Hand,foot and mouth disease(HFMD)is a major public health problem among children in the Asia-Pacific *** optimal specimen for HFMD virological diagnosis remains *** A71(EV-A71)neutralizing antibody titres detected in paired sera were considered the reference standard for calculating the sensitivity,specificity,positive and negative predictive value of throat swabs,rectal swabs,stool,blood samples and cerebrospinal fluid(CSF)by RT-PCR or ELISA *** this study,clinical samples from 276 HFMD patients were collected for analysing the sensitivity of different kind of *** results showed that stool had the highest sensitivity(88%,95%CI:74%–96%)and agreement with the reference standard(91%).The order of diagnostic yield for EV-A71 infection was stool samplerectal swab>throat swab>blood sample>CSF sample,and using a combination of clinical samples improved sensitivity for enterovirus *** sensitivity of ELISA for IgM antibody detection in sterile-site specimens was significantly higher than that of RT-PCR(serum/plasma:62%vs.2%,CSF:47%vs.0%)(P<0.002).In conclusion,our results suggest that stool has the highest diagnostic yield for EV-A71-infected *** stool is unavailable,rectal swabs can be collected to achieve a similar diagnostic ***,throat swabs may be useful in detecting positive *** IgM in blood or CSF is diagnostically accurate,it lacks sensitivity,missing 40%–50%of *** higher proportion of severe cases and shorter interval between onset and sampling contributed to the increase in congruency between clinical testing and the serological reference standard.

Climate change and its impact on population health have been well explored in China in recent years,especially health risk assessments such as studying its impact on the transmission and development of climate-sensitive infectious diseases and non-communicable ***,knowledge gaps including using morbidity as health indicators(***,ED visits,and ambulance call-outs),identifying disease attributable contributions to climate variability and burden of diseases due to climate change,calculating related healthcare costs,and developing climate change adaptation and mitigation strategies need to be *** research directions could focus on both mitigation and health adaptation studies,such as exploring active transportation and green hospitals,studying health co-benefits,identifying vulnerable populations,prioritizing resource allocation,building healthcare capacity and capability,evaluating intervention effect,developing tailored risk communication strategies and community health education packages for vulnerable communities in the context of climate change.

Urinary levels of nickel and chromium associated with dental restoration by nickel-chromium based alloys

This paper aims to investigate if the dental restoration of nickel-chromium based alloy(Ni-Cr) leads to the enhanced excretions of Ni and Cr in *** hundred and ninety-five patients in a dental hospital had single or multiple Ni-Cr alloy restoration recently and 198 controls were recruited to collect information on dental restoration by questionnaire and clinical *** concentrations of Ni and Cr from each subject were measure by graphite furnace atomic absorption *** to the control group,the urinary level of Ni was significantly higher in the patient group of < 1 month of the restoration duration,among which higher Ni excretions were found in those with either a higher number of teeth replaced by dental alloys or a higher index of metal crown not covered with the *** levels of Cr were significantly higher in the three patient groups of <1,1 to <3 and 3 to <6 months,especially in those with a higher metal crown exposure *** curve estimations showed better relationships between urinary Ni and Cr in patients within 6-month *** data suggested significant increased excretions of urinary Ni and Cr after dental *** short- and long-term effects of Ni-Cr alloy restoration need to be investigated.

HIV-specific chimeric antigen receptor(CAR)T-cells have been developed to target HIV-1 infected CD4t T-cells that express HIV Env ***,T cell exhaustion and the patient-specific autologous paradigm of CAR-T cell hurdled clinical ***,we created HIV-specific CAR-T cells using human peripheral blood mononuclear cells and a 3BNC117-E27(3BE)CAR construct that enabled the expression of programmed cell death protein(PD-1)-blocking scFv E27 and the single-chain variable fragment of the HIV-1-specific broadly neutralizing antibody 3BNC117 to target native HIV *** with T cells expressing 3BNC117-CAR alone,3BE CAR-T cells showed greater cytotoxic activity against HIV Envt cells with stronger proliferation capability,higher killing efficiency,and enhanced cytokine secretion in the presence of HIV Env-expressing ***,we manufactured TCR-deficient 3BE CAR-T cells through gene editing and demonstrated that these CAR-T cells could effectively kill HIV Env^(+) cells in vivo without the occurrence of severe graft-versus-host disease(GvHD)in NSG *** data suggest that we have provided a feasible approach to the generation of“off-theshelf”anti-HIV CAR-T cells in combination with PD-1 checkpoint blockade immunotherapy,which can be a powerful therapeutic candidate for the functional cure of HIV.

Polychlorinated biphenyls and omega-3 fatty acid exposure from fish consumption,and thyroid cancer among New York anglers

Fish from the Great Lakes contain polychlorinated biphenyls（PCBs）which have been shown to disrupt endocrine function and mimic thyroid hormones,but they also contain beneficial omega-3 fatty acids that may offer protection against endocrine *** purpose of this study was to examine the effects of Lake Ontario fish consumption and the estimated consumption of PCBs and omega-3 fatty acids on the risk of thyroid cancer in a group of sport *** from the New York State Angler Cohort Study were followed for cancer incidence from 1991–***-seven cases of incident thyroid cancer and 108 controls were included in the *** estimated fish consumption,estimated omega-3 fatty acid consumption,and estimated PCB consumption from Lake Ontario fish were examined for an association with the incidence of thyroid cancer,while matching on sex,and controlling for age and smoking *** from logistic regression indicate no significant associations between fish consumption,short-term estimated omega-3 fatty acids,or estimated PCB consumption from Great Lakes fish and the development of thyroid cancer,but it was suggested that long-term omega-3 fatty acid from Great Lakes fish may be protective of the development of thyroid *** conclusion,fish consumption,with the possible concomitant PCBs,from the Great Lakes does not appear to increase the risk of thyroid cancer in New York *** research is needed in order to separate the individual health effects of PCBs from omega-3 fatty acids contained within the fish.

Knowledge, attitudes and practice survey on Zika virus infection among pregnant women in Brunei Darussalam

To assess the knowledge, attitudes, and practices(KAP) on Zika virus infection among pregnant women in Brunei Darussalam by a cross-sectional survey. Methods: Between February and June 2017, we recruited 234 pregnant women from all government healthcare centres at Brunei-Muara district, using a modified systematic sampling approach. A pre-tested and self-administered questionnaire was used and data analysis was conducted using descriptive statistics and multiple logistic regression analyses. Results: The study participants were mainly Malay(87.2%) and their mean age was 28.0 years. The median knowledge score was 13, out of a possible score of 28. Most participants(92.7%) knew that Zika virus was transmitted by mosquito bites whereas some(34.6%) knew that sexual transmission was also possible. Media(radio, television or newspapers) was the preferred source of updated information on Zika virus, followed by healthcare workers(44.0%), government announcements(43.2%), and social media(38.0%). Pregnant women who were 25 years old or older [Adj. OR=3.62(95% CI: 1.57, 9.51)], not Malays [Adj. OR=3.32(95% CI: 1.35, 8.55)], and had an average monthly household income of more than BND $3 000 [Adj. OR=4.06(95% CI: 1.81, 19.44)] were more likely to score higher for knowledge on Zika virus. The median prevention practice score was 23, out of a possible score of 36. Most participants reported wearing covering clothes(98.3%) and kept their living surroundings clean(99.6%). Most participants(88.0%) agreed that Zika is an important issue in their community. Conclusion: We found a lack of knowledge on Zika virus infection among pregnant women attending government maternal and child healthcare centres in Brunei Darussalam, in particular that Zika virus can be sexually transmitted. Background:Tuberculosis infection still places a great burden on HIV-infected individuals in China and other developing *** of the survival of HIV-infected patients with pulmonary tuberculosis(PTB)would provide important insights for the clinical management of this population,which remains to be well described in current ***:HIV-infected patients with PTB admitted to Shanghai public health Clinical Center from January 2011 to December 2015 were retrospectively *** this cohort,the survival prognosis was estimated by the Kaplan-Meier method,while univariate and multivariate Cox proportional hazards models were used to determine the risk factors affecting ***:After reviewing 4914 admitted patients with HIV infection,359 PTB cases were *** the time of PTB diagnosis,the patients’median CD4+T cell count was 51/mm3(IQR:23-116),and 27.30%of patients(98/359)were on combination antiretroviral therapy(cART).For the 333 cases included in the survival analysis,the overall mortality was 15.92%(53/333)during a median 27-month *** risk factors,including age older than 60 years(HR:3.18;95%CI:1.66-6.10),complication with bacterial pneumonia(HR:2.64;95%CI:1.30-5.35),diagnosis delay(HR:2.60;95%CI:1.42-4.78),CD4+T cell count less than 50/mm3(HR:2.38;95%CI:1.27-4.43)and pulmonary atelectasis(HR:2.20;95%CI:1.05-4.60),might independently contribute to poor *** patients without cART before anti-TB treatment,the later initiation of cART(more than 8 weeks after starting anti-TB treatment)was found to increase the mortality rate(OR:4.33;95%CI:1.22-15.36),while the initiation of cART within 4-8 weeks after starting anti-TB treatment was associated with the fewest deaths(0/14).Conclusions:The subjects in this study conducted in the cART era were still characterized by depressed immunological competence and low rates of cART administration,revealing possible intervention targets for preventing TB reactivation in HIV-in
