Characteristics of implicit schemas in patients with major depressive disorder

Background Many psychotherapy theories emphasise the importance of self-schema and other-schema,but most previous studies focused on the explicit self-schema in major depressive disorder(MDD).However,the limited studies of implicit self-schema in MDD have shown inconsistencies in their findings.Furthermore,only a few studies have investigated the implicit other-schema,and the pathway illustrating how implicit schemas influence depression remains unclear.Aims The primary aim of our study was to explore the characteristics of implicit self-schema and otherschema in patients with MDD.We also examine the chain-mediating effect of attachment relationships and interpersonal trust.Methods The present study included 88 patients with MDD and 88 healthy controls(HCs).The Hamilton Depression Rating Scale-17,Experiences in Close Relationships Inventory—Revised Questionnaire,Trust Scale and the Extrinsic Affective Simon Task(EAST)were used to assess depressive symptoms,attachment relationships,interpersonal trust and implicit schemas,respectively.Paired sample t-test was used to compare the reaction time(RT)for positive and negative words within the two groups.Analysis of covariance was used to explore the difference between two groups from the perspective of implicit schemas and interpersonal patterns.The chain mediation model was verified by bootstrap.Results(1)For interpersonal patterns,patients with MDD scored significantly higher on attachment anxiety(F=82.150,p<0.001)and attachment avoidance(F=23.192,p<0.001)and scored significantly lower on the predictability(F=30.297,p<0.001),dependence(F=39.728,p<0.001)and faith(F=60.997,p<0.001)dimensions of interpersonal trust.(2)As for implicit schemas,no significant difference was found between the RT for positive self-words and negative self-words in patients with MDD(t=−1.056,p=0.294).However,the HC responded faster to positive self-words than negative self-words(t=−3.286,p=0.001).The RT for positive other-words and negative other-words were significantly different in both pati

Background Avoidant paruresis is a common clinical condition in urology and psychosomatic medicine.However,it has limited treatment options that are safe and effective with few side effects.Aims Our study aimed to investigate the effectiveness and safety of the Chinese herbal Yangxin Tongquan decoction combined with cognitive-behavioural therapy(CBT)for avoidant paruresis.Methods Sixty-eight patients with avoidant paruresis were divided into a treatment group(33 patients)and a control group(35 patients).The control group was assigned 10 weeks of CBT and systematic desensitisation.In addition to CBT and systematic desensitisation,the treatment group was given the Chinese herbal Yangxin Tongquan decoction during the 10-week study.The Shy Bladder Syndrome Scale(SBS)and the Self-rating Anxiety Scale(SAS)were administered before and after treatment to measure any change.Results The overall efficacy in the treatment group(n=30)was 80.0%vs 62.5%in the control group(n=33).Comparing pretreatment and post-treatment measures,both groups showed improvement in SBS scores and SAS scores(treatment group:t_((SBS))=8.397,p_((SBS))<0.001,t_((SAS))=8.216,p_((SAS))<0.001;control group:t_((SBS))=6.802,p_((SBS))<0.001,t_((SAS))=5.171,p_((SAS))<0.001).Moreover,both groups’SBS and SAS scores changed significantly over time(SBS scores:F_(time)=118.299,p<0.001;SAS scores:F_(time)=92.114,p<0.001).However,the treatment group performed better than the control group(SBS scores:F_(time*group)=5.709,p=0.020;SAS scores:F_(time*group)=7.235,p=0.009).Conclusions The Chinese herbal Yangxin Tongquan decoction combined with cognitive-behavioural psychotherapy positively affects the treatment of avoidant paruresis without significant adverse effects.

Post-traumatic stress disorder status in a rescue group after the Wenchuan earthquake relief

Previous studies have suggested that the incidence of post-traumatic stress disorder in earthquake rescue workers is relatively high. Risk factors for this disorder include demographic characteristics, earthquake-related high-risk factors, risk factors in the rescue process, personality, social support and coping style. This study examined the current status of a unit of 1 040 rescue workers who participated in earthquake relief for the Wenchuan earthquake that occurred on May 12th, 2008. Post-traumatic stress disorder was diagnosed primarily using the Clinician-Administered Post-traumatic Stress Disorder Scale during structured interviews. Univariate and multivariate sta-tistical analyses were used to examine major risk factors that contributed to the incidence of post-traumatic stress disorder. Results revealed that the incidence of this disorder in the rescue group was 5.96%. The impact factors in univariate analysis included death of family members, contact with corpses or witnessing of the deceased or seriously injured, near-death experience, severe injury or mental trauma in the rescue process and working at the epicenter of the earthquake. Correlation analysis suggested that post-traumatic stress disorder was positively correlated with psychotic and neurotic personalities, negative coping and low social support. Impact factors in mul-tivariate logistic regression analysis included near-death experience, severe injury or mental trauma, working in the epicenter of the rescue, neurotic personality, negative coping and low social support, among which low social support had the largest odds ratio of 20.42. Findings showed that the oc-currence of post-traumatic stress disorder was the result of the interaction of multiple factors.

Computerized cognitive remediation therapy on cognitive impairment and social function in patients with chronic schizophrenia

BACKGROUND Patients with schizophrenia may have various disease manifestations,most of which gradually tend toward incurable chronic decline,leading to mental disability.The basic symptoms of the disease can impair social function,whereas long-term hospitalization produces hospitalization syndrome,causing serious damage to social function.AIM To investigate the effects of Computerized Cognitive Remediation Therapy(CCRT)on cognitive and social functioning in patients with chronic schizophrenia.METHODS A retrospective analysis of 120 patients with chronic schizophrenia in Shanghai Pudong New Area Mental Health Center was performed.They were divided into an intervention group(60 cases treated with CCRT combined with conventional medication)and a control group(60 cases treated with conventional medication).After treatment,effects on cognitive function and social roles were observed in both groups.The Positive and Negative Syndrome Scale(PANSS)was used to assess the patients'psychiatric symptoms.The Wisconsin Card Sorting Test(WCST)was used to assess the patients'cognitive functioning,and the Social Functioning Scale for Psychiatric Inpatients(SSPI)was used to assess the social functioning of the inpatient psychiatric patients.RESULTS No significant differences were observed in the PANSS,WCST,and SSPI intergroup scores before treatment(P>0.05).After 2,4,and 6 wk of therapy,general psychopathological factors,positive symptoms,negative symptoms,and total PANSS scores of PANSS in the intervention group were lower than in the control group(P<0.05).After 2,4,and 6 wk of treatment,the number of false responses,number of persistent bugs,and total responses in the WCST were significantly lower in the intervention group than in the control group(P<0.05),and the amount of completed classification was significantly higher than in the control group(P<0.05).After 2,4,and 6 wk of therapy,the SSPI scores were significantly greater than those of the controls(P<0.05).After 6 wk of treatment,the efficacy rates of the control and interv

Antidepressants for bipolar disorder A meta-analysis of randomized, double-blind, controlled trials

OBJECTIVE： To examine the efficacy and safety of short-term and long-term use of antidepres- sants in the treatment of bipolar disorder. DATA SOURCES： A literature search of randomized, double-blind, controlled trials published until December 2012 was performed using the PubMed, ISI Web of Science, Medline and Cochrane Central Register of Controlled Trials databases. The keywords ＂bipolar disorder, bipolar I disorder, bipolar II disorder, bipolar mania, bipolar depression, cyclothymia, mixed mania and depression, rapid cycling and bipolar disorder＂, AND ＂antidepressant agent, antidepressive agents second- generation, antidepressive agents tricyclic, monoamine oxidase inhibitor, noradrenaline uptake in- hibitor, serotonin uptake inhibitor, and tricyclic antidepressant agent＂ were used. The studies that were listed in the reference list of the published papers but were not retrieved in the above-mentioned databases were supplemented. STUDY SELECTION： Studies selected were double-blind randomized controlled trials assessing the efficacy and safety of antidepressants in patients with bipolar disorder. All participants were aged 18 years or older, and were diagnosed as having primary bipolar disorder. Antidepressants or antidepressants combined with mood stabilizers were used in experimental interventions. Placebos, mood stabilizers, antipsychotics and other antide pressants were used in the control interventions. Studies that were quasi-randomized studies, or used antidepressants in combination with antipsy- chotics in the experimental group were excluded. All analyses were conducted using Review Man- ager 5.1 provided by the Cochrane Collaboration.

Repetitive transcranial magnetic stimulation for hallucination in schizophrenia spectrum disorders A meta-analysis

OBJECTIVE： This study assessed the efficacy and tolerability of repetitive transcranial magnetic stimulation for treatment of auditory hallucination of patients with schizophrenia spectrum disorders. DATA SOURCES： Online literature retrieval was conducted using PubMed, ISI Web of Science, EMBASE, Medline and Cochrane Central Register of Controlled Trials databases from January 1985 to May 2012. Key words were ＂transcranial magnetic stimulation＂, ＂TMS＂, ＂repetitive tran- scranial magnetic stimulation＂, and ＂hallucination＂. STUDY SELECTION： Selected studies were randomized controlled trials assessing therapeutic ef- ficacy of repetitive transcranial magnetic stimulation for hallucination in patients with schizophrenia spectrum disorders. Experimental intervention was low-frequency repetitive transcranial magnetic stimulation in left temporoparietal cortex for treatment of auditory hallucination in schizophrenia spectrum disorders. Control groups received sham stimulation. MAIN OUTCOME MEASURES： The primary outcome was total scores of Auditory Hallucinations Rating Scale, Auditory Hallucination Subscale of Psychotic Symptom Rating Scale, Positive and Negative Symptom Scale-Auditory Hallucination item, and Hallucination Change Scale. Secondary outcomes included response rate, global mental state, adverse effects and cognitive function. RESULTS： Seventeen studies addressing repetitive transcranial magnetic stimulation for treatment of schizophrenia spectrum disorders were screened, with controls receiving sham stimulation. All data were completely effective, involving 398 patients. Overall mean weighted effect size for repeti- tive transcranial magnetic stimulation versus sham stimulation was statistically significant （MD = -0.42, 95%C/： -0.64 to -0.20, P = 0.000 2）. Attribution retraining group therapy for outpatients with major depression disorder,generalized anxiety disorder,and obsessive-compulsive disorder:a pilot study

The aim of this present study is to examine the efficacy of attribution retraining group therapy （ARGT） and to compare the responses of outpatients with major depression disorder （MDD）, generalized anxiety disorder （GAD） and obsessive-compulsive disorder （OCD）. We carried out a prospective uncontrolled intervention study with a 8-weeks of ARGT on sixty three outpatients with MDD, GAD or OCD. Hamilton rating scale for depression, Hamilton rating scale for anxiety, Yale-Brown obsessive-compulsive scale, attribution style questionnaire, self-esteem scale, index of well-being, and social disability screening schedule were administered before and after treatment. Significant improvement in symptoms and psychological and social functions from pre- to posttreatment occurred for all participants. The changes favored MDD patients. Our study suggested that ARGT may improve the symptoms and psychological-social functions of MDD, GAD, and OCD patients. MDD patients showed the best response.

INTRODUCTION While the biological approach remained at the forefront in Russian/Soviet psychiatry,several valuable theoretical approaches were developed in Russian/Soviet psychology.This development,however,did not cross the border of the psychiatric domain.The clinical implications of these psychological constructs for the psychosocial treatment of severe mental illness (SMI) were overlooked.

Late at night on 12 August2015 at Tianjin Port,there was an explosion of chemicals equivalent to more than 20 tons of TNT explosives.Even people living200 km away were jarred awake.Wide attention was given to the people that suffered,including victims,relatives of the martyrs,residents in the disaster area,the injured,and people who may be contaminated by the chemical explosion.

Structural brain volume differences between cognitively intact ApoE4 carriers and non-carriers across the lifespan

Apolipoprotein E4（ApoE4） is a prominent genetic risk factor for Alzheimer＇s disease. The purpose of this review is to explore differences in structural brain volume detected by magnetic resonance imaging between cognitively intact ApoE4 carriers and non-carriers across the lifespan（i.e., older adults, middle-aged adults, young adults, children and adolescents, and neonates）. Consistent findings are found throughout various developmental stages. This area of research may elucidate the mechanisms by which ApoE4 influences risk of developing Alzheimer＇s disease. It could also inform potential treatment strategies and interventions for carriers of the ApoE4 allele.
