Oral Health Status and Gingival Response to Three Different Restorative Materials among Saudi Patients: A Clinical &Histopathological Study

Background: The correlation between oral health and dental restoration is fundamental. For the gingival and periodontal tissues to stay healthy, dental restoration should be in regularity with the surrounding tissues. This study aims to assess the oral health status and histopathological gingival response to three different restorative materials among Saudi patients. Methods: The study groups consist of 240 patients (50% males and 50% females), aged 18 - 45, with inclusion and exclusion criteria in the study. Participants are divided into three equal groups: those with composite resin restorations, those with amalgam restorations and those with glass ionomer restorations. Biopsies were taken from adjacent gingival tissues. Clinical parameters were determined by: plaque index (PLI), gingival index (GI) and clinical attachment loss (CAL). All data were collected and evaluated by through statistical analysis. Results: The clinical findings of the current study revealed that amalgam restorations produce a higher means of PLI, GI and CAL compared with composite resin restorations and glass ionomer restorations, but not insignificant levels, except CAL (p = 0.004*). As for histopathological findings, there were significant differences in gingival tissue response to amalgam restorations, composite resin restorations and glass ionomer cement fillings, where there were statistically significant differences in numbers of chronic inflammatory cells (p Conclusion: At the end of the present study, we concluded that the amalgam restorations are less biocompatible compared to composite resin restorations and glass ionomer restorations.

The hematopoietic stem cell （HSC） niche in the bone marrow has been studied extensively over the past few decades, yet the bone marrow microenvironment that supports the growth of metastatic prostate cancer （PCa） has only been recently considered to be a specialized ＇niche＇ as well. New evidence supports the fact that disseminated tumor cells （DTCs） of PCa actually target the HSC niche, displace the occupant HSCs and take up residence in the pre-existing niche space. This review describes some of the evidence and mechanisms by which DTCs act as molecular parasites of the HSC niche. Furthermore, the interactions between DTCs, HSCs and the niche may provide new targets for niche-directed therapy, as well as insight into the perplexing clinical manifestations of metastatic PCa disease.

Biofilm three-dimensional architecture influences in situ pH distribution pattern on the human enamel surface

To investigate how the biofilm three-dimensional(3D) architecture influences in situ pH distribution patterns on the enamel surface. Biofilms were formed on human tooth enamel in the presence of 1% sucrose or 0.5% glucose plus 0.5% fructose. At specific time points, biofilms were exposed to a neutral pH buffer to mimic the buffering of saliva and subsequently pulsed with 1% glucose to induce re-acidification. Simultaneous 3D pH mapping and architecture of intact biofilms was performed using two-photon confocal microscopy. The enamel surface and mineral content characteristics were examined successively via optical profilometry and microradiography analyses. Sucrose-mediated biofilm formation created spatial heterogeneities manifested by complex networks of bacterial clusters(microcolonies). Acidic regions(pH<5.5) were found only in the interior of microcolonies,which impedes rapid neutralization(taking more than 120 min for neutralization). Glucose exposure rapidly re-created the acidic niches, indicating formation of diffusion barriers associated with microcolonies structure. Enamel demineralization(white spots),rougher surface, deeper lesion and more mineral loss appeared to be associated with the localization of these bacterial clusters at the biofilm-enamel interface. Similar 3D architecture was observed in plaque-biofilms formed in vivo in the presence of sucrose. The formation of complex 3D architectures creates spatially heterogeneous acidic microenvironments in close proximity of enamel surface, which might correlate with the localized pattern of the onset of carious lesions(white spot like) on teeth.

Effects of sleep bruxism on functional and occlusal parameters:a prospective controlled investigation

This study was conducted to verify the results of a preceding retrospective pilot study by means of a prospective controlled investigation including a larger sample size. Therefore, the aim of this clinical investigation was to analyze the relationship between sleep bruxism and several functional and occlusal parameters. The null hypothesis of this study was that there would be no differences among sleep bruxism subjects and non-sleep bruxism controls regarding several functional and occlusal parameters. Fifty-eight sleep bruxism subjects and 31 controls participated in this study. The diagnosis sleep bruxism was based on clinical criteria of the American Academy of Sleep Medicine. Sixteen functional and occlusal parameters were recorded clinically or from dental study casts. Similar to the recently published retrospective pilot study, with a mean slide of 0.77 mm （s.d., 0.69 mm） in the sleep bruxism group and a mean slide of 0.4 mm （s.d., 0.57 mm） in the control group, the evaluation of the mean comparison between the two groups demonstrated a larger slide from centric occlusion to maximum intercuspation in sleep bruxism subjects （Mann-Whitney U-test; P=O.O08）. However, following Bonferroni adjustment, none of the 16 occlusal and functional variables differed significantly between the sleep bruxism subjects and the non-sleep bruxism controls. The present study shows that the occlusal and functional parameters evaluated do not differ between sleep bruxism subjects and non-sleep bruxism subjects. However, as the literature reveals a possible association between bruxism and certain subgroups of temporomandibular disorders, it appears advisable to incorporate the individual adaptive caoacitv of the stomatognathic svstem into future investigations.

Evaluation of the Influence of Fixed Orthodontic Treatment Duration on the Severity of Inflammatory Gingival Enlargement (Fixed Orthodontic Induced Gingival Enlargements) and Some Properties of Saliva

Background: Inflammatory gingival enlargement is a more common clinical feature with orthodontic therapy than other features. Therefore, this study was designed to the evaluation of the influence of fixed orthodontic treatment duration on the severity of inflammatory gingival enlargement (fixed orthodontic induced gingival enlargements) and some properties of saliva. Material and Methods: The sample size comprised 145 patients undergoing fixed orthodontic treatment for at least 6 months aged 13 - 32 years. They were divided according to orthodontic treatment duration into three groups. Group I (n = 47) included the patients who were treated for less than 6 months, group II (n = 51) included the patients who were treated for a period of 6 - 12 months, and group III (n = 47) included the patients who were managed for more than 12 months. Data were obtained from the outpatient clinics, college of dentistry, King Khalid University, Abha, Saudi Arabia, and some dental centers in Sana’a city, the Republic of Yemen. This study was conducted from October 2021 G to January 2022 G. Clinical examination was done for plaque index (PLI), gingival index (GI), and gingival enlargement indexes (GEI). Saliva was collected in sterile test tubes then salivary flow and pH were measured. Statistical analysis was done with SPSS (version 23) and ANOVA test to evaluate the impact of orthodontic treatment duration on the severity of inflammatory gingival enlargement and some properties of saliva. Results: The statistical analysis demonstrated the highest mean plaque index (PLI) was among groups III and I participants whereas, the highest mean gingival index and mean gingival enlargement were among groups II and III participants. The present study revealed an increase in salivary flow with decreased salivary pH values with an increase in orthodontic therapy duration. Daily Lifestyle Habits as Risk Factors for Plaque-Induced Gingivitis and Periodontitis Severity and Grading among Samples of Dental Students at King Khalid University

Objective: To assess daily lifestyle habits as risk factors for plaque-induced gingivitis and periodontitis severity and grading among samples of dental students at King Khalid University. Material and Methods: This study included 150 male participants. They were divided according to the levels into three equal groups: levels 5, 6, and 7 (group I), levels 8, 9, 10 (group II) and levels 11, 12, and interns (group III). This study consisted of questions related to age, gender, educational level, father’s education, mother’s education, home ownership, brushing teeth more than or twice a day, in addition to daily lifestyle habits that were daily smoking, daily use of sugary drinks, and daily use of sugary foods. Some clinical parameters of plaque-induced gingivitis and periodontitis and the percentage of radiographic bone loss were recorded. All data were collected and analyzed using Tukey’s test and Chi-square test. A P-value of less than 0.5 (p Results: One hundred fifty male dental students of different levels in the college of dentistry at King Khalid University participated in the study in three groups (I, II, and III). The mean age of group III is more than that of group II and group I, with highly statistically significant differences (p 30%) were more among the participants in group I than the participants in groups III and II. There was an increase in the percentage of affected participants with stages III and IV of clinical attachment loss (CAL), periodontal pocket depth (PPD), radiographic bone loss (%RBL), tooth mobility, as well as glycated hemoglobin (HbA1c) test values (>7%), and the number of cigarettes smoked per day (>10 cigs) in group III more than in group II and I. Conclusion: We conclude that the severity and grading of plaque-induced gingivitis were higher among the participants in group I, while the severity and grading of periodontitis were higher among the participants in group III due to the different impact of their daily lifestyle habits.

Extra-Oral Digital Panoramic Radiographic Interpretation and Clinical Findings of Stage IV Periodontitis among Some Samples Concerning Saudi/Non-Saudi Nationality

Background: There are associations between the severity of periodontal diseases and some predisposing factors, such as age, race, and income rate. Therefore, the present study was designed to evaluate the extra-oral digital panoramic radiographic interpretation and clinical findings of stage IV periodontitis among some samples concerning Saudi/non-Saudi nationality. Material and Methods: The study comprised a random sample of 298 residents in Saudi Arabia referred to the internship program clinics at the College of dentistry, King Khalid University. They were according to the age of participants as follows: youth (15 - 24 years), adults (25 - 64 years), and seniors (65 years and over) within two groups according to nationality (Saudi and non-Saudi). Clinical periodontal examination and radiographic extra oral digital panoramic radiographs were done, and then the periodontal parameters, the percentage of bone loss, and the pattern of bone loss were recorded. The data was collected, and the statistical analysis was performed with ANOVA test, Tukey’s test, Chi-square test, and T-test and considered a P-value at (p Results: The samples included 62 (20%) youth participants with mean of age (21.806 ± 3.390), 182 (57%) adults’ participants with mean age (42.802 ± 9.094), and 54 (23%) seniors participants with a mean age (69.074 ± 6.810). There were highly statistically significant differences in the comparison between the participants’ ages (P 0.05). Conclusion: At the end of this study, we concluded that there are alterations in clinical and radiographic and clinical findings of stage IV periodontitis with the alteration of participants’ ages and nationality.
