Background:Amanita poisoning as a foodborne disease has raised concerning mortality issues.Reducing the interval between mushroom ingestion and medical intervention could greatly influence the outcomes of Amanita poisoning patients,while treatment is highly dependent on a confirmed diagnosis.To this end,we developed an early detection-guided intervention strategy by optimizing diagnostic process with performingα-amanitin detection,and further explored whether this strategy influenced the progression of Amanita poisoning.Methods:This study was a retrospective analysis of 25 Amanita poisoning patients.Thirteen patients in the detection group were diagnosed mainly based onα-amanitin detection,and 12 patients were diagnosed essentially on the basis of mushroom consumption history,typical clinical patterns and mushroom identification(conventional group).Amanita poisoning patients received uniform therapy,in which plasmapheresis was executed once confirming the diagnosis of Amanita poisoning.We compared the demographic baseline,clinical and laboratory data,treatment and outcomes between the two groups,and further explored the predictive value ofα-amanitin concentration in serum.Results:Liver injury induced by Amanita appeared worst at the fourth day and alanine aminotransferase(ALT)rose higher than aspartate aminotransferase(AST).The mortality rate was 7.7%(1/13)in the detection group and 50.0%(6/12)in the conventional group(P=0.030),since patients in the detection group arrived hospital much earlier and received plasmapheresis at the early stage of disease.The early detection-guided intervention helped alleviate liver impairment caused by Amanita and decreased the peak AST as well as ALT.However,the predictive value ofα-amanitin concentration in serum was still considered limited.Conclusions:In the management of mushroom poisoning,consideration should be given to the rapid detection ofα-amanitin in suspected Amanita poisoning patients and the immediate initiation of medical treatment upon a positive toxin screening re

With the development of hardware devices and the upgrading of smartphones,a large number of users save privacy-related information in mobile devices,mainly smartphones,which puts forward higher demands on the protection of mobile users’privacy information.At present,mobile user authenticationmethods based on humancomputer interaction have been extensively studied due to their advantages of high precision and non-perception,but there are still shortcomings such as low data collection efficiency,untrustworthy participating nodes,and lack of practicability.To this end,this paper proposes a privacy-enhanced mobile user authentication method with motion sensors,which mainly includes:(1)Construct a smart contract-based private chain and federated learning to improve the data collection efficiency of mobile user authentication,reduce the probability of the model being bypassed by attackers,and reduce the overhead of data centralized processing and the risk of privacy leakage;(2)Use certificateless encryption to realize the authentication of the device to ensure the credibility of the client nodes participating in the calculation;(3)Combine Variational Mode Decomposition(VMD)and Long Short-TermMemory(LSTM)to analyze and model the motion sensor data of mobile devices to improve the accuracy of model certification.The experimental results on the real environment dataset of 1513 people show that themethod proposed in this paper can effectively resist poisoning attacks while ensuring the accuracy and efficiency of mobile user authentication.

Objectives Benzimidazoles(BZs)are commonly used for the treatment of soil-transmitted helminth infections in veterinary clinics;however,misuse and overdosing of BZs will cause residual problems and have the potential to damage human health through the food chain.Thus,the existence of BZs in foods needs more attention.This study aims to establish a broad-spectrum immunoassay for rapid detection and to simultaneously monitor BZs in milk.Materials and Methods Based on structure analysis,a‘zero epitope loss’strategy,which introduced a spacer arm into the imino group of the imidazole ring of albendazole,was first adopted for hapten modification to obtain an ultra-sensitive and broad-spectrum antibody.An indirect competitive enzyme-linked immunosorbent assay(icELISA)was established for the detection of 18 BZs in milk sample with a single-step pretreatment.A quantitative structure–activity relationship model was constructed to interpret and predict the recognition.Results The antibody could recognize 20 BZs and the half-inhibitory concentrations ranged from 0.054 to 417.58 ng/mL,the limits of detection of icELISA ranged from 0.4 to 89.4 ng/mL,and the mean recovery rates ranged from 76.49%to 120.40%,with a coefficient of variation<20%.Substituent R1 of BZs was considered to be the main influencing factor for recognition,and the comparative molecular field analysis model(q2=0.724,r2=0.998)was finally chosen for further prediction.Conclusions The results indicated that the established icELISA could simultaneously identify 18 BZs,with good accuracy and precision,which was suitable for rapid detection of BZs in milk.

Death from caustic ingestion:A case report

Rationale:Acute caustic ingestion from suicidal intent is not usual in emergency departments in developed countries.One of the substances commonly ingested by suicidal patients,phosphoric acid,tends to cause multi-system derangements.Patient’s Concern:A 41-year-old male patient presented with complaints of throat discomfort,severe generalized abdominal pain,and multiple episodes of hematemesis after ingesting a restroom cleaning solution.Diagnosis:poisoning by acute caustic ingestion(containing<30%phosphoric acid and<4%ethylene glycol).Interventions:The patient was administered 50 mL of 8.4%sodium bicarbonate solution followed by an isotonic sodium bicarbonate solution running at 500 mL/h,a hyperkalemia kit,ceftriaxone,metronidazole,omeprazole,and atropine.The patient then underwent urgent hemodialysis.Outcomes:The patient suffered gastrointestinal bleeding as a result of local caustic injury.In addition,his course of illness was complicated by severe acidemia from high anion gap metabolic acidosis and deranged electrolytes(hyperphosphatemia,hyperkalemia,and hypocalcemia).He developed multi-organ failure and eventually demised.Lessons:The clinician needs to be mindful of the multi-system complications arising from such a caustic ingestion.These patients need to be monitored closely for deterioration,and have prompt management of the various arising complications,to reduce the high morbidity and mortality associated with this condition.

Following the withdrawal of paraquat,diquat(DQ)has emerged as the predominant herbicide.When people come into contact with or ingest DQ,may lead to poisoning and potentially fatal outcomes.Reports suggest that the mortality of DQ poisoning can be as high as 50%.DQ poisoning can be categorized as mild,moderate to severe or fulminant.In cases of fulminant poisoning,victims often succumb to multiple organ failure within 48 h.This presents a significant challenge in the clinical management.Scholars have discovered that oxidative stress,inflammatory injury,and cell apoptosis play crucial roles in the DQ poisoning.However,the underlying connection of the extensive organ damage remains unknown.The abnormal function and activity of endothelial cells(EC)should play a crucial role in tissue damage caused by DQ due to rich microcirculation and high sensitivity to perfusion in the vulnerable organs.However,reports on DQ-induced EC injury is rare.We made a preliminary discovery-the presence of severe vascular endothelial damage in the kidneys and lungs affected by DQ.Therefore,we hypothesize that DQ poisoning may be attributed to EC damage,ultimately resulting in multiple organ failure.

Pesticide poisoning is one of the most common diseases in the emergency department, characterized by rapid changes in condition, a high misdiagnosis rate, and a poor prognosis. Measures for early removal of poisons are crucial, and gastric lavage is one of the important measures. Regarding the post-gastric lavage effect, abdominal CT scanning has an important application value in the assessment of the gastric lavage effect after pesticide poisoning.

In recent years, there has been an increase in concern regarding the effects of paracetamol poisoning on liver tissues, particularly when consumed in large amounts. Some studies have estimated that paracetamol is involved in 56% of acute liver diseases, whereas 0.4% of paracetamol overdose cases result in fatal-ity. In this study, the effects of Moringa oleifera on paracetamol toxicity in the liver were explored. It has been demonstrated that Moringa oleifera is highly nu-tritious, contains bioactive molecules, and is therapeutically beneficial. Many studies have shown that Moringa oleifera leaves possess a wide range of biologi-cal properties, including antioxidant, tissue protection, analgesic, antihyperten-sive, and immunomodulatory activities. This study highlights the protective role of Moringa oleifera on handling possible paracetamol hepatotoxicity in male rats. .

Association of the genetic polymorphism of EPHX1 and EPHX2 with the susceptibility to chronic benzene poisoning

The aim of this study was to explore the association of the genetic polymorphism of EPHX1 and EPHX2 with the susceptibility to chronic benzene poisoning(CBP).A case-control study of 268 patients with CBP and 268 healthy workers matched by age and sex,all of whom were occupationally exposed to benzene,was conducted.The single nucleotide polymorphisms(SNPs,rs2854451,rs3738047,rs2234922 and rs1051741)of EPHX1 gene and the SNP(rs751141)of EPHX2 gene were tested by the TaqMan PCR method.In the subjects carrying the genotype of EPHX1 rs3738047 GG,the risk of CBP was decreased in the individuals simultaneously carrying EPHX1 rs2234922 G(P=0.02).Alternatively,in the subjects carrying the genotype of EPHX1 rs2234922 AA,the risk of CBP was increased in the individuals simultaneously carrying the allele of EPHX2 rs751141A(P=0.03).It was also found that there were potential interactions between alcohol consumption and the polymorphism of EPHX1 rs1051741(x_(H)^(2)=5.28,P=0.02)or rs2234922(x_(H)^(2)=6.71,P=0.01).Compared to individuals with EPHX1 rs1051741 CC or rs2234922 AA genotype in the drinkers,the risk of CBP in those carrying genotypes of EPHX1 rs1051741 CT+TT or rs2234922 AG+GG was decreased,respectively(P=0.04,Poccupational population and further research is needed for the association between the genetic polymorphism in EPHX2 and the susceptibility to CBP.

A Retrospective Analysis of Intoxicated Patients Admitted to the Intensive Care Unit: A Single-Institution Assessment in the Post-Pandemic Period

Background: The aim of this study was to evaluate the clinical and epidemiological characteristics of patients with acute intoxication admitted to the Intensive Care Unit (ICU). Material and Methods: An observational retrospective study was conducted on intoxicated patients who admitted to ICU between January 2022 and January 2024. Data were collected from the patients medical records. The demographic characteristics, causes of intoxications, clinical parameters, the mean stay in the ICU, treatment modalities and prognosis were recorded. Results: A total of 2875 critically ill patients were admitted during the study period, and 109 (3.79%) of them were acute intoxications. Their mean of ages was 38.09 ± 12.29. The female-to-male ratio was 1.37/1. Drugs were found to be the primary cause (62.39%) of intoxications, and analgesics, antidepressants, and antibiotics were the most frequent agents. Suicidal attempts were present in 66 patients, most of them were female (62.13%) and between 17 - 24 years (40.91%). The other common causes of intoxications were carbon monoxide (CO) (22.02%), methyl/ethyl alcohol (8.26%) and mushroom (5.50%). The mean stay in the ICU was 2.69 ± 0.89 days. Mechanical ventilation was applied to 10 of our patients. Renal replacement therapy was required in 6 patients. Despite all treatments, 6 of our patients died, and we found the mortality rate to be 5.50%. Conclusion: Intoxications were more frequent in young female patients and drugs were the most common cause with suicidal intent. Unfortunately, CO poisoning continues to be a very important problem in our city. These findings provided significant information about the characteristics of intoxications in Karabuk.
