Purpose: To discuss the cyst organization pathological changed for jaw bone giant cyst diseases by the decompression。Methods: Decompression treatment through 72 examples of jaw bone giant cyst diseases in our branch, cut and took the pouch wall organization to pathological examination, made a follow-up for 1-2 years, two issue of surgeries include except the cyst carry on the pathological examination, contrasts two between the organization changes。Results: 72 examples of jaws bone giant cyst diseases underwent decompression treatment, the pouch wall organization presented the chronic inflammation response, 42 examples assume the pouch wall proliferation type to change, 3 examples treatments defeat, 2 examples transform from the cystic ameloblastoma into squamous cell ameloblastoma, 1 example transforms from cyst into Keratocyst, finally transform into ameloblastoma。Conclusion: After the treatment of decompression for the jaw bone giant cyst diseases, the cyst organization has the inflammation type to changes and the partial pouches wall organization proliferation type changes, the minority cases of may appear the organization pathology change, therefore prompts clinically in decompression treatment to the jaw bone giant cyst pathological changes need the strict observation, to pretend the organization changes。

We report here a very rare case of intestinal Behcet＇s disease with pyoderma gangrenosum. A 16-year-old woman who was diagnosed with intestinal Behcet＇s disease by the presence of cutaneous pathergy together with two major criteria （oral and genital aphthoses） and one minor criterion （gastrointestinal manifestations）, was referred to our hospital with a left lower leg ulcer and abdominal pain in September 1989. Colonoscopy demonstrated flare-up colitis involving the entire colon. Her lower leg lesion was a painful destructive ulcer with an irregular margin and a ragged overhanging edge. Based on these clinical and laboratory findings, we diagnosed her cutaneous ulcer as pyoderma gangrenosum developing with exacerbated intestinal Behcet＇s disease. Her cutaneous and intestinal lesions were poorly controlled though she received oral prednisolone treatment for a month. Because of aggravated abdominal symptoms with peritoneal irritation, we performed total colectomy in November 1989. The resected specimen was histologically compatible with intestinal Behcet＇ s disease showing severe inflammation with deep ulcerations and neutrophil accumulation. Subsequently, pyoderma gangrenosum rapidly improved. This clinical course may suggest the close relationship between pyoderma gangrenosum and intestinal Behcet＇s disease.

OBJECTIVE: To investigate the effect of Lingqi Huangban granule(LQHB) plus intravitreal ranibizumab in the treatment of macular edema(ME) induced by retinal vein occlusion(RVO).METHODS: A prospective, randomized controlled study was conducted. A total of 60 subjects with RVO induced ME were randomized into control group(CG)(30 eyes) and LQHB group(LQHBG)(30 eyes). CG patients underwent intravitreal ranibizumab(IVR) injections. LQHBG patients were treated with oral LQHB combined with IVR injections. In order to reduce the financial burden of the injections, we used one injection and pro re nata(PRN)regimen for both groups. The best-corrected visual acuity(BCVA), central macular thickness(CMT), and mean number of injections were evaluated at the beginning of treatment and 3, 6, 9 and 12 months afterward. All the subjects were followed up for 1 ***: At the beginning of treatment, there were no statistically significant differences between the two groups in terms of the general condition of patients(P > 0.05). At 3, 6, 9 and 12 months after treatment, however, the BCVA scores improved and the CMT measurements decreased in all patients(P < 0.05), with the improvement of LQHBG significantly greater than that of CG(P <0.05). The mean numbers of ranibizumab injections were 1.8 ± 0.3 in LQHBG and 2.3 ± 0.6 in CG, respectively(P < 0.05). No adverse events were reported in both ***: LQHB plus intravitreal ranibizumab could be a much more effective and economic treatment for stabilizing and improving vision with fewer intravitreal injections in the treatment of RVO induced ME. This integrative therapy appears to be a promising option for this type of patient.

Focal nodular hyperplasia(FNH) of the liver is a benign lesion occurring in 0.6%-3% of the general population that probably reflects a local hyperplastic response of hepatocytes to a vascular abnormality. Most lesions are diagnosed incidentally and the natural history of the disease remains largely unknown. It has been shown that most FNH remain stable, or even regress, over a long follow-up period. We present a patient with FNH of the liver who was followed up for 7 years. A 26-yearold woman with a 5-year history of oral contraceptive use was referred to our hospital in February 2005 for further examination of a liver tumour. The diagnosis of FNH was made using magnetic resonance(MR) imaging with hepatospecific contrast media; this technique allows a correct diagnosis, in particular distinguishing FNH from hepatic adenoma, avoiding an invasive procedure such as the lesion biopsy. After 7-year from the diagnosis, we observed the complete spontaneous regression of the lesion by enhanced MR scanning. In this patient, discontinuation of oral contraceptive use and two childbirths may have influenced the natural history of FNH. To our knowledge, in the English literature there is no report illustrating a complete regression of giant FNH but only studies of decreasing in size. The present case suggests that a young patient with giant FNH should be managed conservatively rather than by resection, because FNH has the potential for spontaneous regression.

Background:Hepatocellular carcinoma(HCC)with portal vein tumour thrombus(PVTT)signifies advanced disease,whether LT confers any survival superiority over resection remains ***:A propensity score matched(PSM)analysis of liver transplantation(LT)and liver resection(LR)for HCC with PVTT was ***:A consecutive series of 88 patients who received either LT(10 DDLTs and 3 LDLTs)or LR(n=75)respectively were *** PSM,the LT group has a higher MELD score(17.3 vs.7.8,P<0.001),lower serum AFP levels(96 vs.2,164 ng/mL,P=0.017)and smaller tumour size(4 vs.10 cm,P<0.001).The 5-year overall survival for LT and LR were 55.4%and 15.9%respectively(P=0.007).After matching for serum AFP levels and tumour size,1-,3-and 5-year overall survival for LT were 81 ng/mL,3.9 cm,80%,70%and 70%and the corresponding rates for LR were 1,417 ng/mL,5.3 cm,51.8%,19,6%and 9.8%(P value=0.12,0.27 and 0.009 respectively).Conclusions:LT is associated with significantly better oncological outcomes in HCC patients with PVTT involving the lobar or segmental level.A modest expansion of selection criteria to include small HCC with segmental PVTT should be considered.

Antidiarrheal activity of hexane extract of Citrus limon peel in an experimental animal model

OBJECTIVE： Acute diarrhea is one of the major illnesses that cause death in children, despite clinical interventions and the use of oral rehydration therapy. Thus, there is need to discover other effective, affordable and accessible treatments for this disease. Therefore, this study was carried out to investigate the effects of hexane extract of Citrus limon peel （HECLP） on castor oil-induced diarrhea in rats. METHODS： Diarrhea was induced in male albino Wistar rats weighing 100-150 g. The antidiarrheal activity of HECLP at different oral dosages （5, 10 and 20 mg/kg） was investigated by counting the number of wet fecal pellets. Animals were further treated with propranolol, prazosin, nifedipine and atropine to assess the effects of receptor blockers on the activities of the extract. The effects of HECLP on castor oil-induced enteropooling and the intestinal transit time of activated charcoal were also evaluated. RESULTS： Each of the 3 doses of C. limon significantly reduced （P 〈 0.05） the number of wet fecal pellets produced by animals, with 20 mg/kg HECLP producing the highest percentage inhibition （34.2%） Wet fecal pellet inhibition by the standard drug Ioperamide （3 mg/kg p.o.） was 68.4% relative to the negative control. Blockage of 13 adrenergic receptors by propanolol abolished the antidiarrheal effects of HECLP. Intestinal fluid accumulation was inhibited by 68.7% and 78.5% by 20 mg/kg HECLP and Ioperamide respectively. Furthermore, 20 mg/kg HECLP significantly （P 〈 0.05） reduced the percentage intestinal transit time （21.4% ± 1.42%）, relative to the control （34.2% ±4.29%）; atropine （5 mg/kg, intraperitoneal injection） significantly （P 〈 0.001） reduced the percentage intestinal transit time to 11.2% ±0.85%. CONCLUSION： These results suggest that C. limon peel possesses antidiarrheal effects through antisecretory and antimotility mechanisms that act through the β adrenergic system.

Rationale: Acinetobacter radioresistens is a non-fermentative Gram-negative coccobacillus that is environmentally ubiquitous and is an uncommon cause of pneumonia in an immunocompetent patient with no known chronic medical illness. Patient concerns: A middle-aged Asian male with a smoking history presented with fever and cough. Physical examination was unremarkable. Chest imaging was consistent with pulmonary parenchymal infection and blood culture grew Acinetobacter radioresistens. Diagnosis: Community acquired pneumonia with Acinetobacter radioresistens ***: The patient received a combination of intravenous and oral ampicillinsulbactam over 2 ***: Repeat blood cultures showed resolution of bacteremia. Completion of antimicrobial treatment saw resolution of respiratory symptoms and radiological pneumonic changes. Lessons: Acinetobacter radioresistens causing community-acquired pneumonia in an immunocompetent host has never been described before. It may be a novel emerging infectious agent in pulmonary infections. Its clinical course in this immunocompetent patient appears to be relatively benign.

In terms of global cancer-related deaths,hepatocellular carcinoma (HCC) has the fourth highest mortality *** until 2017,treatment of advanced HCC was largely limited to sorafenib,an oral tyrosine kinase inhibitor,with little to no success in the development of alternative treatment ***,in the past two years,there has been an unprecedented increase in both the number and type of treatment options available for *** of 2019,the US FDA has approved four oral tyrosine kinase inhibitors,two immune checkpoint inhibitors,and one anti-angiogenesis antibody for the treatment of *** with this new variety,systemic treatment of advanced HCC remains largely unsatisfactory,and the median survival rate stands at approximately one *** expected breakthrough of using immune checkpoint inhibitors in advanced HCC did not materialize in *** use of immune checkpoint inhibitors in conjunction with oral tyrosine kinase inhibitors or antiangiogenesis medications is the current clinical research trend,the results of which are eagerly *** limited progress in survival,HCC research is currently experiencing a period of growth and innovation,and there is hope for significant advances in the treatment of advanced HCC as the field continues to develop.
