Safety and efficacy of sodium bicarbonate for treating metabolic acidosis in chronic kidney disease:A systematic review and metaanalysis

BACKGROUND Kidney dysfunction and reduced filtration capacity due to chronic kidney disease(CKD)lead to a shift in the body's acid-base balance,ultimately causing metabolic acidosis(MA).Sodium bicarbonate has been used as a supplement to alleviate the symptoms and reverse the acidosis,and it may even slow the progression of ***,its safety profile and overall effectiveness are *** To conduct a systematic review and meta-analysis of clinical trials assessing sodium bicarbonate's safety and efficacy for treating CKD-induced *** Medline,Scopus,EMBASE,and Cochrane Central were systematically searched from inception until May 2024 to select all relevant randomized control trials(RCTs)and non-RCT(NRCTs)evaluating the effectiveness of sodium bicarbonate in correcting MA in end-stage renal disease *** addition,***,***,and Google Scholar were searched for other literature.A random-effects meta-analysis was performed to derive mean differences(MD)and risk ratios(RR)with their 95%CI for continuous and dichotomous outcomes *** Following a systematic search of the databases,20 RCTs and 2 and NRCTs comprising 2932 patients were included in our *** results revealed that sodium bicarbonate significantly increased serum bicarbonate in CKD patients(MD:2.59,95%CI:0.95-4.22;P=0.02;I2=95%).However,there was a non-significant increase in estimated glomerular filtration rate(eGFR)in patients on sodium bicarbonate therapy(MD:0.93,95%CI:-1.88-3.75;P=0.52;I2=93%).Upon assessment of the safety profile of sodium bicarbonate,no significant association was found in the outcomes of death/prolonged hospitalization(RR:1.05,95%CI:0.84-1.32;P=0.66;I2=0%),or gastrointestinal disorders(RR:1.64,95%CI:0.35-7.66;P=0.53;I2=76%),or worsening edema(RR:1.26,95%CI:0.94-1.68;P=0.12;I2=37%)when compared to *** Sodium bicarbonate therapy may halt worsening kidney function by correcting serum bicarbonate levels and treatin

Effect of 5.25% Sodium Hypochlorite on Shear Bond Strength of Orthodontic Brackets Bonded with OBA-MCP

Introduction: Bracket debonding is a frequent issue that clinicians encounter, leading to increased chair time, lost revenue, and material usage. In addition to patient compliance with their diet recommendations, the preparation and conditioning of teeth for bonding significantly influence bond strength and consequently impact orthodontic treatment success and efficiency. Because of OBA-MCP’s (orthodontic bonding adhesive with modified calcium phosphate) decreased shear bond strength (SBS), the purpose of this study was to evaluate the effects of conditioning with 5.25% sodium hypochlorite (NaOCl) before etching in the bonding protocol. materials and Methods: 90 extracted teeth were divided into 3 groups to be bonded with orthodontic brackets with different bonding protocols: 1) Transbond XT with regular bonding protocol (etch + prime + adhesive);2) OBA-MCP with regular bonding protocol;and 3) OBA-MCP with NaOCl prior to acid etching in the regular bonding protocol. SBS (in Newtons) were measured using an MTS universal testing machine with a custom jig to apply a vertical force onto the bracket and ARI (adhesive remnant index) scores were recorded for each sample after de-bond to rate the amount of adhesive remaining. Results: The addition of NaOCl to the bonding protocol statistically significantly increased the SBS of OBA-MCP to comparable levels to Transbond XT. The ARI scores showed that when NaOCl was added, more adhesive remained. Conclusion: The addition of NaOCl to the bonding protocol can increase the SBS of adhesives with historically weaker bond strengths. However, the increased amount of adhesive remaining and the increased time spent during bonding must be considered. Further testing can be done in vivo to demonstrate the practicality of this new procedure.

Maxillofacial Prosthesis for Partial Edentulism with Maxillary Bone Loss

Maxillary bone loss impairs essential functions (chewing, swallowing, speech) and gives patients a very unaesthetic appearance due to the removal of facial support tissues, leading to serious psychological consequences. Treatment is multidisciplinary and requires a resective surgery if the cause is tumor-related, or an additive surgery if the cause is traumatic. This article aims to show the role of making a prosthesis to restore function (chewing, swallowing, speech) and aesthetics following maxillary bone loss. We will eighter present a clinical case involving a right maxillary tumor that was surgically resected followed by radiotherapy, and subsequently rehabilitated with a maxillofacial prosthesis in the consultation and dental treatment center of the university hospital center of Casablanca.

Immunotherapy offers significant potential but is often hampered by the immunosuppressive environment in oral squamous cell carcinoma(OSCC).To address this,we propose an enhanced immunotherapeutic strategy that revitalizes the tumor immune microenvironment(TIME)in OSCC by integrating upconversion-based photodynamic therapy(PDT)with *** a red blood cell membraneinspired biomimetic nanoplatform,our approach concurrently delivers chlorin e6@upconversion nanoparticles(Ce6@UCNP)and doxorubicin(DOX).By leveraging fluorescence resonance energy transfer(FRET)for 980 nm to 660 nm upconversion excitation,we address challenges such as limited tissue penetration and tissue damage,as well as nanoplatform issues including immunogenicity and targeting inaccuracy Our integrated approach enhances PDT and chemotherapy with the goal of transforming immunologically“cold”tumors into“hot”ones through a cascaded therapy,thereby revitalizing the tumor immune microenvironment in OSCC.

Digital Innovation in Oral Health Care: A Comprehensive Review

Synthesizing findings from 125 studies published between 2010 and 2024, the paper identifies key achievements, including improved patient compliance, enhanced diagnostic capabilities, and expanded access to care in underserved areas. Tele-dentistry has been pivotal in bridging geographical gaps, particularly during the COVID-19 pandemic, while AI tools have revolutionized diagnostics and personalized treatment planning. Wearable devices and mHealth applications have empowered patients with real-time feedback, fostering sustained adherence to oral hygiene practices. The review also highlights the potential of digital tools to reduce healthcare disparities and operational costs, paving the way for more equitable and efficient dental care. By outlining critical advancements and future directions, this paper underscores the transformative potential of digital health technologies in dentistry. It advocates for further research on data security, interoperability, and innovative applications of AI to maximize the benefits of these tools, ultimately shaping a more accessible and patient-focused oral health ecosystem.

Sulfur metabolism under stress:Oxidized glutathione inhibits methionine biosynthesis by destabilizing the enzyme cystathionineγ-synthase

Atrial fibrillation(AF)/atrial flutter(AFL)is the most common sustained cardiac *** known risk factors for developing AF/AFL include age,structural heart disease,hypertension,diabetes mellitus,or *** study aims to attribute the trends in AF/AFL-related mortalities over the past two decades 1999-2020 concerning race and sex and disparity among *** the best of our knowledge,this is the first study that estimates the trends and mortality due to AF/AFL from 1999-2020 in older adults in the United *** this 21-year analysis of mortality data,we found a constant increase in mortality rates due to AF/AFL in older *** 1999 to 2020,the overall mortality in older adults aged 65 and above,regardless of sex and race,is found to be almost doubled *** a 50.2%increase in the number of deaths due to AF/***,other confounding risk factors such has obesity,prior myocardial infarction,inflammation,hypertension,birth weight,diabetes mellitus,hyperthyroidism,hormone replacement therapy in menopausal women increases the risk in the occurrence or recurrent occurrence of AF.

Electrochemical behavior of Co-Cr and Ni-Cr dental cast alloys

The cast structures influencing the electrochemical corrosion behavior of Co-Cr and Ni-Cr dental alloys were studied using potentiodynamic polarization and AC impedance in 0.9% (mass fraction) NaCl solution at (37±1) ℃. The phase and microstructure of the alloys that were fabricated using two different casting methods viz. centrifugal casting and high frequency induction casting, were examined using X-ray diffraction analysis, scanning electron microscopy and energy dispersive spectroscopy. The roles of alloying elements and the passive film homogeneity on the corrosion resistance of Co-Cr-Mo and Ni-Cr-Mo dental cast alloys were reviewed. The results of electrochemical study show that the dependence of corrosion resistance on the microstructure associated with the casting methods is marginal. The Co-Cr alloy exhibits more desirable corrosion resistance properties than the Ni-Cr alloy. There is severe preferential dissolution of Ni-rich, Cr and Mo depleted zones in the Ni-Cr alloy.
