Spectrum of Meningitis in Adult at the Douala General Hospital

Objectives: Describe the clinical, aetiological and prognostic features of infectious meningitis in adults at the Douala General Hospital (DGH). Patients and Methods: We carried out a 5-year retrospective cross-sectional study at the DGH, on data from the registers of the bacteriology and biochemistry laboratories. Cases of meningitis were identified from the results of the cerebrospinal fluid (CSF) analysis, and the patient clinical file was obtained. Cases files of patients aged 15 years and above were included. For each patient, sociodemographic, clinical features and hospital mortality data were extracted. Results: During study period, 1877 CSF analyses were recorded and 135 were enrolled for data analysis. Up to 74 patients (55%) were male and the mean age was 40.04 ± 12.5 years. The time lapse between the onset of symptoms and consultation was 12.24 ± 11.16 days. The main clinical signs were meningeal syndrome (96.3%), neck stiffness (71.1%), reduced alertness (64.4%) and confusional states (55.6%). The main aetiology was bacterial (45.19%) withStreptococcus pneumoniae?(45.90%) and?Neisseria meningitis?(29.51%) leading. The other aetiologies were viral (21.48%), fungal with?Cryptococcus neoformans?(20%) and meningeal tuberculosis (13.33%). Fifty eight patients (42.9%) had a positive HIV serology. The mean duration of hospitalization was 9.05 ± 8.1 days, and the total in-hospital mortality was 25.18%. 117 patients (86.8%) had received antibiotherapy before diagnosis. Conclusion: The clinical features of meningitis in the DGH are as in classical description. However, the mortality is high and the major aetiologies appear to be associated with HIV infection at least in part. Prospective multi-centric studies are needed to provide more evidence for the development of staged management guidelines in our resource-limited settings.

Static and Dynamic Properties of Soil Food Web Structure in a Greenhouse Environment

Soil food web structure is fundamental to ecosystem process and function; most studies on soil food web structure have focused on agro-ecosystems under different management practices and natural terrestrial ecosystems,but seldom on greenhouses. This study explored the static and temporal variability of soil food structure in two greenhouses of Shandong Province,North China over a two-year period. The static properties were measured directly by surveying functional group composition and a series of parameters portraying the species properties,link properties,chain properties and omnivory properties of the web,as well as indirectly through calculation of nematode indices,enrichment index(EI),structure index(SI),and channel index(CI). The dynamic variability of greenhouse soil food structure was described by the dynamics of functional groups,Bray-Curtis(BC) similarity and cluster analysis. The results showed that the greenhouse soil food web contained 14 functional groups,with microbes having the highest mean biomass,followed by protozoa. Of the three functional groups of protozoa,flagellates were the dominant group on most sampling dates,amoebae only became the dominant group during the summer,while ciliates were the least prevalent group. All nematodes were assigned into one of the four functional groups,bacterivorous,fungivorous,herbivorous and omnivorous,and the fungivorous nematodes had the lowest mean biomass. Mites were assigned into three functional groups and the omnivorous noncryptostigmatic mites were the dominant group. All the functional groups showed significant seasonal changes. The soil food web connectance was 0.15,the maximum food chain length was 5,and the average food chain length was 3.6. The profiles of the EI and SI showed that the food web was resourcedepleted with minimal structure. The results of CI indicated that the bacterial decomposition pathway was the dominant pathway in the food web of the greenhouse soils studied and the results of BC

Daily Hazelnut Intake Exerts Multiple Reversible Effects on Plasma Profile of Healthy Subjects

Life sciences such as medical, nutritional, and those related to agrarian biotechnologies are experiencing a constant increase. Therefore, food consumption is today related to health state. Aiming to create an innovative spinneret system that defines the tight correlation between high nutritional and nutraceutical value of foods and their beneficial effects on consumer’s health state, we tested the effects related to the administration of 40 g of the Tonda Gentile Romana Italian hazelnut cultivar (Viterbo, Italy). To this end, we enrolled 24 subjects for a period of 14 weeks, based on a crossover design. Although several papers and reviews on hazelnut intake benefits are available, few studies have investigated the effects of hazelnut-enriched diet on plasma key parameters commonly used to ascertain the individual’s well-being status. The primary endpoint of the trial was to assess LDL change related to hazelnut administration, which resulted in significant beneficial effects on cholesterol parameters (LDL dropped from 133.09 mg/dl to 119.18 mg/dl). Interestingly, despite within the normal ranges, uric acid and serum creatinine mean values significantly decreased after hazelnut intake. A significant increment was also observed for ALT (from 30.9 U/L to 35.22 U/L, P = 0.01), while GGT significantly decreased from 38.04 mg/dl at T0 to 35.27 mg/dl at T1 (P = 0.001). No significant changes were also observed in azotemia, serum iron, glycemia, AST, total bilirubin, Hb, WBCs, RBCs, platelet count, and total plasma proteins. All the blood parameters analyzed appeared to be reversible after 6 weeks from the end of the treatment. Mann-Whitney U test did not show any significant differences in T0 - T1 Δ% LDL considering gender, age and baseline LDL. These findings may be of help in the development of strategies for personalized nutrition and diseases management.

Implications of Papyrus (<i>Cyperus papyrus</i>L.) Biomass Harvesting on Nutrient Regulation in Nyando Floodplain Wetland, Lake Victoria, Kenya

Harvesting of papyrus biomass plays a significant role in regulating ecosystem services of which one of them is nutrient uptake and cycling. Despite Lake Victoria’s wetlands being important, little is understood about its role in nutrient uptake and removal. Although there have been studies done in these wetlands, there is inadequate understanding on the implication of unselective biomass harvesting on the water quality of Lake Vitoria. At the same time, these wetlands are threatened by livelihood-related pressures which are driven by extreme hydrological regimes. This study focused on Nyando floodplain wetland located in the Eastern part of the shores of Lake Victoria which is a lifeline to many rural communities living around the lake. Papyrus biomass harvesting was assessed in two study sites of Nyando wetland and at different seasons in order to determine its implication on nutrient regulating services. Participatory tools, field survey, observation, field measurements and laboratory analysis were used. Harvesting of papyrus removed 530.6 Kg N/g DM/ha/day and 97 Kg P/g DM/ha/day in Ogenya while 771.2 Kg N/g DM/ha/day and 109.2 Kg P/g DM/ha/day were removed in Wasare. However, addition of nutrients to the wetland may lead to the problem of eutrophication especially at the site where interaction of wetland and lake water occurs. Understanding the role of papyrus harvesting patterns is crucial for better planning and management of this complex resource in a changing environment.

Geomorphometric Characterization of Upper South Koel Basin, Jharkhand: A Remote Sensing &GIS Approach

The quantitative analysis of drainage system is an important aspect of characterization of watersheds. Morphometry is measurement and mathematical analysis of landforms. The present study is an attempt to evaluate the drainage morphometrics of Upper South Koel Basin using Remote Sensing and GIS approach. A morphometric analysis was carried out to describe the topography and drainage characteristics of Upper South Koel watershed. The stream numbers, orders, lengths and other morphometric parameters like bifurcation ratio, drainage density, stream frequency, shape parameters etc. were measured. The drainage area of Upper South Koel watershed is 942.4 sq km and the drainage pattern is dentritic. The watershed was classified as 6th order drainage basin. The low values of bifurcation ratio and drainage density suggest that the area has not been much affected by structural disturbances. The study reveals that the different geomorphic units in the study area i.e. Structural hills, Pediments, Valley fills, Pediplains formed under the influence of permeable geology, are moderate to nearly level plains, with medium to low drainage density (studies can be of immense help in planning and management of river basins.

Nutrient Use Efficiency of Three Fast Growing Hardwood Species across a resource Gradient

Attitudes regarding traditional energy sources have shifted toward renewable resources. Specifically, short-rotation woody crop supply systems have become more prevalent for biomass and biofuel production. However, a number of factors such as environmental and inherent resource availability can limit tree production. Given the intensified demand for wood biomass production, forest and plantation management practices are focusing on increasing productivity. Fertilizer application, while generally one of the least expensive silvicultural tools, can become costly if application rates exceed nutrient uptake or demand of the trees especially if it does not result in additional biomass production. We investigated the effect of water and varying levels of nitrogen application (56, 112, and 224 kg·N·haǃ·yrǃ) on nutrient content, resorption efficiency and proficiency, N:P and the relationship with ANPP, as well as leaf- and canopy-level nutrient use efficiency of nitrogen, phosphorus, and potassium for Populus deltoides, Quercus pagoda, and Platanus occidentalis. P. deltoides and P. occidentalis reached their maximum nitrogen budget with the application of water suggesting old agricultural fields may have sufficient nutrient levels to sustain short-rotation woody crops negating the application of additional nitrogen for these two species. Additionally, for P. deltoides and Q. pagoda application of nitrogen appeared to increase the uptake of phosphorus however, resorption efficiency for these two species were more similar to studies conducted on nutrient poor sites. Nutrient resorption proficiency for all three nutrients and all three species were at levels below the highest rates of nitrogen application. These findings suggest maximum biomass production may not necessarily be tied to maximum nutrient application.
