Objective:To determine their reproductive health behaviour concerning fidelity and psychosocial intimacy in the face of current trends in sexually transmitted infections(STIs) and human immunodeficiency virus(HIV)/ acquired immune deficiency syndromes(AIDS) ***:A cross-sectional comparative male/female cohort study using a semi-structured questionnaire to interview sexually activity of undergraduates in University of Calabar(UNICAL) was conducted over a three(3) month period in *** used were number of sexual partners,frequency of sexual intercourse in the 6 months preceding,knowledge of last menstrual period(LMP),methods of self-protection against STIs,number of unwanted pregnancies terminated,and contraceptive ***:Of the 1 337 respondents,648 out of 684 males(94.7%) and 543 out of 653 females (83.2%) had multiple sex *** gender difference was statistically significant(P 0.05).While 275(42.0%) of the females could state the exact last menstrual period(LMP),only 14(2.1%) males could remember their mates LMP,and 496(72.5%) of males did not know anything about mates’menstrual *** was no statistical significance difference(P >0.05 ) between both groups in their safe sex practices. Ageing in man is associated with a decline in testosterone following changes in the hypothalamo-pituitary testicular axis. This may offset the physiologic equilibrium between oestrogen and androgen and at some point when the ratio of free testosterone to oestradiol reaches a critical level, the oestrogenic gonadotropin suppressive effect predominates with decreased release of FSH and LH. Adding to this endocrinal complexity is the continued peripheral conversion to oestradiol through aromatisation. Although the androgen deficiency is not the sole cause for impotence in the elderly, there is a gradual decrease in nocturnal penile tumescence (NPT) and spontaneous morning erections with ageing. Despite the age related increase in oestrogen levels, the information on the pathophysiological role of the 'female hormone' in erectile dysfunction has been scanty. Together with our identification of oestrogen receptors within the penile cavernosum, we have delineated dysfunctional changes on male erection mediated by oestradiol. These findings parallel the recent concerns over environmental oestrogens on fertility declines in young men. Oestrogenic activity is also present in plants and thereby in human diet. These phytoestrogens are structurally and functionally similar to oestradiol and more potent than the environmental oestrogenic chemicals such as organochlorine and phenolic compounds. Thus in the light of growing concerns of possible compromising effects on sexuality by endogenous and environmental oestrogens, we are faced with the scientific need to delineate their role on the mechanism of male erectile pathway in health and disease for clinical correlates and prognostics.

Objective： Vigna subterranea is widely consumed as a traditional staple food in Nigeria and some West African countries. The ethanolic seed extract of V. subterranea （EEVS） was investigated for its gastroprotective effects on aspirin plus pylorus ligation-induced gastric ulcerated rats using an in vivo assay. Methods： Gastric mucosal ulceration was induced experimentally in Groups 2 to 5 using aspirin plus pylorus ligation. Rats in Group I were orally pretreated with 3% Tween 80 only as normal control. Groups 2 to 5 were pretreated with 3% Tween B0 （ulcer group）, 20 mg/kg of omeprazole （positive group）, and 200 and 400 mg/kg of EEVS （experimental groups）, respectively, once daily for 21 days before ulcer induction. Parameters including those for gastric secretions, ulcerated areas and gastric wall histology were assessed. Levels of superoxide dismutase （SOD）, glutathione peroxidase （GPx）, and malondialdehyde （MDA） in the gastric tissue homogenate were also determined. Results： Pretreatment with EEVS significantly （P 〈 0.05） reduced the ulcer index, gastric volume and total acidity in rats with aspirin plus pylorus ligation-induced ulcer. The pH and mucus of gastric content increased significantly （P 〈 0.05） while the levels of SOD and GPx were observed to be elevated with a reduced amount of MDA. Significant severe gastric mucosal injury was exhibited in the ulcer group and EEVS or omeprazole offered significant （P〈0.05） protection against mucosal ulceration. Histologically, the gastric submucosal layer showed remarkable decrease in edema and leucocytes infiltration compared with ulcer group. Conclusion： The study suggests that EEVS offered a protective action against aspirin plus pylorus ligationinduced gastric ulcers in Wistar rats. The protective effect might be mediated via antisecretory, cytopro- tective and antioxidative mechanisms.

CERVICAL CYTOLOGICAL SCREENING AND MANAGEMENT IN PREGNANT AND POSTPARTUM WOMEN

Objeotive To examine and follow up cervical cytology of pregnant and postpartum women and study their cytopathologie characteristics, so as to determine screening and managing programs for abnormal cervical cytology. Mothods Totally 5296 patients in pregnancy and postpartum, in which 3729 by computer-assisted cytology test and 1567 by liquid-based monolayers cytology test, were examined and diagnosed by the Bethesda System made in 2001. Those proven epithelial abnormalities patients were followed up until the lesions regressed to normal. The remaining patients, who exhibited persistent abnormalities or progression, were given further examinations. Results The positive rate of cervical cytological test was 8.72% （462/5296）, including squamous carcinoma （SCA） 1 case （ 0.02% ）, high grade squamous intraepithelial lesion （HSIL） 34 cases （ 0.64% ）, low grade squamous intraepithelial lesion （LSIL） 145 cases （ 2.74% ）, atypical glandular cells （AGC） 5 cases （ 0.09% ）, atypical squamous cells cannot exclude HSIL （ASC-H） 14 cases （0.26%）, atypical squamous cells of undetermined significance （ASC-US） 263 cases （4.97%）. The 419 proven cytological abnormality cases were followed up successfully. The total transnegative rate in three months was 73.74% （309/419）, in which 303 cases （72.32%） persisted normal status for more than six months after regression. And the transnegative rate of ASC-US, ASC-H, AGC, LSIL, and HSIL were 79.56%, 64.29%, 100%, 72.14% and 44.12%, respectively. Forty-six cases received biopsy directed by colposcopy. The distribution of coincidence of cytopathologic and histopathologic diagnosis was： SCA 1 case （100%）, HSIL 10 cases （76.92%）, LSIL 13 cases （65%）, ASC-H 2 cases （50%）, ASC-US 3 cases （37.50%）, total 29 cases （63.04%）. Conclusions We should cast more attention to screening cervix lesions in pregnant and postpartum women. Gasless laparoscopy for benign gynecological diseases using an abdominal wall-lifting system

Objectives: The use of gasless laparoscopy with an abdominal wall-lifting device for benign gynecological diseases was compared to conventional laparoscopy with CO2 pneumoperitoneum. Methods: From February 2007 to July 2007,76 women with uterine and/or adnexal benign diseases and candidates for laparoscopic surgery were recruited in this study. Thirty-two women underwent gasless laparoscopic surgery and 44 women underwent pneumoperitoneum laparoscopic surgery. Results: Diverse pathologies,including adnexal cyst,uterine myoma and ectopic pregnancy,were treated successfully with gasless laparoscopic surgery. Compared with the patients in the pneumoperitoneum group,the similar hospital stay (P=0.353) and intraoperative blood loss (P=0.157) were observed. However,the mean operative time in the gasless group was significantly longer than that in the pneumoperitoneum group (P=0.003). No severe intraoperative or postoperative complications were found in either group,except for one case of laparotomic conversion in the pneumoperitoneum group due to dense pelvic adhesions. The total hospital charges were significantly less in the gasless group than in the pneumoperitoneum group (P=0.001). In 38 cases of ovarian cyst resection,the mean operative time in the gasless group remained longer than that in the pneumoperitoneum group (P=0.017). The total hospital charges were also significantly less in the gasless group than in the pneumoperitoneum group (P<0.001). Conclusion: Our preliminary results demonstrated that the laparoscopic procedure using the gasless technique was a safe,effective method to treat benign gynecological diseases. Moreover,it was easy to master. As a minimally invasive treatment,gasless laparoscopic surgery provides a good choice to patients in the undeveloped regions in China without increasing the patients’ and the government’s burden significantly.

Despite controversy regarding the clinical value of semen analysis, male fertility investigation still relies on a standardized analysis of the semen parameters. This is especially true for infertility clinics in both developing and developed countries. Other optional tests or sophisticated technologies have not been widely applied. The current review addresses important changes in the analysis of semen as described in the new World Health Organization （WHO） manual for semen analysis. The most important change in the manual is the use of evidence-based publications as references to determine cutoff values for normality. Apart from the above mentioned changes, the initial evaluation and handling methods remain, in most instances, the same as in previous editions. Furthermore, the review evaluates the importance of quality control in andrology with emphasis on the evaluation of sperm morphology. WHO sperm morphology training programmes for Sub-Saharan countries were initiated at Tygerberg Hospital in 1995. The external qualitY control programme has ensured that the majority of participants have maintained their morphological reading skills acquired during initial training. This review reports on current sperm functional tests, such as the induced acrosome reaction, and sperm-zona pellucida binding assays, as well as the impact of sperm quality in terms of DNA integrity, and the relationship of sperm function tests to sperm morphology.
