Partial aortic root remodeling for root reconstruction in patients with acute type A dissection

In the present study, we reported our experience with partial aortic root remodeling for root reconstruction in patients with acute type A dissection, which involves in non-coronary sinus and/or the right coronary sinus with just one trimmed Dacron graft. Between February 2001 and May 2010, we performed partial aortic root remodeling in 40 patients, who underwent emergency surgical intervention. The dissected sinuses were excised leaving a 3-5 mm rim of the aortic wall from the attached aortic valve cusps. A short piece （4-5 cm） of collagen coated woven polyester vascular prosthesis was trimmed with one or two ＂tongues＂ to reconstruct the non-coronary sinus and/ or the right coronary sinus, but without using separated patches. Additional procedures were including hemi-arch replacement in 11 patients, and total arch replacement plus stent-elephant trunk in 20 patients. The mean follow-up time was 36.4~3.6 months. In-hospital mortality was only 5.0% （2/40）; furthermore, 3 （8.6%） patients underwent re-operation of the aortic valve and 2 （5.7%） patients died during follow-up. At the end of follow-up, trivial or no aortic regurgitation was found in 33 patients, but mild aortic regurgitation was found in 2 patients. Our data suggest that the early and mid-term results of partial aortic root remodeling were favorable, and it restored valve durability and function. Thus, the use of technique for root reconstruction in patients with acute type A dissection should be vigorously encouraged.

Calcific aorta is a disease of old age and is an independent risk factor for morbidity and mortality. Here, we present two patients with calcific aorta at different levels. One with a descending porcelain aorta, and modified Bentall＇s procedure was done. Second is a patient who is having a calcific ascending aorta and coronary artery. Coronary artery bypass grafting from left internal mammary artery to left anterior descending was done for the patient. The calcification and its morbidity had been discussed briefly.

Systemic air embolism through a bronchovenous fistu-la(BVF) has been described in patients undergoing positive-pressure ventilation. However, no report has mentioned the potential risks of systemic air embolism through a BVF in patients undergoing extracorporeal membrane oxygenation(ECMO). Positive-pressure ventilation and ECMO support in patients with lung injury can increase the risk of systemic air embolism through a BVF. Increased alveolar pressure, decreased pulmonary venous pressure, and anticoagulation are thought to be the factors that contribute to this complication. Here, we present a case of systemic air embolism in a patient with ECMO and mechanical ventilator support.

Complicated hydatid cysts of the lung:surgical treatment in Kurdistan of Iraq

Objective:To review the problems encountered in surgical treatment of complicated pulmonary hydatid cysts and to evaluate the functional results in the surgery of complicated hydatid ***:The medical records for 89 patients with complicated pulmonary hydatidosis were retrospectively *** series consisted of 47 male and 42 female patients with a mean age of 32±8 *** performed during January 2000 to December 2007,all patients were treated *** related to surgical procedures performed,postoperative morbidity,hospitalization time,and cyst recurrence were collected from each individuals records,and the group findings were ***:Among these cysts,58 were perforated,23 were infected,and 13 were cysts with pleural *** plus capitonnage was the most frequently performed operative technique(n=43),followed by cystotomy plus closure of bronchial openings(n = 28),pericystectomy plus capitonnage(n=13),decortications(n=7),lobectomy and segmentectomy(n=3).The 11 cases with coexisting liver cysts were approached by right *** complications developed in 12 patients(13.4%).Conclusion:surgery is the primary mode of treatment for patients with pulmonary hydatid *** cases have higher rates of preoperative and postoperative complications and require longer hospitalization time and more extensive surgical procedures than uncomplicated *** underlines the need for immediate surgery in any patient who is diagnosed with pulmonary hydatidosis when it is indicated.
