Introduction: Obesity and pregnancy is a major public health problem worldwide, both maternal and fetal. Objective: This is to describe the epidemiological and prognostic aspects of obesity and pregnancy in the gynecology-obstetrics department at the Sylvanus Olympio university hospital Center (CHU SO) in Lomé. Methodology: This was a descriptive cross-sectional study concerning obesed pregnant women. The survey was conducted from the 1st to the 30th of June 2022 at the CHU SO. Results: We enrolled 55 obese pregnant women. The frequency of obesity and pregnancy was 5.14%. Resellers were represented at 41.8%. The average age was 31 years old. As risk factors, 85.5% claimed to have a fatty diet and 76% did not practice sports. The gestational pathologies found during pregnancy were hypertension in 47.4% of cases, preeclampsia in 24.6% and gestational diabetes in 7%. Caesarean section was the way of delivery in 63.6% of cases and those who gave birth vaginally presented a tear of the soft tissues in 85% of cases. Birth weight was abnormal (low weight and excess weight) in 61.8% of cases. Conclusion: The association between obesity and pregnancy constitutes an important risk factor for the mother and the fetus.

Background:Massive bleeding is the main concern for the management of placenta percreta(PP).Intra-abdominal aortic balloon occlusion(IABO)is one method for pelvic devascularization,but the efficacy of IABO is uncertain.This study aims to investigate the outcomes of IABO in PP patients.Methods:We retrospectively reviewed the clinical data of PP cases from six tertiary centers in China between January 2011 and December 2015.PP cases with/without the use of IABO were analyzed.Propensity score matching analysis was performed to reduce the effect of selection bias.Postpartum hemorrhage(PPH)and the rate of hysterectomy,as well as neonatal outcomes,were analyzed.Results:One hundred and thirty-two matched pairs of patients were included in the final analysis.Compared with the control group,maternal outcomes,including PPH(68.9%vs.87.9%,χ^(2)=13.984,P<0.001),hysterectomy(8.3%vs.65.2%,χ^(2)=91.672,P<0.001),and repeated surgery(1.5%vs.12.1%,χ^(2)=11.686,P=0.001)were significantly reduced in the IABO group.For neonatal outcomes,Apgar scores at 1 minute(8.67±1.79vs.8.53±1.68,t=-0.638,P=0.947)and 5 minutes(9.43±1.55vs.9.53±1.26,t=0.566,P=0.293)were not significantly different between the two groups.Conclusions:IABO can significantly reduce blood loss,hysterectomies,and repeated surgeries.This procedure has not shown harmful effects on neonatal outcomes.

Melatonin receptor 1B(MT2,encoded by the MTNR1B gene),a high-affinity receptor for melatonin,is associated with glucose homeostasis including glucose uptake and transport.The rs10830963 variant in the MTNR1B gene is linked to glucose metabolism disorders including gestational diabetes mellitus(GDM);however,the relationship between MT2-mediated melatonin signaling and a high birth weight of GDM infants from maternal glucose abnormality remains poorly understood.This article aims to investigate the relationship between rs10830963 variants and GDM development,as well as the effects of MT2 receptor on glucose uptake and transport in trophoblasts.TaqMan-MGB(minor groove binder)probe quantitative realtime polymerase chain reaction(qPCR)assays were used for rs10930963 genotyping.MT2 expression in the placenta of GDM and normal pregnant women was detected by immunofluorescence,western blot,and qPCR.The relationship between MT2 and glucose transporters(GLUTs)or peroxisome proliferator-activated receptorγ(PPARγ)was established by western blot,and glucose consumption of trophoblasts was measured by a glucose assay kit.The results showed that the genotype and allele frequencies of rs10830963 were significantly different between GDM and normal pregnant women(P<0.05).The fasting,1-h and 2-h plasma glucose levels of G-allele carriers were significantly higher than those of C-allele carriers(P<0.05).Besides,the protein and messenger RNA(mRNA)expression of MT2 in the placenta of GDM was significantly higher than that of normal pregnant women(P<0.05).Melatonin could stimulate glucose uptake and GLUT4 and PPARγprotein expression in trophoblasts,which could be attenuated by MT2 receptor knockdown.In conclusion,the rs10830963 variant was associated with an increased risk of GDM.The MT2 receptor is essential for melatonin to raise glucose uptake and transport,which may be mediated by PPARγ.

EZH2 Contributes to Anoikis Resistance and Promotes Epithelial Ovarian Cancer Peritoneal Metastasis by Regulating m6A

Objective:Histone modification has a significant effect on gene expression.Enhancer of zeste homolog 2(EZH2)contributes to the epigenetic silencing of target chromatin through its roles as a histone-lysine N-methyltransferase enzyme.The development of anoikis resistance in tumor cells is considered to be a critical step in the metastatic process of primary malignant tumors.The purpose of this study was to investigate the effect and mechanism of anoikis resistance in ovarian adenocarcinoma peritoneal metastasis.Methods:In addition to examining EZH2 protein expression in ovarian cancer omental metastatic tissues,we established a model of ovarian cancer cell anoikis and a xenograft tumor model in nude mice.Anoikis resistance and ovarian cancer progression were tested after EZH2 and N6-methyladenosine(m6A)levels were modified.Results:EZH2 expression was significantly higher in ovarian cancer omental metastatic tissues than in normal ovarian tissues.Reducing the level of EZH2 decreased the level of m6A and ovarian cancer cell anoikis resistance in vitro and inhibited ovarian cancer progression in vivo.M6a regulation altered the effect of EZH2 on anoikis resistance.Conclusion:Our results indicate that EZH2 contributes to anoikis resistance and promotes ovarian adenocarcinoma abdominal metastasis by m6A modification.Our findings imply the potential of the clinical application of m6A and EZH2 for patients with ovarian cancer.

Protective Effect and Mechanism of Kuntai Capsule on Angiotensin II-Induced Hypertension in Ovariectomized Rats

Objective To explore the protective effect and mechanism of Kuntai(KT)Capsule on angiotensin II(Ang II)-induced hypertension in ovariectomized(OVX)rats.Methods Fifty-four rats were randomly divided into 6 groups according to a random number table,9 in each group:control,OVX sham+Ang II,OVX,OVX+Ang II,OVX+Ang II+E2,and OVX+Ang II+KT.OVX rats model was constructed by retroperitoneal bilateral ovariectomy.After 4 weeks of pretreatment with KT Capsule[0.8 g/(kg·d)and 17-β-estradiol(E2,1.2 mg/(kg·d)]respectively,Ang II was injected into a micro-osmotic pump with a syringe to establish a hypertensive rat model.Blood pressure of rat tail artery was measured in a wake state of rats using a non-invasive sphygmomanometer.Blood pressure changes were compared between the intervention groups(OVX+Ang II+KT,OVX+Ang II+E2)and the negative control group(OVX+Ang II).Serum malondialdehyde(MDA)level and superoxide dismutase(SOD)activity were detected respectively.The expressions of oxidative stress-related protein superoxide dismutase2(SOD2)and anti-thioredoxin(TRX),autophagy marker protein[beclin1,light chain(LC)3 II/I ratio and autophagy canonical pathway protein phosphatidylinositol 3-kinase(PI3K)/serine/threonine kinase(AKT)-mammalian target of rapamycin(mTOR)]were evaluated by Western blotting.Results Compared with the OVX+Ang II group,the systolic blood pressure of OVX+Ang II+KT group was significantly lowered(P<0.05)but not the diastolic blood pressure.Besides,SOD2 and TRX protein levels in mycardial tissues were significantly reduced in the OVX+Ang II+KT group compared with the OVX+Ang II group(P<0.05).Oxidative stress serum markers MDA and SOD were down-and up-regulated in the OVX+Ang II+KT group,respectively(P<0.05).Compared with OVX+Ang II group,the levels of cardiac proteins beclin-1 and LC3II/LC3 I in OVX+Ang II+KT group were also up-regulated(P<0.05),and the expression levels of p-PI3K,p-AKT and mTOR protein were down-regulated(P<0.05).Conclusion KT could protect blood pressure of Ang II-induced OVX rats by inhibiting

Efficacy of Chinese Herbal Medicines on Pregnancy Outcomes in Patients with Endometriosis in Long-Term Management:A Multicenter Retrospective Cohort Study

Objective:To analyze the factors related to pregnancy of endometriosis and whether Chinese herbal medicines(CHMs)can improve pregnancy outcomes in patients with endometriosis in long-term management.Methods:This multicenter cohort study retrospectively analyzed the clinical data of endometriosis patients with fertility needs from January 2019 to November 2019.A total of 252 patients with endometriosis from 5 level-ⅢGrade A hospitals in Beijing were included in this study.Univariate and multivariate logistic regression analysis were performed for the relevant factors.The propensity score matching(PSM)function of SPSS software was used to match the CHMs group with the non-CHMs group.The pregnancy rate and live birth rate were analyzed.Results:The results of univariate analysis showed that age,disease course,presence of infertility,presence of adenomyosis,time after surgery or use of gonadotropin-releasing hormone agonist(Gn RH-a),use of CHMs and follow-up time were influencing factors of pregnancy in endometriosis patients(P6 months were independent risk factors(OR=0.445,0.348,0.140,respectively,P<0.05),time after surgery or use of Gn RH-a 6 months and use of CHMs were independent protective factors(OR=3.839,3.842,respectively,P<0.05).After PSM,99 pairs of two groups were matched successfully.The pregnancy rate of the CHMs group was higher than that of the non-CHMs group[55.56%(55/99)vs.36.36%(36/99),P<0.05].The live birth rate of the CHMs group was higher than that of the non-CHMs group[49.49%(49/99)vs.35.35%(35/99),P<0.05].Conclusion:CHMs can effectively improve clinical pregnancy rate and live birth rate of patients with endometriosis in the chronic disease management.

Effects of menopausal hormone therapy-based on the role of estrogens,progestogens,and their metabolites in proliferation of breast cancer cells

Menopausal hormone therapy(MHT)has been widely used for the clinical treatment of symptoms associated with menopause in women.However,the exact nature of the relationship between MHT and the increased risk of breast cancer has not been fully elucidated.The results of the Women’s Health Initiative’s randomized controlled clinical studies showed that estrogen monotherapy was associated with a lower incidence of breast cancer as compared to estrogen-progesterone combined therapy,with an elevated risk of breast cancer.The evidence currently available from randomized trials and observational studies is based on data from different populations,drug formulations,and routes of administration.Even though the risks of MHT and breast cancer have received a great deal of attention,information regarding the unpredictable toxicological risks of estrogen and progestogen metabolism needs to be further analyzed.Furthermore,the diversity and complexity of the metabolic pathways of estrogen and different progestogens as well as the association of the different estrogen and progestogen metabolites with the increased risk of breast cancer need to be adequately studied.Therefore,this review aimed to describe the biological effects of estrogen,progesterone,and their metabolites on the proliferation of breast cancer cells,based on relevant basic research and clinical trials,to improve our understanding of the biological functions of estrogen and progestogen as well as the safety of MHT.

To the Editor:Cerclage reduces the rate of preterm birth and improves perinatal outcomes in patients with cervical insufficiency.However,previous findings on this subject are discordant.Many clinical factors affect the outcome of cerclage.The morbidity rate of neonates born after 32 weeks of gestation is reported to be significantly reduced.[1]Therefore,the use of a gestational age of<32 weeks at delivery as the main efficacy measure for cerclage is of practical significance.

With the rapid socioeconomic development, improvements in living standards, and changes in lifestyle, adult diseases, such as cardiovascular diseases, type 2 diabetes mellitus (T2DM), hypertension, obesity, and so on, are increasing dramatically over the past decades, which have caused a huge economic and health burden at both population and individual levels in China.[1] The increased burden of adult diseases has become the main public health issue, and prevention of adult diseases in China has become a formidable task during recent years. In 2016, China has put forward the concept of whole life-course health and efforts have been made on shifting the focus from treating diseases to prevention. In view of the poor effects of prevention of diseases in adulthood, it is suggested that prevention and management of adult diseases should be extended to early life, which is the critical window for physiological programming and has profound and enduring effects on the whole life-cycle health.[2,3,4] In 2018, an article named "To Prevent Cardiovascular Disease, Pay Attention to Pregnancy Complications" published in Journal of the American Medical Association (JAMA) also illustrated that pregnancy is a window to future health[5].

Acupuncture Enhances Chances of Pregnancy in Unexplained Infertile Patients Who Undergo A Blastocyst Transfer in A Fresh-Cycle

Objective: To analyze the effects of acupuncture on in vitro fertilization patients with unexplained infertility.Methods: We retrospectively analyzed the charts of a total of 302 cycles performed in 273 patients attending Assisted Reproductive Technology(ART) Centre from August 2013 to August 2016.During the study period,embryo transfer with acupuncture(Acupuncture group,46 cases) and without acupuncture(Control group,42 cases) were applied.Prior to embryo transfer,the following points were used in the acupuncture group: Neiguan(CX 6),Diji(SP 8),Taichong(Liv 3),Baihui(Gv 20),and Guilai(S 29).These sessions were carried out two times before and after embryo transfer in a single day.In addition,auricular acupuncture was also performed at ear points,including ear point 55(Shenmen),ear point 58(Zhigong),ear point 22(Neifenmi)and ear point 34(Naodian).The biochemical pregnancy rate and clinical pregnancy rate after transplantation were compared between two groups.Results: The clinical pregnancy rate in the acupuncture group was higher than that in the control group [60.9%(28/46) vs.33.3%(14/42),respectively,P<0.05].Likewise,the live birth rate in the acupuncture group was also higher than that in the control group [71.7%(33/46) vs.31.0%(12/42),P<0.01).Conclusion: Administration of acupuncture on the day of embryo transfer dramatically improved fertility results in women who underwent in vitro fertilization/intra-cytoplasm sperm injection for reproduction.
