囊性胰岛素瘤的临床与诊断现状

Cystic endocrine tumors of the pancreas rarely occur, and only a few cases of cystic insulinoma have been reported to date. Diagnosis of insulinoma could be difficult if the functional activity is incomplete, possibly leading to blunted symptoms of hypoglycemia and failure in the laboratory to provide evidence of hyperinsulinemia. We report a clinical case of cystic insulinoma confirmed by histological examination after surgery, characterized by a high intracystic insulin concentration despite normal blood basal levels of the hormone. New diagnostic findings from dynamic tests and cystic fluid examination have been carefully focused on.

自从几十年前豪斯菲尔德因其突破性工作而赢得诺贝尔奖以来,X线CT已经被广泛的应用到了临床前以及临床应用,并产生了海量的灰度断层图像。由于其固有的光子计数问题涉及到较高的辐射剂量,这些灰度断层图像的软组织对比度差,经常难以提供一些关键性的细节而满足诊断的需要。从物理的角度来看,X线的谱是多能的,获得多能CT(亦被称为谱CT,真彩色CT)图像是可行的。这种谱图像提供了强有力的新诊断信息。近年来,新兴的Medipix技术提供了一种准确而快速的高分辨率能量选择的X线检测技术。本文将以我们小组的研究结果为重点来综述基于Medipix的谱微CT的研究进展。主要内容包括:先进的MediPix检测器,商业化MARS谱微CT的系统与方法,以及混合谱微CT的设计以及彩色扩散现象。

May-Thurner syndrome(MTS) is the pathologic compression of the left common iliac vein by the right common iliac artery, resulting in left lower extremity pain, swelling, and deep venous thrombosis. Though this syndrome was first described in 1851, there are currently no standardized criteria to establish the diagnosis of MTS. Since MTS is treated by a wide array of specialties, including interventional radiology, vascular surgery, cardiology, and vascular medicine, the need for an established diagnostic criterion is imperative in order to reduce misdiagnosis and inappropriate treatment. Although MTS has historically been diagnosed by the presence of pathologic features, the use of dynamic imaging techniques has led to a more radiologic based diagnosis. Thus, imaging plays an integral part in screening patients for MTS, and the utility of a wide array of imaging modalities has been evaluated. Here, we summarize the historical aspects of the clinical features of this syndrome. We then provide a comprehensive assessment of the literature on the efficacy of imaging tools available to diagnose MTS. Lastly, we provide clinical pearls and recommendations to aid physicians in diagnosing the syndrome through the use of provocative measures.

As changes in hard or soft oral tissues normally have a microbiological component,it is important to develop diagnostic techniques that support clinical evaluation,without destroying microbio-logical *** optical coherence tomography(OCT)represents an alternative to analyze tissues and microorganisms without the need for *** imaging technique could be defined as a fast,real-time,in situ,and non-destructive ***,this study proposed the use of the OCT to visualize biofilm by Candida albicans in reline resins for removable *** reline resins(Silagum-Comfort,Coe Comfort,and Soft-Confort),with distinct char-acteristics related to wa ter sorption and fungal inhibition were used.A total of 30 samples(10 for each resin group)were subjected to OCT scanning before and 96 h after inoculation with Candida albicans(URM 6547).The biofilm analysis was carried out through a 2D optical Callisto SD-OCT(930 nm)operated in the spectral ***,the images were preprocessed using a3×3 Gaussian filter to remove the noise,and then Otsu binarization,allowing segmentation and pixel *** layer's biofilm formed was clearly defined and,indeed,its visualization is modified by water sorption of each ***-Comfort and *** showed some similarities in the scattering of light between the clean and inoculated samples,in which,the latter samples presented higher values of light signal ***-Comfort samples were the only ones that showed no di®erences between the clean or inoculated ***,the results of this study suggest that OCT is a viable technique to visualize the biolm in reline *** in the literature are still scarcely using the OCT technique to visualize biolm in reline resins,further studies are *** should not contain any references or displayed equations.

Endoscopy of the gastrointestinal and biliary tract is a common procedure and is routinely performed for therapeutic and diagnostic ***,bleeding and infection are some of the more common reported side *** embolism on the other hand,is a rare complication of gastrointestinal *** report a 77-year-old African-American female with a history of pancreatic cancer,which was resected with a Whipple *** part of diagnostic and therapeutic procedure,an endoscopic retrograde cholangiopancreatography was planned several months after the *** patient’s heart rate suddenly slowed to 40 bpm during the procedure and she became cyanotic and difficult to oxygenate after the endoscope was introduced and CO2 gas was insufflated.A forensic autopsy was performed with post-mortem computed tomography(PMCT)and revealed extensive systemic air *** detailed PMCT and autopsy findings are presented and current literature is reviewed.

Clinical analysis of parotid gland sarcoidosis

Purpose: To summarize related clinical experiences in diagnosis and treatment by analyzing 10 cases of parotid gland sarcoidosis。Methods: We retrospectively analysed 10 cases of parotid gland sarcoidosis treated in hospital between 1995 and 2007。Selected patients were 4 males and 6 females,28-51 years old。In 7 cases,sarcoidosis was developed in both sides of parotid glands,which were diagnosed by biopsy and then treated by prednisone。The other patients,who just had one-side gland involved,had been extirpated superficial lobes。Results:The incidence rate of parotid gland sarcodosis was 8.7%。4 out of 5 cases were initially diagnosed wrong。2 patients were combined with lung sarcoidosis。Biopsic and surgical wounds of parotid glands were all healed by first intention。Both parotid glands of patients who were treated by Prednisone decreased in size and softened obviously,further resumed to normal at the end of curative phase。During 8 months to 11 years tracing observation, all 10 patients had not recurrence。Conelusion:Parotid gland sarcoidosis is not a common clinical disease and does not have typical symptoms。Most of them are bilateral。Thus, integrative analysis should be done to eliminate diagnostic wrong。Surgical operation can be used to treat one-side sarcoidosis;however,assured curative effects can be provided by drug treatments for patients who have both-side sarcoidosis。

Aim:This study aims to assess the infection risks of flashlight contamination in a stomatology hospital and compare the disinfection effectiveness of alcohol(75%) and disinfecting ***:The flashlight is a basic non-critical medical device in oral and maxillofacial surgery *** are mostly found in oral cavities;therefore,reusable flashlights may be a potential source of nosocomial infections(NIs).However,the microbial flora present in flashlights used in hospitals has not yet been ***:This study investigated the microbial contamination of 41 flashlights used in a stomatology hospital in Guangzhou in March ***:Results indicated that 75.6%(31/41) of the flashlights had microbial *** bacteria accounted for 72.7%(24/33)of the microbial groups contaminating the flashlights,and Gram-negative bacteria(21.2%,7/33),and fungi(6.1%,2/33) constituted the re maining *** predominantly isolated species was Staphyloccus(66.7%,22/33),especially Staphylococcus aureus(24.2%,8/33).Approximately 77.3%(17/22) of the types of bacteria detected in the hands were same as those in the corresponding *** the bacterial overstandard and *** detection rates of doctors' flashlights were higher than those of nurses' flashlights(16/17 vs.14/23,7/17 vs.1/23,respectively)(P 0.05).Conclusion:Flashlights are potential causes of *** flashlights could be an effective and practical infection control method.
