AIM: To compare safety and efficacy of intravitreal dexamethasone(IVD) implant with topical nepafenac(TN) 0.1% in previously untreated Irvine-Gass syndrome(IGS) in clinical practice. METHODS: This was a retrospective study of 62 eyes with IGS after phacoemulsification with posterior chamber intraocular lens(IOL) implantation. None of the patients used treatment before IVD or TN. Best-corrected visual acuity(BCVA) with Early Treatment Diabetic Retinopathy Study chart(ETDRS), slit-lamp, intraocular pressure(IOP) measurement, fundus examination, spectral-domain optical coherence tomography(OCT) and fundus florescein angiography were performed to all subjects at baseline, 1, 3 and 6 mo. RESULTS: The mean BCVA of the IVD group was 49.3±6.8, and the mean BCVA of the TN group was 32.9±7.3 ETDRS letters in post-treatment month 6. The mean central macular thickness(CRT) of IVD group was 266.6±53.5 μm and the mean CRT of TN group was 364.9±56.3 μm in posttreatment month 6. Baseline BCVA has correlation with final BCVA in TN group however there was no correlation between baseline BCVA and final BCVA in IVD group. CONCLUSION: IVD is found to be better than TN in controlling pseudophakic macular edema and improving visual acuity. IVD group also has significantly lower CRT however IOP is not significantly different between two groups in post-treatment month 6.

Background This study aimed to compare the efficacy of intravenous,intranasal fentanyl and oral sucrose in reducing the pain response during retinopathy of prematurity examinations using premature infant pain profile(PIPP)scores.Method The study included 42 infants who underwent retinopathy screening examinations.The infants were divided into three groups:oral sucrose,intranasal fentanyl,and intravenous fentanyl.Vital signs(heart rate,arterial oxygen saturation,and mean arterial pressure)were recorded.The PIPP was used to determine pain severity.Cerebral oxygenation and middle cerebral artery blood flow were evaluated using near-infrared spectroscopy and Doppler ultrasonography,respectively.The data obtained were compared between groups.Results There was no significant difference between the three groups regarding postconceptional and postnatal ages or birth weights and weight at the time of examination.All babies had moderate pain during the examination.No correlation was observed between analgesia method and pain scores(P=0.159).In all three groups,heart rate and mean arterial pressure increased,whereas oxygen saturation decreased during the exam compared with pre-examination values.However,heart rate(HR),mean arterial pressure(MAP)and arterial oxygen saturation(sPO2)values did not differ between groups(HR,P=0.150;MAP,P=0.245;sPO2,P=0.140).The cerebral oxygenation(rSO2)values between the three groups were found to be similar[rSO2:P=0.545,P=0.247,P=0.803;fractional tissue oxygen extraction(FTOE):P=0.553,P=0.278].Regarding cerebral blood flow values,we also did not find any difference between the three groups[mean blood flow velocity(Vmean):P=0.569,P=0.975;maximum flow velocity(Vmax):P=0.820,P=0.997].Conclusions Intravenous and intranasal fentanyl and oral sucrose were not superior to each other in preventing pain during the examination for retinopathy of prematurity(ROP).Sucrose may be a good alternative for pain control during ROP examination.Our findings suggest that ROP exam may not affect cerebral oxygenatio

Background:More than ten genome-wide association studies have identified the significant association between the gap junction delta-2 (GJD2) gene and myopia.However,no functional studies have been performed to confirm that this gene is correlated with myopia.This study aimed to observe how this gene changed in mRNA and protein level in the form-deprivation myopia (FDM) animal model.Methods:Four-week-old guinea pigs were randomly divided into two groups:control and FDM groups (n =12 for each group).The right eyes of the FDM group were covered with opaque hemispherical plastic lenses for 3 weeks.For all the animals,refractive status,axial length (AL),and corneal radius of curvature were measured at baseline and 3 weeks later by streak retinoscope,A-scan ultrasonography,and keratometer,respectively.Retinal GJD2 mRNA expression and connexin 36 (Cx36) levels in FDM and control groups were measured by quantitative real-time PCR and Western blot analyses,respectively.Those results were compared using independent t test,Mann-Whitney U test,or paired t test.A significance level of P < 0.05 was used.Results:Three weeks later,the FDM group (form-deprived eyes) showed about a myopic shift of approximately-6.75 (-7.94 to -6.31) D,while the control group remained hyperopic with only a shift of-0.50 (-0.75 to 0.25) D (Z =-3.38,P < 0.01).The AL increased by 0.74 (0.61-0.76) and 0.10 (0.05-0.21) mm in FDM and control groups,respectively (Z =-3.37,P < 0.01).The relative mRNA expression of GJD2 in the FDM group decreased 31.58% more than the control group (t =11.44,P < 0.01).The relative protein expression of CX36 on the retina was lowered by 37.72% in form-deprivation eyes as compared to the controls (t=17.74,P < 0.01).Conclusion:Both the mRNA expression of GJD2 and Cx36 protein amount were significantly decreased in the retina of FDM guinea pigs.This indicates that Cx36 is involved in FDM development,providing compensating evidence for the results obtained from genome-wide association studies.

Efficacy of Carbamazepine Combined with Botulinum Toxin A in the Treatment of Blepharospasm and Hemifacial Spasm

Purpose:To observe the efficacy of the combined treatment of carbamazepine and botulinum toxin A for blepharospasm and hemifacial spasm. Methods:Fifty-eight patients with either blepharospasm or hemifacial spasm were randomly divided into treatment and control groups. In the treatment group, 30 patients were administered with local intramuscular injections of botulinum toxin A and oral carbamazepine 100 mg/time,3 times/day for 60 days. Twenty-eight subjects in the control group under-went local intramuscular injections of botulinum toxin A only. Results:After combined treatment, the complete remission rate was 90%, which was significantly higher than that of the of the control group (67.9% , P<0.05,χ2 =4.733). However, no statistical significance was noted regarding the duration of therapeutic effects between the treatment group (range 14～40 weeks; 19.2 weeks on average) and control group (range 12～36 weeks; 18 weeks on average). Conclusion:The combined therapy of carbamazepine and topical injections of botulinum toxin A had increased efficacy in the treatment of blepharospasm or hemifacial spasm, but had no significant effect in terms of the duration of the therapeutic effect.

AIM： To evaluate total antioxidant status （TAS）, total oxidant status （TOS）, and the oxidative stress index （OSI） of the aqueous humor （AH） in patients with glaucoma. METHODS： The prospective study was composed of a study group （n-31） and a control group （n=31）. Fifteen patients in the study group were diagnosed with primary open angle glaucoma （POAG）, and 16 patients were diagnosed with pseudoexfoliation glaucoma （PEG）. The control group was composed of non-glaucomatous patients with cataracts. AH samples were collected and analyzed for TAS, TOS, and OSI levels. RESULTS： Mean AH TAS level was significantly higher in patients with glaucoma than that in the control group （P〈0.01）. Mean TOS and OSI levels tended to increase in patients with glaucoma. No significant differences in TAS, TOS, or OSI levels were observed between patients with POAG and PEG. CONCLUSION： High levels of TAS were observed in patients with glaucoma, which was likely a response to the increased oxidative stress observed in these patients.
