Focal atrial tachycardia （AT） is an uncommon chronic supraventricular tachycardia in children, often resists medical therapy and untreated may progress to tachycardiomyopathy. In adults, the responsible foci cluster around the crista terminalis, ostium of the coronary sinus, tricuspid annulus, parahisian region, superior vena cava, the mitral annulus, the ostia of the pulmonary veins and left atrial septum. In contrast, major source of half of AT during childhood were determined to be one of the atrial appendages （AA）.~ Radiofrequency catheter ablation （RFCA） is becoming the preferred treatment for symptomatic, medically refractory patients with AT,2 however, it can recur after ablation because the origin in the left AA （LAA） or the right AA （RAA） is not accessible with the abtation catheter. In this study, we review the clinical results of atrial appendectomy for recurrent AT originating from the AA after RFCA in children at our hospital.

Gastrointestinal stromal tumor of the stomach with axillary lymph node metastasis: A case report

Gastrointestinal stromal tumors (GISTs) are the most common type of gastrointestinal mesenchymal tumors, although metastasis to the perigastric lymph nodes is relatively rare, compared with liver or peritoneal metastasis. In this report, we describe a case of stomach GIST with a solitary simultaneous metastasis in the left axillary lymph node. A 68-year-old man was diagnosed with a large upper-stomach GIST, and computed tomography and positron emission tomography revealed masses in the left axilla and right mediastinum. We did not detect evidence of metastases to the liver, or other sites including the perigastric lymph nodes, although findings from the surgically resected axillary lymph nodes were compatible with GIST metastasis. Treatment using imatinib markedly reduced the gastric and mediastinal lesions, and this response persisted for 3 years. The patient subsequently experienced rapid growth of the gastric lesion without mediastinal or axilla recurrence, which required palliative surgery. Despite continuing medical treatment(sunitinib and regorafenib), the patient died of liver metastases 23 mo after the surgery. Based on our findings, it appears that the axillary lymph nodes can be a potential metastatic site for GIST metastasis.

Objective:To compare Roux-en-Y hepatico-jejunostomy with complete resection of the cyst or incomplete resection with 1-cm remnant proximal cyst wall in treating adult type I choledochal cyst(CC).Methods:The medical records of 267 adult patients with type I CC from January 1998 to December 2015 were reviewed *** them,171 underwent Roux-en-Y hepatico-jejunostomy with complete resection(PBD 0-cm group)and 96 underwent Roux-en-Y hepatico-jejunostomy with 1-cm proximal cyst wall left(PBD 1-cm group).The short-and long-termpost-operative complications were compared between the two ***:No significant difference was observed in operative time or anastomotic diameter between the two *** incidence of perioperative complications was significantly higher in the PBD 1-cm group than that in the PBD 0-cm group(28.1%vs 14.0%,p¼0.005),especially post-operative cholangitis(7.3%vs 1.2%,p¼0.021).The incidence of long-term post-operative complications was not significantly different,including anastomotic stricture,reflux cholangitis,intra-hepatic bile duct stones and bile leak(all p>0.05).Post-operative intra-pancreatic biliary malignancy occurred in one patient in the PBD 0-cm group at 25 months and one patient in the PBD 1-cm group at 5 month,*** site malignancy was observed in one patient in the PBD 1-cm group at 10 ***:Ease of performing anastomosis does not justify retaining a segment of choledochal cyst in type I CC due to its higher risk of post-operative complication and malignancy.A complete excision of the CC with anastomosis to the healthy proximal bile duct is necessary in treatment of type I CC.

Objective: To investigate the expression of Fas ligand in human colon carcinoma cell lines. Methods: A total of six human colon cancer cell lines were examined for the expression of Fas ligand mRNA and cell surface protein by using RT-PCR and flow cytometry respectively. Results: The results showed that Fas ligand mRNA was expressed in all of the six cancer cell lines and Fas ligand cell surface protein was expressed in part of them. Conclusion: These data suggest that Fas ligand was expressed, at least in part, in human colon cancer cell lines and might facilitate to escape from immune surveillance of the host.
