Roles of serum uric acid on the association between arsenic exposure and incident metabolic syndrome in an older Chinese population

Growing evidences showed that heavy metals exposure may be associated with metabolic ***,themechanism underlying arsenic(As)exposure and metabolic syndrome(MetS)risk has not been fully *** we aimed to prospectively investigate the role of serum uric acid(SUA)on the association between blood As exposure and incident MetS.A sample of 1045 older participants in a community in China was *** determined As at baseline and SUA concentration at follow-up in the Yiwu Elderly *** events were defined according to the criteria of the International Diabetes Federation(IDF).Generalized linear model with log-binominal regression model was applied to estimate the association of As with incident *** investigate the role of SUA in the association between As andMetS,amediation analysiswas *** the fully adjusted log-binominal model,per interquartile range increment of As,the risk of MetS increased *** with the lowest quartile of As,the adjusted relative risk(RR)of MetS in the highest quartile was 1.42(95%confidence interval,CI:1.03,2.00).Additionally,blood As was positively associated with SUA,while SUA had significant association with MetS *** mediation analysis demonstrated that the association of As and MetS risk was mediated by SUA,with the proportion of 15.7%.Our study found higher As was remarkably associated with the elevated risk of MetS in the Chinese older adults *** analysis indicated that SUA might be a mediator in the association between As exposure and MetS.

Changes in Depression,Anxiety,and Stress Levels during a Religious Period:A Prospective Cohort Study

Toxic effects in terms of mitochondria and hereditary substances have been characterized in vitro for individual rare earth elements,while,the joint effects of mixed elements exposure in the population remain *** on the Occupational Chromate Exposure Dynamic Cohort of China,this study investigated the relationship between 15 blood rare earth elements(cerium,dysprosium,erbium,europium,gadolinium,holmium,lanthanum,lutetium,neodymium,praseodymium,samarium,terbium,thulium,yttrium,and ytterbium)and mitochondrial DNA copy number(MtDNACN)as well as peripheral blood lymphocyte micronucleus frequency(MNF).The elastic net was used to select elements highly correlated with effect indicators,whose dose-response relationships were further illustrated by restricted cubic *** kernel regressionwas employed to explore the combined effects of elements and the contributions of single *** results showed that most rare earth elementswere positively correlatedwith effect indicators,with yttrium showing the strongest association(β(95%CI):0.139(0.1089–0.189)for MtDNACN,0.937(0.345–1.684)for MNF).In the mixed exposure model,with the exposure level fixed at the 50th percentile as the reference,the effect estimates on MtDNACN and MNF increased by 0.228 and 0.598 units,respectively,at the 75th *** single effect analysis implied that yttrium,lanthanum and terbium contributed the most to the elevation of MtDNACN,while yttrium posed the highest risk for genetic damage,accordingly,we provided recommendations to prioritize these elements of *** addition,we observed a chief mediating effect of MtDNACN on the elevation of MNF caused by lanthanum,whereas further mechanistic exploration is required to confirm this finding.

Objective: To examine the effectiveness of Chinese medicine(CM) Lianhua Qingwen Granule(LHQW) and Jingyin Gubiao Prescription(JYGB) in asymptomatic or mild patients with Omicron infection in the shelter hospital. Methods: This single-center retrospective cohort study was conducted in the largest shelter hospital in Shanghai, China, from April 10, 2022 to May 30, 2022. A total of 56,244 asymptomatic and mild Omicron cases were included and divided into 4 groups, i.e., non-administration group(23,702 cases), LHQW group(11,576 cases), JYGB group(12,112 cases), and dual combination of LHQW and JYGB group(8,854 cases). The length of stay(LOS) in the hospital was used to assess the effectiveness of LHQW and JYGB treatment on Omicron infection. Results: Patients aged 41–60 years, with nadir cycle threshold(CT) value of N gene <25, or those fully vaccinated preferred to receive CM therapy. Before or after propensity score matching(PSM), the multiple linear regression showed that LHQW and JYGB treatment were independent influence factors of LOS(both P<0.001). After PSM, there were significant differences in LOS between the LHQW/JYGB combination and the other groups(P<0.01). The results of factorial design ANOVA proved that the LHQW/JYGB combination therapy synergistically shortened LOS(P=0.032). Conclusions: Patients with a nadir CT value <25were more likely to accept CM. The LHQW/JYGB combination therapy could shorten the LOS of Omicron-infected individuals in an isolated environment.

Real-world cost-effectiveness of targeted temperature management in out-of-hospital cardiac arrest survivors: results from an academic medical center

BACKGROUND: Targeted temperature management(TTM) is a common therapeutic intervention, yet its cost-effectiveness remains uncertain. This study aimed to evaluate the real-world cost-effectiveness of TTM compared with that of conventional care in adult out-of-hospital cardiac arrest(OHCA) survivors using clinical patient-level ***: We conducted a retrospective cohort study at an academic medical center in the USA to assess the cost-effectiveness of TTM in adult non-traumatic OHCA survivors between 1 January, 2019 and 30 June, 2023. The primary outcome was survival to hospital discharge. Incremental cost-effectiveness ratios(ICERs) were calculated and compared with various decision makers' willingness to pay. Cost-effectiveness acceptability curves were utilized to evaluate the economic attractiveness of TTM. Uncertainty about the incremental cost and effect was explored with a 95% confidence ***: Among 925 non-traumatic OHCA survivors, only 30(3%) received TTM. After adjusting for potential confounders, the TTM group did not demonstrate a significantly lower cost(delta cost-$5,141, 95% confidence interval [95% CI]: $-35,347 to $25,065, P=0.79) and higher survival to hospital discharge(delta effect 6%, 95% CI:-11% to 23%, P=0.41). Additionally, a 95% confidence ellipse indicated uncertainty reflected by evidence that the true value of the ICER could be in any of the quadrants of the cost-effectiveness ***: Although TTM did not demonstrate a clear survival benefit in this study, its potential cost-effectiveness warrants further investigation with larger sample sizes. These findings highlight the need for additional research to optimize TTM use in OHCA care and inform resource allocation decisions.

AIM:To investigate the effects of adenosine triphosphate(ATP)and melatonin,which have antioxidant and antiinflammatory activities,on potential 5-fluorouracil(5-FU)-induced optic nerve damage in ***:Twenty-four rats were categorized into four groups of six rats:healthy(HG),5-FU(FUG),ATP+5-FU(AFU),and melatonin+5-FU(MFU).ATP(4 mg/kg)and melatonin(10 mg/kg)were administered intraperitoneally and orally,*** hour after ATP and melatonin administration,rats in the AFU,MFU,and FUG were intraperitoneally injected with 5-FU(100 mg/kg).ATP and melatonin were administered once daily for 10d.5-FU was administered at a single dose on days 1,3,and 5 of the *** 10d,the rats were euthanized and optic nerve tissues were *** nerve tissues were biochemically and histopathologically ***:ATP and melatonin treatments inhibited the increase in malondialdehyde(MDA)and interleukin-6(IL-6)levels,which were elevated in the *** treatments also prevented the decrease in total glutathione(tGSH)levels and the superoxide dismutase(SOD)and catalase(CAT)activities(P<0.001).This inhibition was higher in the ATP group than in the melatonin group(P<0.001).ATP prevented histopathological damage better than melatonin(P<0.05).CONCLUSION:ATP and melatonin have the potential to be used in alleviating 5-FU-induced optic nerve *** addition,ATP treatment shows better protective effects than melatonin.

The measurement of amino acid and monoamine neurotransmitters(NTs)in serum plays a central role in the identification and monitoring of clinical panic disorder(PD).However,the strong polarity and chemical heterogeneity of NTs make direct extraction and rapid detection extremely ***,we developed a strategy that integrated solid phase extraction based on magnetic Fe_(3)O_(4)coated with polydopamine(Fe_(3)O_(4)@PDA)and direct analysis of mass spectrometry in real time(DART-MS)for quantification of 10 *** systematically investigated the optimal conditions of this strategy to achieve good accuracy and *** with existing magnetic solid phase extraction methods for NTs detection in biological samples,this strategy could adsorb more targets while having similar adsorption efficiencies,and the total process time was reduced by an average of 69.42%.We rapidly determined the metabolic abnormality of 6 types of NTs in the serum of PD patients and demonstrated their diagnostic *** addition,an outstanding feature of PD in which tryptophan was more metabolized by the KYN pathway and the 5-hydroxytryptamine pathway was inhibited could be elucidated by this *** results highlighted the potential ability of Fe_(3)O_(4)@PDA coupled with DART-MS to assist in the clinical diagnosis of PD.

Effect of nursing intervention on improving awareness of high-risk patients about stroke prevention

Objective: Stroke is perceived as a serious illness although it is preventable, and lifestyle alterations can be made to reduce its risk. Improvement of stroke care should be achieved through awareness of public, especially through high-risk group. Hencefor th, the aim of this study was to evaluate the effect of nursing intervention on improving awareness of high-risk people about stroke prevention. Methods: A quasi-experimental design was used to carry out this study, where a purposive sample of 80 adult patients at high risk for stroke were enrolled while they were attending the out-patient clinics of Zagazig University Hospitals for follow-up treatment; they were divided into two groups: case(40 patients) and control(40 patients). An interview questionnaire was used to assess patients' knowledge regarding warning signs, risk factors, and immediate treatment; attitude; and their practice for prevention for stroke at pretest and posttest. Results: The study revealed the poor level of knowledge, bad level of preventive behavior, and negative attitude regarding stroke prevention at pre-intervention. However, after intervention, the high-risk people had fair level of knowledge with moderate practice for stroke prevention and positive attitude, with statistically significant difference between pre–post intervention and between the case and control groups. Conclusions: The study revealed that the nursing intervention seemed to change the knowledge, practice, and attitude of subjects regarding stroke prevention. This, in turn, implies that awareness of prevention of stroke and high-risk factor control could be impor tant to reduce the burden of stroke in Egypt. Thus, fur ther research on a larger scale is required to improve population knowledge and behavior for stroke prevention and control.
