Differentiated super-enhancers in lung cancer cells

Background:Whether minimally invasive esophagectomy(MIE)is superior to open esophagectomy(OE)in the treatment of esophageal squamous cell carcinoma(ESCC)is still ***,this multicenter prospective study aimed to compare MIE with OE in postoperative parameters and long-term ***:All hospitalized patients with cT1b-3N0-1M0 thoracic ESCC treated by MIE or OE were enrolled from 19 selected centers from April 1,2015 to December 31,*** propensity score matching(PSM)was performed to minimize the selection *** basic clinicopathological characteristics and 3-year overall survival(OS)as well as disease-free survival(DFS)of two groups were compared by R version ***:MIE were performed in 1,387 patients and OE in 335 patients.335 cases in each group were finally matched by PSM,and no significant differences in the essential demographic characteristics were observed be-tween the MIE and OE groups after *** with OE,MIE had significantly less intraoperative bleeding,less total drainage volume,shorter postoperative hospital stay,and harvested significantly more lymph nodes(LNs)(all P<0.001).There were no significant differences in the major postoperative complications and death rates between MIE and *** 3-year OS and DFS were 77.0%and 68.1%in the MIE group versus 69.3%and 60.9%in the OE group(OS:P=0.03;DFS:P=0.09),and the rates were 75.1%and 66.5%in the MIE group versus 66.9%and 58.6%in the OE group for stage cII patients(OS:P=0.04,DFS:P=0.09),***:Compared with OE,MIE is a safe and effective treatment approach with similar mortality and *** has the advantages in harvesting more LNs,improving postoperative recovery and survival of stage cII ESCC patients.

Segmental liver incarceration through a recurrent incisional lumbar hernia

BACKGROUND:Lumbar hernia is a rare congenital or acquired defect of the posterior abdominal wall. The acquired type is more common and occurs mainly as an incisional defect after flank surgery. Incarceration or strangulation of hernia contents is uncommon. METHOD:Segmental liver incarceration through a recurrent incisional lumbar defect was diagnosed in a 58 years old woman by magnetic resonance imaging. RESULTS:The patient underwent an open repair of the com-plicated hernia. An expanded polytetraflouoroethylene(e-PTFE) mesh was fashioned as a sublay prosthesis. She had an uncomplicated postoperative course. Follow-up examinations revealed no evidence of recurrence. CONCLUSIONS:Although lumbar hernia rarely results in incarceration or strangulation,early repair is necessary because of the risks of complications and the increasing difficulty in repairment as it enlarges. Surgical repair is often difficult and challenging.

Midterm outcome of stapled transanal rectal resection for obstructed defecation syndrome:A single-institution experience in China

AIM:To assess midterm results of stapled transanal rectal resection(STARR)for obstructed defecation syndrome(ODS)and predictive factors for ***:From May 2007 to May 2009,75 female patients underwent STARR and were included in the present *** and postoperative workup consisted of standardized interview and physical examination including proctoscopy,colonoscopy,anorectal manometry,and *** and functional results were assessed by standardized questionnaires for the assessment of constipation constipation scoring system(CSS),Longo’s ODS score,and symptom severity score(SSS),incontinence Wexner incontinence score(WS),quality of life Patient Assessment of Constipation-Quality of Life Questionnaire(PAC-QOL),and patient satisfaction visual analog scale(VAS).Data were collected prospectively at baseline,12 and 30 ***:The median follow-up was 30 mo(range,30-46 mo).Late postoperative complications occurred in 11(14.7%)*** of these patients required procedure-related reintervention(one diverticulectomy and two excision of staple granuloma).Although the recurrence rate was 10.7%,constipation scores(CSS,ODS score and SSS)significantly improved after STARR(P<0.0001).Significant reduction in ODS symptoms was matched by an improvement in the PAC-QOL and VAS(P<0.0001),and the satisfaction index was excellent in 25(33.3%)patients,good in 23(30.7%),fairly good in 14(18.7%),and poor in 13(17.3%).Nevertheless,the WS increased after STARR(P=0.0169).Incontinence was present or deteriorated in 8(10.7%)patients;6(8%)of whom were new *** analysis revealed that the occurrence of fecal incontinence(preoperative,postoperative or new-onset incontinence;P=0.028,0.000,and 0.007,respectively)was associated with the success of the ***:STARR is an acceptable procedure for the surgical correction of ***,its impact on symptomatic recurrence and postoperative incontinence may be problematic.

Hospital type- and volume-outcome relationships in esophageal cancer patients receiving non-surgical treatments

AIM: To study the "hospital type-outcome" and "volumeoutcome" relationships in patients with esophageal cancer who receive non-surgical ***: A total of 6106 patients with esophageal cancer diagnosed between 2008 and 2011 were identified from a national population-based cancer registry in Taiwan. The hospital types were defined as medical center and non-medical center. The threshold for high-volume hospitals was based on a median volume of 225 cases between 2008 and 2011(annual volume, > 56 cases) or an upper quartile(> 75%) volume of 377 cases(annual volume > 94 cases). Cox regression analyses were used to determine the effects of hospital type and volume outcome on patient ***: A total of 3955 non-surgically treated patients were included in the survival analysis. In the unadjusted analysis, the significant prognostic factors included c T, c N, c M stage, hospital type and hospital volume(annual volume, > 94 vs ≤ 94). The 1- and 3-year overall survival rates in the non-medical centers(36.2% and 13.2%, respectively) were significantly higher than those in the medical centers(33.5% and 11.3%, respectively; P = 0.027). The 1- and 3-year overall survival rates in hospitals with an annual volume of ≤ 94(35.3% and 12.6%, respectively) were significantly higher than those with an annual volume of > 94(31.1% and 9.4%, respectively; P = 0.001). However, in the multivariate analysis, the hospital type was not statistically significant. Only c T, c N, and c M stages and hospital volume(annual volume > 94 vs ≤94) were independent prognostic ***: Whether the treatment occurs in medicalcenters is not a significant prognostic factor. Highvolume hospitals were not associated with better survival rates compared with low-volume hospitals.

Berberine displays antitumor activity in esophageal cancer cells in vitro

To investigate the effects of berberine on esophageal cancer (EC) cells and its molecular mechanisms. METHODS Human esophageal squamous cell carcinoma cell line KYSE-70 and esophageal adenocarcinoma cell line SKGT4 were used. The effects of berberine on cell proliferation were evaluated using the 3-(4,5-dimethylthiazol-2-yl)2,5-diphenyltetrazolium bromide (MTT) assay. For cell cycle progression, KYSE-70 cells were stained with propidium iodide (PI) staining buffer (10 mg/mL PI and 100 mg/mL RNase A) for 30 min and cell cycle was analyzed using a BD FACSCalibur flow cytometer. For apoptosis assay, cells were stained with an Annexin V-FITC/PI apoptosis detection kit. The rate of apoptotic cells was analyzed using a dual laser flow cytometer and estimated using BD ModFit software. Levels of proteins related to cell cycle and apoptosis were examined by western blotting. RESULTS Berberine treatment resulted in growth inhibition of KYSE-70 and SKGT4 cells in a dose-dependent and time-dependent manner. KYSE-70 cells were more susceptible to the inhibitory activities of berberine than SKGT4 cells were. In KYSE-70 cells treated with 50 mu mol/L berberine for 48 h, the number of cells in G2/M phase (25.94% +/- 5.01%) was significantly higher than that in the control group (9.77% +/- 1.28%, P < 0.01), and berberine treatment resulted in p21 upregulation in KYSE-70 cells. Flow cytometric analyses showed that berberine significantly augmented the KYSE-70 apoptotic population at 12 and 24 h post-treatment, when compared with control cells (0.83% vs 43.78% at 12 h, P < 0.05; 0.15% vs 81.86% at 24 h, P < 0.01), and berberine-induced apoptotic effect was stronger at 24 h compared with 12 h. Western blotting showed that berberine inhibited the phosphorylation of Akt, mammalian target of rapamycin and p70S6K, and enhanced AMP-activated protein kinase phosphorylation in a sustained manner. FACTORS AFFECTING REMOVAL AND PROGNOSIS OF THYMIC TUMORS

One hundred and ten cases of thymic tumors were intervened surgically, including 92 thymoma, 8 thymic carcinoid, and 10 thymic carcinoma. In this series, 50. 9% of the cases were complieated with various syndromes, 44. 5 % with myasthenia gravis (MG). Resection rate was correlated with the size and invasion of the tumor. There was significant difference in resection rate among thymoma, thymic carcinoid and thymic carcinoma. The degree of invasion undoubtely influenced on resection. The 3 -,5- and 10- year suvival rate of the thymoma were 82. 7 %, 68. 1% and 40. 0 %, respectively. The prognosis depended on the pathological classification and the severity of the neighbouring invasion, but MG had no significant effect on prognosis. Recurrence and metastasis of the tumor were the main cause of late death.
