Constraint-induced movement therapy is an effective rehabilitative training technique used to improve the restoration of impaired upper extremity movement after stroke. However, whether constraint-induced movement therapy is more effective than conventional rehabilitation.in acute or sub-acute stroke remains controversial. The aim of the present study was to identify the optimal time to start constraint-induced movement therapy after ischemic stroke and to explore the mechanisms by which constraint-induced movement therapy leads to post-stroke recovery. Sixty-four adult male Sprague-Dawley rats were randomly divided into four groups: sham-surgery group, cerebral ischemia/reperfusion group, early constraint-induced movement therapy group, and late constraint-induced movement therapy group. Rat models of left middle cerebral artery occlusion were established according to the Zea Longa line embolism method. Constraint-induced movement therapy was conducted starting on day 1 or day 14 in the early constraint-induced movement therapy and late constraint-induced movement therapy groups, respectively. To explore the effect of each intervention time on neuromotor function, behavioral function was assessed using a balance beam walking test before surgery and at 8 and 21 days after surgery. The expression levels of brain-derived neurotrophic factor, nerve growth factor and Nogo receptor were evaluated using real time-polymerase chain reaction and western blot assay to assess the effect of each intervention time. The results showed that the behavioral score was significantly lower in the early constraint-induced movement therapy group than in the cerebral ischemia/reperfusion and late constraint-induced movement therapy groups at 8 days. At 21 days, the scores had significantly decreased in the early constraint-induced movement therapy and late constraint-induced movement therapy groups. Combination Treatment with Gua Sha and Blood-letting Causes Attenuation of Systemic Inflammation,Activated Coagulation,Tissue Ischemia and Injury during Heatstroke in Rats

Objective： Gua Sha and Blood-letting at the acupoints were Chinese traditional therapies for heatstroke. The purpose of present study was to assess the therapeutic effect of Gua Sha on the DU Meridian and Bladder Meridian combined with Blood-letting acupoints at Shixuan （EX-UE 11） and Weizhong （BL 40） on heatstroke. Methods： Anesthetized rats, immediately after the onset of heatstroke, were divided into four major groups： Gua Sha group, Blood-letting group, Gua Sha combined with Blood-letting group and model group. They were exposed to ambient temperature of 43℃ to induce heatstroke. Another group of rats were exposed to room temperature （26℃） and used as normal control group. Their survival times were measured. In addition, their physiological and biochemical parameters were continuously monitored. Results： When rats underwent heatstroke, their survival time values were found to be 21-25 min. Treatment of Gua Sha combined with Blood- letting greatly improved the survival time （230 ± 22 rain） during heatstroke. All heatstoke animals displayed and activated coagulation evidenced by increased prothrombin time （PT）, activated partial thromboplastin time （aPTT）, D-dimer, and decreased platelet count, protein C. Furthermore, the animals displayed systemic inflammation evidenced by increased the serum levels of cytokines interleukin-1β （IL-1β）, tumor necrosis factor α （TNF-α） and malondialdehyde （MDA）. Biochemical markers evidenced by cellular ischemia and injury/dysfunction included increased plasma levels of blood urea nitrogen （BUN）, creatinine, serum glutamic oxaloacetic transaminase （SGOT）, serum glutamic pyruvic transaminase （SGPT）, and alkaline phosphatase （ALP） were all elevated during heatstroke. Core temperatures （Tco） were also increased during heatstroke. In contrast, the values of mean arterial pressure were significantly lower during heatstroke. Effect of Acupuncture Cooperated with Low-frequency Repetitive Transcranial Magnetic Stimulation on Chronic Insomnia： A Randomized Clinical Trial

The effect of acupuncture cooperated with low-frequency repetitive transcranial magnetic stimulation （rTMS） on chronic insomnia was explored. Seventy-eight patients with chronic insomnia were randomly allocated into two groups： treatment group and control group. In the treatment group, the patients received acupuncture combined with rTMS treatment, and those in the control group were given acupuncture cooperated with sham rTMS treatment, 3 days per week for 4 weeks. Before and after treatment, the primary outcomes including the scores on Insomnia Severity Index （ISI） and Pittsburgh Sleep Quality Index （PSQI） and the secondary outcomes including total sleep time （TST）, sleep onset latency （SOL）, wake after sleep onset （WASO）, sleep efficiency （SE%） recorded by sleeping diary and actigraphy were observed in both groups. Seventy-five participants finished the study （38 in treatment group and 37 in control group respectively）. After treatment, the scores in the two groups were improved significantly, more significantly in the treatment group than in the control group. It can be inferred that acupuncture cooperated with rTMS can effectively improve sleep quality, enhance the quality of life of patients and has less side effects.

Effect of Electroacupuncture on the Pathomorphology of the Sciatic Nerve and the Sensitization of P2X_3 Receptors in the Dorsal Root Ganglion in Rats with Chronic Constrictive Injury

Objective： To explore the effect of electroacupuncture （EA） on the pathomorphology of the sciatic nerve and the role of P2X3 receptors in EA analgesia. Methods： The chronic constriction injury （CCI） model was adopted in this study. A total of 32 rats were randomly divided into four groups： sham CCI, CCI, CCI plus contralateral EA （CCI ＋ conEA） and CCI plus ipsilateral EA （CCI ＋ ipsEA）. Mechanical withdrawal threshold （MWT） and thermal withdrawal latency OWL） were measured. EA began at day 7 after the CCI operation and was applied to the Zusanli （ST 36） and Yanglingquan acupoints （GB 34）. At day 14, the pathomorphologic changes of the operated sciatic nerve were demonstrated by hematoxylin and eosin staining. In addition, dorsal root ganglion （DRG） neurons isolated from rats were examined by electrophysiological recording to determine if the P2X3 receptor agonists, adenosine 5＇-tdphosphate disodium （ATP） and α, 13 -methylen-ATP （or, 13 -meATP） evoked inward currents. Results： Pain thresholds in the CCI group were obviously decreased post CCI surgery （P〈0.01）. In the EA groups, thermal and mechanical threshold values were increased after the last EA treatment （P〈0.05, P〈0.01）. There was no significant difference in light microscopic examination among the four groups （P〉0.05）. Current amplitude after application of ATP and or, 13 -meATP in DRG neurons were much larger in the CCI group compared to those obtained in sham CCI （P〈0.05）. ATP and α, 13 -meATP invoked amplitudes in the CCI ＋ EA groups were reduced. There was no significant difference between the CCI ＋ conEA group and the CCI ＋ ipsEA group （P〉0.05）. Conclusion： EA analgesia may be mediated by decreasing the response of P2X8 receptors to the agonists ATP and or, 13 -meATP in the DRG of rats with CCI. No pathological changes of the sciatic nerve of rats were observed after EA treatment.

Alzheimer's disease(AD) is a progressive and degenerative neurological disease characterized by the deterioration of cognitive functions. While a definitive cure and optimal medication to impede disease progression are currently unavailable, a plethora of studies have highlighted the potential advantages of exercise rehabilitation.for managing this condition. Those studies show that exercise rehabilitation.can enhance cognitive function and improve the quality of life for individuals affected by AD. Therefore, exercise rehabilitation.has been regarded as one of the most important strategies for managing patients with AD. Herein, we provide a comprehensive analysis of the currently available findings on exercise rehabilitation.in patients with AD, with a focus on the exercise types which have shown efficacy when implemented alone or combined with other treatment methods, as well as the potential mechanisms underlying these positive effects. Specifically, we explain how exercise may improve the brain microenvironment and neuronal plasticity. In conclusion, exercise is a cost-effective intervention to enhance cognitive performance and improve quality of life in patients with mild to moderate cognitive dysfunction. Therefore, it can potentially become both a physical activity and a tailored intervention. This review may aid the development of more effective and individualized treatment strategies to address the challenges imposed by this debilitating disease, especially in low-and middle-income countries.

OBJETIVE: To know the prevalence of acupuncturists within physical and rehabilitation.medicine specialists in Europe and their characteristics,as well as to analyze the pathologies treated and therapeutic techniques ***: This is a cross-sectional descriptive study based on a structured and closed questionnaire. The study was carried out from October 2011 to May ***: A total of 115 specialists from 19 countries completed the questionnaire. The mean characteristics of the physical and rehabilitation.medicine(PRM) specialists who are also acupuncturists in Europe were: women(62.6 %); mean age of47.91 years old; 4.20 years of physical and rehabilitation.medicine school and 2.00 years of acupuncture training; 15.03 years of mean time of clinical practice as physical and rehabilitation.medicine specialists and 10.42 years of experience as acupuncturists; 35.65 % had a doctorate(Ph D) degree,obtained at a mean age of 33.55 years old; working in a public hospital(80%); treating mainly neurological conditions(48.1%) and using mainly techniques such as kinesiotherapy(11.81%),orthosis(10.40%) and electrotherapy(9.92%). Acupuncture was applied by 32.2% of the participants mainly in musculoskeletal conditions(27.38%). A mean number of more than 20 patients a day were treated by47.04% of the specialists,being 41.8% of the patients(adults and children). Significant differences were found between the variables,acupuncture and gender(Pearson's correlation Index = 0.007).However,no differences were observed in relation to acupuncture and age,doctorate degree,years of clinical practice and workplace(all with P values >0.05).CONCLUSION: In the European area,physical and rehabilitation.medicine specialists who are also acupuncturists are mainly female,middle-aged,with15 years of professional experience,Ph D holders,working in public hospitals,treating mainly neurological conditions with kinesiotherapy and acupuncture,and treating over 20 patients a day(adults and child

Protective effects of Batroxobin on spinal cord injury in rats

Expansion of the secondary injury following primary spinal cord injury is a major pathological event that increases destruction in the spinal cord, so measures to reduce secondary injury are needed. Our previous study demonstrated that, at the front of the expanding secondary injury in the spinal cord, there is an ischemic area in which many neurons can still be rescued. Therefore, enhancement of blood circulation in the cord may be helpful, and indeed, we found that a traditional Chinese medicine, shu-xue-tong, efficiently reduces the secondary injury. The aim of the present study was to investigate the effect of reducing fibrinogen with Batroxobin, a drug widely used clinically for ischemia, in rats with spinal cord contusion. We found that both 2 and 4 Batroxobin units （BU）/kg efficiently decreased the plasma fibrinogen, and 2 BU/kg significantly increased spinal blood flow, enhanced neuronal survival, mitigated astrocyte and microglia activation, and improved locomotor recovery. However, 4 BU/kg had no effect on the secondary spinal cord injury. These data suggest that Batroxobin has multiple beneficial effects on spinal cord injury, indicating a potential clinical application.
