New insights into the immunopathogenesis of systemic lupus erythematosus： the role of T follicular helper cells

Objective To review the development of T follicular helper （TFH） cells and their role in systemic lupus erythematosus （SLE） pathogenesis, the effect of dendritic cells （DCs） on TFH cells in SLE, as well as the potential use of TFH cells as a new therapeutic target in clinical practice. Data sources The data used in this review were retrieved mainly from the PubMed database （1989-2013）. The terms used in the literature search were ＂T follicular helper cells,＂＂systemic lupus erythematosus,＂ and ＂dendritic cells.＂ Study selection Relevant publications about the TFH cells development, the interaction between the TFH cells and the DCs, and the clinical applications of TFH cells were identified, retrieved, and reviewed. Results TFH cells, a novel distinct CD4＋ T cell subset, are specialized in providing help to B cells in the formation of germinal centers （GCs） and long-term protective humoral immune responses. The development of TFH cells from naive CD4＋ T cell is a multistep process. As the pivot of immunoregulation, DCs are indispensable for TFH cells generation. In addition to receptor-ligand interactions between TFH cells and DCs, the cytokines secreted by DCs are also necessary for TFH cell generation. TFH cell dysregulation has been implicated in the development of SLE. More evidence from animal models of SLE and SLE patients suggests that TFH cells are necessary for pathogenic autoantibody production. Therefore, therapeutically targeting TFH cells can be a promising approach to treat antibody-mediated autoimmune diseases including SLE. Conclusion TFH cells play a critical role in the pathogenesis of SLE, making them attractive therapeutic targets in clinical practice.

Background Paroxysmal kinesigenic dyskinesia （PKD） is characterized by recurrent brief episodes of chorea and dystonia induced by sudden movement. Whether the central nervous system is hyper- or hypoexcitable in PKD remains undetermined. The aim of our study was to compare the somatosensory evoked potential （SEP） recovery cycle, a marker of somatosensory system excitability, in PKD patients and controls.

Analysis on hidden blood loss of total knee arthroplasty in treating knee osteoarthritis

Background Total knee arthroplasty （TKA） is an important therapy for the treatment of various late-stage knee diseases. However, it has been observed that patients have lower hemoglobin （HB） counts postoperatively, which are significantly inconsistent with the measured blood loss. Although the concept of hidden blood loss has been presented in 2000, very little clinical attention has been paid since then. In this study, we investigated the characteristics and influential factor of hidden blood loss after TKA in treating knee osteoarthritis.

Metastases to the breast from extramammary neoplasms are very rare, constituting 2.7% of all malignant breast tumours. The most common primary tumor metastatic to the breast is primary breast cancer. Rectal cancer metastasizing to the breast is extremely rare. We report a case of aggressive rectal carcinoma with metastasis to the breast.

Progressive loss of striatal dopamine terminals in MPTP-induced acute parkinsonism in cynomolgus monkeys using vesicular monoamine transporter type 2 PET imaging([^(18)F]AV-133)

The 1-methyl-4-phenyl-1,2,3,4-tetrahydropyridine （MPTP）-induced parkinsonism model, particularly in non-human primates, remains the gold-standard for studying the pathogenesis and assessing novel therapies for Parkinson＇s disease. However, whether the loss of dopaminergic neurons in this model is progressive remains controversial, mostly due to the lack of objective in vivo assessment of changes in the integrity of these neurons. In the present study, parkinsonism was induced in cynomolgus monkeys by intravenous administration of MPTP （0.2 mg/kg） for 15 days; stable parkinsonism developed over 90 days, when the symptoms were stable. Noninvasive positron emission tomographic neuroimaging of vesicular monoamine transporter 2 with 9-[18F] fluoropropyl-（＋）-dihydrotetrabenazine （[18F]AV-133） was used before, and 15 and 90 days after the beginning of acute MPTP treatment. The imaging showed evident progressive loss of striatal uptake of [18SF]AV-133. The dopaminergic denervation severity had a significant linear correlation with the clinical rating scores and the bradykinesia subscores. These findings demonstrated that [18F]AV-133 PET imaging is a useful tool to noninvasively evaluate the evolution of monoaminergic terminal loss in a monkey model of MPTP-induced parkinsonism.

Effects of Structural Changes in Subchondral Bone on Articular Cartilage in a Beagle Dog Model

Objective Using MR T2-mapping and histopathologic score for articular cartilage to evaluate the effect of structural changes in subchondral bone on articular cartilage. Methods Twenty-four male Beagle dogs were randomly divided into a subchondral bone defect group （n = 12） and a bone cement group （n = 12）. Models of subchondral bone defectin the medial tibial plateau and subchondral bone filled with bone cement were constructed. In all dogs, the left knee joint was used as the experimental sideand the right knee as the sham side. The T2 value for articular cartilage at the medial tibial plateau was measured at postoperative weeks 4, 8, 16, and 24. The articular cartilage specimens were stained with hematoxylin and eosin, and evaluated using the Mankin score. Results There was a statistically significant difference （P 〈 0.05） in Mankin score between the bone defect group and the cement group at postoperative weeks 16 and 24. There was a statistically significant difference in the T2 values between the bone defect group and its sham group （P 〈 0.05） from week 8, and between the cement group and its sham group （P 〈 0.05） from week 16. There was significant difference in T2 values between the two experimental groups at postoperative week 24 （P 〈 0.01）. The T2 value for articular cartilage was positively correlated with the Mankin score （ρ = 0.758, P 〈 0.01）. Conclusion Structural changes in subchondral bone can lead to degeneration of the adjacent articular cartilage. Defects in subchondral bone cause more severe degeneration of cartilage than subchondral bone filled with cement. The T2 value for articular cartilage increases with the extent of degeneration. MR T2-mapping images and the T2 value for articular cartilage can indicate earlycartilage degeneration.

The Relevance between Symptoms and Magnetic Resonance Imaging Analysis of the Hippocampus of Depressed Patients Given Electro-acupuncture Combined with Fluoxetine Intervention - A Randomized,Controlled Trial

Objective： To probe the relevance between depressive symptoms and hippocampal volume and its metabolites detected by magnetic resonance imaging （MRI） in depressed patients who were given electro- acupuncture （EA） combined with Fluoxetine before and after treatment. Methods： A randomized, controlled trial was conducted. A total of 75 cases of mild or moderate depression were randomly assigned to two groups： the EA group which received EA combined with Fluoxetine; the Fluoxetine group which received Fluoxetine only as the control. The 17-item Hamilton Scale for Depression （HAMD） was used to assess the depression level. The relevance between the changes of the hippocampal volume and its metabolites, including N-acetyl aspartate （NAA）/creatine （Cr） and choline containing compounds （Cho）/Cr, and the reduction rate of the HAMD score before and after treatment of the two groups were analyzed. Results： At the end of the treatment, the therapeutic response rates were not statistically different between the two groups （73.53% for the Fluoxetine group and 83.33% for the EA group, respectively）. Compared to that of the Fluoxetine group, a significant difference was shown in the EA group in the reduction rate of the HAMD scores （P〈0.05）. There was a negative correlation between the therapeutic effect and the HAMD scores before treatment in both groups of patients. There was no significant difference in the hippocampal volume before and after treatment. The NAA/Cr ratio of both groups increased after treatment, with the EA group increasing more. There was a negative correlation between the rate of change of the NAA/Cr after treatment and the HAMD scores before treatment in the two groups. In the Fluoxetine group, the Cho/Cr ratio showed no significant difference before and after treatment, which had no relevance with the HAMD scores before treatment either. Association between clinical features and prognosis of patients with limb-shaking transient ischemic attack

Objective This review aims to illustrate the relationship between clinical features and the prognosis of patients with limb-shaking transient ischemic attack （LS-TIA）. Data sources Relevant articles published in two main Chinese medical periodical databases （China National Knowledge Infrastructure and China Science Periodical Database） from 1986 to June 2013 were identified with keywords ＂limb shaking＂ and ＂transient ischemic attack＂. Study selection Original articles and case reports about LS-TIA were selected. Results A total of 63 cases collected from 19 articles were included in the pooled analysis. Ls-rlA presented in two cerebrovascular diseases, of which atherosclerotic high-grade stenosis or occlusion in carotid artery system and moyamoya disease formed 95.2% and 4.8%, respectively. Of 63 patients, 11 （17.5%） were once misdiagnosed as epileptic and prescribed useless antiepilepsy drugs. The multivariable Logistic regression model showed a significant protective effect of patients with revascularization therapy on prognosis, compared with patients treated with drugs （odds ratio 0.20, 95% CI 0.05-0.74, P=0.016）. Conclusions Chronic carotid artery system hypoperfusion can induce limb（s） shaking, followed by high possibility of ischemic stroke in the same brain territorial. Revascularization of the responsible artery may work better than conservative drug-based therapy.
