Invasive <i>Aspergillus</i>Infections in a Thai Tertiary-Care Hospital during 2006-2011

From an increase in the number of immunocompromised hosts including AIDS patients, organ transplantation, solid-organ tumor, hematological malignancy, corticosteroid use, and others underlying diseases, it leads to increasing the incidence of invasive aspergillosis (IA) as one of the most prevalent opportunistic mould infections. However, the epidemiological data are still limited. Our objective is to study the epidemiology of IA, patients’ characteristics in a tertiary-care hospital, King Chulalongkorn Memorial Hospital, Bangkok, Thailand. The retrospective study of IA as principal diagnosis in both medical and laboratory records in a tertiary-care hospital, King Chulalongkorn Memorial Hospital, from January 1, 2006 to December 31, 2011, was performed. There were 69 patients who were diagnosed as IA during 2006 till 2011. They were classified as proven (45 patients), probable (3 patients), and possible (21 patients) invasive aspergillosis following the criteria of European Organization for Research and Treatment of Cancer/Mycoses Study Group (EORTC/MSG), 2008. The numbers of patients in 2006 to 2011 were 3, 11, 12, 10, 10, and 23 respectively. Male patients were 58 percent. The age range was from 8 months to 87 years old. Most of patients were from Medicine ward. Others were derived from Pediatrics, Surgery, and Ear Nose Throat wards. The most common underlying disease was diabetes mellitus type 2 in the proven group. The main predisposing factors of patients were the history of pulmonary tuberculosis and using of immunosuppressive drugs. The sites of infection were lung (62%), sinus (28%), and brain (8%). Aspergillus fumigatus (69%) and Aspergillus flavus (15%) were common species from the isolated culture. The treatment used mostly was surgery and followed by amphotericin B or voriconazole. The Prevalence of Human Cytomegalovirus Viremia among HIV-1 Infected Individuals Undergoing Antiretroviral Therapy

HIV infection is an emerging health issue in Libya, particularly among young adults. Human cytomegalovirus (HCMV) is a prevalent infectious agent that presents with subclinical and fatal diseases in immunosuppressed individuals including HIV-infected individuals. Although the impact of HCMV infection in HIV-positive patients is well documented in several regions, epidemiologic estimates concerning HCMV co-infection among HIV-infected individuals remain limited in Libya. Hence, this cross-sectional study was undertaken to derive data regarding the prevalence of active HCMV viremia among HIV-infected individuals undergoing antiretroviral therapy (ART) from Libya. A total of 90 consented HIV-infected subjects followed by the National Center for Disease Control (NCDC) of Benghazi/Libya were recruited in this study and investigated for HCMV-IgG, HCMV-IgM specific antibodies, detection of HCMV lower matrix phosphoprotein (pp65) antigen, and detection of HCMV-DNA using qPCR to assess the prevalence of HCMV viremia. We determined that 77 (85.56%) of subjects were seropositive for HCMV-IgG antibodies, whereas the seropositivity for HCMV-IgM was 3.33% (3/90 subjects). Our results also revealed that 4.44% (4/90) of participants had viral antigenemia based on the laboratory diagnosis of HCMV-pp65. Regarding the PCR, we were able to detect the DNA of HCMV only in 3/90 subjects (3.33%) suggesting an active viremic condition. The detection of HCMV DNA along with the HCMV-pp65 in HIV-positive individuals highlights the necessity of early diagnosis to manage the progression of the disease. Furthermore, we highly recommend the use of anti-HCMV therapy in viremic individuals in combination with ART to reduce the burden of HCMV complications.

Increasing Incidence of Extended-Spectrum Beta-Lactamase-Producing Enterobacteriaceae (ESBL) and the Relation to Consumption of Broad-Spectrum Antimicrobial Agents 2003-2011 in a Large Area of Copenhagen, Denmark

Purpose: To investigate 1) the development in the incidence of ESBL-producing bacteria in hospitals and primary health care, 2) the contribution of primary health care to the incidence of ESBL-producing bacteria, and 3) the development in resistance patterns for all Escherichia coli and Klebsiella pneumoniae isolates in relation to antimicrobial consumption in hospitals and primary health care. Methods: ESBL-data were retrospectively collected from bacterial isolates from all specimens received at the department of Clinical microbiology from 2003 to 2011 together with the corresponding patient data. ESBL-production was detected in isolates from 1067 of 59,373 patients (1.8%) with an E. coli infection and in 263 of 8660 patients (3.0%) with a K. pneumoniae infection. Results: From 2003 to 2009, an increase in patients with an ESBL-producing isolate occurred in both hospitals and primary health care at the same time as an increased consumption of broad-spectrum antimicrobial agents was seen. Interventions to reduce prescription of cephalosporins and ciprofloxacin at the hospitals from 2010 resulted in a remarkable decrease in patients with ESBL-producing K. pneumoniae whereas a continuing increase was seen in patients with ESBL-producing E. coli both at hospitals and in primary health care. The proportion of patients with community-acquired ESBL-producing E. coli was stable with an increase of only 1.4% from 2007 to 2011. Conclusions: Reduction in prescription of broad-spectrum antimicrobial agents at the hospital level had an important impact on the incidence of ESBL-producing K. pneumoniae, but not on ESBL-producing E. coli.

Anthropogenic disturbances and their impact on vegetation in Western Himalaya,India

The present study assesses anthropogenic disturbances and their impacts on the vegetation in Western Himalaya,India on the basis of various disturbance parameters such as density,Total Basal Cover(TBC) of cut stumps,lopping percentage and grazing *** the basis of canopy cover and frequency of disturbances(%),the studied forests were divided into highly disturbed(HD),moderately disturbed(MD) and least disturbed(LD) *** HD forests had the lowest canopy cover,lowest density and lowest TBC and the LD had the highest canopy cover,highest density and highest *** MD forests occupied the intermediate position with respect to these *** richness was least in HD forests,highest in one of the MD forests while LD forest occupied an intermediate *** percentage of regenerating species was lowest(54%) in HD and highest(72%) in *** density of seedlings and saplings was higher in one of the MD forests as compared to HD and *** concluded that the moderate disturbances do not affect the vegetation adversely;however the increased degree of disturbance causes loss in plant diversity;affects regeneration and changes community *** of hydroelectric projects at various places in the study area was found to be one of the most important sources of anthropogenic disturbances in addition to the routine anthropogenic disturbances like grazing,fuelwood collection and fodder *** all proposed dams in the Indian Himalaya are constructed combined with weak national environmental impact assessment and implementation,it will result in a significant loss of ***,various agents of disturbances should be evaluated in cumulative manner and any developmental activities such as hydropower projects,which trigger various natural and anthropogenic disturbances,should be combined with proper cumulative environmental impact assessment and effective implementation to minimise the anticipated loss of vegetation.

Characterization and function analysis of a Halo-alkaline-adaptable Trk K^+ uptake system in Alkalimonas amylolytica strain N10

By functional complementation of Escherichia coli mutants defective in potassium (K+) uptake, two genes that are required for K+ uptake in halo-alkaliphilic Alkalimonas amylolytica strain N10 were cloned. These two genes, Aa-trkA (1337 bp) and Aa-trkH (1452 bp), were adjacent on the A. amylolytica N10 chromosome and transcribed in opposite directions. Complementation experiments revealed that Aa-TrkA and Aa-TrkH from A. amylolytica strain N10 restored the ability to grow at low K+ concentration in E. coli △trkA and △trkG △trkH strains, respectively. In addition, Aa-TrkAH supported the growth of an E. coli △sapD strain, indicating that the ATP-binding protein TrkE was dispensable for the Trk system of A. amylolytica strain N10. The net K+ uptake was detected at different pH levels and the critical NaCl concentration indicated that Aa-TrkAH is an alkaline-adaptable and partially halo-adaptable K+ transporter. Kinetics determined by heterogeneous K+ transport experiments with an E. coli △trkA strain revealed that Aa-TrkAH has an alkaline pH optimum close to 8.5 or higher. Site-directed mutagenesis of Aa-TrkH showed that Phe103 and Ser229 play certain key roles in K+ selection and transportation. The molecular chaperones groES-groEL and tig promoted Aa-TrkH and Aa-TrkA overexpression in vitro.

Interferon-alpha-2b induces autophagy in hepatocellular carcinoma cells through Beclin1 pathway

Objective: To determine whether Interferon-alpha-2b(IFN-α2b) can modulate the autophagic response in hepatocellular carcinoma ***: Hepatocellular carcinoma cells were treated with IFN-α2b. Autophagy was assessed by acridine orange staining, GFP-LC3 dotted assay, transmission electron microscopy and ***: Acridine orange staining showed that IFN-α2b triggered the accumulation of acidic vesicular and autolysosomes in HepG2 cells. The acridine orange HepG2 cell ratios were(4.3±1.0)%,(6.9±1.4)%, and(13.1±2.3)%, respectively, after treatment with 100, 1,000, and 10,000 IU/mL IFN-α2b for 48 h. A markedly punctate pattern was observed in HepG2 cells treated with 10,000 IU/mL IFN-α2b for 48 h, but only diffuse and weakly fluorescent GFP-LC3 puncta was observed in control cells. HepG2 cells treated with 10,000 IU/mL IFN-α2b for 48 h developed autophagosome-like characteristics, including single- or double-membrane vacuoles containing intact and degraded cellular debris. The Beclin1 and LC3-II protein expression was up-regulated by IFN-α2b ***: Autophagy can be induced in a dose-dependent manner by treatment with IFN-α2b in HepG2 cells, and the Beclin1 signaling pathway was stimulated by IFN-α2b.
