Critical Factors in Managing Relationally Demanding Jobs, in Care for Very Ill and/or Dying Patients: A Phenomenological Study among Public Hospital Nurses

Background: workload, interpersonal relationships, professional conflict and the emotional cost of providing care are potential sources of stress and burnout among nurses. Based on experiences of hospital nurses, this paper aims to identify critical factors for nurses in managing relationally demanding situations in care for very ill and/or dying patients. Methods: In-depth interviews were carried out with six nurses, working in a medium-sized hospital in Norway. The interviews were analysed using an interpretative phenomenological analysis. Results: The lack of identification with the core aspect of the job, relational contact with patients and relatives, and external motivation were found as potential barriers for managing relationally demanding jobs. The results also indicate that the nurses’ experiences of symptoms of burnout can be a result of demands that exceeded the nurses’ resources. A match between personal capacity and demands, mutual support among colleagues and leadership support, and contextual factors are critical to prevent the negative process of burnout. Conclusion: The results underscore the importance of an early response to employees who are beginning to struggle at work and the relevance of a true match between personal capacity and job demands. Moreover support among colleagues and leadership seems critical to prevent a negative process of burnout and help to manage relationally demanding jobs.

The Human Capital Return and social Integration of Migrants in China

From the perspective of migrants ＇self-selection and skill transferability theories, this paper compares human capital return rates of different migrant groups in urban China and discuss the possibility of economic status identified by migrants as local labors. Results suggest that positive self-selection works for all types of migrant labors. Migrant workers with non-agricultural household registration or feinong hukou have no trouble to be integrated into the labor market of destination. They even have higher income and human capital return rates than local urban workers. Newly-arrived migrant peasants earn less than local labors. However, after a certain period, the skilled migrant workers catch up with local labors and are economically integrated into the local labor market while the low-skilled migrant group still earns less. Therefore the integration of migrant labors in urban labor market is yet a kind of segregated integration.

CYP2C19 polymorphism has no influence on rabeprazole-based hybrid therapy for Helicobacter pylori eradication

AIM To evaluate the impact of cytochrome P450 2C19(CYP2C19) and interleukin-1β(IL-1β) polymorphisms on the efficacy of Helicobacter pylori(H. pylori) eradication by using rabeprazole-based hybrid *** A total of 88 H. pylori-infected patients were recruited to receive 14-d of hybrid therapy from March 2013 to May 2014. Three patients were excluded from analysis because of incomplete compliance. Either a follow-up endoscopy or 13 C-urea test was performed to determine the results of H. pylori eradication therapy. The genotypes of CYP2C19 and IL-1β were analyzed to investigate the impact on treatment effect. RESULTS The total eradication rate of H. pylori was 92.94%(79/85). According to the CYP2C19 genotypes, the rates of H. pylori eradication were 89.19% in extensive metabolizers(EM) and 95.83% in non-EM. The *** eradication rates regarding the IL-1β genotypes were 92.59% in the normal acid secretion group and 93.10% in the low acid secretion group. After multivariable logistic regression analysis, both the genotypes of CYP2C19 and IL-1β had no significant influences on the eradication rates of H. *** The CYP2C19 and IL-1β polymorphisms are not significantly independent factors of *** eradication using rabeprazole-based hybrid therapy.

Objective To evaluate the efficacy and safety of simultaneous administration of dutasteride,tadalafil and solifenacin in the treatment of benign prostatic hyperplasia(BPH)with overactive bladder symptoms and lower urinary tract obstruction in previously unsuccessfully treated *** Patients in Group A(n=97)received dutasteride 0.5 mg/day,tadalafil 2.5 mg/day,and solifenacin 2.5 mg/day;Group B(n=95)received dutasteride 0.5 mg/day,tadalafil 5 mg/day,and solifenacin 5 mg/day;Group C(n=103)received dutasteride 0.5 mg/day,tadalafil 20 mg/day,and solifenacin 10 mg/*** functional status of the lower urinary tract was assessed using the International Prostate Symptom Score(I-PSS),Overactive Bladder Questionnaire(OABq),International Index of Erectile Function(IIEF),and Male Sexual Health Questionnaire Ejaculatory Dysfunction(MSHQ-EjD)as well as *** The total score of the sexual function remained unchanged in Group B of patients 81.3 points vs.80.2 points(p>0.05)according to MSHQ-EjD,61.4 points vs.51.2 points(p>0.05)according to IIEF *** total assessment of symptoms of hyperactivity significantly decreased in Group C according to OABq data after the 4th week of the study(17.5 points vs.26.1 points,p<0.05)and remained below the baseline until the end of the study(15.2 points).Conclusions The simultaneous administration of standard doses of dutasteride,solifenacin,and tadalafil for 3 months is safe,effective,and can be recommended for patients with BPH to reduce symptoms of obstruction and hyperactivity of the bladder and maintain sexual function.

Subgroup comparison of COVID-19 case and mortality with associated factors in Mississippi: findings from analysis of the first four months of public data

We compared subgroup differences in COVID-19 case and mortality and investigated factors associated with case and mortality rate(MR)measured at the county level in *** were based on data published by the Mississippi State department of Health between March 11 and July 16,*** COVID-19 case rate and case fatality rate(CFR)differed by gender and race,while MR only differed by *** aged 80 years or older and those who live in a non-metro area had a higher case rate,CFR,and *** controlling for selected factors,researchers found that the percent of residents who are obese,low income,or with certain chronic conditions were associated with the county COVID-19 case rate,CFR,and/or MR,though some were negatively *** findings may help the state to identify counties with higher COVID-19 case rate,CFR,and MR based on county demographics and the degree of its chronic conditions.

Psychological Outcomes in Disaster Responders: A Systematic Review and Meta-Analysis on the Effect of social Support

Disaster response work is associated with various psychological outcomes. In post-disaster conditions,social support is generally observed to impact mental health, particularly for survivors. This review was conducted to survey the extent of social support effectiveness on disaster responder groups. Published quantitative social support studies on police, emergency medical responders,rescue and recovery workers, firefighters, and military responders were searched in various academic databases using keyword searches, a reference list search, and a citation search that resulted in 24 studies with 90 effect sizes being included in the final data base. Articles were coded and effect sizes were averaged using the Hedges–Vevea Random Effects model. Nineteen categories of psychological outcomes(for example, anxiety, depression,posttraumatic stress symptoms, and psychological distress)and eight classifications of support were coded. social support was found to be associated with anxiety, burnout,depression, job control, job satisfaction, psychological distress, turnover intentions, and work engagement, with mean effect sizes from-0.36 to 0.57. Most studies measured perceived social support and negative *** support correlated with outcomes in police responders and rescue and recovery workers. This review discusses the breadth of effect of social support, as well as other elements, such as temporal factors, that may affect the effectiveness of social support in disaster responders.

Supporting farmer-managed irrigation systems in the Shigar valley, Karakorum： Role of the government and Aga Khan Rural Support Programme

Farmer-managed irrigation systems(FMIS) in the high altitude valleys of the Karakorum,Pakistan, continue to be managed effectively despite increased pressure on the social arrangements that sustain them. Colonial era records shows that over a century ago government agencies undertook irrigation support projects. In the past three decades,government agencies and the non-government agency Aga Khan Rural Support Programme(AKRSP), which channels foreign funds into the region, have actively engaged in the provision of irrigation support. This article seeks to explore whether such projects support or undermine farmer-managed irrigation systems and the complex institutional arrangements that underpin them. Field research using ethnographic and participatory methods was conducted in spring 2013 in the upper Shigar valley, Skardu district, GilgitBaltistan. The findings show that irrigation development is a political activity that involves village-based actors, religious leaders, local politicians,and government and non-government *** agencies operate in a largely top-down,engineering mode, their larger projects limited to villages suffering water scarcity. The local government provides small funds for renovation work of FMIS,though allocation of funds is highly politicized. Nongovernment agencies, for a variety of reasons including donor-funding cycles, apply a one-size-fitsall ‘participatory' model in an attempt to socially engineer rules and institutions. In communities divided by factionalism the use of such external models that stress formation of committees are unlikely to yield positive results, and could instead contribute to undermining the very systems they seek to support. This research argues that irrigation interventions should take care to build upon the rich and complex social arrangements that have sustained FMIS through the centuries.

Émile Durkheim’s Le Suicide(1897/2005)paved the way for the development of the sociology of ***,Durkheim’s positivistic approach in studying suicide was trenchantly challenged by,amongst others,Jack Douglas,who,in social Meanings of Suicide(1968),advocated extricating the social meanings of suicide from the interpretive tradition of social *** on Douglas’approach to the sociology of suicide,this paper reports and reflects on part of a study conducted across several rural areas of Jhenaidah,an area in Bangladesh highly affected by suicide,with 17 males who have attempted *** responses of the participants were interpreted with a view to understanding the social meanings associated with their suicide *** there was a diversity of individual experiences among the participants with regard to suicide attempts,the paper synthesises these and reflects on three themes which most closely represent the commonalities of experience of the 17 men,namely:(i)inability to provide;(ii)intimate relationship stress;and(iii)loss of social *** conclusion,the paper contributes to understanding suicide attempts by men as social meaningfully events,delineating the roles and complexities associated with constructions of masculine identities.

Assessing the Factors Influencing Migration Decision of Climate Refugees in Coastal Areas of Bangladesh

The major impacts of climate change play a substantial role in triggering human migration, especially in the coastal areas. The individual or combined effects of climate change are likely to trigger mass human movement both within and across international borders. People rarely move for a single reason;the motivation to migrate is complex of many factors. The main goal of this article is to identify the factors related to the decision to migrate taken by refugees in the coastal area. To assess this objective we employ exploratory factor analysis and structural equation modeling (SEM) and find that different factors influence refugees’ migration decision differently. From the findings, it is seen that loss of shelter, extreme events, decreasing soil fertility and food shortage, variability in temperature patterns and exhaustion of natural resources are the most important environmental factors that affect the decision to migrate of climate refugees. Low income, increasing price, decreasing purchasing power are the most important economic factors that influence migration decision. No social factors have significant effect on migration decision while safety as a political factor has a moderate influence on refugees’ decision to migrate. Finally, this article provides some recommendations for recognition of and protection for migrants forced to move to safer places due to certain direct impacts of climate change, notwithstanding the existence of multi-causality.
