In this editorial,we provide a critical review of the article by Tang et al published in the World J Clin Cases,focusing on the utilization of butorphanol for epidural analgesia during *** discussion encompasses recent research developments in epidural labor analgesia,specifically highlighting the current status of clinical applications of butorphanol and associated treatment *** analgesia is widely acknowledged as the primary method for pain management during labor,offering effective and prolonged pain relief while allowing mothers to remain alert and actively participate in the delivery *** the various drugs utilized for epidural labor analgesia,butorphanol has received increasing attention due to its potential efficacy and distinctive pharmacological *** a synthetic opioid analgesic,butorphanol exhibits both agonistic and antagonistic activity on opioid receptors,striking a balance between analgesia and minimizing side ***,the safety and efficacy of butorphanol in epidural labor analgesia remains *** certain studies have reported positive outcomes with butorphanol,including effective pain relief and a reduced incidence of side effects,others have raised concerns about its safety and efficacy compared to traditional opioids or alternative *** addition,the optimal dosing strategy and regimen of butorphanol as an adjuvant in epidural labor analgesia still need to be *** comprehensive synthesis and analysis of existing literature,we aim to evaluate the current evidence regarding the use of butorphanol for epidural labor analgesia,delineate areas of consensus and controversy,and propose future avenues for research and clinical practice in this domain.

Aims:Patients with liver disease may exhibit higher infection rates and mortality rates from coronavirus disease 2019(COVID-19)than healthy individuals,and vaccination against severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is an effective prevention *** metaanalysis aimed to assess the effectiveness and safety of SARS-CoV-2 vaccines in patients with chronic liver disease(CLD)and post-liver transplantation(LT).Methods:The PubMed,Embase,and Cochrane databases were searched.A random-effects model meta-analysis was used to determine the seropositivity rates of SARS-CoV-2 antibodies,odds ratio(OR)compared with healthy controls(HC),risk ratio(RR)between the booster and standard vaccination regimen,and the rate of adverse reactions(ADR).Results:In the standard vaccination regimen analysis,17 controlled articles were included for effectiveness analysis,and six articles for ADR *** pooled seropositivity rates of SARS-CoV-2 antibodies in patients with CLD and post-LT were 93.3%(95%confidence interval[CI]:89.0%-97.6%)and 69.1%(95%CI:63.0%-75.3%),*** rates were lower than those in HC(p<0.001).The differences remained significant after sorting by detection interval,vaccine type,antibody type,or CLD *** recipients showed much lower seropositivity rates of antibodies than patients with CLD(69.1%vs.93.3%)or HC(OR:0.055).The pooled total ADR rate of patients was 24.0%(95%CI:16.2%-31.8%).In the booster vaccination regimen analysis,11 prospective studies were enrolled,and the seropositivity rates of antibodies after the booster dose were increased by 27%compared with those of the standard vaccination regimen(RR:1.27,95%CI:1.15-1.41,p<0.001).Conclusion:Patients with CLD and post-LT can gain protection against COVID-19 from standard vaccines,demonstrating a potentially weaker immunogenic response than *** vaccines can compensate for this ***,patients with CLD and post-LT should be prioritized for receiving the COVI

Cities are increasingly vital in global carbon mitigation efforts,yet few have specifically tailored carbon neutrality ***,out-of-boundary indirect greenhouse gas(GHG)emissions,aside from those related to electricity and heat imports,are often overlooked in existing pathways,despite their significance in comprehensive carbon mitigation *** this gap,here we introduce an integrated analysis framework focusing on both production and consumption-related GHG *** to Wuyishan,a service-oriented city in Southern China,this framework provides a holistic view of a city's carbon neutrality pathway,from a full-scope GHG emission *** findings reveal the equal importance of carbon reduction within and outside the city's boundaries,with out-of-boundary emissions accounting for 42%of Wuyishan's present total GHG *** insight highlights the necessity of including these external factors in GHG accounting and mitigation strategy *** framework serves as a practical tool for cities,particularly in developing countries,to craft effective carbon neutrality roadmaps that encompass the full spectrum of GHG emissions.

Aquatic vegetation is crucial for improving water quality,supporting fisheries and preserving biodiversity in *** the spatiotemporal dynamics of aquatic vegetation is indispensable for the assessment and protection of lake ***,a comprehensive global assessment of lacustrine aquatic vegetation is *** study introduces an automatic identification algorithm (with a total accuracy of 94.4%) for Sentinel-2 MSI,enabling the first-ever global mapping of aquatic vegetation distribution in 1.4 million lakes using 14.8 million images from 2019 to *** show that aquatic vegetation occurred in 81,116 lakes across six continents over the past four years,covering a cumulative maximum aquatic vegetation area (MVA) of 16,111.8 *** global median aquatic vegetation occurrence (VO,in%) is 3.0%,with notable higher values observed in South America (7.4%) and Africa (4.1%)compared with Asia (2.7%) and North America (2.4%).High VO is also observed in lakes near major rivers such as the Yangtze,Ob,and Paraná*** historical data with our calculated MVA,the aquatic vegetation changes in 170 lakes worldwide were *** shows that 72.4%(123/170) of lakes experienced a decline in aquatic vegetation from the early 1980s to 2022,encompassing both submerged and overall aquatic *** most substantial decrease is observed in Asia and *** findings suggest that,beyond lake algal blooms and temperature,the physical characteristics of the lakes and their surrounding environments could also influence aquatic vegetation *** research provides valuable information for the conservation and restoration of lacustrine aquatic vegetation.

Based on the comprehensive analysis of core, thin section, logging and seismic data, this study carried out the identification and comparison of Permian Changxing Formation sequences, clarified the typical sedimentary architectures of intra-platform shoal, investigated the vertical and horizontal development and distribution of intra-platform shoal in each sequence, and thus established the sedimentary evolution model of shoal body. The study results are reflected in four ***, there are two complete third-order sequences(SQ1 and SQ2) in Changxing Formation in central Sichuan Basin. SQ1 is generally thick in the north and thin in the south, and SQ2 shows a thickness differentiation trend of “two thicknesses and three thinnesses”. Second, the Changxing Formation in central Sichuan Basin mainly develops intra-platform shoal, inter-shoal sea and intra-platform depression subfacies. In the vertical direction, the intra-platform shoal mainly presents two typical sedimentary sequences: stable superposed and high-frequency interbedded. Third, the stable superimposed sedimentary sequence is developed in the shoal belt at the edge of intra-platform depression, which is composed of two shoal-forming periods and located in the highstand systems tracts(HSTs) of SQ1 and SQ2. The high-frequency interbedded sedimentary sequence is developed in the southern shoal belt of intra-platform depression, which is composed of four shoal-forming periods and mainly located in the HST of SQ2. Fourth, during the SQ1 deposition, the intra-platform shoal was mainly developed at the edge of the intra-platform depression on the north side of the study area, and the inter-shoal sea subfacies was mainly developed on the south side. AIM: To identify the predictors of rebleeding after initial hemostasis with epinephrine injection (EI) in patients with high-risk ulcers. METHODS: Recent studies have revealed that endoscopic thermocoagulation, or clips alone or combined with EI are superior to EI alone to arrest ulcer bleeding. However, the reality is that EI monotherapy is still common in clinical practice. From October 2006 to April 2008, high-risk ulcer patients in whom hemorrhage was stopped after EI monotherapy were studied using clinical, laboratory and endoscopic variables. The patients were divided into 2 groups: sustained hemostasis and rebleeding. RESULTS: A total of 175 patients (144, sustainedhemostasis; 31, rebleeding) were enrolled. Univariate analysis revealed that older age (≥ 60 years), advanced American Society of Anesthesiology (ASA) status (category Ⅲ , Ⅳ and Ⅴ ), shock, severe anemia (hemoglobin surgical hemostasis, 15 (48.4%) suffered from delayed hemostasis causing major complications and 13 (41.9%) died of these complications. CONCLUSION: Endoscopic EI monotherapy in patients with high-risk ulcers should be avoided. Initial hemostasis with thermocoagulation, clips or additional hemostasis after EI is mandatory for such patients to ensure better hemostatic status and to prevent subsequent rebleeding, surgery, morbidity and mortality.

Cecal bascule is a rare cause of bowel obstruction in which a mobile cecum folds anteriorly and superiorly over the ascending colon. Herein, we present the first case of internal herniation of a cecal bascule into the lesser sac through the foramen of winslow, aiming at discussing radiological findings, differential diagnosis,and surgical management of this uncommon condition.A 75-year-old female presented to the emergency room with an 18-h history of sudden onset sharp, progressively worsening abdominal pain associated with vomiting. Physical exam revealed abdominal distention and epigastric tenderness while initial laboratory tests were unremarkable. Computed tomography of her abdomen and pelvis showed a loop of distended colon within lesser sac without signs of bowel ischemia or perforation. On exploratory laparotomy, a cecal bascule was found herniating into lesser sac via foramen of *** reduction, the cecum appeared viable therefore a cecopexy was performed without bowel *** cecal volvulus, cecal bascule consists of no axial rotation of the bowel with no mesenteric vascular compromise and therefore ischemia would only occur from intraluminal tension or extraluminal compression from the borders of foramen of winslow. The management of internal herniation of a cecal bascule is always surgical including anatomic resection or cecopexy.

Relationships among productivity,leaf phenology,and seasonal variation in moisture and light availability are poorly understood for evergreen broadleaved tropical/subtropical forests,which contribute 25% of terrestrial *** the one hand,as moisture availability declines,trees shed leaves to reduce transpiration and the risk of hydraulic *** the other hand,increases in light availability promote the replacement of senescent leaves to increase ***,we provide a comprehensive framework that relates the seasonality of climate,leaf abscission,and leaf productivity across the evergreen broadleaved tropical/subtropical forest *** seasonal correlation between rainfall and light availability varies from strongly negative to strongly positive across the tropics and maps onto the seasonal correlation between litterfall mass and productivity for 68 *** rainfall and light covary positively,litterfall and productivity also covary positively and are always greater in the wetter sunnier *** rainfall and light covary negatively,litterfall and productivity are always greater in the drier and sunnier season if moisture supplies remain adequate;otherwise productivity is smaller in the drier sunnier *** framework will improve the representation of tropical/subtropical forests in Earth system models and suggests how phenology and productivity will change as climate change alters the seasonality of cloud cover and rainfall across tropical/subtropical forests.
