Background:A new staging system for patients with hepatocellular carcinoma(HCC)associated with portal vein tumor thrombus(PVTT)was developed by incorporating the good points of the BCLC classification of HCC,and by improving on the currently existing classifications of HCC associated with ***:Univariate and multivariate analysis with Waldχ2 test were used to determinate the clinical prognostic factors for overall survival(OS)in patients with HCC and PVTT in the training *** the conditional inference trees analysis was applied to establish a new staging ***:A training cohort of 2,179 patients from the Eastern Hepatobiliary surgery Hospital and a validation cohort of 1,550 patients from four major liver centers in China were enrolled into establishing and validating a new staging *** system was established by incorporating liver function,general health status,tumor resectability,extrahepatic metastasis and extent of *** staging system had a good discriminatory ability to separate patients into different stages and *** median OS for the two cohorts were 57.1(37.2-76.9),12.1(11.0-13.2),5.7(5.1-6.2),4.0(3.3-4.6)and 2.5(1.7-3.3)months for the stages 0 to IV,respectively(P<0.001)in the training *** corresponding figures for the validation cohort were 6.4(4.9-7.9),2.8(1.3-4.4),10.8(9.3-12.4),and 1.5(1.3-1.7)months for the stages II to IV,respectively(P<0.001).The mean survival for stage 0 to 1 were 37.6(35.9-39.2)and 30.4(27.4-33.4),respectively(P<0.001).Conclusions:A new staging system was established which provided a good discriminatory ability to separate patients into different stages and substages after *** can be used to supplement the other HCC staging systems.

Treatment of severe acute pancreatitis through retroperitoneal laparoscopic drainage

A treatment method based on drainage via retroperitoneal laparoscopy was adopted for 15 severe acute pancreatitis(SAP)patients to investigate the feasibility of the *** patients received only drainage via retroperitoneal laparoscopy,four patients received drainage via both retroperitoneal and preperitoneal laparoscopy,and one patient received drainage via conversion to *** patients exhibited a good drainage effect and were successfully cured without any other surgical *** patients had encapsulated effusions or pancreatic pseudocysts after surgery,but were successfully cured after lavage and B ultrasound-guided percutaneous catheter *** treatment via retroperitoneal laparoscopic drainage is an effective surgical method,resulting in minor injury.

Relationship between CYP1A1 polymorphisms and invasion and metastasis of breast cancer

Objective:To investigate the relationship between CYPIA1 genetic polymorphisms and the invasion and metastasis of breast ***:The CYP1A1 gene polymorphism(an T-C transversion at nucleotide position 3801)was detected by the polymerase chain reaction and restriction fragment length polymorphism in 80 cases with breast cancer and 60 samples of normal breast *** difference in genotypic distribution frequency between the groups,the correlation between the genotypes and the factors related to prognosis were ***:The incidence of homozygous and variant genotypes had no difference between the breast cancer group and controls group(P=0.746).The proportion of variant genotype increased as clinical stage(P=0.006)advanced,as well as with increased numbers of lymph node metastases(P=0.010).Conclusions:In patients with breast cancer there is a correlation between the CYP1A1 CC allele and some factors indicating poor prognosis,including more lymph node metastases as well as a more advanced clinical stage.

Multicentric hyaline-vascular Castleman's disease in the retroperitoneum

BACKGROUND:Castleman’s disease is a rare lymphoproliferative disease of unknown *** multicentric cases described have been of the plasma-cell *** article presents a case of multicentric hyaline-vascular Castleman’s disease in the retroperitoneum with the ultrasonographic and computed tomography(CT)imaging manifestations. METHODS:During routine physical examination,a mass was detected in the left abdomen of a 53-year-old man with no signs or *** patient underwent ultrasoundguided aspiration biopsy and operative excision after laboratory examination,ultrasonography,and CT. RESULTS:Ultrasonography demonstrated a dominant hypoechogenic mass with hypervascularity in the *** detected a relatively homogenous enhanced lesion and several satellite *** the mass and several lymph nodes were resected, histopathologic examination demonstrated a lymphocytepredominant infiltrate surrounding the germinal centres and extensive capillary proliferation,consistent with the hyaline-vascular type of Castleman’s *** patient received postoperative chemotherapy and remained free of recurrence 3 months later. CONCLUSION:Ultrasonography and contrast-enhanced CT can provide a positive differential diagnosis of hyalinevascular Castleman’s disease which is a kind of giant lymph node hyperplasia with hypervascularity.

A new targeting anticancer system was prepared by using hydroxyapatite particles (2 mm in diameter) as carrier material and adriamycin as anticancer agent. The adsorption and release properties of the complexes were assayed by fluorometry in vivo and in vitro and the curative effect on W 256 sarcoma of rat was observed. The results showed that one particle of hydroxyapatite could adsorb approximately 0.08 mg adriamycin and they can maintain a steady and slow release of adriamycin from hydroxyapatite for one month. When hydroxyapatite adriamycin complexes were implanted into the liver of rat, liver adriamycin concentration at the implanted region was obviously higher than that achieved by injection of adriamycin solution. The locally implanted complexes obviously inhibited the growth of subcutaneous implanted tumor of rat, and increased the survival rate of rat with implanted liver tumor.

Experimental Study on Polymer Artificial Bone with HA Coated and Fiber Liners

Layers of ante-polymerized PMMA （ Polymethylmethacrylate ） ore manually smeared on both sides of carbon fiber and polyester fiber to a certain thickness. It was pre-solidified, stripped and cut for sterilization. Based on the results of a series of experiments, HA coated artificial bone is considered to be a non-senshizing, non- irritant, and non- toxic biomaterial for medical applications. The artificial bone has excellent mechanical and biological property. And it conforms to the national standard requirement. Safety analysis guarantees h a prosperous future of clinical application.

BACKGROUND In a previous study,autologous bone marrow infusion(ABMI)was performed in patients with decompensated liver cirrhosis(DLC)and acquired immunodefi-ciency syndrome and achieved good results,but whether splenectomy affected outcome was *** To investigate the efficacy of ABMI combined with splenectomy for treatment of *** Eighty-three patients with DLC were divided into an intervention group(43 cases)and control group(40 cases)according to whether splenectomy was *** control group was treated with ABMI through the right omental RESULTS After ABMI,the prothrombin time,serum total bilirubin levels,ascites volume and model for end-stage liver disease score in both groups were significantly lower,while the albumin levels were significantly higher than before ABMI(P0.05).After ABMI,the white blood cell and platelets counts in both groups were significantly higher than before ABMI(P<0.01),and the counts in the intervention group were significantly higher than in the control group(P<0.01).After ABMI the CD4+and CD8+T cell counts in both groups were significantly higher than before ABMI(P<0.01).The CD8+T cell counts in the intervention group increased continuously and the increase had a shorter duration compared with control *** ABMI through the portal vein in patients with DLC can significantly improve liver synthetic and secretory functions,and splenectomy promotes improvement of bone marrow hematopoietic and cellular immune functions.
