who were younger,female gender,white race,and covered by private health *** patients diagnosed with non-complex CHD lesions(i.e.,shunts,valves,or shunts+valves)were at greater risk of dying compared to those with severe complex *** CHD type was assessed separately,those with valve lesions were more likely to die compared to those with complex CHD *** adjustment for age,gender,race,insurance and CHD complexity,ACHD patients with HF,with or without HTN,were equally likely to die during the study ***,ACHD patients with HF,without or without HTN,who had valve defects were more likely to die during the three-year study period compared to patients with complex CHDs.

Background:Data regarding tuberculosis(TB)treatment outcomes,proportion of TB/HIV co-infection and associated factors have been released at different TB treatment facilities in Ethiopia and elsewhere in the world as part of the auditing and surveillance ***,these data are missing for the TB clinic offering directly observed treatment short-course(DOTs)at Debre Tabor General Hospital(DTGH).Methods:The authors analysed the records of 985 TB patients registered at the DTGH from September 2008 to December *** on patients’sex,age,type of TB,and treatment outcomes were extracted from the TB treatment registration *** treatment outcome of patients was categorized according to the National TB and Leprosy Control Program guidelines:cured,treatment completed,treatment failed,died,and not evaluated(transferred out and unknown cases).Results:Around half of the registered patients were males(516,52.4%).In terms of TB types,381(38.7%),241(24.5%),and 363(36.9%)patients had smear-negative pulmonary TB,smear-positive pulmonary TB,and extra pulmonary TB,*** hundred and seventy-two patients(90.1%)had successful treatment outcomes(cured and treatment completed),while 74 patients(9.9%)had unsuccessful treatment outcomes(death and treatment failure).TB treatment outcome was not associated with age,sex,type and history of TB,or co-infection with HIV(P>0.05).The proportion of TB/HIV co-infection was at 24.2%,and these were found to be significantly associated with the age groups of 25-34,35-44 and≥65 years:(aOR:0.44;95%CI:0.25-0.8),(aOR:0.39;95%CI:0.20-0.70),(aOR:4.2;95%CI:1.30-12.9),***:The proportion of patients with successful treatment outcomes was above the World Health Organization target set for Millennium Development Goal of 85%and in line with that of the global milestone target set at>90%for *** higher proportions of transfer-out cases were recorded in the present ***,the proportion of TB/HIV co-i

Interferon regulatory factor 1(IRF-1)is a member of the IRF *** is the first transcription factor to be identified that could bind to the interferon-stimulated response element(ISRE)on the target gene and displays crucial roles in the interferoninduced signals and ***-1,as an important medium,has all of the advantages of full cell cycle regulation,cell death signaling transduction,and reinforcing immune surveillance,which are well *** studies indicate that IRF-1 is of vital importance to the occurrence and evolution of multifarious liver diseases,including but not limited to inhibiting the replication of the hepatitis virus(A/B/C/E),alleviating the progression of liver fibrosis,and aggravating hepatic ischemiareperfusion injury(HIRI).The tumor suppression of IRF-1 is related to the clinical characteristics of liver cancer patients,which makes it a potential indicator for predicting the prognosis and recurrence of liver cancer;additionally,the latest studies have revealed other effects of IRF-1 such as protection against alcoholic/non-alcoholic fatty liver disease(AFLD/NAFLD),cholangiocarcinoma suppression,and uncommon traits in other liver diseases that had previously received little ***,several compounds and drugs have featured a protective function in specific liver disease models in which there is significant involvement of the IRF-1 *** this paper,we hope to propose a prospective research basis upon which to help decipher translational medicine applications of IRF-1 in liver disease treatment.

Clinico-Pathological Characterization of Hereditary, Familial and Sporadic Prostate Cancer

Aim: To characterize familial prostate cancer including hereditary prostate cancer and assess the disease-free survival following radical prostatectomy. Methods: A self-administered written questionnaire was forwarded to 709 prostatectomized patients from the Aarhus Prostate Cancer Study containing questions about cases of prostate cancer (PC), age at diagnosis, vital status, and age at death for all first-degree relatives. Patients were then divided into groups according to their family history: hereditary prostate cancer (HPC), familial prostate cancer (FPC), and sporadic prostate cancer (SPC) groups. The information from a subset of both FPC (n = 17) and SPC (n = 17) groups were validated in the Danish Cancer Register and the Civil registration System. Between groups, we described the association of age, prostatespecific antigen (PSA), postoperative Gleason score and T Stage. A Kaplan-Meier curve demonstrated postoperative disease-free survival in each group. Results: The response rate was 81% (574/709). About 21% of the patients were categorized in the FPC group, of which 7% were identified as having HPC. The median follow-up time was 63 months for HPC, 65 months for FPC and 88 months for SPC. Overall, there was no significant difference between groups in clinical features and disease-free survival except that patients with HPC were significantly associated with younger age than sporadic cases (p = 0.02). The proportion of self-reported PC diagnoses confirmed in the cancer register was 27.8%. The index persons with SPC reported no PC in first-degree relatives and none was found the cancer register. Conclusion: Overall, we found no difference in clinical characteristics and survival, following radical prostatectomy except that patients with HPC were younger at diagnosis. These results are in line with previously reported data.

Clinicopathological and therapeutic comparisons of esophageal cancer between China and the USA:a multicenter hospital-based study

Background:Esophageal cancer(EC)remains a global health challenge due to its poor *** and the United States of America(USA)represent two distinct epicenters of EC *** the EC disparities in these two countries is vital for tailoring prevention strategies,optimizing treatment,and enhancing outcomes in both ***,there lacks a comprehensive comparison of EC characteristics between the two ***:In this multicenter,retrospective hospital-based study,we enrolled primary EC patients who received their initial treatment at one of 23 hospitals in China during *** electronic medical records and cancer registration records,information on demographics,lifestyle,and clinicopathological characteristics(in-cluding tumor site,pathology,stage,metastases,differentiation,and treatment)were ***,we compared these data with the clinicopathological information of invasive EC patients diagnosed in 2016-2017 from the surveillance,Epidemiology,and End Results(SEER)database in the ***:A total of 6,658 EC patients in China and 8,555 EC patients in the USA were included finally.85.5%(n=5,694)of EC were esophageal squamous cell carcinoma(ESCC)in China,while esophageal adenocarcinoma(EAC)was prominent in the USA(58.9%,n=5,041).Among EC patients with known staging,the proportion of early stage was higher in China compared to the USA(48.3%vs.30.5%).Among ESCC patients,early-stage cases were higher in China than in the USA(49.8%vs.31.8%),while among EAC patients,late-stage cases were higher in China than in the USA(77.3%vs.68.5%)(all P<0.001).In China,EC mainly occurred in the middle third(60.2%)of the esophagus,whereas in the USA,it was more common in the lower third(59.9%)of the *** with EC patients with known metastatic status in the USA,China had fewer cases of lymph node metastases(51.4%vs.57.7%)and distant metastases(7.9%vs.33.8%).Regarding treatment,China had more surgical therapy(53.7%vs.22.6%
