Radiographic characteristics of neuroendocrine liver metastases do not predict clinical outcomes following liver resection

Background: Previous research has demonstrated that specific radiographic criteria, including the presence of calcifications and the enhancement pattern on computed tomography (CT) imaging, correlates with clinicopathologic features and outcomes of patients with gastroenteropancreatic neuroendocrine tumors (NET). We sought to investigate whether these radiographic characteristics were prognostic among patients with neuroendocrine liver metastases (NELM) undergoing surgical resection. Methods: The preoperative contrast-enhanced CT scans of all patients who underwent resection of NELM at a single institution between 2000-2015 were retrospectively reviewed. The presence of calcifications was determined on non-contrast phase imaging. Enhancement on the arterial phase scan was categorized as hyperenhancing, hypoenhancing, or mixed. Relevant clinicopathologic characteristics as well as recurrence-free survival (RFS) and overall survival (OS) were compared between groups. Results: Among 82 patients who underwent resection of NELM, 57 had available data on calcifications while 51 had data available on arterial enhancement patterns. Among all patients, median age was 58 (IQR:47-63) and the majority were female (N=48, 59.5%). The most common primary tumor locations were pancreas (N=25, 30.5%) and small bowel (N=27, 32.9%). The most commonly performed operations were right hepatectomy (N=29, 35.4%), bisegmentectomy (N=15, 18.3%), and segmentectomy (N=14, 17.1%). Median tumor number was 4 (IQR: 2-9), median Ki-67 was 5% (IQR: 2-10%), and median size of the largest liver metastasis was 4.5 (IQR: 2.8-7.7) cm. Twelve (21%) patients had tumor calcifications. Among patients with and without calcifications there were no differences in demographics, clinicopathologic characteristics, RFS (P=0.772) or OS (P=0.095). Purpose: Craniospinal axis irradiation (CSI) is a method of treating various central nervous system malignancies. The large target volume typically includes entire spinal cord and whole brain. Dosimetric comparison was performed between tomotherapy, volumetric modulated arc therapy (VMAT), and 3D conformal radiation therapy (3D-CRT) for CSI. Methods and Materials: Five (n = 5) CSI patients were planned using 3D-CRT, VMAT, and tomotherapy (normalized such that 95% of PTV received at least 23.4 Gy in 13 fractions). Plans were compared using PTV conformity number (CN) and homogeneity index (HI), normal tissue (NT) dose statistics, integral dose, and treatment time. Results: On average, tomotherapy plans showed higher CN (0.932 vs. 0.860 and 0.672 for SmartArc and 3D-CRT). In terms of HI, VMAT plans consistently showed better dose homogeneity (1.07 vs. 1.15 and 1.13 for tomotherapy and 3D-CRT). SmartArc delivered lower maximum dose for majority of NT, but higher mean dose. 3D-CRT plans delivered higher maximum dose but lower mean dose to NT. Conclusions: SmartArc treatments achieved better PTV homogeneity and reduced maximum dose to NT. Tomotherapy showed better target conformity, but 3D-CRT was shown to reduce mean dose to NT. Integral doses were similar between treatment modalities, but tomotherapy treatment times were much longer.

BACKGROUND There are several surgical options for treating early gastric cancers(EGCs), such as endoscopic resection, laparoscopic or open gastrectomy with D1 or D2 lymphadenectomy. Endoscopic resection for EGC with low risk of lymph node metastasis has been widely accepted as a therapeutic alternative. The role of endoscopic submucosal dissection(ESD) in treating EGC is not well established,especially when compared with resection surgery cases in a long-term follow-up *** To compare the safety and efficacy of the short-and long-term outcomes between ESD and resection *** We searched the databases of PubMed, EMBASE, Web of Science, and the Cochrane Library from January 1990 to June 2018, enrolling studies reporting short-or long-term outcomes of ESD in comparison with resection surgery for EGC. The quality of the studies was assessed by the Newcastle-Ottawa Quality Assessment Scale. Stata software(version 12.0) was used for the analysis. Pooling analysis was conducted using either fixed-or random-effects models depending on heterogeneity across *** Fourteen studies comprising 5112 patients were eligible for analysis(2402 for EGC and 2710 for radical surgery). Our meta-analysis demonstrated that the ESD approach showed advantages through decreased operation time [weighted mean difference(WMD):-140.02 min, 95%CI:-254.23 to-34.82 min, P = 0.009], shorter hospital stay(WMD:-5.41 d, 95% CI:-5.93 to-4.89 d, P < 0.001), and lowerpostoperative complication rate [Odds ratio(OR) = 0.39, 95%CI: 0.28-0.55, P <0.001).
